3.Transcription and expression of excision repair cross complementing 1 in endemic arsenism caused by coal-burning

Objective To study the transcription and expression of excision repair cross complementing 1(ERCC1) in the peripheral blood and the skin tissue in coal-burning borne endemic arsenism, and to explore the role of arsenism in its pathogenic or carcinogenesis mechanism. Methods According to "Endemic arsenism diagnostic criteria" (WS/T 211-2001), 110 arsenism patients were chosen as case group in Xingren county,Guizhou province and they were divided into 3 groups according to their hnir arsenic: < 2(31 cases),2 ~< 4(31 cases),≥4 mg/kg(48 cases), respectively. Another 36 healthy residents about 13 km away from the endemic area were chosen as healthy control group. Under the principle of informed consent, hair samples were collected for arsenic analysis by Ag-DDC and blood samples were collected to determine mRNA expression levels of ERCCI by real-time fluorescence quantitative PCR. At the same time, skin tissue samples were collected from the voluntary surgical treatment of 62 patients with endemic arsenism as case group which were divided into 3 groups according to their hair arsenic: < 2(16 cases), 2 ~< 4(20 cases) and ≥4 mg/kg(26 cases), respectively, and these patients were also divided into general pathological changes (32 cases), precancerous (19 cases) and cancerous groups( 11cases), respectively, according to their skin pathologic diagnosis of skin lesions. Another 13 cases pathologically normal without skin cancer surgery from a certain hospital were chosen as control group. Skin samples were collected to detect the ERCC1 protein by immunohistochemical method. Results The mRNA levels of ERCC1 were 0.7156(0.2158 ~ 1.2405),0.5772(0.0843 ~ 1.1234) and 0.5490(0.1895 ~ 0.8431 ), respectively, among < 2, 2 ~< 4and ≥4 mg/kg groups, which were lower than the mRNA levels of ERCC1 in the control group[1.5128(1.0000 ~2.1295)], and the difference was statistically significant(all P < 0.05). The expression rate of ERCC1 protein were 87.5%, 80.0% and 77.0%, respectively, among < 2, 2 ~< 4 and ≥4 mg/kg groups. The expression rate of ERCC1 protein in 2 ~< 4 and ≥4 mg/kg groups were lower than the rate in the control group(100.0%), and the difference was statistically significant (all P < 0.05). The expression rate of ERCC1 protein were 84.4%, 79.0%and 72.8%, respectively, among general pathological changes, precancerous and cancerous groups compared with the control group( 100.0% ), and the difference was statistically significant(all P < 0.05). Conclusions Arsenic from coal-burning can lead to abnormal ERCC1 gene transcription and protein expression, which may inhibit DNA repair through influencing the removal of damaged DNA and promoting the incidence of arsenism development and even skin carcinogenesis.
5.Analysis of factors relating to serum lutein level in elderly patients with early age-related macular degeneration

Objective To explore the factors relating to serum lutein level in elderly patients with early age-related macular degeneration (AMD).Methods 102 early AMD patients diagnosed by international diagnostic criteria were selected.The level of serum lutein was measured by HPLC,and dietary intake was estimated by food-frequency questionnaire (FFQ).The factors relating to serum lutein level were analyzed by Pearson correlation and multiple regression analysis models.Results Serum lutein level was higher in female AMD patients than in male AMD patients [(0.33±0.31) μmol/L vs.(0.21 ±0.19) μmol/L,t=2.21,P<0.05].Serum lutein level was positively correlated with dietary lutein (r=0.49,P< 0.001),but not correlated with age,BMI,and other nutrient intake after adjustment for caloric intake.Dietary lutein was mainly from corn,spinach,leeks,eggs,chrysanthemum,pumpkin,broccoli,lettuce,asparagus and peas.Intakes of spinach,pumpkin,asparagus were significantly associated with serum lutein level.Conclusions Elderly patients with AMD should increase the intakes of lutein-rich vegetables to increase the serum lutein level.
7.Accuracy of Corvis ST for the measurement of central corneal thickness and intraocular pressure in myopia

Background Corvis ST corneal biomechanical analyzer (Corvis ST) can offer corneal biomechanical parameters,intraocular pressure (IOP) and central corneal thickness (CCT),and measured IOP value was corrected based on CCT and biomechanical factors.Corvis ST is applied abroad,but the study on its accuracy is few in China.Objective This diagnostic trial was to evaluate the accuracy of Corvis ST for CCT and IOP measurement in myopic population.Methods Fifty-six eyes from 56 myopic patients were prospective recruited in Visual Science and Optometry Center of Guangxi from November to December in 2012.IOP was measured by using Corvis ST and Goldmann applanation tonometer (GAT),and CCT was measured by Corvis ST and A type ultrasonic pachymetry.The CCT difference between Corvis ST and A type ultrasonic pachymetry as well as IOP between Corvis ST and GAT were compared by using paired-t test,and agreements of measured outcomes were analyzed by Bland-Altman method.This study was approved by the Ethic Committee of People's Hospital of Guangxi and written informed consent was obtained from all subjects.Results The CCT from Corvis ST was (539.82± 19.79) μm,which was significantly higher than (535.34± 19.41) μm from A type ultrasonic pachymetry (t =4.19,P<0.001).Bland-Altman analysis revealed that the CCT measured by Corvis ST was 4.5 μm higher than that of A type ultrasonic pachymetry,with the 95% limit of agreement ranged from-11.2 to 20.2 μm,and 7.1% (4/56) of points were located at the outside of the 95% confidence interval.The IOP measured by Corvis ST and GAT were (15.75±1.60) mmHg and (16.23 ±2.40) mmHg,respectively,showing statistically significant difference between the two methods (t=2.15,P =0.04).Bland-Altman analysis revealed that the IOP measurement of Corvis ST was 0.5 mmHg lower than that of GAT,with the 95% limit of agreement ranged from-3.8 to 2.8 mmHg,and 3.57% (2/56) of points were located at the outside of the 95% confidence interval.Conclusions CCT obtained by Corvis ST is higher than that by A type ultrasonic pachymetry with poor agreement between these two outcomes,and the two methods cannot replace each other clinically in myopic eyes.IOP value from Corvis ST is slightly lower than that from GAT,showing a good agreement between these two outcomes.The IOP value of Corvis ST shows satisfactory accuracy.
8.Clinical studies on different acupuncture time for primary dysmenorrhea

Objective: To seek the optimal acupuncture time for primary dysmenorrhea and provide clinical basis for optimal acupuncture treatment protocol. Methods:A total of 90 eligible cases were randomly allocated into three groups, 30 cases in each group. Points Guanyuan (CV 4), bilateral Zusanli (ST 36) and Sanyinjiao (SP 6) were selected for patients in all three groups, with a different treatment duration: 15 min in group A, 30 min in group B and 45 min in group C. Then the clinical efficacy in each group was evaluated by pain symptom scoring. Results:As for the pain symptom scores, there were statistically significant intra-group differences between before and after treatment in three groups (allP<0.05); coupled with statistically significant inter-group differences between group B and the other two groups (bothP<0.05). As for clinical efficacy, there were statistical differences between group B and the other two groups (bothP<0.05), indicating that 30 min of acupuncture is the optimal duration in the treatment of dysmenorrhea. Conclusion:With the same needling manipulation, 30 min of acupuncture treatment achieves a better efficacy for primary dysmenorrhea.
9.Expression of DNA methyltransferase 1 mRNA in patients of endemic arsenism and its clinical significances

Objective To investigate the transcription and expression of DNA methyltransferase 1 (DNMT1) mRNA in endemic arsenism patients by burning coal usage,to probe its effects on the development and carcinogenesis of arsenism. Methods In 2008,68 arsenism patients(including 24 mild cases,28 moderate cases and 16 severe cases) were selected in the areas with endemic arsenism according to Standarding of Diagnosis for Endemic Arsenism from Xingren county,Guizhou province. Among the subjects,40 cases were diagnosed by pathological methods,and they were divided into general pathological changes(20),precancerous(14) and cancerous group(6). Tweleve kilometer away from the endemic arsenism area,23 controls were selected in Daguoduo village (non-arsenism exposure). Under the principle of informed consent,blood samples were collected from individuals. The mRNA expression of DNMTI was detected by real-time quantitative reverse transcription polymerase chain reaction(FQ-PCR). At the same time,skin tissue samples were collected from the voluntary surgical treatment patients with endemic arsenism (total 61 cases,including 34 general pathological changes cases,21 precancerous cases and 6 cancerous cases) and from the control(15 cases). DNMT1 protein was detected by immunohistochemical method.Results Average level of DNMT1 mRNA were 0.221 83±0.595 09,0.246 11±0.509 79 and 0.389 27±0.411 33 respectively among mild,moderate and severe arsenism group. DNMT1 mRNA level of mild and moderate group were obviously lower than the control group(0.695 95±0.463 98,all P < 0.01). The mRNA average level of DNMT1 were 0.320 64±0.547 46,0.313 09±0.529 13 and 0.159 07±0.342 56 individually among general pathological changes,precancerous and cancerous group,which were obviously lower than the control group(0.695 95±0.463 98,all P < 0.05). The expression rates of DNMT1 protein in skin were 88.24%(30/34),100%(21/21) and 100% (6/6) among general pathological changes,precancerous and cancerous group were higher than the control group [0(0/15),all P < 0.01],and the extent of expression gradually increased with the aggravation of skin damage(r,= 0.740,P < 0.01). Conclusions DNMT1 participated in the development of the arsenism. High expression of its protein was an early event during the process of the arsenism. DNMT1 may be the new target markers for early diagnosis and treatment of arsenism.
10.Study on the late stage of autophagy in primary gouty arthritis

Objective:To explore the expression and clinical significance of late autophagy in per-ipheral blood mononuclear cells (PBMCs) of patients with primary gouty arthritis (GA).Methods:Peripheral blood, clinical data, and laboratory tests were collected from 30 patients with acute gout (AG), 30 patients with intermittent gout (IG), and 50 healthy controls (HC). Quantitative polymerase chain reaction (RT-qPCR) was used to detect mRNA expression levels of autophagy-related genes (ATG5, ATG12, ATG16, ATG3, ATG7, ATG10, ATG4B, LC3-2/LC3B). Measurement data conformed to normal distribution were tested using t test or analysis of variance (ANOVA), and non-normal distribution data were tested using Mann-Whitney test or Kruskal-Wallis H test. SNK was used for pairwise comparison among the three groups. Correlation between variables was tested by Spearman correlation analysis. The expression level of ATG4B mRNA in the AG and IG group was higher than that in HC group, and there was significant difference between IG group and AG group, IG group and HC group[9.95×10 -3(6.32×10 -3, 12.23×10 -3) vs 10.86×10 -3 (8.80×10 -3, 17.03×10 -3) vs 8.07×10 -3(5.52×10 -3, 11.63×10 -3), H=8.531, P=0.014]. There was no significant difference between the ATG3 mRNA and ATG7 mRNA groups ( H=0.539, 3.739, bothall P values >0.05). ② The results of Spearman correlation analysis suggested that in patients with acute gout, ATG3 was negatively correlated with PDW and MPV ( r=-0.499, P=0.006; r=-0.463, P=0.011); ATG4B was positively correlated with HDL-C ( r=0.408, P=0.048); ATG7 was negatively correlated with GLOB ( r=-0.554, P=0.001); ATG10 was positively correlated with ALB ( r=0.412, P=0.024) and negatively correlated with Crea and hsCRP ( r=-0.459, P=0.011; r=-0.375, P=0.045); ATG12 was negatively correlated with MO ( r=-0.434, P=0.017); ATG16 was negatively correlated with ALT and AST ( r=-0.389, P=0.034; r=-0.366, P=0.047); LC3-2 was positively correlated with UA ( r=0.381, P=0.041) and negatively correlated with MPV and PDW ( r=-0.413, P=0.026; r=-0.449, P=0.015). In patients with intermittent gout, ATG3 and ATG4B were negatively correlated with apoB100 ( r=-0.555, P=0.011; r=-0.462, P=0.040); ATG5 was negatively correlated with Crea ( r=-0.456, P=0.011); ATG10 was negatively correlated with TC, LDL-C, and apoB100 ( r=-0.526, P=0.017; r=-0.556, P=0.011; r=-0.515, P=0.020). Conclusion:Autophagy is involved in the development of gout, and is correlated with ibflammatory and metabolic indicators, suggesting that autophagy is an important feature in the pathogenesis of GA.
