1.Rapid Determination of Four Illegally Added Staining Substances in Safflower by Solid Phase Extraction-Electrospray Ionization-High Performance Ion Mobility Spectrometry

Objective: To establish a fast quantitative detection method for tartrazine, sunset yellow, orange Ⅱ sodium salt and allura red in safflower by solid phase extraction-electrospray ionization-high performance ion mobility spectrometry (SPE-ESI-HPIMS).Methods: The pigments were extracted by 70% ethanol with ultrasonic treatment, and then a polyamide SPE column was used to remove the complex matrix interference in safflower.The purified sample was then dissolved in 90% methanol and analyzed under the optimized IMS parameters.Results: The detection time of all the pigments was less than 20 ms.The limit of detection of tartrazine, sunset yellow, orange Ⅱ sodium salt and allura red was 0.17 , 0.15 , 0.30 and 0.25 μg·ml-1 , respectively.All the pigments showed excellent linearity within the range of 0.5-20 μg·ml-1 (r>0.990 0), the method recovery was 88.0%-98.9% , and the RSD was 1.5%-5.2% (n =6).Conclusion: The method is rapid, simple, highly sensitive and reproducible, and suitable for the rapid quantitative detection of illegal added staining substances in safflower.
2.Efficacy of memantine on moderate to severe Alzheimer's disease: a meta-analysis

Objective To systematically assess the efficacy of memantine on moderate to severe Alzheimer's disease (AD).Methods With the evaluation method of the Cochrane system,searches were made in the Cochrane Library,MEDLINE,Embase,Forest Laboratories,CNKI,Wanfang Data,and VIP Data up to February 2013 for double blind,randomized,and placebo-controlled trials (RCTs) evaluating the efficacy of memantine for moderate to severe AD.A meta-analysis of included clinical trials was conducted using the Revman 5.2 software to evaluate the efficacy of memantine on overall clinical status,cognitive function activities of daily living,and behavioral and psychological disturbances.Results A total of 8 RCTs were included (2 527 patients with moderate to severe AD).Results of the meta-analysis showed that,for patients with moderate to severe AD,memantine had better efficacy than placebo on overall clinical status,cognitive function,and activities of daily living (MD=-0.24,95%CI:0.340.15;SMD=-0.26,95%CI:-0.340.18;SMD=-0.13,95%CI:-0.21-0.05),but there was no significant difference in efficacy on behavioral and psychological function between memantine and placebo (P =0.08).Analysis of subgroups showed that memantine had better efficacy than placebo on cognitive function in moderate AD patients (SMD =-0.22,95%CI:-0.37 0.06) and on overall clinical status,cognitive function,and activities of daily living in severe AD patients (MD-0.29,95%CI:-0.40 0.18;SMD=-0.31,95%CI:0.46-0.15;SMD=-0.16,95% CI:-0.25 0.06;MD=-3.13,95% CI:-4.88-1.39;respectively).Conclusions Memantine has efficacy on overall clinical status,cognitive function and activities of daily living in patients with moderate to severe AD,especially in patients with severe AD.
3.The olfactory function evaluation of Parkinson disease and its influencing factors

Objective To explore the incidence of olfactory dysfunction in patients with Parkinson disease and the characteristic as well as its possible influencing factors. Methods The SS-12 was used to evaluate the olfactory function of 106 patients with Parkinson's disease and 110 healthy volunteers. The data was then compared between the two groups to investigate the correlation of olfactory function with age, gender, education, smoking, disease duration, Hohn-Yahr stage, UPDRSⅢscores, the dosage of levodopao and olfactory scores. Results Mean identification scores were significantly lower in patients(5.97 ± 2.27)than in controls(8.04 ± 2.00)(t=7.108, P=7.108). Parkinson's disease group did worse than the control group in identifying some odors including peppermint, bananas,liquorice,coffee,pineap?ple,rose and fish (P<0.05). Receiver Operating Characteristic curves revealed that 7.5 was the best olfactory dysfunc?tion diagnostic boundary value with 67.3%sensitivity and 73.6%specificity. Thus, the incidence of olfactory dysfunc?tion in Parkinson's disease was 73.6%. Correlation analysis showed that gender(rs=0.243, P=0.012), education(rs=0.208, P=0.032) and smoking(rs=0.279, P=0.279) were associated with odor identification ability, while age, disease du?ration, Hohn-Yahr stage, UPDRSⅢscores and the dosage of levodopa were not related with odor identification abilityin Parkinson disease (P>0.05). Conclusion Olfactory dysfunction occurs in Parkinson disease with an hign incidence rate. Olfactory function has nothing to do with disease duration, Hohn-Yahr stage, UPDRSⅢscores and the dosage of levodopa in Parkinson disease.
4.Asiaticoside attenuates bleomycin-induced interstitial pulmonary fibrosis

OBJECTIVE To study the therapeutic effect and the underlying mechanism of asiatico?side on bleomycin-induced rat interstitial pulmonary fibrosis(IPF). METHODS Male Sprague-Dawley (SD)rats were divided into normal control group,bleomycin 5 mg·kg-1 model group and asiaticoside 50 mg · kg-1 group. The model and asiaticoside group were administrated with bleomycin 5 mg · kg-1 to induce IPF,while the asiaticoside group was administrated with asiaticoside 50 mg·kg-1 by gastric perfusion. Hematein eosin(HE)and Masson staining were carried out to analyze the histopathological changes in the lung. Lung homogenates were used to examine hydroxyproline(HYP) content,and serum samples were used to measure the concentration of interferon-γ(IFN-γ),interleukin-4(IL-4) and tumor necrosis factor-α(TNF-α). In addition,immunohistochemical methods were used to locate lung transforming growth factor-β1(TGF-β1)and adenosine 2A receptor(A2AR)expression,and Western blotting was used to examine the expression levels of TGF-β1 and A2AR. RESULTS On the 7th,14th and 28th days,the scores of pulmonary inflammation were higher in model group than in control group (P<0.01),and the asiaticoside group showed mitigated alveolitis(P<0.01,P<0.05) compared with model group. Compared with control group,the scores of pulmonary fibrosis in model group were elevated(P<0.01),and the asiaticoside group showed reduced pulmonary fibrosis(P<0.05). On the 14th and 28th days,HYP content in the model group〔1.85±0.10,(2.48±0.18)mg·g-1〕was higher than in the control group〔0.79 ± 0.07,(0.84 ± 0.08)mg · g-1〕(P<0.01),but HYP content in the asiaticoside group〔1.32±0.131,(1.71±0.13)mg·g-1〕was lower than in the model group(P<0.05). IL-4 and TNF-αin the asiaticoside group were lower than in model group(P<0.05),but were higher in the model group than in the control group(P<0.01,P<0.05). The expression level of TGF-β1 protein in the asiaticoside group was lower than in the model group(P<0.05),but was higher in the model group than in the control group(P<0.05). The expression level of A2AR protein in the asiaticoside group was higher than in the model group(P<0.05),but was lower in the model group than in the control group(P<0.05). CONCLUSION Asiaticoside can mitigate bleomycin-induced IPF by inhibiting the expression of IL-4, TNF-αand TGF-β1,and raising the level of A2AR.
5.Effect of Three-dimensional Echocardiography for Evaluating Right Atrial Function in Patients After Atria Sepal Defect Closure

Objective: To explore the effect of three-dimensional echocardiography (RT-3DE) for evaluating right atrial function in patients after atrial septal defect (ASD) closure.
Methods: Our research included 2 group: ASD group,n=50 patients with successful ASD closure and Control group, n=30 normal subjects from physical examination. RT-3DE was conducted at pre-operation and at 3 days, 1 month, 3 months post-operation; the volume of right atrium before contraction (Vpre), minimum volume of the right atrium (Vmin), maximum volume of the right atrium (Vmax), total emptying volume of right atrium (Vt), total emptying volume fraction of right atrium (Ft), active emptying volume fraction (Fa) and passive emptying volume fraction (Fp) were measured and compared between 2 groups at pre- and post-operation respectively.
Results: At pre-operation, compared with Control group, ASD group had increased Vmax, Vmin, Vpre, Vt, and Fa, while decreased Ft and Fp, allP<0.05. At 3 days post-operation, compared with Control group, ASD group had increased Vmax, Vmin, Vpre, Vt, and Fa, while decreased Ft and Fp, allP<0.05. Compared with pre-operation, ASD group had decreased post-operative Vmax, Vmin, Vpre, Vt and Fa while increased Ft and Fp at 3 time points, allP<0.05. At 3 months post-operation, the above 7 indexes were similar between ADS group and Control group,P>0.05.
Conclusion: ADS patients had abnormal volume and function in right atrium, the abnormalities could be improved at 3 days after ASD closure and could be recovered to normal level at 3 months after operation. RT-3DE has the important role for evaluating right atrial volume and function in patients after ASD closure.
6.The correlation between periventricular white matter changes and matrix metalioproteinase-3 expression after chronic cerebral hypoperfusion in rats

Objective To observe periventricular white matter changes and matrix metalloproteinase-3 (MMP-3) expression after two-vessel occlusion in rats.Methods A total of 40 Wistar rats were randomly divided into four groups:sham operation group,model group 1 (20 days after operation),model group 2 (40 days after operation),model group 3 (60 days after operation)(n =10 per group).Model of chronic cerebral hypoperfusion was induced by permanent ligation of the bilateral common carotid arteries in the rat.The change of periventricular white matter was observed by electron microscope.The myelin basic protein (MBP) levels was determined by Western blotting,and the expression of MMP-3 was analysed by immunohistochemical stain and Western blotting.Results Permanent ligation of the bilateral common carotid arteries resulted in demyelination in the corpus callosum and internal capsule in rats.The expressions of MMP-3 in model groups were markedly elevated compared with sham group (model group 1,39.17± 4.167; model group 2,43.33±3.502; model group 3,54.16±2.787; sham group,26.67±3.830; all P<0.01).The content of MBP in model groups was gradually decreased compared with sham group (model group 1,54.50±4.087; model group 2,47.83±4.875; model group 3,36.50±4.231; sham group,65.01±7.688; all P<0.01).Additionally,the upregulation of MMP-3 had significant correlation with the loss of MBP in the periventricular white matter (r =-0.883,P<0.01).Conclusions Chronic cerebral hypoperfusion induces progressive periventricular white matter demyelination,and the upregulation of MMP-3 may involve in the pathological process.
7.Preventive effect of laminar flow bed on hospital infections in patients with leukemia after chemotherapy:A Meta-analysis

Objective To compare the effect of laminar flow bed in the prevention of hospital infections in patients with leukemia after chemotherapy. Method The Meta-analyses were done on Meta RevMan5.0 by retrieving the domestic databases from January 1999 to January 2014 to look into and compare the hospital infections in leukemic patients using laminar flow beds and common beds, including of the rate of hospital infection, duration of fever, days for antibiotic use, hospital stay. Results A total of 16 papers were included. The Meta analyses showed that the rate of hospital infection in the wards with laminar flow beds was significantly lower than that in the wards with common beds (RR=0.54,95%CI (0.47, 0.62), P<0.01). The fever duration, time for antibiotics use and hospital stay in patients in the wards with laminar flow beds were all significantly shorter than those in the patients in the wards with common beds [WMD=-3.64, 95%CI (-4.83,-2.46);WMD=-4.13, 95%CI (-5.95,-2.32)];WMD=-5.36, 95%CI (-7.52,-3.20);P<0.01,respectively)]. Conclusion Laminar flow beds are effective in preventing hospital infections among acute leukemic patients having undergone chemotherapy.
8.Pharmaceutical Care on an H Hypertension Case with MTHFR Gene Mutation

Objective To explore the role of clinical pharmacist in individualized treatment of hypertension.Methods A patient withH hypertension receiving pharmaceutical care from clinical pharmacists was retrospectively analyzed.Results Patient's MTHFR (C677T) gene type was TT homozygous.Clinical pharmacist suggested doctor modify treatment,and then patient's plasma homocysteine dropped from 61.5 to 16.0 μmol·L-1,and blood pressure dropped from 173/ 111 mmHg(1 mmHg =0.133 kPa) to 130/80 mmHg.Conclusion Clinical pharmacist provides individualized treatment for patient with hypertension to ensure the safety and effectiveness of the drug by genotyping.
9.Research on speckle reduction in ophthalmology high frequency ultrasound imaging

Objective Speckle is the inherent problem exists in B-Mode ultrasound image,and effective speckle noise reduction will improve the image quality and contribute greatly to clinical doctors to diagnose,especially in fine ophthalmic examination with high frequency ultrasound.Methods This paper proposed a new speckle reduction method based on the Laplacian pyramid and multiscale analysis.In the Laplacian pyramid,true clinical features were separated from noise,according to the different bandpass ultrasound image characteristics in each layer,and the advanced eight directions speckle reduction anisotropic diffusion (SRAD)was adapted to suppressed the speckle noise,and the identified noise and the extracted features were selectively emphasized by suitable edge,coherence and contrast enhancement filtering from fine to coarse scales.The performance of the proposed method was compared to the traditional SRAD method and coherence-enhancing diffusion method by measuring the equivalent number of looks (ENL) and Time cost.Results The ENL and Time cost value of the proposed method were higher compared to the SRAD and the cedif method,i.e 1.172 3 vs 1.122 3,0.929 3 and 0.864 0 vs 1.396 0,1.468 3.Conclusions In summary,the proposed method can more effectively remove the speckle noise while preserving the edge and details of the images.
10.The impact of different body posture and bed time on the headache of lumbar puncture: a Meta-analysis

Objective To compare the effect of different body position and length of bed time after lumbar puncture on the postoperative headache.Methods Literatures were searched at home and abroad through the database,and provided comprehensive quantitative analysis to compare the effect of different body position and length of bed time after lumbar puncture on the postoperative headache by the Cochrane collaboration Meta-analysis.Results A total of 20 studies (3 514 patients) were included for Meta-analysis.Mter lumbar puncture for the low supine pillow and the pillow recumbent position,headache incidence rate had no significant difference between the two positions,combined odds ratio (OR) and 95% confidence intervals were 0.87 (0.41-1.87)(P> 0.05);After lumbar puncture to stay in bed for a long time to rest (≥ 4 h) and short time in bed rest (<1 h),headache incidence rate had no significant difference between the two different time,the combined odds ratio (OR) and 95% confidence intervals were 0.77 (0.58-1.02)(P > 0.05).Conclusions After lumbar puncture,the occurrence rate of headache is not increased after taking a low pillow lying position or a short time in bed rest (<1 h).
