Objective To explore a suitable mode of prenatal ultrasound diagnosis of fetal malformations with early neonatal surgical treatment. Methods Nine fetus with surgical malformations diagnosed prenatally by ultrasound in our hospital from Mar. 1998 to Dec. 2000 were analysed. Results There were two cases diagnosed in the second trimester, 7 in the third trimester, including 1 duodeneal atresia with annular pancreas, 1 abdominal wall defect, 1 congenital diaphragmatic hernia, 1 huge omphalocele, 1 high imperforate anus, 4 hydronephrosis (2 mild and 2 severe). Six cases were operated in neonatal period, two were treated conservatively, and one did induced abortion. All cases recovered except 1 death. Conclusions The mode of prenatal ultrasound diagnosis with surgery in early neonatal period is a suitable way in China before the fetal surgery is applied. It is important to make effort to increase sensitivity of sonographic diagnosis of fetal malformation and to get early treatment in perinatal period.

OBJECTIVE To develop a rapid method for simultaneously identifying Staphylococcus aureus(SA) and its resistance against meticillin. METHODS The target strains authenicated to Staphylococcus by traditional methods(appearance,Gram-stain,catalase) first,then using Slidex~staph.plus to authenticate SA;establishing traditional method or coagulase test and ATB-ID32STAPH system for verification.The penicillin-binding protein 2a(PBP2a) was examined by Slidex MRSA Detection,establishing resistance oxacillin sieving test and mecA gene was examined by PCR for verification. RESULTS The 135 strains were positive and 321 strains were negative for Slidex~staph.plus from 456 strains.The 127 S.aureus strains and eight others were confirmed from 135 positive strains finally,11 SA strains and 310 other strains were confirmed from 321 negative strains,there were 92.0% for sensitivity and 97.5% for specificity in this method.The 52 strains PBP2a positively confirmed to meticillin-resistant S.aureus(MRSA) using Slidex MRSA Detection and 57 MRSA strains were confirmed using resistance oxacillin sieving test or PCR from 138 strains.There were 91.2% for sensitivity and 100% for specifity in this method. CONCLUSIONS The duplex Slidex~-staph monoclonal antibody examinated to SA and confirmed to MRSA has higher sensitivity and specificity.

Objective To investigate the efficacy of laparoscopic surgery for intestinal atresia in neonates.Methods A perspective study,from February 2002 to November 2006,a series of 61 newborns with intestinal atresia were randomly divided into laparoscopic and open surgery groups.The intra-and post-operative records of the two groups were compared.Results The survival rate and postoperative complications of the two groups were similar (29/31 vs 28/30,x~2=0.001,P=0.973;and x~2=1.298,P= 0.862).In the laparoscopic group,the intestine was pulled out through the umbilical trocar and then resected;the umbilical area was normal after intestinal anastomosis;no scars or only small scars were observed on the abdomen after the operation.The two groups were followed up for 6 to 18 months.During the period,3 patients showed enterocolitis (2 in the laparoscopic group and 1 in the open surgery group);3 patients developed adhesive intestinal obstruction (1 in the laparoscopic group and 2 in the open surgery group), both the complications were cured by conservative therapies.Conclusions Laparoscopic surgery is simple,safe,and effective for intestinal atresia in newborn.

Protein poly ADP-ribosylation (PARylation) is a widespread post-translational modifica-tion at DNA lesions, which is catalyzed by poly(ADP-ribose) polymerases (PARPs). This modifi-cation regulates a number of biological processes including chromatin reorganization, DNA damage response (DDR), transcriptional regulation, apoptosis, and mitosis. PARP1, functioning as a DNA damage sensor, can be activated by DNA lesions, forming PAR chains that serve as a docking platform for DNA repair factors with high biochemical complexity. Here, we highlight molecular insights into PARylation recognition, the expanding role of PARylation in DDR path-ways, and the functional interaction between PARylation and ubiquitination, which will offer us a better understanding of the biological roles of this unique post-translational modification.

Objective To study the clinical efficacy and safety of leflunomide combined with irbesartan in the treatment of lupus nephritis . Methods 80 cases of patients with lupus nephritis in Yueqing Hospital Affiliated to Wenzhou Medical University from May 2014 to May 2016 were selected and randomly divided into leflunomide group ( leflunomide combined with irbesartan group ) and cyclophosphamide group ( cyclophosphamide combined with irbesartan group),40 cases in each group.The urine indexes and blood indexes levels,clinical curative effect,adverse reaction of the two groups were statistically analyzed.Results The 24h urine protein,urine beta 2-MG,urine red blood cell count,blood beta beta2-MG,ESR,Cr levels of the leflunomide group were significantly lower (P<0.05),the serum albumin,C3 levels were significantly higher (P<0.05),the total treatment efficiency 97.5%was significantly higher than the cyclophosphamide group 82.5%(P<0.05),the incidence of adverse reactions 5.0%was significantly lower than the cyclophosphamide group 22.5%(P<0.05).Conclusion Leflunomide combined with irbesartan is safe and effective in the treatment of lupus nephritis.

Objective To explore the histological properties of isolated chondrons and chondrocytes from rabbit knee cartilage,and to determine if these properties vary with age.Methods Three groups of rabbit knees were evaluated according to different age:(1) young (2 months,n=10);(2) adult (8 months,n=10);and (3) old (31 months,n=10).The cartilage structure,proteoglycan,collagen-2,and collagen-6 content were determined by light microscopic using hematoxylin-eosin (HE),Toluidine Blue,and col-2,6 staining.The chondrons were enzymatically isolated using 0.3 g/L dispase and 0.2 g/L collagenase-2 by shaking for 3 hours.The morphology and composition of isolated chondrons were observed by HE and collagen-6 immunostaining staining after overnight coverslip monolayer culture under a microscopy.Results The chondrocytes became sparser and the total content of proteoglycans and collagen-2 were decreased in the articular cartilage with age.Compared to the chondrocytes,the surrounding rim or capsule was more obvious in the isolated chondrons,and they exhibited obvious differences in shape.The cells within one cluster from different age groups were similar to the morphology observed in cartilage in situ.The adult and old chondrons generally possessed a thicker pericellular matrix with more enclosed cells,and the chondrons contained more cells can reach 47％.Conclusion These findings further suggest that the properties of the chondrons and pericellular matrix have an important influence on the biomechanical microenvironment of the knee joint cartilage degeneration that occurs with age.

OBJECTIVE:To evaluate the efficacy and safety of Fluvastatin vs. Xuezhikang for hyperlipidemia in patients after renal transplantation. METHODS: 56 hyperlipidemia patients after renal transplantation were enrolled: 32 were assigned to receive Fluvastatin (40～80 mg) po qd for 8 weeks and 24 to receive Xuezhikang (0.6 g) po bid for 8 weeks. Total cholesterol (TC),triglyceride (TG),low-density lipoprotein-cholesterol (LDL),high-density lipoprotein-cholesterol (HDL),liver function and renal function in two groups were measured before and after treatment. RESULTS: In Fluvastatin-treated group,the TC decreased from(7.39?1.98)mmol?L-1 to(5.62?0.93)mmol?L-1,LDL reduced from(3.68?1.13)mmol?L-1 to (2.86?0.83)mmol?L-1;in Xuezhikang-treated group,TC decreased from(6.82?1.29)mmol?L-1 to (5.56?1.19) mmol?L-1 and LDL decreased from (3.26?0.73) mmol?L-1 to (2.78?0.80) mmol?L-1,all showing significant differences as compared with before treatment(P 0.05). No obvious adverse effect was noted in either group during treatment. CONCLUSION: Both Fluvastatin and Xuezhikang are safe and effective for hyperlipidemia in patients after renal transplantation.

Objective To explore the incidence of olfactory dysfunction in patients with Parkinson disease and the characteristic as well as its possible influencing factors. Methods The SS-12 was used to evaluate the olfactory function of 106 patients with Parkinson's disease and 110 healthy volunteers. The data was then compared between the two groups to investigate the correlation of olfactory function with age, gender, education, smoking, disease duration, Hohn-Yahr stage, UPDRSⅢscores, the dosage of levodopao and olfactory scores. Results Mean identification scores were significantly lower in patients(5.97 ± 2.27)than in controls(8.04 ± 2.00)(t=7.108, P=7.108). Parkinson's disease group did worse than the control group in identifying some odors including peppermint, bananas,liquorice,coffee,pineap?ple,rose and fish (P<0.05). Receiver Operating Characteristic curves revealed that 7.5 was the best olfactory dysfunc?tion diagnostic boundary value with 67.3%sensitivity and 73.6%specificity. Thus, the incidence of olfactory dysfunc?tion in Parkinson's disease was 73.6%. Correlation analysis showed that gender(rs=0.243, P=0.012), education(rs=0.208, P=0.032) and smoking(rs=0.279, P=0.279) were associated with odor identification ability, while age, disease du?ration, Hohn-Yahr stage, UPDRSⅢscores and the dosage of levodopa were not related with odor identification abilityin Parkinson disease (P>0.05). Conclusion Olfactory dysfunction occurs in Parkinson disease with an hign incidence rate. Olfactory function has nothing to do with disease duration, Hohn-Yahr stage, UPDRSⅢscores and the dosage of levodopa in Parkinson disease.

Objective To evaluate the therapeutic efficacy of hook plate and fascia lata graft in management of chronic acromioclavicular joint dislocation.Methods From May 2012 to March 2014,a total of 16 cases of chronic acromioclavicular joint dislocation (Rockwood type Ⅲ) were treated with hook plate and fascia lata graft.They were 10 men and 6 women,from 19 to 57 years of age (mean,34.7 years).Seven cases were at the left side and 9 at the right side.All injuries resulted from direct violence.The time from injury to surgery ranged from 8 to 54 weeks (mean,12.4 weeks).At the final follow-ups,the shoulder function was assessed using the Quick Version of the Disabilities of the Arm,Shoulder and Hand Score (DASH) and visual analogue scale (VAS),and the efficacy using Karlsson's evaluation system.Results All surgical wounds primarily healed.The dislocation of acromioclavicular joint was corrected,without any vascular or neural damage.The patients were followed up for an average of 12 months (from 6 to 18 months).No plate breakage,screw loosening or redislocation was observed.The DASH and VAS scores of the shoulder at the final follow-ups (25.42 ± 8.34 and 2.22 ± 1.70,respectively) were significantly improved,compared with the preoperative ones (43.72 ± 12.35 and 6.33 ± 1.37,respectively) (P ＜ 0.05).According to the Karlsson's criteria,10 cases were excellent,4 good and 2 moderate.Conclusion In management of Rockwood type Ⅲ chronic acromioclavicular joint dislocation,hook plate and fascia lata graft can restore dynamic balance and stability of the acromioclavicular joint,resulting in solid fixation,early joint motion and limited complications.

Objective To explore the clinical effect of the elderly far lateral lumbar disc herniation via Quadrant channel system under the Wiltse approach. Methods Thirty-three elderly patients suffering from far lateral lumbar disc herniation were treated by surgery, among whom 18 patients were treated by Quadrant minimally invasive channel system (Quadrant group), and 13 patients were treated by percutaneous transforaminal endoscopic discectomy (PTED group). The patients were followed up for 12 months, and the operation time, blood loss and length of incision were investigated. The JOA scores were compared preoperatively and postoperatively. Results The patients in two groups were followed up for 24 months, except 1 patient withdrew in Quadrant group. The incision length and blood loss in PTED group were superior to those in the Quadrant group:2.0-2.5 cm vs. 0.7 cm, (46.50± 15.20) ml vs.(23.87±6.58) ml, P<0.01, but the operation time in PTED group was longer than that in Quadrant group: (77.17 ± 8.71) min vs. (63.20 ±10.87) min, P<0.01.The two groups had achieved good curative effect, while JOA score in PTED group was superior to the Quadrant group 1 week after operation ( P<0.05), but in 3 month and 12 month two groups had no significant difference ( P>0.05). Conclusion Surgical treatment on elderly far lateral lumbar disc herniation via quadrant channel under wiltse approach is an effective method, and can achieve similar effect with PTED.
