1.Glucagon-Like PeptideⅠand Targeted Differentiation of Stem Cells

Diabetes mellitus has become one of the diseases which threaten the heath of human being in the 21st century.A goal of research in diabetes is to find a way to increase the number of functional insulin-producing cells. Islet transplantation has been considered to be the most effective approach to cure type Ⅰ and part of type Ⅱ diabetes mellitus.This approach, however, is severely limited by an inadequate supply of donor islets available for transplantation.Moreover, recent progress of stem cells research has shown that stem cells may act as a new source of islet transplantation in diabetes mellitus treatment. Recent evidence indicates that Glucagon-Like PeptideⅠ(GLP-1) plays a very important role in targeted differentiation of stem cells into Insulin-Producing Cells and pancreatic development. GLP-1 is an intestine-derived insulinotropic hormone that stimulates glucose dependent insulin production and secretion. GLP-1 can induce differentiation of stem cells into insulin-producing cells, which is achieved by up regulation of PDX-1 expression.PDX-1 is a transcription factor critical for pancreatic development and endocrine cell neogenesis and a marker for pancreatic stem cells. These new findings suggest an approach to create Insulin-Producing cells in vitro by expanding stem/progenitor cells and then to convert them into Insulin-Producing cells by treatment with GLP-1. Thus GLP-1 may be a means by which to create Insulin-Producing cells ex vivo for transplantation into patients with insulinopenic type Ⅰ diabetes and severe forms of type Ⅱ diabetes. This article reviews recent progress about GLP-1 and targeted differentiation of stem cells induced by GLP-1.
2.Two cases of Avian Influenza A(HSNI) in Anhui province of China

Objective To analyze the clinical features,epidemiologic information and outcome in confirmed cases of human infection with a highly pathogenic influenza A(H5N1).Methods The clinical features and epidemiologic findings in 2 confirmed cases of avian influenza A(H5N1)in Anhui province,in November 2005.Clinical data on vital signs,physical findings,laboratory tests and roentgenology were obtained by means of retrospective review of the hospital records.Epidemiologic data were collected through interviews of the patients and their relatives.Results In both cases,the diagnosis of influenza A(H5N1)was confirmed by means of viral culture and real-time polymerase chain reaction with specific primers for H5 and N1 in samples obtained from tracheal aspiration.All patients were previously healthy young women and resided in village.They had a clear history of di- rect contact with sick/dead poultry and prepared dead chickens at home for eating(removed feathers, washed,cut)hut no report of confirmed HSN1 animals in the village.The time between exposure and onset of illness were 5 days,no one else in family sick.The time between the onset of illness and hos- pitalization were 5 days and 6 days,respectively.Two patients have initial symptoms of high fever (typically a temperature more than 38℃).The prominent clinical features were those of influenza syndrome,including fever,cough,and shortness of breath.Upper respiratory tract symptoms were absent.The platelet counts were decreased.In both patients,there were marked abnormalities on chest radiography,radiographic changes include bilateral and unilateral lobular consolidations with air bronchograms,and had dramatic worsening of findings.Two patients developed acute respiratory dis- tress syndrome(ARDS)complicated multiple organ dysfunction syndrome(MODS)and died of pro- gressive respiratory failure.Conclusions Exposure to dead poultry within a week before the onset of illness was associated with Influenza A(H5N1)infection in humans,but no definitive evidence of hu man-to-human transmission has been found yet.Influenza A(HSNI)infection,characterized by fe- ver,an influenza-like illness with lower respiratory tract symptoms,carries a high risk of death.
3.Influence of Weight - Lost on Glucose Intolerance Test and Insulin Secretion in Obese Children

Objective To observe the influence of weight - lost therapy on obese children with impaired glucose tolerance( IGT), insulin resistance. Methods Fasting,post- prandail 2 hours blood glucose and insulin were measured in 14 obese children (age 8 - 15 years) with IGT before and after two - month weight - lost iherapy. Glucose were measured with enzymeoxidize assay, and insulin were measured with radio-immunity assay(RIA). Results Among 14 obese children with IGT, after two-month weight - lost the-rapy, there were 9 children becoming normal OGTT. There were significantly lower in the concentration of fasting insulin and post - prandail 2 hours blood insulin and glucose[(14.23?2.35) mIU/L,(47.20?10.26) mIU/L,(5.36?0.91) mmol/L] than before weight -lost therapy[ (32.54?7. 13) mIU/L,( 164.53?33.60) mIU/L, (8.75?1.09) mmol/L](P
5.Comprehensive Healing Treatment of 30 Cases with Small and Short Penis in Simple Obesity Children

Objective To investigate the therapy for small and short penis in simple obesity children.Methods Sixty simple obesity children with short and small penis were randomly divided into 2 groups(experiment group 30 cases and control group 30 cases).The experiment group received comprehensive treatment(consisting of massage of traditional Chinese medicine,behavior modification,dietetic and sport adjustment) to lose weight and treated by short and small penis healing apparatus.The control obesity children were given conducted behavior,dietetic and sport adjustment,and treated by short and small penis healing apparatus.Results After treatment,weight of 30 cases in the experiment group decreased,with a decrease of(7.75?3.50) kg.Body mass index(BMI) decreased from(31.10?3.88) before treatment to(27.82?3.49),and BMI before and after treatment changed significantly(t=12.68 P
6.Epidemiological Investigation of Child Simple Obesity in Zhengzhou Area and Intervention Research on Massage Along Channel of Traditional Chinese Medicine

Objective To explore incidence of child obesity in Zhengzhou area and intervention measures.Methods In 2001, spot check was conducted on 5688 cases of high and primary school students ,including 2848 boys and 2840 girls at the age of 7-18.Child obesity was diagnosed by meeting reference BMI value published by Cole et al,conducted comprehensive treatment consisting of massage intervention along channel of traditional Chinese medicine, behavior modification, dietetic and sport adjustment for 22 cases of simple obesity children (7-15 years old, 18 boys, 4 girls) selected for one month, and follow-up survey 6 months after treatment.Results Five thousands,six hundreds and eighty-eight high and primary school students investigated had an overweight incidence of 15.4%, an obesity incidence of 3.2%,boys' overweight incidence and obesity incidence(19.2%,4.6%) were remarkably higher than those of girls (11.5%,1.7% P<0.001). Overweight incidence of various age groups evidently differed(χ2=42.88 P<0.001) with the group of 8-15 years old children as popular. Incidence of various age groups also differed(χ2=21.28 P<0.05) with 7-10 years old and 14-15 years old children as popular. After one-month treatment, weight of all the 22 cases of fat children decreased from (76.45±19.87) kg upon hospitalized to (69.06±17.98) kg with a decrease of (7.43±2.58)kg, BMI value decreased from (31.05±3.96) before treatment to (27.72±3.54).Weight and BMI value before and after treatment differed evidently (t=13.6,12.88 P<0.01), and weight and BMI value decrease were remarkably related with those upon hospitalized (r=0.77 P<0.01;r=0.49 P<0.05).Conclusions Incidence of child obesity has been increasing in recent years, comprehensive treatment consisting of massage intervention along channel of traditional Chinese medicine, behavior modification, dietetic and sport adjustment have good curative effect and are comfortable, well received by children and suitable to promote and apply.
7.Biological characteristics of arsenic resistance cell chronic arsenic exposure human bone marrow mesenchymal stem cells

Objective To study the biological characteristics of arsenic resistance cell model chronic arsenic exposure human bone marrow mesenchymal stem cells (CAsE-hFBMSCs) and discuss the consequence of chronic arsenite exposure to human mesenchymal stem cells (hFBMSCs). Methods hFBMSCs cultivated under general conditions,hFBMSC cell survival rate was detected in 48 hours with arsenite toxicity test under different doses arsenic [0(control),0.25,0.50,1.00,2.00,4.00,8.00,20.00,40.00,80.00,120.00 μmol/L]of the fist 2-generation(P2). According to the test results,1.00 μmol/L sodium arsenite was chosen to stimulate hFBMSCs for 14 weeks as experimental group,simultaneous 0 μmol/L sodium arsenite as the control group. And then,the phenotype was detected by fluorescence-activated cell sorting,and the cell cycle by flow cytometry. Finally,the cell malignant transformation was detected by soft-agar assay. Results Arsenite low than 10 μmol/L promoted cell proliferation,but inhibited cell proliferation when exceeding 10 μmol/L. Half of the lethal dose (LC_(50)) in experimental and control groups were (89.42±0.64),(52.48±0.71)μmol/L. The difference between two groups was statistically significant(t = 123.89,P < 0.05). The phenotype of CAsE-hFBMSCs was CD29,CD90,CD166 positive and CD34,CD45 negative. The phenotype of CAsE-hFBMSCs was the same as the control. Comparing to control group[(8.44±0.45)%,(9.14μ0.14)%,(82.42±0.60)%],G2/M phage[(17.72±5.47)%]and S phage [(25.34±3.36)%]cell increased,G0/G1 phage[(56.96±8.83)%]cell decreased in P2 CAsE-hFBMSCs. The cell cycle became nearly the same as the control group after adaption. CAsE-hFBMSCs did not show clone formation in soft agar clone formation assay. Conclusion Long last and low level exposure to arsenite does not influence the biologic features of hFBMSCs.
8.Cross-sectional association of serum C-reactive protein and uric acid with albuminuria in Chinese type 2 diabetic patients.

BACKGROUNDEvidences show that subclinical chronic inflammation is involved in the pathogenesis of diabetic nephropathy. The aim of this study was to examine the relationship between serum C-reactive protein (CRP), serum uric acid, and albuminuria in Chinese type 2 diabetic patients.
METHODSA total of 1162 type 2 diabetic patients were recruited. All participants had relevant clinical and laboratory measurements. CRP was measured using a particle enhanced immunoturbidimetric assay.
RESULTSIn the multiple linear regression model, natural log-transformed CRP (lnCRP) and uric acid were independent predictors of natural log-transformed urinary albumin to creatinine ratio (lnACR) (β = 0.18, 95% CI 0.10-0.27, P < 0.001 and β = 0.18, 95% CI 0.09-0.27, P < 0.001). The interaction of lnCRP with uric acid was also associated with lnACR (β = 0.04, 95% CI 0.02-0.06, P < 0.001). In the full-adjusted logistic regression model, the OR for albuminuria of the patients in the third tertile levels of CRP and uric acid was 3.94 compared with patients in the first tertile levels of CRP and uric acid (95% CI 1.73-8.94, P < 0.001).
CONCLUSIONSElevated serum CRP and increased serum uric acid level were associated with albuminuria in Chinese type 2 diabetic patients. Moreover, CRP and uric acid had an interactive effect on albuminuria.

9.Experimental Study on Therapeutic Effects of Xileisan Pessary on Rats with Cervicitis

Objective To study the therapeutic effects of Xileisan Pessary on rats with cervicitis based on the idea of treating different diseases with the same therapy. Methods Vagina perfusion of hydroxybenzene mucilage was used to establish rat cervicitis models. Rats were randomly divided into normal group, model group, positive medicine group, low-, medium-, and high-dose Xileisan Pessary groups. Rats with cervicitis were treated with Xileisan Pessary from the next day after modeling for 10 days. The amount of neutrophilic granulocytes and leukocytes were detected by blood routine analysis meter; cervial histopathologic examination was conducted by HE staining. Results Compared with normal group, the numbers of granulocyte and leucocyte increased significantly in the model group (P<0.01). Compared with the model group, the numbers of granulocyte and leucocyte in the low-dose Xileisan Pessary group showed a decreasing trend (P<0.05, P<0.01); cervical inflammation mucosal epithelium and tissue necrosis were signally alleviated; granulocyte in the medium-dose Xileisan Pessary group showed a decreasing trend (P>0.05);the content of leucocyte hardly changed;granulocyte in the high-dose Xileisan Pessary group increased observably (P<0.01); the number of leucocyte showed an increasing trend (P>0.05), and the numbers of granulocyte and leucocyte in positive medicine group decreased observably (P<0.01), and cervical inflammation mucosal epithelium and tissue necrosis were also signally alleviated. Conclusion Xileisan Perssary can relieve the inflammation of cervix and repair injured mucosa. It has the function of treating different diseases with the same therapy.
10.Clinical significance of flow cytometry in diagnosis of immunorelated pancytopenia.

This study was purposed to explore the diagnostic role of flow cytometry in immunorelated pancytopenia (IRP). After 50 IRP patients were hospitalized, the concentration of serum ferritin, folic acid and vitamin B(12), immunologic test, platelet antibody, test of hepatitis A, B and C, haemolysis test and bone marrow smear examination were carried out, meanwhile the chromosome karyotype analysis and some routine examinations were performed. The 50 patients were divided into group A and group B. Group A consisted of 22 patients who were undefinedly diagnosed and intended to diagnosed as IRP, group B consisted of 28 definedly diagnosed patients with hematologic malignancies, including 7 cases of aplastic anemia, 2 of paroxysmal nocturnal hemoglobinuria, 10 of myelodysplastic syndrome, 9 of megaloblastic anemia. In addition, 30 normal people were used as normal control group (group C). For groups A and B, the binding autoantibodies of bone marrow stem/progenitor cells, erythroblasts and myelocytes were detected by flow cytometry, meantime the ratio of total B-(CD10(+)) and CD5(+) B-lymphocytes in peripheral blood was assayed. For control group, the ratios of CD19(+) and CD5(+) B lymphocytes in peripheral blood were determined alone. The results indicated that the detection of bone marrow autoantibodies in 20 patients of group A showed positive with 90.90%. The IgG type was found mostly in antibody binding types, next the IgM type, the IgA type was fewer. The detection of bone marrow autoantibodies of 2 patients in group B showed positive with 7.14%. The positive rate in group A was obviously higher than that in group B (p < 0.01). The ratios of CD19(+) and CD5(+) B lymphocyte in peripheral blood were significant higher in group A than that in group B and control group (p < 0.01), but there was no significant difference between groups B and control. It is concluded that the application of flow cytometry in detecting the autoantibodies of bone marrow cells and CD19(+) B-and CD5(+) B-lymphocyte in peripheral blood can provide reliable diagnostic evidence and detection measure for diagnosis and differential diagnosis of IRP, as well as may contribute to draw up more effective therapeutic strategy.
