Nonalcoholic fatty liver disease(NAFLD) is the most common liver disorder in the world.A significant,albeit relative small,proportion of NAFLD patients can advance from steatohepatitis to liver cirrhosis and hepatocellular carcinoma.Moreover,NAFLD is associated independently with an increased incidence of type 2 diabetes and cardiovascular disease.The mortality of patients with NAFLD is significantly higher than that among the general population and cardiovascular risk may compete with liver-related risk in dictating the final outcome.From a clinical point of view,it has become mandatory to evaluate the metabolic risk factors in NAFLD patients and to consider careful surveillance and aggressive treatment of both hepatic and cardiovascular outcomes.Nowadays,there are substantial advances in the ability to make the diagnosis of NAFLD as well as both grade and stage the disease,however,liver biopsy remains the gold standard.Lifestyle changes are the first line and mainstay of management of NAFLD,weight loss and treatment of insulin resistant remain central to the therapeutic process.Specific pharmacological treatment is currently not recommended for routine clinical practice.

Objective To evaluate the safety,middle or short -term effect and indication of the endovascular treatment for ischemic stroke patients with extracranial carotid stenosis.Methods The clinical datas with percutaneous transluminal angioplasty and stenting for extracranial carotid artery stenosis cases were retrospectively reviewed,from which 106 cases with ischemic stroke in the past were selected for study.And theinformation including operation opportunity,operation experience,serious complications such as cerebral hemorrhage and secondary ischemic stroke were summarized.Their clinical features,methods of treatment and prognosis were analyzed specially.Results The placement of 106 cases with 120 stents was satisfaction.Cerebral hemorrhage occurred in 3 cases while thrombosis fol-lowing stent placement in 2 cases,hypotension and bradycardia in 6 cases,vasospasm in 11 cases,gastrointestinal stress bleeding in 1 case,restenosis occurred in 8 cases and no cerebral infarction occurred after operation.Apart from one case of death cerebral hemorrhage,8 cases of restenosis,the,middle or short -term results of the remaining 97 patients were satisfactory.83 cases were followed up for 3 months to 5years,there were no ischemic attack and cerebral infarction occurred.Conclusion The satisfactory effect and safety are achieved in the therapy of carotid stenosis by percutaneous transluminal angioplasty and stenting.Angioplasty and stenting may improve the cerebral hemodynamics of patients with severe /moderate internal carotid artery stenosis.It is suitable for previous ischemic stroke patients.We must be individualized to more carefully assess these patientsˊsurgical risk,strictly adhere to the indications of surgery,and general measures is the key of preventing the complications during the perioperative period.

ObjectiveTo investigate the difference in cell apoptosis between keratoacanthoma and squamous cell carcinoma.Methods Ninety specimens were obtained from the lesions of 30 patients with regressing keratoacanthoma (rKA) and 30 patients with well-differentiated squamous cell carcinoma (wdSCC) as well as from the normal skin of 30 human controls.Immunohistochemistry was carried out to measure the expressions of B-cell leukemia/lymphoma 2(Bcl-2),cysteine-aspartic acid protease(Caspase)-3,second mitochondria-derived activator of caspase/direct inhibitor of apoptosis-binding protein with low pl (Smac/DIABLO) and inhibitor of apoptosis protein Livin.ResultsThe Bcl-2 protein expression was significantly lower in rKA specimens than in normal control specimens(t =3.1572,P ＜ 0.05).A lower expression of Caspase-3 protein was observed in rKA specimens compared with wdSCC specimens (t =2.1364,P ＜ 0.05).The expression of Smac/DIABLO protein was significantly increased in normal control specimens than in rKA and wdSCC specimens(t =7.6141,9.5666,respectively,both P ＜ 0.05).Livin was absent in normal skin,but highly expressed in rKA and wdSCC specimens,and the difference was significant between the normal skin and lesional (rKA and wdSCC) specimens ( t =4.7913,12.7737,respectively,both P ＜ 0.05).The expression of Livin protein was significantly reduced in rKA specimens than in wdSCC specimens(t =7.9824,P ＜ 0.05).Conclusions Cell apoptosis plays a certain role in the natural regression process of KA.The detection of apoptosis-regulating proteins Caspase-3 and Livin are beneficial for the differentiation between rKA and wdSCC.

Objective To evaluate whether the MDR1 exon 26 genotype is associated with the dosage of FK506 and acute rejection in renal transplant patients. Methods One hundred and six patients who administrated FK506 after renal transplantation were retrospectively analyzed. The patients were investigated for MDR1 genotype determined by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP) assays. According to the genotype, the patients were divided into 3 subgroups, wild homozygous-CC, mutant homozygous-TT and heterozygous-CT. At 3rd, 6th and 12th month after renal transplantation, enzyme-linked immunosorbent assay (ELISA) was used to measure the FK506 whole blood concentration. Whole blood FK506 concentration per dose/kg/day and acute rejection within one month postoperation were compared among 3 subgroups. Results Of the 106 patients, 32 (30.2 %), 44 (41.5 %) and 30 (28.3 %) were carriers of CC, CT, TT in MDR1 exon 26, respectively. AIM:To establish the fingerprint for Cacumen Platycladi (carbonized) by HPLC. METHODS:The column of Lichrospher C 18 (250 mm?4.6 mm 5 ?m)was used. The mobile phase consisted of 0.5‰ trifluoroactic acid-methanol with gradient elution. The detective wavelength was at 375 nm,and the flow rate was 1.0 mL/min. Different habitats were compared by Similarity Evaluation System for Chromatographic Fingerprint of CMM Version 2004A. RESULTS:The fingerprint consisted of 14 common peaks. The mutual mode of HPLC fingerprints was set up and the similarity of the crude drugs was in the range of 0.178-0.963. The standard HPLC fingerprint of Cacumen Platycladi (carbonized) was established too. CONCLUSION:This method is accurate and reliable and provides a scientific basis for the quality control of Cacumen Platycladi (carbonized).
