1.Pathological Analysis and Therapeutic Evaluation of Early Colorectal Carcinoma Treated by Endoscopic Resection or Surgery

Background:Prognosis of early colorectal carcinoma( ECC)is well because of low rate of lymph-node metastasis ( LNM). How to improve the detection rate and select appropriate therapy for ECC has been an eager task in clinical practice. Aims:To analyze the pathological features of ECC treated by endoscopic resection(ER)or surgery,and evaluate the therapeutic efficacy of ER. Methods:Pathological data of 503 ECC lesions treated by ER or surgery were retrospectively analyzed. Risk factors of infiltration depth,LNM of ECC were analyzed,and therapeutic efficacy of ER was evaluated. Results:The overall detection rate of ECC was 10. 7%. The incidence of LNM was 1. 2%(6/503);the LNM incidence of mucosal high-grade neoplasia was 0%( 0/247 ),while was 2. 3%( 6/256 ) in submucosal carcinoma. The LNM incidence of submucosal superficial carcinoma treated by ER was 0%( 0/31 ). Tumor location, size,histological type and LNM had significant impacts on infiltration depth(P<0. 05). The rates of en bloc,complete and curative resection by ER were 96. 0%,94. 2% and 82. 1%,respectively. Infiltration depth was the risk factor of piecemeal,incomplete and noncurative resection of ER(P<0. 05). Conclusions:The incidence of LNM in ECC is extremely low. Accurate evaluation of character and infiltration depth of lesion before operation is helpful for selecting appropriate therapy. It should be cautious to choose ER for lesions infiltrated into submucosa,thereby to improve the therapeutic efficacy of ER.

Long-term potentiation(LTP)is an important form of synaptic plasticity and an objective indicator to investigate learing and memory synaptic mechanisms.With the development of brain slice technology,more and more experiments associated with LTP are carried out on brain slices,which aim to investigate the mechanism in biology and the change in physiology or biochemistry are carried out on the brain slice.This paper gives an overview of recent advances in research of LTP with technology of brain sliceby suchexamples as follows:The regulated expression mechanisms of long-term potentiation at CA1 synapses,the characteristics of LTP induced in hippcampal slices and its relation with the slice-recovery conditions,the enhancement of the magnitude of early longterm potentiation at CA1 hippocampal synapse by the activation of dopamine receptor,and the enhancement of associative long-term potentiation by the activation of β-adrenergic receptors at CA1 synapses in rat hippocampol slices.

4.Influence of sea sickness elixir on changes in cardiac rhythm in subjects susceptible to sea-sickness susceptibility

Objective To observe the influence of sea sickness elixir on changes in cardiar rhythm in subjects susceptible to sea-sickness. Methods In a double-blind control test, 50 volunteers, 10 persons not susceptible to sea-sickness (controll group) and 40 persons susceptible to sea-sickness to travel their first sea voyage were enrolled. They were divided into three groups, control group (n=10), placebo group (n=20) and test group (n=20). Sea sickness elixir or placebo was administered half an hour before the sea voyage. The 4h dynamic electrocardiogram was collected and the changes in heart rhythm was analyzed using time-domain, frequency domain analysis methods. Meanwhile the symptoms of sea sickness were recorded. 5.Spiral enteroscopy in diagnosis and managements of small bowel diseases

Objective To evaluate the use of spiral enteroscopy in diagnosis and treatment of smallbowel diseases.Methods The data of 8 patients who underwent spiral enteroscopy from July to September 2009 were retrospectively studied.The variables including maximal insertion depth,total procedure time,complications,and outcome were evaluated.Results The average maximal depth of intubation was 2.2 m beyond the Ligament of Triez (0-3.6 m beyond Ligament of Triez),with a mean procedure time at 41 min (25 to 77 min).Small bowel Crohn's disease was diagnosed in 2 cases with biopsy suggesting active inflammation and granular formation.Small intestinal tumor was detected in 1 patient with pathological finding of high grade dysplasia.Jejunal ulcer was detected in 1 patient.Multiple polyps were found in 1 patient after jejunal anastomosis,which were then treated with endoscopic argon plasma coagulation (APC).No abnormalities were found in 3 other patients.No complications occurred during and after the procedure.The maneuver of spiral enteroscopy and APC were same as that of balloon enteroscopy.Conclusion Spiral enteroscopy is simple and convenient to operate,which is of great potential in clinical use.
6.Effect of early rehabilitation on the hip joint function in patients with comminuted posterior wall fractures of the acetabulum after internal fixation

BACKGROUND: Treatment for acetabular fracture becomes a hot point in orthopaedic trauma, but ignorance of rehabilitative treatment that effect therapeutic efficacy is to be seen repeatedly. Normative rehabilitation therapy is one indispensable procedure of acetabular fracture treatment.OBJECTIVE: To investigate the effect of early rehabilitation on the hip joint function in patients with comminuted posterior wall fractures of the acetabulum after internal fixation.METHODS: Forty patients of comminuted posterior wall fractures of the acetabulum at the Department of Orthopedics, Third Hospital of Hebei Medical University, were selected, including 36 males and 4 females, aged 23-61 years, mean aged 35.3 years. All patients were randomly divided into rehabilitation and control group, with 20 patients in each group. All the patients were fixed with reconstruction plate, the patients in the rehabilitation group received early standard rehabilitation exercises intervention and those in control group received usual care. The clinical result was evaluated with the scores of Merle d'Aubigne and Postel.RESULTS AND CONCLUSION: The scores of Merle d'Aubigne and Postel of the rehabilitation group were (13.10±2.05),(14.50±1.32), and (15.80±1.36) points at months 3, 6, and 12 after operation, which were greater than those of the control group (11.20±2.80), (13.00±2.10), and (14.25±1.74) points, respectively (P<0.05). The early rehabilitation exercises may improve the hip joint function for patients of the comminuted posterior wall fractures of the acetabulum after surgery.
7.Quantitative detection of Porphyromonas gingivalis by real-time PCR of orthodontic patients after removal of orthodontic appliances

Objective To detect Porphyromonas gingivalis and clinical index in subgingival specimens after removal of orthodontic appliances and to evaluate clinical index and P.gingivalis factors associated with orthodontic appliances during an episode of gingival inflammation and the impact of appliance removal on periodontal health.Methods Choose 20 patients prepare to removal of orthodontic appliances.Plaque index(PLI),sulcus bleeding index(SBI) and probing depth(PD) of observed teeth were examined and subgingival plaques were collected at prior to appliance removal(baseline) and 1,3 and 6 months after appliance removal.The numbers of P.gingivalis,total bacteria,the proportion of P.gingivalis and the prevalence of P.gingivalis in the samples were detected with the TaqMan real-time PCR.Results The prevalence of P.gingivalis decreased from baseline to the 6th month after fixed appliance removal in two groups,and significant change was found in 6th month(P < 0.05); The numbers of P.gingivalis significantly reduced from baseline to the 6th month(P < 0.05); The proportion of P.gingivalis in subgingival plaque decreased from baseline to the 6th month(P <0.05).PLI,SBI and PD significantly decreased from baseline to the 6th month after fixed appliance removal.Conclusion After fixed appliance removal,most of patients can recover periodontal tissue health in good oral hygiene.The percentage of P.gingivalis are closely associated with periodontal status.TaqMan real-time polymerase chain reaction has a high sensitivity and specificity.It was demonstrated the broad potential of application on detection of oral microbes.
8.Role of itopride in the improvement of bowel preparation before colon endoscopy examination in constipated patients

Objective To investigate the role of prokinetic agent itopride in colonic preparation before colonoscopy examination in patients with constipation.Methods A total of 115 outpatients with history of chronic constipation who requested colonoscopy were collected.According to colonic preparation proposal,patients were divided into three groups.Group A (39 cases) took standard dosage of PEG-E solution six hours before colonoscopy examination.Group B (38 cases) took 150 mg itopride 30 minutes before administration of lavage solution.Group C (38 cases) took itopride 150 mg at 7 am,12 am and 8 pm the day before the examination and on the examination day took the same medicine as that of group B.The blood pressure,heart rate and blood electrolytes were monitored before and after taking medicine in the patients of three groups.Quality of colon cleansing of each group was observed and side effects were also observed.One-way analysis of variance (least significant difference,LSD) test was performed for pairwise comparisons among the three groups.Chi-square test was applied for count data.Results Both group A and group B excluded one patient because of malignant carcinoma with colon stricture under colonoscopy,and 113 patients completed the whole colon examination.There was no significant difference in the baseline patients' data of three groups.The colon cleaning score of group C (7.28±1.11) was higher than those of group A and B (6.55±1.18 and 6.51±1.16,LSD test,both P<0.05).The frequency of bowel movements defecation of group C (8.31± 1.32) was more than those of group A and group B (7.11± 1.41 and 6.94± 1.51,LSD,test,both P<0.05).There was no significant difference in terms of intestinal bubble scores,blood pressure,heart rate,blood electrolytes the uncomfortable degree of colonic preparation and rate of side effects of the three groups.Conclusion The colonic preparation can be safely and effectively improved by taking high dose of itopride one day before and on the day of colonoscopy examination.
9.Analysis of distribution and drug resistance of pathogens in premature infants with ventilator-associated pneumonia

Objective To investigate the distribution and characteristics of drug resistance of pathogens in premature infants with ventilator‐associated pneumonia(VAP) ,and explore the therapeutic measures for premature infants with VAP ,so as to provide references for clinically reasonable administration of antibacterial agents .Methods A total of 54 cases of premature infants diag‐nosed with VAP in the First Affiliated Hospital of Xiamen University from January 2013 to June 2014 were enrolled in this study . Specimens of respiratory tract secretion were collected ,and species identification of pathogens and drug sensitivity test were per‐formed by VITEK 2 Compact system .Results A total of 69 strains were isolated .Gram negative bacteria was accounted for 94 .20% ,and gram positive bacteria was accounted for 5 .80% .Four of the most common pathogenic bacteria were Klebsiella pneu‐monia (29 strains ,42 .03% ) ,Enterobacter aerogenes (12 strains ,17 .39% ) ,Pseudomonas aeruginosa (10 strains ,14 .49% ) and Stenotrophomonas maltophilia(7 strains ,10 .14% ) .The results of antibiotics‐sensitivity tests showed that these strains were multi‐drug resistant to most commonly used antibiotics ,but sensitive to only a few antibiotics .Conclusion Gram‐negative bacteria are the major pathogens for premature infants with VAP in this hospital .Mixed infection and multidrug resistance are common .Therefore , the rational use of antibiotics according to drug sensitivity tests′results is critical in treating VAP .
10.Antibiotics Susceptibility Analysis of Sixty-two Clinical Streptococcus pneumoniae Isolates

OBJECTIVE To investigate the in vitro antimicrobial susceptibility of Streptococcus pneumoniae and guide the rational use of drug clinically. METHODS We performed statistical analysis of the susceptibility of 62 strains of S.pneumoniae isolated in our hospital from May 2007 to Aug 2008. RESULTS S.pneumonia e could be isolated from various specimens,most of them were isolated from sputum(84.0%).S.pneumoniae could be detected from many hospital wards,but more strains isolated from pediatric(67.8%) and respiratory(11.4%) departments.A total of 67.7% of S.pneumoniae isolates were penicillin non-susceptible,the resistance prevalence to tetracycline was 87.1%,to erythromycin 79.0% and to trimethoprim/sulfamethoxazole 49%,while they were highly susceptible to ofloxacin(90.3%),vancomycin(91.9%),levofloxacin(95.2%),moxifloxacin(96.8%) and rifampicin(98.4%),and more strains showed multi-resistance from penicillin non-susceptible isolates. CONCLUSIONS The resistance to penicillin of S.pneumoniae is serious in our hospital.The tetracycline and erythromycin are not the best ckoice in treating S.pneumoniae infection,and the new fluroquinolones show strong activity against S.pneumoniae.
