3.Hospital Infection Administration in Chinese Level Ⅱ Hospital of UN Peacekeeping Army in Lebanon:Current Situations and Measures

OBJECTIVE To improve efficiency of hospital infection administration and take strict precautions against medical malpractice in Chinese level Ⅱ hospital of UN Peacekeeping Army in Lebanon.METHODS The current situations of its hospital infection administration were enumerated,compared and analyzed,and corresponding measures of administration were suggested.RESULTS Under the influence of characters of medical missions during peacekeeping,the hospital infection administration presented the problems that patients and disease spectra were diversified,the rate of hospital infection was influenced by peacekeeping mission,the positive rate of HIV was high,medical workers protection awareness was lagged behind,and the disposition of hospital was illegitimate;many measures should be taken to enhance the administration level.CONCLUSIONS In order to well develop the hospital infection administration in Chinese level Ⅱ hospital of UN Peacekeeping Army in Lebanon,we should reform and improve the system of hospital infection administration,and establish an effective and systematic prevention system of hospital infection.

The production of mature functional T cells requires many signals from the thymus such as Wnt,Notch and Hedgehog,et al.The Hedgehog protein family signals for development,patterning and organogenesis of many tissues during mammalian embryogenesis.In recent years,more and more research groups focus their attention on it because of its relationship between tumors.Abnormal differentiation and development of T cells may cause various immunological diseases and tumors.Illuminating the roles of Hedgehog signaling in T cell differentiation and development can provide a theoretical guide in the treatment of tumor.
6.Continuous irrigation with recombinant human acid fibroblast growth factor combined with negative-pressure drainage for the treatment of refractory skin ulcer

Objective To evaluate the clinical efficacy of continuous irrigation with recombinant human acid fibroblast growth factor (rh-aFGF) combined with negative-pressure drainage in the treatment of refractory skin ulcer.Methods A clinical trial was performed from January 2010 to December 2012.Sixty patients with refractory skin ulcer were randomly and equally classified into three groups:group A receiving continuous irrigation with rhaFGF combined with negative-pressure drainage,group B receiving irrigation of raw surfaces with rh-aFGF followed by bandaging after debridement,group C receiving covering of raw surfaces with Vaseline gauze and absorbent gauze followed by bandaging after debridement.Dressings were changed once daily in every group.The treatment lasted 21 days.The efficacy of these regimens was evaluated on day 7,14 and 21.Results At the above time points for observation,both the granulation and epithelium tissues of ulcers grew well in group A,with a decrease in the volume of wounds and area of exposed deep tissues.There was a significant increase in the cure rate and reduction rate of wound volume in group A compared with the other two groups (all P < 0.01).Conclusion Refractory dermal ulcer can be treated with continuous irrigation with rh-aFGF combined with negative-pressure drainage.

Objective To investigate the effect of bone marrow mesenchymal stem cells (bone marrow stem cells,BMSCs) through different approaches in the treatment of transplanted cirrhotic model in rabbit effect difference.Methods Establishment of rabbit model of carbon tetrachloride cirrhosis with liquid olive oil mixture of subcutaneous inj ection and ethanol fed,the liver cirrho-sis models were randomly divided into cirrhosis model group (n=10),hepatic artery transplantation group (n=10),ear vein trans-plantation group (n=10).Healthy New Zealand rabbits bone marrow,using the method of whole bone marrow adherent cultured bone marrow BMSCs,and then were transplanted and serum albumin,alanine aminotransferase in 3 ways;by HE staining,the de-gree of hepatic cirrhosis and detection of serum albumin by Western blot expression in liver.Results Through the vein group and ar-tery transplantation group BMSCs to cirrhotic model in rabbit,4 weeks after the discovery of liver transplantation group rabbits were significantly improved,serum albumin is liver cirrhosis without transplantation group increased significantly(P<0.05),transami-nase is liver cirrhosis without transplantation group decreased significantly (P<0.05).The therapeutic effect of 2 ways of BMSCs transplantation on rabbit liver cirrhosis had a significant difference (P<0.05).Conclusion Transplantation of BMSCs through the ear vein and the hepatic artery of rabbit after 4 weeks of treatment of liver cirrhosis,liver function can be improved.But the hepatic artery transplantation therapy is obviously better than that of intravenous transplantation of treatment effect.
9.Study on the expression of ?_2-adrenergic receptor gene and enhanced green fluorescent protein gene in cardiomyocytes transferred by adeno associated virus vector

Objective To investigate the way of gene therapy on congestive heart failure and other heart diseases. Methods Recombinant adeno associated viruses (AAV) encoding the human ? 2 adrenergic receptor(AR) and enhanced green fluorescent protein (EGFP) were constructed and the ? 2 AR and EGFP cDNA were transfected in vitro to the neonate rat ventricular myocytes. The expression of ? 2 AR and EGFP on cultured neonate rat ventricular myocytes and the ability of ? 2 AR to potentiate the beta adrenergic signaling were assessed. Results The presence of human ? 2 AR mRNA was demonstrated by Northern blot, the expression of the ? 2 AR gene was demonstrated by Westernimmuno blot. The density of ? AR in the myocytes was greater than that in the control 〔(204.0 ? 6.4) fmol/mg protein vs (76.0 ? 2.8) fmol/mg protein〕 according to a ligand binding assay. Human ? 2 AR gene significantly increased isoproterenol stimulated cAMP as compared with the control〔(116.2?5.8) pmol/10 6 cell vs (58.4 ? 4.6) pmol/10 6 cell〕. EGFP was also expressed in the transfected cardiomyocytes. Conclusions Human ? 2 AR gene could be transfected into cardiomyocytes by AAV vector and could potentiate the beta adrenergic signaling.
10.THE SIGNIFICANCE AND CHARACTERISTICS OF DISTRIBUTION OF EPIDERMAL STEM CELLS DURING WOUND HEALING PROCESS

To investigate the distribution of epidermal stem cells in regenerating wound tissues, and to elucidate the role of epidermal stem cells during wound repair. 80 circular full-thickness wounds were produced on both sides of the back in 20 male Wistar rats labeled 60 days previously with 5-Bromodeoxyuridine (BrdU) (4 wounds in each animal). Then these 80 wounds were randomly divided into 2 groups as follows: group A: with topical treatment of epidermal growth factor (n=40) and group B: no-treatment (n=40). BrdU, ? 1 integrin and keratin19 (K19) were employed to determine the epidermal stem cells with streptavidin-peroxidase (SP)immunohistochemical method, and the speed and quality of epithelialization were determined with routine histological methods with HE staining on the 3rd, 7th, 14th, and 21st day after the wounding. Results showed that the healing rate of wounds was 80% in group A (32/40) and 60% in group B (24/40). No cells with positive immunostaining for BrdU, ? 1 integrin, or K19 were found in the granulation tissue of all wounds in both groups during the healing process. However, a few BrdU, ? 1 integrin and K19 positive cells, bearing no anatomic relation with the epidermal stem cells in the basal layer, were found scattering in the stratum spinosum and stratum granulosum of the epidermis on the wound edges. The results suggested that epidermal stem cells appearing on the wound edges were the main source of re-epithelialization of granulating wounds.
