1.The changes of serum IFN -gamma,TNF alpha and IL -10 levels in ulcerative colitis and its relevance to helicobacter pylori infection

Objective To study the changes of serum IFN -gamma,TNF alpha and IL -10 levels in ulcera-tive colitis and its relevance to helicobacter pylori infection.Methods 80 UC patients were selected as the research object,and 50 cases of 80 patients were in the activity stage of UC,30 patients were in remission.In the 50 patients with active UC:14 cases were mild,26 cases were moderate,and 10 cases were severe.30 healthy subjects were set as healthy controls.Detection of serum IFN -gamma,TNF alpha,IL -10 levels and Hp infection were made.Results The serum TNF alpha and IL -10 levels of the activity group were higher than those of the remission group and the control group,the difference was statistically significant[TNF alpha :(54.75 ±5.92)ng/L vs.(23.69 ±4.15)ng/L vs.(20.45 ±4.01)ng/L,F =36.79,P =0.000;IL -10 :(35.16 ±6.72)ng/L vs.(25.12 ±5.37)ng/L vs.(23.96 ± 5.21)ng/L,F =35.68,P =0.000],the serum IFN -gamma level of the activity group was lower than those of the remission group and the control group,the difference were statistically significant[(14.98 ±3.15)ng/L vs.(26.06 ± 4.31)ng/L vs.(27.16 ±4.39)ng/L,F =35.68,P =0.000].As the aggravating of patients with UC in activity stage,TNF alpha and IL -10 levels showed a trend of increase(F =32.81,24.95,all P <0.05),while IFN -gamma showed a trend of reduce(F =22.29,P <0.05).In 50 cases of active UC patients,18 patients with Hp positive, 32 cases were Hp negative.In active UC group Hp positive patients'serum TNF alpha and IL -10 levels higher than the Hp negative patients,the difference was statistically significant(t =4.62,4.35,all P <0.01).Serum levels of IFN-gamma of active UC patients with Hp positive group was lower than the Hp negative patients,the difference was sta-tistically significant(t =5.91,P <0.01).Conclusion In active UC patients expression of TNF alpha and IL -10 are high,and IFN -gamma expression is low.The expression of IFN -gamma,TNF alpha and IL -10 are closely related to Hp infection.
2.Qualitative study of in depth interview on 24 hours treatment and management model of children with cerebral palsy in Shanghai

Objective To investigate the feasibility of implementing the 24 hours treatment and management model of children with cerebral palsy in Shanghai,in order to provide a set of effective and saving manpower,material and financial rehabilitation and management model for children with cerebral palsy.Methods Firstly,the 10 exports engaged in cerebral palsy rehabilitation were selected as the in depth interviewees by the sampling method of grounded theory.Secondly,applying in depth interviews,the 10 exports were interviewed by designing interview outline and subjects.Last,the interview data were collected and analyzed.Results By analyzing the interview data in three-stage coding mode,three factors affecting the feasibility of 24 hours treatment and management model of children with cerebral palsy in Shanghai were obtained,including the favorable factors,obstacles and necessary factors.Conclusions The 24-hour treatment and management model of children with cerebral palsy in Shanghai is feasible,but it will encounter some obstacles in the implementation process.Through the policy support for health care and education sectors,the feasibility of the pattern will be greatly enhanced.
3.Autologous leukocyte′s labeling for the diagnosis of patients with suspected intraabdominal infection

Objective To evaluate the effectiveness of scanning with labeled autologous leukocytes for the detection of abdominal inflammation in equivocal patients. Methods From July 1998 to April 2000 sixteen patients with equivocal abdominal inflammation were recruited into this study. ResultsThere were 10 cases with positive scanning. The diagnosis of intraabdominal infection was consequently confirned by laparotomy and or good response to antibiotic therapy.The 6 cases with negative result were eventualy proven to have no intraabdominal infection. 4.EFFECT OF ZINC DEFICIENCY ON THE GENE EXPRESSION OF VITAMIN D RECEPTOR AND CaBP IN DUODENAL MUCOSA OF RATS

Objective: To study the effect of zinc deficiency(ZD)on the gene expression of vitamin D receptor(VDR)and CaBP in duodenal mucosa of rats. Methods: Thirty weaning male rats were randomly divided into three groups:ZD,paired-fed(PF),zinc adaquate(ZA). After 15 d feeding, took the duodenal mucosa to extract RNA,and measured the levels of VDR mRNA and CaBP mRNA in duodenal mucosa by real time fluorescent quantity PCR. Results: The VDR gene expression of ZD group was downregulated 88.89% as that of PF group,and 50.00% as that of ZA group. CaBP gene expression of ZD group was downregulated 80.16% as that of the PF group,and 88.50% as that of ZA group. Conclusion: Zinc deficiency, by changing the activity of VDR and the mRNA expression of VDR,affects the transcription of the target gene CaBP,and then the absorption of calcium that causes allo-osteogenesis.
5.Analysis of Contemporary Rehabilitation Research

In this paper, 6 kinds of SCI-indexed international rehabilitation journals and all the articles published in 3 kinds of China Rehabilitation Source Journals in recent 1 year were counted. This article compared the difference of research methods and topics of the direction between our country and foreign countries were compared, analyzed the basic principles of rehabilitation medicine and influencing factors about the topics, and the future trends in rehabilitation research.
6.Infliximab versus disease modifying antirheumatic drugs in the treatment of ankylosing spondylitis

Objective To evaluate the efficacy and safety of infliximab and traditional disease modifying antirheumatic drugs (DMARDs) in the treatment of ankylosing spondylitis (AS). Methods Sixty patients with definite AS were treated with infliximab 5 mg/kg infusion at 0, 2, 6, 12 weeks and were followed up for 12 weeks. The primary endpoint was proportion of ASAS 20 responders at week 12. The secondary endpoints were the proportion of ASAS 50, the change from baseline in Bath AS functional index (BASFI).The improvement of signs and symptoms of AS and physical function were evaluated. The statistical treatments were used t-test andA2 test. Results The proportion of ASAS 20 responders at 2, 6, 12 week was 70%, 83% and 93% respectively. The proportion of ASAS50 responders at patients at 2, 6, 12 week was 13%, 37% and 57% respectively. Results for other secondary efficacy endpoints showed that infliximab could provide substantial benefits to patients with AS by reducing clinical signs and symptoms and improving range of motion, physical function and quality of life. Ten percent of the subjects reported treatment- related adverse events. The most frequently occurred were upper respiratory tract infection, followed by gastrointestinal adverse events and infusion reaction. Most treatment-related adverse events were mild to moderate in severity and disappeared after drug withdrawal. Conclusion Infliximab has been demonstrated to be effective and is well tolerated in the treatment of AS.
7.Influence of acupuncture on parturition process

Objective To observe the influence of acupuncture treatment on parturition process.Methods 545 pregnant women delivered in our hospital were selected,excluding those with cesarean section indication.On voluntary principle and randomized principle,primipara aged from 22 to 30 who agreed with natural labor were divided into two groups:observation group and control group.Pregnant women in observation group were received acupuncture treatment during latent phase.The following indexes of the two groups were recorded and compared:labor time of the first stage and the second stage,the case for inertia of uterus,cesarean sections,postpartum hemorrhage amount and Apgar score of new-born.Results The labor time of the first stage and the second stage of the control group were (178.55 ± 56.87) min and (130.70 ± 77.22) min,and which of the observation group were (53.87 ±22.33) min and (33.25 ± 15.55) min,there was significant differenec between the two groups(t =3.97,4.11,all P <0.05).The differences in the pilot failed to cesarean section rate(14.2% and 13.6%),postpartum hemorrhage and the perinatal outcome were not significant between the two groups.Conclusion Acupuncture and moxibustion can accelerate labor.
8.Golgi dispersal in irradiated cells and the protective effect of vanillin derivatives

Objective To determine the Golgi dispersal in radiation damaged cells and the protective effect of vanillin derivatives.Methods Immunofluorescence, cell cycle analysis of flow-cytometry,Western blot,and clone formation were used.Results Immunofluorescence observation showed that the Golgi dispersal caused by 2 Gy 60 Coγ-ray was significantly increased in a dose-dependent manner in the range of 4-10 Gy as was demonstrated by the fact that the Golgi area was significantly increased. When the irradiated cells were treated with the radioprotective agent VND3207, a vanillin derivative,the Golgi dispersal induced by radiation was significantly reduced.The radiation-induced Golgi dispersal was also displayed in a pattern of time-course after irradiation in the HeLa cells, and persisted at least to 36 h post-irradiation. Cell cycle test results indicated that the Golgi dispersal was not associated with the G2/M arrest triggered by radiation-induced DNA damage response.VND3207 could promote cell survival by plate colony formation assay.Conclusion The Golgi dispersal can be caused byγ-ray irradiation in a dose-and time-dependent manner, and VND3207 can provide a good protection against radiation injury associated with inhibited Golgi dispersal.
9.Optimization of the Formulation of Dimemorfan Phosphate Tablets by Orthogonal Test

OBJECTIVE:To optimize the formulation of Dimemorfan phosphate tablets. METHODS:Using 60 min dissolution rate of dimemorfan phosphate as index,L9(34) orthogonal test was used to optimize the amount of starch,microcrystalline cellu-lose,croscarmellose sodium and concentration of HPMC E5 solution. The friability,hardness,60 min dissolution rate and main component were detected. The similarity of dissolution curves of Dimemorfan phosphate tablets was compared with that of imported tablets in 0.1 mol/L hydrochloric acid,pH 6.8 phosphate buffer,water and pH 4.0 acetate buffer. RESULTS:The optimized formu-lation of Dimemorfan phosphate tablet(1 000 tablets)was composed of dimemorfan phosphate 10 g,starch 60 g,microcrystalline cellulose 40 g,10% HPMC E5 solution and croscarmellose sodium 25 g. The friability,hardness,60 min dissolution rate and main component of 3 batches of Dimemorfan phosphate tablets prepared by optimized prescription were 0.42%-0.58%,9.8-10.5 kg,94.89%-96.21% and 99.21%-99.52%,respectively. In 4 dissolution mediums,similar factors f2 of dissolution curves between prepared tablets and imported tablets were above 50. CONCLUSIONS:Dimemorfan phosphate tablets were prepared successfully. The optimized formulation is rational. The dissolution behavior of prepared tablets is similar to that of imported tablets in vitro.
10.Effect of Acupuncture Combined with Doctor-nurse Cooperation Mode on Stroke-associated Pneumonia

Objective To investigate the efficacy of acupuncture combined with doctor-nurse cooperation mode on stroke-associated pneumonia in patients with acute stroke. Methods 67 patients of acute stroke within 1 week in our hospital from March, 2013 to January, 2015 were randomly divided into control group (n=35) and treatment group (n=32). Both groups received neurological routine treatment and systematic rehabilitation training by professional physiotherapists under general nursing mode. The treatment group received acupuncture treatment under doctor-nurse cooperation mode additionally. Incidence of stroke-associated pneumonia, swallow function and aspiration rate were recorded. Results Compared with the control group, the incidence of stroke-associated pneumonia significantly decreased in the treat-ment group after treatment (χ2=4.615, P=0.032). The retention volume of epiglottic vallecula and pyriform sinus (tdilute barium=21.01, tthick barium=19.85), the duration of pharynx period (tdilute barium=17.03, tthick barium=13.68), and the start time of pharynx period (tdilute barium=19.63, tthick barium=17.86) were less in the treatment group than in the control group (P<0.05). In the test of dilute barium, the aspiration rate was lower in the treatment group than in the control group (χ2=4.03, P=0.044), however, there was no significant difference between 2 groups in the test of thick bari-um and solid food (P>0.05). Conclusion Acupuncture combined doctor-nurse cooperation mode may decrease the rates of aspiration and stroke-associated pneumonia in acute stroke patients through improving their swallow function.
