Objective To compare open reduction and internal fixation (ORIF) and minimally invasive percutaneous plate osteosynthesis (MIPPO) in the operative treatment of complicated proximal humeral fractures.Methods From January 2014 to October 2015,55 complicated proximal humeral fractures were treated at our department.They were 32 men and 23 women,from 21 to 81 years of age (average,51 years).According to Neer classification,34 cases belonged to three-part fractures and 21 ones to four-part fractures,14 of which were complicated with dislocation.Of them,35 received ORIF and 20 underwent MIPPO.The 2 groups were compared in terms of operation time,intraoperative blood loss,incision length and Neer scoring of the shoulder function at the final follow-ups.The 2 groups were compatible without significant differences in preoperative demographic data (P ＞ 0.05).Results The 55 patients were followed up for 6 to 12 months(mean,9 months).The operation time (100.5 ± 10.2 min),intraoperative blood loss (80.0 ± 10.5 mL),and incision length (7.5 ± 3.2 cm) in the MIPPO group were significantly better than those in the ORIF group (120.0±10.1 min,200.5±10.2mL,and10.5±5.3cm,respectively) (P ＜0.05).According to the Neer scoring of the shoulder function at the final follow-ups,the good to excellent rate was 71.4％ (25/35) in the ORIF group and 85.0％ (17/20) in the MIPPO group,showing a significant difference (P ＜ O.05).The incidence of humeral head necrosis was 15 in the ORIF group,significantly higher than that (3) in the MIPPO group (P ＜ 0.05).Conclusion In treatment of complex proximal humeral fractures,compared with ORIF,MIPPO may lead to less damage to local blood supply,less trauma,and quicker functional recovery due to early functional exercise.

Objective To observe the targeted migration of glycosylated bone marrow mesenchymal stem cells (BMSCs) in rabbit bone defect models.Methods A New Zealand rabbit of 2 months old and 1.5 kg in weight was used to separate,extract and amplify BMSCs.α-1,3 fucosyltransferase Ⅵ (FUT-6) gene was transfected to the BMSCs by liposome method to screen the positive clone cell lines.ELISA was used to detect FUT-6 expression and Sialyl Lewis X (sLeX) of FUT-6/BMSCs and BMSCs.Flow cytometry was used to detect the binding force of FUT-6/BMSCs and BMSCs with E/P selectin.FUT-6/BMSCs and BMSCs were labeled with eGFP in vitro and then intravenously re-infused respectively to 2 equal groups of bone defect models made of 12 New Zealand rabbits.Fluorescence microscopy was used at 6,12 and 24 hours postoperation to observe the number of labeled cells in the medullary cavity tissue at the bone defect sites to evaluate the targeted migration.Results BMSCs were amplified by in vitro culture.Transfected BMSCs expressed a significantly higher level of FUT-6,and increased significantly generation of sLeX (P ＜ 0.05).Compared with BMSCs,FUT-6/BMSCs had an obviously higher binding force with E/P selectin (15.0％ and 12.7％ versus 68.9％ and 59.7％).Fluorescence microscopy at 6,12 and 24 hours showed that there were significantly more FUT-6/BMSCs than BMSCs in the medullary cavity tissue at the bone defect sites (P ＜ 0.05).Conclusion Glycosylated BMSCs possess an obvious ability to migrate to the sites of lesion.

[Objective]To find a simple and direct feasible method to detect bone tissue vascularize from comparing advantage or disadvantage of ink intravascular Dabble-VG dyeing and other tissue dyeing in the tissue section capillary vessel display.[Methods]One side femoral artery of 6 normal goat was mixed with liquor composed of 10 percentink,10 percent formaldehyde and 20 percent mannitol,volume ratio in 7∶2∶1.Stopped femoral vein effluent was black.To get off goat tibia,10 percent formaldehyde was fixed with.EDTA decalcification,cedar oil douse,fiat 50 to 100 micrometer pachy-section and compound ink-VG dyeing to observe capillary vessel.[Results]Compound ink-VG dyeing ould display relations of capillary vessel and surrounding tissue.But in control group,single HE,Masson and Weigert Resorcinol-aniline red dyeing after-stain VG dyeing ould display few capillary vessel and surrounding tissue,and recognize difficultly.[Conclusion]Compound ink-VG dyeing can display capillary vessel of bone clearly,and can disclose relationships of surrounding tissue as well.This method can try on in tissue vascularization detect.

[Objective] To study the results of three dimensional orthomorphia treatment of king type Ⅲ idiopathic scoliosis transthoracic anterolateral.[Method]From Sep.2003 to Sep.2005,clinical data of applications of TSRH,CDH-M8 to 12 patients of king type Ⅲ adolescent idiopathic scoliosis transthoracic anterolateral were investigated with practicing ertebral solution centrum,centrum three dimensional orthopedic,bone graft fusion internal fixation,and analyzing and to appraising their operation complications,scoliosis correction rats and follow-up results.[Result]Postoperative complications of bellows were found in two examples,one was pulmonary closure and one was respiratory tract obstruction,and they had been soothed by trachea cannula and assistor respiration of respirator.[Conclusion]There weren't injury of great blood vessel,spinal injury,pneumonia and chylothorax in all cases.Cobb's angle of thoracic vertebra was 48?～66? preoperatively,and the average was 52.6?.The average Cobb's angle of remnant vertebra was 15.2? postoperatively,the rectification rat was 77.8%.The follow-up ranged 12 to 36 months,12 patients of king type Ⅲ gained good segment bone fusion.The complications of pseudarthmsis,internal fixation loosening and fracture and broken compensation haven't been found.Three dimensional orthomorphia treatment of King type Ⅲ idiopathic scoliosis transthoracic anterolateral is a safe and effective method of internal fixation.

Objective: To study retrospectively 20 hip revison patients treated by cementless total hip arthroplasty with structural allograft.
Methods: Twenty patients suffering from aseptic loosening of an uncemented cup complicated by a large defect underwent cementless total hip arthroplasty with structural allograft and were followed up for at least 5 years. Clinical results were evaluated by Harris score and leg length measurements. Radiographic analysis included implants migration, graft absorbance, osteolysis and liner wear.
Results: No cup loosening or graft reabsorption was found at final follow-up. Clinical improvements in pain and functional status were demonstrated during the follow-up period. The mean Harris hip scores improved from 29 preoperatively (range 20-41) to 81 postoperatively (range 73-89).
Conclusion: Our study shows that cementless total hip arthroplasty with allograft is a good way for massive defect in acetabular bone stock.

ObjectiveTo explore the morbidity rate of deviation of nasal septum about personnels who take part in physical examination in the enterprise and facilities of Qinhuangdao,and clinic symptom.MethodsTransverse questionnaire investigation and normal physical examination were adopted.2604 personnels who take part in physical examination were choiced.ResultsMorbidity rate of deviation of nasal septum was 17.6％.The rate of men' s exceeds that of women' s.ConclusionMorbidity rate of deviation of nasal septum was rather high,clinic symptom of snuffle was primary.

Objective To compare the effect and safety between cemented and uncemented long-stem hip arthroplasty for treatment of unstable intertrochanteric fracture in the elderly.Methods A retrospective review was made on 70 cases (70-87 years old) undergone artificial long-stem hip arthroplasty of unstable intertrochanteric fracture between June 2012 and June 2014.According to the material of prostheses,the cases were assigned to cemented group (n =36) and uncemented group (n =34).Operation-related index,postoperative complications,ambulation time,Harris hip score and prosthesis stability and excellent results were compared between the two groups.Results There were no significant differences in operation time,bleeding volume and ambulation time between the two groups (P ＞ 0.05).Postoperative drainage volume and blood transfusion volume in cemented group [(277.4 ± 22.0) ml and (245.1 ± 17.8) ml] were lower than these in uncemented group[(294.2 ± 33.7) ml and (262.8 ± 30.9)ml] (P ＜0.05).Harris scores in cemented group were higher than these in uncemented group 1 and 3 months after operation(P ＜0.05),and there were no significant differences 6 and 12 months after operation(P ＞ 0.05).Complication rate had no significant difference between the two groups (P ＞ 0.05).Two cases were died of cement poisoning,one case of subsequent acute myocardial infarction,and two cases of subsequent acute cerebral infarction.Sixty-five cases were followed up for 12-36 months,which showed no complications such as prosthesis loosening,displacement or infection.Prosthesis stability and treatment excellent rate between the two groups were not significantly different (P ＞ 0.05).Conclusions Clinical efficacy of the two treatments is comparable,including good function recovery and early off-bed activity.Cemented long-stem hip arthroplasty reduces blood requirements,but the risk of cement poisoning exists.

OBJECTIVE:To evaluate therapeutic efficacy and ADR of amphotericin B in the treatment of AIDS combined with fungal infections retrospectively. METHODS:The cases of amphotericin B in the treatment of AIDS combined with fungal in-fections were collected our hospital. Total therapeutic efficacy,distribution of pathogenic bacteria,clearance rate and ADR were evaluated. RESULTS:89 patients were involved totally,among which 45 cases were marked effect,32 cases improved and 12 cas-es failed,with effective rate of 86.5%. The fungus were found or cultured in 76 cases,among which 64 strains were cleared,but 12 strains were not,with total clearance rate of 84.2%. There were 41 ADR cases(46.1%),while all completed the treatment ex-cept 2 developing severe renal failure(2.2%). CONCLUSIONS:Domestic amphotericin B could effectively treat AIDS complicat-ing with fungal infections,and the ADRs were acceptable.

Objective:To observe the effects of sonicated extract of Porphyromonas endodontalis(P.endodontalis)on the cell cy-cle and apoptosis of human osteoblastic hFOB 1 .1 9 cells.Methods:hFOB 1 .1 9 cells were treated with sonicated extract of P.end-odontalis at 0 (the control),1 ,1 0 and 1 00 μg/ml for 1 2,24 and 48 h respectively.MTT assay was used to examine cell prolifera-tion.The cell cycle distribution and apoptosis were examined by flow cytometry.Results:The sonicated exact of P.endodontalis in-hibited the proliferation of hFOB 1 .1 9 cells in a time-and dose-dependent manner.24 h treatment of 1 0 μg/ml and 1 00 μg/ml ex-act arrested the cell cycle of the cells in G1 phase,48 h treatment induced apoptosis in a dose-dependent manner.Conclusion:Sonicated exact of P.endodontalis may arrest hFOB 1 .1 9 cells in G1 phase and induce cell apoptosis,leading to inhibition of the cell proliferation.
