1.Protection and Rehabilitation of Forearm Rotation Function after Fracture of Radius: 53 Case Report

ObjectiveTo study the rehabilitation for the forearm rotation function handicap after the fracture of radius. Methods53 patients with the forearm rotation function handicap after operation for the fracture of radius were reviewed. ResultsThe patients were followed up for 2~7 years. The angles of pronation and supination in forearm were 23.47° and 34.42° before treatment, and were 66.59° and 78.56° after treatment (P<0.01). The patients with the course of disease less than 2 months procured better forearm rotation function the those more than 2 months (P<0.01). ConclusionThe forearm rotation function need to be prevented and rehabilitated early after the fracture of radius.
2.Efficacy of non-surgical treatment of lumbar spinal stenosis in elderly patients

目的探讨应用非手术方法治疗老年患者腰椎管狭窄症的疗效。方法对74例老年(>60岁)腰椎管狭窄的患者采用手法矫正椎体的假性滑移、药物治疗、适当的腰背肌功能锻炼综合治疗。随访至少2年。采用自定标准评定疗效。结果优(无疼痛及间歇性跛行,无功能限制,不用止痛剂)34例,良(偶然疼痛及间歇性跛行,轻度功能受限,偶尔使用止痛剂)27例,一般(中度的疼痛及间歇性跛行,中度功能限制,无运动,经常使用止痛剂)11例,差(中度每天疼痛及间歇性跛行,或偶尔严重疼痛,每天日常生活活动受限,应用强止痛剂)2例。结论对老年腰椎管狭窄症采取综合的非手术方法能取得一定疗效。
3.Expression and significance of serum osteoprotegerin and related inflammatory factors in patients with coronary heart disease

Objective:To investigate the expression and significance of serum osteoprotegerin and related inflammatory factors in patients with coronary heart disease.Methods:According to case-control study method, 236 subjects with chest pain admitted to Nanyang Second General Hospital Affiliated to Xinxiang Medical University from March 2018 to July 2019 were collected.According to the results of coronary angiography, they were divided into 132 cases of coronary heart disease group and control group(non-coronary Patients with heart disease) 104 cases.Questionnaire survey to collect general data of patients; 5 mL of fasting peripheral venous blood was collected in the morning, and serum was collected by centrifugation.Enzyme-linked immunosorbent assay was used to detect osteoprotegerin and monoclonal antibody to receptor activator of nuclear factor kappa B(RANK) ligand, interleukin 6 (IL-6), C-reactive protein, insulin-like growth factor 1 (IGF-1), monocyte chemoattractant protein 1 (MCP-1), matrix metalloproteinase 9 (MMP-9) levels.Results:The levels of serum osteoprotegerin, IL-6, C-reactive protein, IGF-1, MCP-1, and MMP-9 in patients with coronary heart disease were (1.85±0.49) μg/L, (65.93±5.18) ng/L, (15.74±2.52) mg/L, (725.19±13.36) μg/L, (302.16±15.92) μg/L and (58.31±7.94) μg/L, respectively, which were significantly higher than those in control group (1.42±0.44) μg/L, (47.56±3.51) ng/L, (1.91±0.67) mg/L, (228.61±12.05) μg/L, (246.39±10.28) μg/L and (37.09±4.76) μg/L.Soluble RANK ligand (332.69± 14.91) ng/L was significantly lower than the control group (380.85±19.56) ng/L.The difference was statistically significant (t value were 4.739, 21.065, 29.721, 27.637, 18.911, 16.463 and 17.085, respectively, all P<0.05). The levels of serum osteoprotegerin, IL-6, C-reactive protein, and IGF-1 were statistically significant among different lesion groups (all P<0.05). The levels of osteoprotegerin, IL-6, C-reactive protein and IGF-1 in three vessel disease group ((2.05±0.51) μg/L, (80.96±25.70) ng/L, (19.79±2.03) mg/L, (849.07±18.95) μg/L) were significantly higher than the double-vessel disease group ((1.83±0.45) μg/L, (62.74±20.61) ng/L, (13.82±1.75) mg/L, (714.84±19.06) μg/L) and single-vessel disease group ((1.61±0.42) μg/L, (53.09±18.37) ng/L, (9.67±1.40) mg/L, (507.51±17.83) μg/L), and the double-vessel disease group was significantly higher than the single-vessel disease group, the difference was statistically significant (all P<0.05). Multivariate unconditional Logistic regression analysis showed that serum osteoprotegerin and IGF-1 are the influencing factors of coronary heart disease.ROC curve analysis showed that the AUC of serum osteoprotegerin was 0.827.At the best cut-off point of 1.54 μg/L, the diagnostic sensitivity was 84.09% (111/132) and the specificity was 73.48% (97/132) ; the AUC of serum IGF-1 was 0.883, which was at the best cut-off point.At 395.78 μg/L, the diagnostic sensitivity was 71.21% (94/132) and the specificity was 96.21% (127/132). Conclusion:Serum osteoprotegerin and related inflammatory factor IGF-1 are influential factors in the occurrence of coronary heart disease, and are positively correlated with the severity of coronary artery disease, and have a good diagnostic value for the occurrence of coronary heart disease.
4.Diagnosis and treatment of knee patellar plica under arthroscopy

Objectives To discuss the diagnosis and treatment of patellar plica under arthroscopy.Methods A retrospective survey was performed in 75 cases with patellar plicas under arthroscopy from Jun.2008 to June.2012.All patients were admitted by the clinic.All the patients diagnosised by arthroscopy and clinical evidence, and cuted off under arthroscopy.After the operation, patients were followed-up in the clinic and by telephone for 12 to 60 months, according to the knee joint Lysholm scoring system to assess curative effect.Results All the patients with successful operation, the operation time was 15-25 minutes.All cases after the operation without complications such as infection, quicker recovery of joint function.After 12 to 60 months, 70 patients were followed up, the knee joint Lysholm excellent score was 92.9%.Conclusions Inferior patellar plica under is a type of knee joint synovial plica syndrome, the arthroscopic surgery can determine diagnosis and do effective treatment.
5.The treatment of the fractures of the talar neck

Objective To study the characteristics and classifications of the fractures of the talar neck as well as to define the appropriate treatments and its complications. Methods 47 patients of the fractures of the talar neck were referred to our department of orthopaedics from July 1996 to November 2001. There were 32 males and 15 females with an average age of 31.6 years. The injuries were caused by fall in 13, traffic accident in 28, and others in 6. According to Hawkins classification, there were type Ⅰwith nondisplaced vertical fracture in 6 patients, which were treated non-operatively, and immobilized with a non weight bearing short leg cast; type Ⅱ in 26 patients, and type Ⅲ in 15 patients, all of which were treated in emergency with anatomic reduction and fixation with 4.0 mm lag screws through anteromedial or anterolateral approaches of the ankle. Otherwise, the arthrodesis of subtalar joint in type Ⅲ fracture were primarily performed as well. The ankles were immobilized until the fracture healed. Results The patients were followed up 2 to 5 years (average, 3.6 years). By the assessment of clinical effects depending on the complaints, functions and radiological results of the ankle and subtalar joint, 17 patients were rated as excellent, 22 patients good, 5 fair, and 1 poor, the total rate of excellent and good was 83.0%. Osteonecrosis occurred in 8 patients with 3 of type Ⅱ and 5 of type Ⅲ fractures. 3 of 4 patients with talar displacement were found with osteonecrosis or painful arthritis. The subtalar arthrosis occurred in 6 patients, 4 of which were associated with ankle arthrosis. 3 patients underwent arthrodesis because of painful arthritis of the subtalar joint or osteonecrosis of the talar body. Anteromedial skin necrosis of the ankle developed in 1 patient. Conclusion If the fracture of talar neck is not managed appropriately as early as possible, its complications are common, the fractures should be treated in emergency to reduce the incidence of posttraumatic osteonecrosis, arthrosis and malunion of the fractures.
6.Role of injection of different doses of melatonin for scoliotic deformity in the pinealectomized chickens

Objective To find out if different doses of melatonin have different roles in the cause or treatment of idiopathic scoliosis, and to illuminate whether melatonin can be a substitution in the treatment of pinealectomized chickens. Methods Fifty Little-Shaoxing chickens, in which pinealectomy was performed when 3-day-old, were divided into five groups. Ten served as control group, were kept in light-dark (12h: 12h) cycle, 500 lx in daytime and 0-5 lx in nighttime after surgery. Ten served as M-5 group, were given 5 mg/kg (5 milligrams per kilogram of body weight) melatonin twice every day. Ten as M-10 group, given 10 mg/kg melatonin abdominally twice every day, ten as M-20 group, given 20 mg/kg melatonin twice every day, and ten as M-40 group, given 40 mg/kg melatonin twice every day. The light-dark (12h: 12h) cycle was the same as control group. Radiological examinations were performed on all chicken spines for scoliosis when at 3 months old. Measured of the angle of the scoliosis was taken by Cobb's technique, and the data was analyzed by statistical software. Results Overall, there was 25 chickens developed to scoliosis in all when the chickens were 3 months. The attack rate was only 56%; this rate was consistent throughout all experimental groups. In the control group, 6 chickens had obvious scoliosis, and 5 in M-5 group, 5 in M-10 group, 4 in M-20 group, and 5 in M-40 Group. And the average Cobb's angle of control group was 26.7?. There were 23.5?, 21.7?, 24.5? and 23.2? in M-5, M-10, M-20 and M-40 respectively. There was no statistical significance in the average Cobb's angle and the incidence of scoliosis among all groups. It was determined that neither the prevalence nor the pattern of the scoliosis was affected by the therapy in any of the experimental groups. Conclusion Different dose of melatonin can't reduce the rate of scoliosis in the pinealectomized chickens, and also can't prevent the development of the experimental scoliosis. The results of this study raise doubts regarding the role of melatonin in the development of scoliosis after pinealectomy in the young chicken. Melatonin acting as a vicarious therapeutic tool in the treatment of idiopathic scoliosis is premature, deserved further research.
7.Indoor Air Pollution and Health Effects in Newly Decorated Offices

Objective To research the indoor air pollution in offices induced by decoration and its health effects. Methods According to the completed time, the offices were divided into 6 groups (2 month group, 2-4 month group, 5-8 month group, 9-11 month group, 12-18 month group, over 5 year group). The concentration of indoor air pollutants was determined and 617 people from the decorated offices were randomly selected to finish the questionnaires on health. Results The over standard rate of formaldehyde, benzene and ammonia in indoor air was the highest in 2 month group, 100%, 87.3%, 83.9% respectively. The concentration of indoor air pollutants decreased as the time went on, about one year after decoration, the concentrations were lower than the standard limits (GB/T 18883-2002). Objective To study the forearm angulation deformities and observe its influence on the rotation function of the forearm.The importance of the interosseous to forearm rotation function was also analyzed. Methods Eight human cadaver specimens, with the forearm superfical and deep fascia, antebranchial extensor and flexor groups dissected off were used to study the origin and insertion of the pronator teres and supinator, and the movements of the fibers of the interosseous membrane and oblique cord during forearm rotation. Single and double fracutures of the radius and the ulna were produced in ten other human cadaver upper limb specimens with the pronator teres, supinator, interosseous membrane, oblique cord, proximal and distal radio-ulnar joint preserved. The fractures were fixed with plates and screws, and were made into models with different directions and degrees of angulation deformity. The upper limb were fixed with elbow at 90? of flexion in order to observe the effects of angulation deformities upon the forearm rotation function. Results The oblique cord and the upper third interosseous membrane fibres control the excessive pronation of the forearm while the middle and lower third interosseous membrane fibres control the excessive supination; so the oblique cord and interosseous membrane limited the rotation range of the forearm. The forearm angulate deformity resulted from single or double fractures are both be able to cause the forearm rotational disturbance; when the angular deformity exceeds 5? , disturbance of rotational function of the forearm will occur. The disorder of interosseous membrane and the oblique cord, the slope of the articulus radio-ulnaris distalis and proximalis and the osseous obstruction are the main causes of the disturbance of rotation function. Conclusion The interosseous membrane limits the scope of forearm rotation and the deformity of angulation can cause the hindrance of the forearm rotation.

Objective:Defensins from human neutrophils exhibit a broad antimicrobial activity.The transfection of the gene fused by human neutrophil peptide 1(HNP1) with the J chain into other cells may induce the in vitro expression of HNP1 to play antimicrobial roles.The authors analyzed the influencing factors on the expressed proteins of HNP1 and the J chain gene in COS-7 cells.Methods: Western blot was used to analyze the technical factors influencing the HNP1 protein expression.Results: Recombinant J-HNP1 was expressed in the cells successfully.Conclusion: The expression of J-HNP1 can be detected inside and outside the cells by Western blot.
10.Effect of alendronate on OPG and RANKL in periprosthetic osdeolysis induced by polyethylene particles: an experimental study

Objective To investigate effect of alendronate on OPG and RANKL in periprosthetic osdeolysis induced by polyethylene particles.Methods Twelve rabbits which had been implanted a titanium plug in femur by intercondylar notch were divided into two groups randomly,polyethylene particles were injected into the left knee joint,one received alendronate,and the other placebo as control.After 12 weeks,all rabbits were sacrificed.Periprosthetic tissues were observed by ELISA.Results The concentration of OPG in the experimental group was not higher than that of the control (P >0.05 ).But the concentration of RANKL in the experimental group was lower(P <0.01 ).And specific value of OPG/RANKL was higher in the experimental group ( P < 0.05 ).Conclusion The therapy of alendronate can change the concentration of RANKL and specific value of OPG/RANKL in periprosthetic osdeolysis induced by polyethylene particles,inhibit aseptic loosening of prosthesis in rabbits.
