1.Hospital Infection Administration in Chinese Level Ⅱ Hospital of UN Peacekeeping Army in Lebanon:Current Situations and Measures

OBJECTIVE To improve efficiency of hospital infection administration and take strict precautions against medical malpractice in Chinese level Ⅱ hospital of UN Peacekeeping Army in Lebanon.METHODS The current situations of its hospital infection administration were enumerated,compared and analyzed,and corresponding measures of administration were suggested.RESULTS Under the influence of characters of medical missions during peacekeeping,the hospital infection administration presented the problems that patients and disease spectra were diversified,the rate of hospital infection was influenced by peacekeeping mission,the positive rate of HIV was high,medical workers protection awareness was lagged behind,and the disposition of hospital was illegitimate;many measures should be taken to enhance the administration level.CONCLUSIONS In order to well develop the hospital infection administration in Chinese level Ⅱ hospital of UN Peacekeeping Army in Lebanon,we should reform and improve the system of hospital infection administration,and establish an effective and systematic prevention system of hospital infection.
2.The inactivation of duck hepatitis virus B in blood cell suspenion induced by specific photosensitizer

Objective:To study the specific photochemical effects of a newly designed target photosensitizer. Methods Based on the technique of antisense nucleic acid and the principle of photochemical reaction effects,a specific sensitizer,TFO P has been designed and synthesized.When in coordination with long wave ultraviolet ray(UVA) ,this decorated complex (TFO P) was added into the blood cell suspension to inactivate the contaminating virus( duck hepatitis virus B,DHBV).The efficacy of specific binding to DHBV DNA and viral inactivation by TFO P was detected by gel shift blot assay and infection of primary culture of duck hepatocyte.Results The designed TFO P could specifically bind to different DHBV DNA line sample and present different linking level.With a TFO P concentration of 0.1 nmol/ml and UVA intensity of 1800 ?W/cm 2,the DHBV in blood cell suspension could be reduced by 1.90~5.40 logs.Conclcusion The photochemical effects of TFO P could significant inactivate DHBV in blood.
3.Concomitant chemoradiotherapy for prevention of recurrence and metastasis in the surgical treatment of salivary adenoid cystic carcinoma

Objective: To evaluate the effect of concomitant chemoradiotherapy for the prevention of recurrence and metastasis of postoperative salivary adenoid cystic carcinoma(SACC).Methods: 14 cases of salivary adenoid cystic carcinoma after operation were treated with concomitant chemoradiotherapy. Regular radiotherapy of 60 Gy and chemotherapy with hydroxycamptothecin (HCPT) at 10 mg/m 2 , 3 times a week, were administered. HCPT was given in 1st, 2nd, 4th and 5th week. Recurrence and metastasis of SACC after operation were observed using CT, MRI, ultrasonography and X ray. Results: After 3~17 months' follow up, relapse free survival was 100%. No recurrence and metastasis of SACC after operation were found. Adverse reactions of the chemotherapy were nausea, vomiting and leukopenia. After suspended in a short time and using ondansetron and /or G CSF the chemotherapy of HCPT was continued and completed. Conclusion: concomitant chemoradiotherapy of HCPT and radiotherapy can prevent recurrence and metastasis of SACC after operation. The efficacy of long term needs further observation.
4.The design of bionic left ventricular auxiliary pump.

This paper reports a novel design of bionic left ventricular auxiliary pump, and the characteristic is that elastic diaphragm of pump driven by hydraulic, having smooth, reliable blood supply, can prevent blood clots, can use the flow sensor, pressure sensor detection showing the blood pressure and blood volume at the inlet and outlet of the pump. The pump can go with heart rate synchronization or asynchronous auxiliary by the R wave of human body's ECG. The design goal is realization of bionic throb. Through the animal experiment, the blood pressure waveforms are close to expectations, stable flow can stroke according to the set value, which prove that the pump can meet the requirement for heart disease patients for bionic left ventricular assistant.

5.DIGITAL ANATOMY OF THE DEEP CIRCUMFLEX ILIAC ARTERY OSTEOCUTANEOUS PERFORATOR FLAP

Objective To determine the anatomical basis of an algorithm to safely elevate the deep circumflex iliac artery osteocutaneous perforator(DCIAP) flap. Methods 1.Six unfixed corpses underwent whole body gelatine/lead oxide injection.Specimens were dissected by layers.Angiography and photography were used to document the precise course,size,location,and type of individual perforators in the lateral lumbar region.The surface areas of cutaneous territories and perforator zones were measured and calculated with Photoshop and Scion Image.2.One specimen also underwent whole body carboxymethylcellulose/lead oxide injection,CT scan and 3D-Reconstrution. Results An average of 1.6 DCIA perforators with a diameter of 0.7mm was present in 92% of specimens.Perforators were located 5~10 cm posterior to the anterior superior iliac spine,12~35mm above the crest,with a perforator zone of 31 cm~2.The DCIA reliably perfused the medial aspect of the iliac crest.Conclusion The DCIA reliably perfused the medial aspect of the iliac crest and lateral lumbar region.It offers a large quantity of bone on a pedicle of large diameter.The mobility of the skin component allows better tissue positioning during complex reconstructions.
6.Application of prone-straddle position for transvaginal hysteromyomectomy in the posterior wall of the uterus

Objective To discuss the safety and advantages of prone-straddle position applied in transvaginal hysteromyomectomy of the posterior wall of the uterus. Methods The clinical data of patients who were admitted into our hospital from March 2013 to Janaury 2015 and received transvaginal hysteromyomectomy were retrospectively analyzed. They were divided into group A ( prone-straddle position, 30 cases) and group B (traditional lithotomy position,24 cases). The exposure of operative field, convenience of operation, time of the placement, time of operation, patient satisfaction, as well as patients’ heart rate, blood pressure, and oxyhemoglobin saturation were observed and ana-lyzed. Results Compared with the lithotomy position, there were significant differences in the time of the placement and the time of opera-tion in the patients treated by prone-straddle position. The patients with prone-straddle position cooperated well, and there was no obvious discomfort. Their vital signs were stable during the operation. The operative field during prone-straddle position exposed better and it was more convenient which make the operation became easier for both the operators and nurses. Conclusion Prone-straddle position applied in transvaginal hysteromyomectomy of the posterior wall of the uterus is safe and practicable, and it is valuable for clinical application.
7.The application of HA330-Ⅱ microporous resin plasma adsorption in the treatment of chronic severe hepatitis

Objective The aim of the study is to discuss the application value of HA330-Ⅱ microporous resin plasma adsorption in liver support and the treatment of complications of chronic severe hepatitis.Methods The treatment group has 41 patients of chronic severe hepatitis B,which were treated for 3 times by HA330-Ⅱ microporous resin plasma adsorption.We compared the improvement of symptom,the fadeamay and rebound of icterus,the change of hepatic encepllalOpathy and brain oedema,the change of blood routine test,blood ammonia level,blood-gas analyzing result,cruor index,inflammatory factors(IL-1b,IL-6,IL-8,TNF-α,etc.),side effects,etc.The control group has 37 patients of the same type.Results HA330-Ⅱ microporous resin plasma adsorptioin is partly effective for the improvement of symptom,improvernent rate 43.9%,and without new discomfort due to the treatment. It obtains some extent fadeaway of icterus which has a low rebound,reduce TBiL(17.83±3.65)% and there be no statistics difference of rebound.It lowers the blood ammonia level and improves the hepatic encephalopathy and brain oedema.It absorbs cytokine IL-8.Atthough it has some extent effect to albunfin,blood paltelet,haemoglobin.Conclusion HA330-Ⅱ microporous resin plasma adsorption has good application value in the treatment of chronic severe hepatitis B.It is a new treatment choice of severe hepatitis.
8.Diagnosis value of MRI to cerebellopontine angle tumor: analysis of 96 cases

Objective To improve the preoperative diagnosis accuracy of cerebellopontine angle tumors through analyzing MRI findings. Methods Ninety-six cases with cerebellopontine angle tumors, confirmed by pathology and surgery, were collected and underwent MRI scan plus enhanced MRI. Among the 96 capes, we observed acoustic neurinoma in 55 cases, meningioma in 20 cases, cholesteatoma in 9 cases, trigeminal neuroma in 7 cases,cavernous hemangioma in 3 cases,arachnoid cyst in 2 cases. The MRI characteristics of all cases were analyzed retrospectively. Results The chief type of tumor happened in the cerebellopontine angle zone was acoustic neurinoma,followed in order by meningioma,cholesteatoma,trigeminal neuroma,cavernous hemangioma and arachnoid cyst. The accuracy of preoperative localization and qualitative diagnosis were 100% and 94.7%respectively.Conclusion MRI has a high value in the diagnosis and differential diagnosis of cerebellopontine angle tumors,which can be used as a preferred preoperative examination method in cerebellopontine angle tumors.
9.Photochemical effect of MB on the inactivation of E. coli in blood and biologic functions of blood cells

Objective To observe the photochemical effect of methylene blue (MB) on the inactivation of E. coli in blood and on biological functions of blood cells. Methods The blood was treated with 15 ?mol/L MB, 40 000 Lux halogen lamp irradiation and 0.35 mmol/l rutin for 10~40 min. The photochemical effect of MB on the inactivation of E.coli in blood was observed. The changes of biologic functions of blood cells were measured by means of RBC membrane binding to poly L lysine, reaction promoted by ATP enzyme and neutrophil phagocytosis tests. Results After treatment for 40 min, the E.coli count in blood decreased by 5 log units. The value of T/E of MB and irradiation of halogen lamp was higher than 1. No obvious changes of the morphology of blood cell were found. The negative electrical charges on membrane of RBC decreased by 11.23%~25.49%. After treatment of the blood for 20~30 min, the activity of Na + K +ATP enzyme of RBC membrane increased by 7.60%~20.07%. When the blood had been treated for 10~20 min, the positive rate of neutrophil phagocytosis enhanced by 9.00%. Conclusion MB photochemical treatment can effectively inactivate E. coli in blood. Under morphology specific condition of effectively inactivated blood bacteria, the shapes and functions of blood cells are not altered, but the RBC adhesion may increase by about 11.23%~25.49%.
10.Migration and localization of umbilical cord mesenchymal stem cells implanted into brain injury model rats

BACKGROUND:Choosing an effective means to label and trace the distribution, differentiation and migration of celsin vivo help to further explore the specific mechanism of cels that exert a therapeutic effect. OBJECTIVE:To understand the migration and localization of BrdU-labeled human umbilical cord mesenchymal stem cels in brain injury model rats. METHODS:Human umbilical cord blood samples were obtained, and the isolation of human umbilical cord mesenchymal stem cels was carried out. The primary and passage culture were performed. The phenotype of cels was detected by flow cytometry. Passage 3 human umbilical cord mesenchymal stem cels were labeled using BrdU, and the cel proliferation was detected using MTT method. BrdU-labeled cels were injected into brain injury ratsvia the tail vein. At 14 days after transplantation, brain tissues in the injury region were cut into sections and the migration and location of the umbilical cord mesenchymal stem cels were observed under inverted
fluorescence microscope. RESULTS AND CONCLUSION: Cel surface specific markers CD45 and CD34 were detected by flow cytometry, but the cels could not express CD44, CD105 and CD29. Based on the cel growth curve, the cels came into a conditioning period at 1-3 days of seeding and came into a logarithmic phase at 3-5 days. BrdU-positive cels were visible at the injury region after 14 days, indicating that in the rats, transplanted human umbilical cord mesenchymal stem cels migrated from the peripheral blood to the site of brain injury to achieve the effective repair of injured parts.
