Objective To investigate the feasibility of determining the surface density of RGD-containing peptide on allogenic bone by isotopic tracing technique using RGD peptide labelled with 125Ⅰ,and the impact of the input concentration of RGD-containing peptide on the surface density, and to obtain the history between the surface density and the input concentration of RGD-containing peptide. Methods The synthetic RGD-containing peptide was labelled with 125Ⅰ,and the specific radioactivity was calculated. The reactive solutions of RGD-containing peptide with the radioactive 125Ⅰ-RGD as a probe were prepared at the input concentrations of 0.01 mg/ml,0.10 mg/ml,0.50 mg/ml,1.00 mg/ml,2.00 mg/ml,4.00 mg/ml.Using EDC as the cross-linking agent, the reaction was carried out by placing the allogenic bone pieces into the reactive solutions of RGD-containing peptide with different input concentrations. After the reaction, the surface density of RGD-containing peptide grafted onto the allogenic bone pieces was calculated by evaluating the radioactivity and the surface area of the bone pieces. The impact of the input concentration of RGD-containing peptide on the surface density was measured and the curve was ascertained. Results After measuring in the radiodensity γ-counter,the result showed the RGD peptides have been marked with 125Ⅰ successfully. The allogenic bone pieces were radioactive after the reaction, which demonstrated that the RGD-containing peptide had been grafted onto the surface of bone pieces successfully. It was also found that with the increasing of input concentration, the surface density raised. Conclusion The surface density of RGD-containing peptide is related to its input concentration. With the increasing of input concentration, the surface density raises to the saturation value gradually.

Objective To investigate the effect of different bone cement volume injected after vertebroplasty on stress distribution of the upper and lower endplates of the injured vertebra in the flexion and extension positions by finite element analysis. Methods Two patients with osteoporotic compression fractures of L2 vertebral body were chosen.Bone cement volumes of 25% and 50% were respectively injected into the fractured vertebral body bilaterally.Pre-and post-operative finite element models of fractured and adjacent vertebral bodies were developed after extracting the CT data.Stress changes at L2 superior-and inferior-vertebral endplates were observed in the vertical, flexion, and extension loading. Results Stress at the upper and lower endplates of the fractured vertebra increased after bone cement injection and more increase was observed in 50% bone cement group than in 25% bone cement group. In the neutral position, the stress mainly concentrated at posterior vertebral body, with stress of the upper endplate improving from 0.91 × 106 N · mm to 0.93 × 106 N · mm and stress of the lower endplate from 0.98 × 106 N ·mm to 1.03 × 106 N · mm.In the anterior flexion, the stress mainly concentrated at anterior vertebral body, with stress of the upper endplate improving from 4.44 × 106 N · mm to 5.55 × 106 N · mm.In the posterior extension, the stress mainly concentrated at medial vertebral body, with stress of the upper endplate improving from 21.95 × 106 N · mm to 22.10 × 106 N · mm and stress of the lower endplate from 30.60 ×106 N · mm to 30.88 × 106 N · mm.The finite element model revealed stress at the upper and lower endplates was higher in the anterior flexion and posterior extension than in the neutral position, while in the anterior flexion the stress was comparatively higher. Conclusions A small amount of implanted bone cement act no evident effect on stress distribution of the upper and lower vertebral endplates. However, the stress changes are significant as cement volume reached up to 50%,especially in the flexion position,as may suggest an enhanced risk of adjacent intervertebral disc degeneration and adjacent vertebral body fractures.

Journal of Chinese Physician 2011;13(7):930-934
Objective To quantify the morphometric characteristics of the pedicles of the infantile thoracolumbar spine and to determine the feasibility and safety of pedicle screw fixation in very young children. Methods A total of 52 patients younger than three years of age underwent standard spiral computed tomography (CT) were enrolled in this study. The patients were grouped according to age, group 1 (0 ~ 18months of age) and group 2 (18 ~ 36 months of age). Images were reformatted, and multiplanar reconstructions were used to attain images of thoracic and lumbar pedicles on sagittal, coronal and transverse planes. The measurements included the inner and outer pedicle diameters on the transverse plane, pedicle sagittal diameter, pedicle length and the pedicle angle on the transverse. Characteristics of the pedicles of the immature thoracic and lumbar spine were studied. Results Pedicle diameters on the transverse plane decreased gradually from T1 to T5 and increased gradually from T6 to L5. The shortest transverse diameter of the thoracic pedicle was T5 or T6. The inner diameter of the segment of L4 in infant (younger than eighteen months) was less than 3.0 mm and the external diameter less than 3.5 mm from T3 to T9. The inner diameter of the segment from T5 to T8 in infant (more than eighteen months) was less than 3.0 mm and the external diameter from T4 to T7 was less than 3.5 mm. The sagittal diameter was significantly more than the transverse diameter. Pedicle diameters on the transverse plane, the sagittal diameter, and the length of the pedicle changed significantly with age, but the pedicle angle did not changed significantly with age. Conclusions The pedicles in the segment from T3 to T9 of less than eighteen months and from T4 to T7 of more than eighteen months were too small, there was no feasibility and safety of pedicle screw fixation. So pedicle screw fixation was feasible and safe for below T9 segment in the infants who are younger than three years old.

OBJECTIVE:To evaluate the effectiveness, safety and economics of antibiotics for inpatients with acute exacerba-tion of chronic obstructive pulmonary diseases (AECOPD),in order to promote rational use of antibiotics. METHODS:In retro-spective study,AECOPD inpatients were selected as research objects during 2013-2015 to evaluate the use of antibiotics. RE-SULTS:Three thousand one hundred and eighty-six AECOPD patients were enrolled in the study, and utilization ratio of antibiot-ics was above 90%. DDDs of Cefordizime sodium for injection ranked the first place in 3 years and was decreased in 2015,while those of β-lactam and carbapenems were increased. DDC of Meropenem for injection and Imipenem/cilastatin sodium for injection were the highest. Drug utilization index(DUI)of Cefodizime for injection was far more than 1.0,while DUI of Piperacillin sodi-um and sulbactam sodium for injection was much less than 1.0. Sort ratio(B/A)of Cefodizime sodium for injection ranged 1-2 in 3 years. B/A of Piperacillin sodium and sulbactam sodium for injection,Imipenem and cilastatin sodium for injection,Meropenem for injection were lower than 1.0. CONCLUSIONS:The indication of anti-infective treatment in AECOPD patients have been mas-tered strictly in our hospital;the utilization ratio of antibiotics is decreased year by year;the types of antibiotics are rational rela-tively,and the number of antibiotics which is rationally used is increased year by year. But the rationality of Cefodizime sodium for injection and Piperacillin sodium/sulbactam sodium for injection need to be improved.

OBJECTIVE:To investigate the relationship of drug utilization index(DUI)of antibiotics with therapeutic efficacy of patients with acute exacorbation of chronic obstructive pulmonary disease(AECOPD). METHODS:By the method of drug utili-zation evaluation, inpatients with AECOPD in our hospital during 2013-2015 were selected as the research object. Diagnosis and treatment information prescribing information of patients were collected, and inpatients only receiving one kinds of antibiotics dur-ing hospitalization stay were selected to evaluate the relationship of therapeutic efficacy with rehospitalization indexes. RESULTS:A total of 2155 AECOPD patients were enrolled in the study. Among antibiotics with DDDs>500,antibiotics with DUI close to 1.0 was cefotiam hydrochloride for injection, and that with DUI far higher than 1.0 was Cefodizime sodium for injection, while that with DUI much less than 1.0 was Piperacillin sodium and sulbactam sodium for injection. There was statistical significance in therapeutic efficacy between Cefotian hydrochloride for injection and Piperacillin sodium and sulbaactam for injection (P<0.05);rehospitalization had no statistical significance(P>0.05). There was no statistical significance in therapentic efficacy or rehospital-ization between cefotiam hydrochloride for injection and cefodizimes sodium for injection(P>0.05). CONCLUSIONS:DUI is as-sociated with therapeutic efficacy, but the rationality of antibiotics can not be simple judged according to the distance between DUI and 1.0. A variety of confounding and bias factors should be integrated to avoid misreading and misjudgment.
