1.Diagnosis and treatment for gastrointestinal stromal sarcomas

Objective To explore the clinical behavior,pathological and immunohistochemical characteristics and diagnosis, treatment methods of gastrointestinal stromal sarcomas (GISS). Methods Retrospective analysis was perfomed over 36 GISS patients based on clinical data and pathological data. Results GISS occurred more in the stomach and small intestine, than colon and rectum. Immunohistochemical staining showed CD117, CD34 expression,the positive rates were 91.7% and 86. 1%. Conclusion GISS is a rare malignant tumor, GISS diagnosis depends on the histologic and immunohistochemical test. CD34 and CD117 in the positive diagnosis has its special significance. Surgery is the main treatment.
2.Perioperative nursing of patients receiving colorectal tumor surgery under peritoneoscope

Objective To discuss the perioperative nursing method for patients receiving colorec-tal tumor surgery under peritoneoscope. Methods Using the nursing procedure, through appraisal of patients' health situation→listing the nursing diagnosis or the nursing questions→formulation of nursing plan→ implementation of nursing plan→appraisal of the expected effect, combination of the surgical nursing's characteristic, to practice the idea of "the perfect person health and concern", cooperation with medical and nursing team, we provided a complete treatment and nursing plan for 38 patients receiving colorectal tumor surgery under peritoneoscope. Results 38 patients made satisfactory results with no surgery complications. The average in-hospital time was 9 days. Conclusions This technology has slight wound,light hemorrhage,slight draw-off, less pain, short exposure time of intestine, fast recovery of intestine function and early food intake. Systematic and intact perioperative nursing can improve the nursing quality and the success rate and reduce the complications so that the patients can live through the surgery safely.
3.The mechanism and prevention of bronchial pulmonary dysplasia

Bronchopulmonary dysplasia is one of the causes of affecting children′s health,which has a complicated etiology,including the gene level of internal aspect and some external factors, such as premature birth inducing the pulmonary hyperplasia,infectious diseases caused by low immunity,complications caused by clinical interventions,and the low quality of respiratory tract management. In the article,we review the research progress about mechanism and prevention of BPD in domestic and overseas in recent years.
4.The sensation and transduction of mechanical stimulus in cells

Mechanical stresses modulate almost all aspects of cell function, including growth, differentiation, migration, gene expression, protein synthesis and apoptosis. Thus, uncovering the mechanisms by which living cells sense and transduce mechanical stress lies at the core of understanding how they respond and adapt to their physical environments. In this review, the possible mechanism about cell response to mechanical stimulus was discussed from outer cell membrane to inner nucleus.
5.Clinical observation and thinking of procedure for prolaps and hemofrhoids in treatment of mixed hemorrhoids combined with non-functional obstructed defecation syndromes

Objective To observe the clinical effect of procedure for prolaps and hemofrhoids (PPH) in treatment of mixed hemorrhoids combined with non-functional obstructed defecation syndromes (ODS).Methods The clinical data of 50 patients with mixed hemorrhoids combined with non-functional ODS treated by PPH were retrospectively analysed.All these patients were followed up for 12 months,the treatment outcome was observed.Results Eleven cases were basic cured,apparently effective in 12 cases,and effective in 12 cases,invalid in 15 cases,the total effective rate was 70% (35/50),inefficacy patients were mainly combined with rectocele.None patients had serious complications such as death,anastomotic infection and pelvic sepsis.Conclusion PPH convenient operation,small trauma,for the treatment of mixed hemorrhoids combined with internal rectal prolapse is safe and effective,but the effective is poorer in treatment of mixed hemorrhoids combined with rectocele.
6.Expression and Correlated Analysis of Autophagy-related Proteins in Human Squamous Cell Carcinoma and Adenocarcinoma of Lung

Objective To explore expression of Beclin1, autophagy-related protein 1 (Atg1), mammalian target of ra-pamycin (mTOR) in squamous cell carcinoma (SCC) and adenocarcinoma of lung and to analyse relationship among these three factors. Methods Immunohistochemical stainning was applied to detect expression of Beclin1, Atg1 and mTOR in 82 samples of SCC and adenocarcinoma of lung (lung cancer group) and 17 samples of paraneoplastic normal lung tissues (con-trol group). Correlations of expression of Beclin1, Atg1 and mTOR were analyzed in SCC and adenocarcinoma of lung. Re-sults The expression of Beclin1 was significantly lower in lung cancer group than that in control group(52.44%vs 94.12%, P<0.01). There were significant differences of the differentiation, TNM stages and lymph node metastasis in lung cancer group compared with those in control group. And Beclin1 expression showed a decreasing trend with malignant and invasive of tumors. mTOR expression in lung cancer tissue with lymph nod metastasis is higher than that in the tissue without lymph node metastasis(P<0.01). Atg1 expression in lung cancer tissue with larger mass is less than that in lung cancer tissue with small lumps(P<0.01). Beclin1 Expression was positively correlated with the Atg1 expression in SCC(r=0.609,P<0.01). Beclin1 and Atg1 expressions were both negatively correlated with mTOR expression in SCC(r=-0.617,-0.444,P<0.01). Beclin1 expression was positively correlated with Atg1 expression in adenocarcinoma(r=0.458,P<0.01). Beclin1 and Atg1 expression were both negatively correlated with mTOR expression in adenocarcinoma(r=-0.497,-0.541,P<0.01). Conclu-sion Beclin1, Atg1 and mTOR expression showed a strong correlation in lung cancer, and clinicopathological change of lung SCC and adenocarcinoma may be related to autophagy.
7.Expression of podoplanin-positive stromal cells in esophageal squamous cell carcinoma and its clinical significance

Purpose To study the correlation between the expression of podoplanin-positive stromal cells and clinicopathologic features in squamous cell carcinoma ( SCC) of esophagus, and to explore the role of podoplanin-stromal cells in the progression of SCC of e-sophagus. Methods Immunohistochemical EnVision method was used to detect the expression of podoplanin in the stromal cells of e-sophageal SCC. The correlation between the expression of podoplanin-positive stromal cells and clinicopathologic features in SCC of e-sophagus was analyzed. Results Of 69 cases of SCC, podoplanin-positive stromal cells were seen in 43 and the positive rate was 62. 3%. Lymphatic vessel density in SCCs with podoplanin-positive stromal cell was 45. 72 ± 14. 36, which was higher than that with-out podoplanin-positive stromal cell (21. 23 ± 10. 49) (P<0. 01). Expression of podoplanin in stromal cells in SCCs with lymphatic vessel invasion was higher than those without lymphatic vessel invasion (P<0. 05). There was no correlation between podoplanin-posi-tive stromal cells and age, sex, tumor size, the depth of invasion, differentiation, lymph node metastasis, and clinical stage ( P>0. 05). Conclusion Podoplanin-positive stromal cells may promote the lymphangiogenesis and lymph vessel invasion of esophageal SCC.
8.Empirical study of treatment of peritoneal carcinomatosis with a combination of adenoviral p53 gene therapy and thymosin ?1

Objective To evaluate the cooperative effect of intraperitoneal adenoviral p53 gene therapy together with thymosin ?1 against peritoneal carcinomatosis. Methods KM mouse model bearing peritoneal carcinomatosis was established using H22 cell line. 48 hours after inoculation, adenoviral-mediated p53 gene was administrated intraperitoneally alone, or combined with thymosin ?1 within 7 days. The abdominal girth, body weight, tumor cell number, apoptosis and cell cycle were tested using methods of cell counting and flow cytometry. Results Adenoviral p53 gene transfer alone arrested H22 cell in G0/G1. In combination therapy group, ascitic fluid was obviously halted, tumor cells in peritoneal cavity decreased in number and ascites formation, dead cell ratio was elevated. Conclusion Adenoviral-mediated p53 gene could block cell proliferation. Intraperitoneal adenoviral p53 gene therapy combined with thymosin ?1 showed synergism for the lethal damage and inhibition for peritoneal carcinomatosis.

Eleven salt-losers (SL group) and eleven non-salt-losers (NSL group) of congenital a-drenal hyperplasia due to 21-hydroxylase deficiency (21-OHD) were evaluated for sodium status under routine regimen of glucocorticoid (4 salt-losers with mineralocorticoid at the same time). Venous samples were collected for the measurement of plasma renin activity (PRA), aldosterone (Aldo) , and a-drenocorticotropin (ACTH), and for serum cortisol (F), 17-OH progesterone (17-OHP), and testosterone (T). Urine samples were collected for 17-ketosteroid (17-KS). 10 age-matched normal children were chosen as control. 10.Evaluation of risk factors of adrenal incidentaloma during follow up

Objective To assess the risk factors of adrenal incidentalomas during follow up. Methods In 43 patients with adrenal incidentalomas, the significant risk factors were screened by univariate analysis, and the multivariate analysis was evaluated by logistic regression. Results After an average of (4.0?1.2)years of follow up,4 out of in 21 patients with adrenalectomy were verified to be malignant. By means of multivariate analysis, tumor size ( P =0.018)and abnormal level of hormonal or its metabolic products ( P =0.039)were the risk factors for adrenal incidentalomas and adrenalectomy should be considered when indicated. Conclusions The gains and losses of open surgery for adrenal incidentalomas should be weighed up.If the evaluation of risk factors reveals a possibility of malignancy,adrenalectomy should be undertaken.
