1.Prenatal Ultrasonic Diagnosis and the Neonatal Surgical Treatment for Fetus Related Surgical Malformations

Objective To explore a suitable mode of prenatal ultrasound diagnosis of fetal malformations with early neonatal surgical treatment. Methods Nine fetus with surgical malformations diagnosed prenatally by ultrasound in our hospital from Mar. 1998 to Dec. 2000 were analysed. Results There were two cases diagnosed in the second trimester, 7 in the third trimester, including 1 duodeneal atresia with annular pancreas, 1 abdominal wall defect, 1 congenital diaphragmatic hernia, 1 huge omphalocele, 1 high imperforate anus, 4 hydronephrosis (2 mild and 2 severe). Six cases were operated in neonatal period, two were treated conservatively, and one did induced abortion. All cases recovered except 1 death. Conclusions The mode of prenatal ultrasound diagnosis with surgery in early neonatal period is a suitable way in China before the fetal surgery is applied. It is important to make effort to increase sensitivity of sonographic diagnosis of fetal malformation and to get early treatment in perinatal period.
2.Rapid Identification of Staphylococcus aureus and Its Resistance Against Meticillin Using Slidex-staph Monoclonal Antibody

OBJECTIVE To develop a rapid method for simultaneously identifying Staphylococcus aureus(SA) and its resistance against meticillin. METHODS The target strains authenicated to Staphylococcus by traditional methods(appearance,Gram-stain,catalase) first,then using Slidex~staph.plus to authenticate SA;establishing traditional method or coagulase test and ATB-ID32STAPH system for verification.The penicillin-binding protein 2a(PBP2a) was examined by Slidex MRSA Detection,establishing resistance oxacillin sieving test and mecA gene was examined by PCR for verification. RESULTS The 135 strains were positive and 321 strains were negative for Slidex~staph.plus from 456 strains.The 127 S.aureus strains and eight others were confirmed from 135 positive strains finally,11 SA strains and 310 other strains were confirmed from 321 negative strains,there were 92.0% for sensitivity and 97.5% for specificity in this method.The 52 strains PBP2a positively confirmed to meticillin-resistant S.aureus(MRSA) using Slidex MRSA Detection and 57 MRSA strains were confirmed using resistance oxacillin sieving test or PCR from 138 strains.There were 91.2% for sensitivity and 100% for specifity in this method. CONCLUSIONS The duplex Slidex~-staph monoclonal antibody examinated to SA and confirmed to MRSA has higher sensitivity and specificity.
3.Comparison of Laparoscopic and Open Surgery for Intestinal Atresia in Newborn

Objective To investigate the efficacy of laparoscopic surgery for intestinal atresia in neonates.Methods A perspective study,from February 2002 to November 2006,a series of 61 newborns with intestinal atresia were randomly divided into laparoscopic and open surgery groups.The intra-and post-operative records of the two groups were compared.Results The survival rate and postoperative complications of the two groups were similar (29/31 vs 28/30,x~2=0.001,P=0.973;and x~2=1.298,P= 0.862).In the laparoscopic group,the intestine was pulled out through the umbilical trocar and then resected;the umbilical area was normal after intestinal anastomosis;no scars or only small scars were observed on the abdomen after the operation.The two groups were followed up for 6 to 18 months.During the period,3 patients showed enterocolitis (2 in the laparoscopic group and 1 in the open surgery group);3 patients developed adhesive intestinal obstruction (1 in the laparoscopic group and 2 in the open surgery group), both the complications were cured by conservative therapies.Conclusions Laparoscopic surgery is simple,safe,and effective for intestinal atresia in newborn.
4.Functions of PARylation in DNA Damage Repair Pathways

Protein poly ADP-ribosylation (PARylation) is a widespread post-translational modifica-tion at DNA lesions, which is catalyzed by poly(ADP-ribose) polymerases (PARPs). This modifi-cation regulates a number of biological processes including chromatin reorganization, DNA damage response (DDR), transcriptional regulation, apoptosis, and mitosis. PARP1, functioning as a DNA damage sensor, can be activated by DNA lesions, forming PAR chains that serve as a docking platform for DNA repair factors with high biochemical complexity. Here, we highlight molecular insights into PARylation recognition, the expanding role of PARylation in DDR path-ways, and the functional interaction between PARylation and ubiquitination, which will offer us a better understanding of the biological roles of this unique post-translational modification.
5.Clinical efficacy and safety of leflunomide combined with irbesartan in lupus nephritis

Objective To study the clinical efficacy and safety of leflunomide combined with irbesartan in the treatment of lupus nephritis . Methods 80 cases of patients with lupus nephritis in Yueqing Hospital Affiliated to Wenzhou Medical University from May 2014 to May 2016 were selected and randomly divided into leflunomide group ( leflunomide combined with irbesartan group ) and cyclophosphamide group ( cyclophosphamide combined with irbesartan group),40 cases in each group.The urine indexes and blood indexes levels,clinical curative effect,adverse reaction of the two groups were statistically analyzed.Results The 24h urine protein,urine beta 2-MG,urine red blood cell count,blood beta beta2-MG,ESR,Cr levels of the leflunomide group were significantly lower (P<0.05),the serum albumin,C3 levels were significantly higher (P<0.05),the total treatment efficiency 97.5%was significantly higher than the cyclophosphamide group 82.5%(P<0.05),the incidence of adverse reactions 5.0%was significantly lower than the cyclophosphamide group 22.5%(P<0.05).Conclusion Leflunomide combined with irbesartan is safe and effective in the treatment of lupus nephritis.
6.Surgical treatment of elderly far lateral lumbar disc herniation via Quadrant channel system under the Wiltse approach

Objective To explore the clinical effect of the elderly far lateral lumbar disc herniation via Quadrant channel system under the Wiltse approach. Methods Thirty-three elderly patients suffering from far lateral lumbar disc herniation were treated by surgery, among whom 18 patients were treated by Quadrant minimally invasive channel system (Quadrant group), and 13 patients were treated by percutaneous transforaminal endoscopic discectomy (PTED group). The patients were followed up for 12 months, and the operation time, blood loss and length of incision were investigated. The JOA scores were compared preoperatively and postoperatively. Results The patients in two groups were followed up for 24 months, except 1 patient withdrew in Quadrant group. The incision length and blood loss in PTED group were superior to those in the Quadrant group:2.0-2.5 cm vs. 0.7 cm, (46.50± 15.20) ml vs.(23.87±6.58) ml, P<0.01, but the operation time in PTED group was longer than that in Quadrant group: (77.17 ± 8.71) min vs. (63.20 ±10.87) min, P<0.01.The two groups had achieved good curative effect, while JOA score in PTED group was superior to the Quadrant group 1 week after operation ( P<0.05), but in 3 month and 12 month two groups had no significant difference ( P>0.05). Conclusion Surgical treatment on elderly far lateral lumbar disc herniation via quadrant channel under wiltse approach is an effective method, and can achieve similar effect with PTED.
7.Association between SLC30A8 gene polymorphisms and diabetes mellitus after renal transplantation

BACKGROUND:Previous studies have found that the susceptibility genes of adiponectin gene and calpain 10 gene of type 2 diabetes are closely related with the incidence of diabetes in Chinese renal transplantation patients. So, are other susceptibility genes of type 2 diabetes also associated with posttransplantation diabetes mel itus? OBJECTIVE:To investigate the association between the zinc transporter solute carrier family 30 member 8 (SLC30A8) gene polymorphism and the posttransplantation diabetes mel itus. METHODS:A total of 97 patients with posttransplantation diabetes mel itus and 301 patents without posttransplantation diabetes mel itus (control group) were selected, and then the SLC30A8 gene rs13266634 genotype was detected with real-time PCR method. The association between gene polymorphism and posttransplantation diabetes mel itus was analyzed with Logistic regression test. RESULTS AND CONCLUSION:There were significant differences in al ele frequencies and genotype distributions of rs13266634 between the patients with and without posttransplantation diabetes mel itus (P<0.05). After adjustments of age, sex, body weight and body mass index, the incidence of posttransplantation diabetes mel itus of the CC genotype patients was 2.108 times to that of the TT genotype patients (odds ratio=2.108, 95%confidence interval:1.075-4.131, P=0.044);and the incidence of posttransplantation diabetes mel itus of the CC+CT genotype patients was 1.862 times to that of the TT genotype patients (odds ratio=1.862, 95%confidence interval:1.049-3.306, P=0.034). The results suggest that the C-al ele in rs13266634 of SLC30A8 gene is the independent risk factor of posttransplantation diabetes mel itus.

Aim Myocardial cells of neonatal rats were cultured in vitro to study aim of morphine on serum hungry-induced apoptosis in cardiac myocytes and its mechanism.Methods Myocardial cells of neonatalrats were cultured in vitro.48 hours later,different agents were added to cardiac myocytes.The cellular survival was determined with MTT colormeteric assay;apoptosis rates were determined by Annexin V-FITC/PI;cell cycle was determined by flow cytometry;Caspase-3 and PKC were investigated by Western blot.Results Free-serum induced apoptosis in cardiac myocytes was shown after 48 hours;morphine(1 ?mol?L-1)could inhibit apoptosis in cardiac myocytes,manifestation,apoptosis rates were decreased,Caspase-3 experssion was decaeased Naloxone at 10 ?mol?L-1 inhibited the promoting effects of morphine;GF109203X at 10 ?mol?L-1 or Staurosporine at 1 ?mol?L-1 could inhibit the promoting effects of morphine,manifestation,apoptosis rates were increased,Caspase-3 experssion was incaeased,PKC expression was decreased.Conclusion morphine inhibited serum hungry-induced apoptosis in cardiac myocytes via PKC signal transduction pathway.

Aim To investigate the possible mechanism of ?-opioid receptor inhibiting serum deprivation-induced apoptosis of neonatal cardiac myocytes.Methods Myocardial cells of neonatal rats were cultured in vitro,after 48 h,the medium was changed to serum-free DMEM.The experimental groups were:A:Normal;B: Model;C: Model+DADLE(0.1 ?mol?L-1);D:Model+DADLE(0.1 ?mol?L-1)+naltrindole(10 ?mol?L-1);E:Model+DADLE(0.1 ?mol?L-1)+GF109203X(10 ?mol?L-1);F:Model+DADLE(0.1 ?mol?L-1)+staurosporine(1 ?mol?L-1).The cell viability was determined with MTT colormeteric assay;apoptosis index and the percentage of G0 in cell cycle were determined by flow cytometry;Caspase-3 and PKC signal pathway was investigated by Western blot.Results ?-opioid receptor agonist [D-Ala2,D-Leu5]-enkephalin(DADLE) could significantly inhibit serum deprivationinduced apoptosis of neonatal cardiac myocytes,which increased the survival index of cardiac myocyte,percentage of G2/M in cell cycle and the expression of PKC,decreased the apoptotic index of cardiac myocyte,percentage of G0/G1 in cell cycle and the expression of activate caspase-3.The protective effect of DADLE was obviously blocked by ?-opioid receptor antagonist Naltrindole at 10 ?mol?L-1,the PKC inhibitor GF109203X at 10 ?mol?L-1 and staurosporine at 1 ?mol?L-1,which decreased the survival index of cardiac myocyte,percentage of G2/M in cell cycle and the expression of PKC,increased the apoptotic index of cardiac myocyte,percentage of G0/G1 in cell cycle and the expression of caspase-3.Conclusion These findings suggested that ?-opioid receptor activation might be a potential survival factor against serum deprivation-induced myocardial cell apoptosis and this cardioprotective effect might be via PKC pathway.
10.Effects of berberine on the transport of P-gp substrates across Caco-2 and L-MDR1 cell monolayers

Aim To investigate effects of berberine (Ber) on the transport of P-gp substrates including cyclosporine A (CsA) and digoxin across Caco-2 and L-MDR cell monolayers. Methods Permeability coefficients and transport rates of digoxin and CsA across Caco-2 and L-MDR1 monolayers were measured in the presence of Ber(50 ?mol?L-1~5 mmol?L-1). Results The inhibition studies for digoxin transport showed a dose-dependent decrease in basal-to-apical direction across Caco-2 and L-MDR1 monolayers in the presence of Ber (50 ?mol?L-1~5 mmol?L-1). Both a dose-dependent decrease in basal-to-apical direction and an increase in apical-to-basal direction were observed for CsA transport across Caco-2 monolayers with different concentrations of Ber. The IC50 values calculated for Ber-induced inhibition of digoxin transport are 1.44 mmol?L-1 in Caco-2 cells and 1.24 mmol?L-1 in L-MDR1 cells, respectively. The IC50 value for Ber-induced inhibition of CsA transport is 607 ?mol?L-1 in Caco-2 cells. Conclusion It was suggested that the inhibition and saturation of P-gp transport activity might be involved in interactions between Ber and CsA.
