1.Enhanced dissolution of nimodipine from the preparations of the drug-PVP precipitates

OBJECTIVE: To enhance the dissolution rate and efficacy of nimodipine (NMDP) which is a poorly water-soluble substance, and to design the formulations with fast-release properties. METHOD: NMDP-PVP-k30 coprecipitate and physical mixture were prepared. The physical states of NMDP in both newly-made and one-year-old samples were investigated by X-ray diffraction analysis. The dissolution rates of NMDP from coprecipitate and from physical mixture were also compared. Five formulations were prepared on the basis of NMDP-PVP-k30 coprecipitate and their in vitro drug dissolution behaviors were examined. Also, the dissolution property of the capsules with one selected composition was examined. The selected capsules were compared with two commercial tablets on their drug release processes. RESULTS: NMDP-PVP-k30 coprecipitate gave much higher improvement in the dissolution rate than the physical mixture, and NMDP was released 89% from the coprecipitate and 45% from the physical mixture in five minutes respectively. There was no appearance of crystallization in the coprecipitate after one year storage under experimental conditions. The tablet formulation with the highest drug dissolution rate was selected. The capsules with the same composition as the selected tablet showed a higher drug dissolution rate, which indicated that drug release property was influenced by the compressing pressure. The results showed that the dissolution rate of NMDP from the selected capsules was about three to four times of that from the two commercial tablets.CONCLUSION: The dissolution rate of NMDP can be improved greatly by coprecipitation using PVP-k30 as a carrier.
2.Analysis of omission diagnosis and misdiagnosis on the cervical high-grade lesions

Objective To discuss the clinical value of the "three-step" diagnostic routine in the diagnosis of cervical high-grade lesions. Method The women with the abnormal thinprep cytology test (TCT) underwent colposcopy and biopsy,the pathobiology result was the diagnostic standard. The sensitivity and specificity of the cervical high-grade lesions was calculated. Results The identify rate of the cervical high-grade lesions from the abnormal TCT was 22.3% (340/1524), the sensitivity was 27.1% (92/340), the specificity was 97.5% (1154/1184). The sensitivity of colposcopy was 47.9% (163/340), the specificity of colposcopy was 96.4% (1141/1184). Conclusion TCT with high sensitivity is used for preliminary screening, the colposcopic examination is used to pinpoint and evaluate the lesions,the detection rate and accuracy of cervical high-grade lesions can be improved.

4.Application of tension-free hernia repair for umbilical hernia in adults

Objective To discuss the value of tension-free hernia repair for umbilical hernia in adults.Methods Fourty-five adult patients with umbilical hernia were treated with tension-free hernia repair,including mesh plug-patch in 29 cases and prolene hernia system in 16 cases.The operative procedure,postoperative pain,patient's ability recovery time,and complication and recurrence rate were studied.Results All cases were operated successfully.The mean operation time was 60 min.The only postoperative complications were 4 cases of subcutaneous seroma.The follow-up period ranged from 6 to 48 months and no recurrence was observed.Conclusions Tension-free hernia repair is a better surgical operation for umbilical hernia in adults,and it has many advantages,such as safety,rapid recovery,good early therapeutic effect and lower recurrence rate.

[Objective] To study the results of three dimensional orthomorphia treatment of king type Ⅲ idiopathic scoliosis transthoracic anterolateral.[Method]From Sep.2003 to Sep.2005,clinical data of applications of TSRH,CDH-M8 to 12 patients of king type Ⅲ adolescent idiopathic scoliosis transthoracic anterolateral were investigated with practicing ertebral solution centrum,centrum three dimensional orthopedic,bone graft fusion internal fixation,and analyzing and to appraising their operation complications,scoliosis correction rats and follow-up results.[Result]Postoperative complications of bellows were found in two examples,one was pulmonary closure and one was respiratory tract obstruction,and they had been soothed by trachea cannula and assistor respiration of respirator.[Conclusion]There weren't injury of great blood vessel,spinal injury,pneumonia and chylothorax in all cases.Cobb's angle of thoracic vertebra was 48?~66? preoperatively,and the average was 52.6?.The average Cobb's angle of remnant vertebra was 15.2? postoperatively,the rectification rat was 77.8%.The follow-up ranged 12 to 36 months,12 patients of king type Ⅲ gained good segment bone fusion.The complications of pseudarthmsis,internal fixation loosening and fracture and broken compensation haven't been found.Three dimensional orthomorphia treatment of King type Ⅲ idiopathic scoliosis transthoracic anterolateral is a safe and effective method of internal fixation.
7.Methylation and aberrant expression of Wnt antagonist SFRP1 in bladder cancer

Objective To measure the expression of SFRP1 in bladder cancer and explore the corresponding mechanism,in order to study the roles of SFRP1 in the pathogenesis of bladder cancer.Methods SFRP1 mRNA was detected by RT-PCR.Methylation status of SFRP1 was detected by methylation specific PCR.SFRP1 protein was determined by Western blotting.Results SFRP1 was methylated in bladder cancer cell lines T24 and 5637,not in SCaBER.SFRP1 mRNA and protein were detected in SCaBER,but not in T24 and 5637.Six hours after treating T24 and 5637 with 5'-aza-deoxycytidine,both T24 and 5637 expressed SFRP1 mRNA and protein.In 45 bladder cancers,methylation of SFRP1 was detected in 28 (62.2%).In matched cancer adjacent tissues,6 (13.3%) were found with SFRP1 methylation.The methylation rate in bladder cancers was significant higher than that in matched cancer adjacent tissues (P < 0.01).Conclusions SFRP1 is downregulated in some bladder cancers due to methylation.SFRP1 methylation and aberrant expression may be involved in the pathogenesis of bladder cancers.
8.Relationship between daytime sleepiness and oxygen desaturation in the male elderly with obstructive sleep apnea/hyponea syndrome

Objective To analyze the relationship between daytime sleepiness and oxygen desaturation in the male eldcrly with obstructive sleep apnea /hyponea syndrome (OSAHS).Methods 147 male elderly (aged 60-80 years) with OSAHS in our sleep laboratory from March 2008 to February 2011 were divided into daytime sleepiness and daytime non-sleepiness groups. Epworth sleeping scale (ESS) and polysomnography (PSG) were employed to detect the degree of sleepiness.Oxygen desaturation were evaluated by oxygen desaturation index (ODI),average and minimum blood oxygen,time percentage of 90% oxygen desaturation (CT90). Resulls There were significant differences between the two groups in body mass index (BMI) (t=4.157),ODI (t=-10.063),average (t=5.800) and minimum (U=916) blood oxygen,CT90(U=887)and apnca/hypopneaindex (AHI) (t =-11.361) (all P<0.001).ESS scores were related with desaturation parameters of ODI,average and minimum blood oxygen and CT90 (r=0.683,-0.450,-0.583,0.507,all P<0.001).ODI was correlated with average and minimum blood oxygen and CT00 (r=-0.628,-0.763,0.689,all P<0.001).Among parameters of oxygen desaturation,only ODI had influence on ESS scores (odds ratio=1.11,95%CI:1.07 1.15),The sensitivity and specificity predicting daytime sleepiness at ODI ≥23 times/h were about 79.2% and 84.3%,respectively.Conclusions Daytime sleepiness is associated with oxygen desaturaion and may be predicted by ODI through which average and minimum blood oxygen and CT00 are related with daytime sleepiness in the male elderly with OSAHS.
9.Effect of bone mesenchymal stem cells transplantation on neurological function and learning and memory of traumatic brain injury rats by the external carotid artery

ObjectiveTo study the effect of bone mesenchymal stem cells (BMSC) transplantation into traumatic brain injury(TBI) rats by the external carotid artery on neurological function and learning and memory.MethodsTen adult SD rats were randomly divided into TBI group ( n =5 ) and BMSC transplantation group ( n=5).Feeney free falling method was used to establish TBI models.The experimental rats were administrated with BMSC via external carotid artery (ECA),while TBI rats were injected with sterile liquid medium of equal volume via right ECA.Neurological function were evaluated according to the modified neurological severity score (NSS) at 1,3,7,15 days.Morris water maze test was used to observe the animal capabilities of place navigation and space exploration at 15 days,then animals were sacrificed.Survival and migration of implanted BMSC in brains under fluorescence microscope. ResultAfter traumatic brain,varying degrees convulsions,paralysis,loss of balance function in rats were found.Compared with TBI group,BMSC transplantation decreased significantly NSS (P <0.01 ).BMSC transplantation significantly decreased on escape latency ( ( 20.48 ± 2.29 ) s ) than the TBI group ( ( 85.93 ± 47.48 ) s) (P < 0.01 ).Moreover,BMSC group in the target quadrant dwell time ( ( 28.62 ± 1.72) % )and distance ( (29.05 ± 3.08 )% ) as well as the number of passing the platform (8.00 ± 2.45 ) were significantly higher than the TBI group ( ( 19.37 ± 2.81 ) %,(21.78 ± 3.06) %,(2.00 ± 1.87) respectively,P < 0.01 ).Transplanted BMSC could survive and migrate around injury brain through Hochest mark immunofluorescence.ConclusionBMSC can survive and migrate around injury brain by transplantation of external carotid artery,which results in a significant neurological function improvement and learning and memory increase in rats with traumatic brain injury.
10.Transurethral resection of prostate in the treatment of high risky and senile patients with large volume benign prostatic hyperplasia

Objective To improve the skills and level of TURP and peri-operative period managements to the high risky senile patients with large volume benign prostatic hyperplasia.Methods The clinical data of 318 high risky and senile patients whose ages above 80ys,ASA score > 2 and prostate volume > 60g were analyzed retrospectively.They underwent the treatment of TURP.ResultsTotal 318 patients underwent TURP were safe.The operating time ranged from 40 to 85 minutes,averaged 58.2minutes;the volume of blood transfusion ranged from 200ml to 600ml;No serious complications happened during and after the operation.With follow up of 1 ~ 12 months,the International Prostate Symptom Scores(I-PSS) decreased 14.7 averagely,Quality of Life(QOL) decreased 3.3 averagely,Maximal flow rate( Qmax ) increased 6.4ml/s averagely,and Post-voided Residual(PVR) decreased 85.3ml averagely.Conclusion The actions including sufficient preparations and evaluations pre-operatively,the maintenance to the stability of circulatory system during the operation,guaranteeing the demands of blood exchange in the vital organs such as heart,lungs and brain,are the key points to the success.
