1.Clinical study of nicergoline and aniracetam in treatment of mild and moderate cognitive dysfunction after cerebral infarction

Objective To observe the clinical efficacy and safety of nicergoline and aniracetam in treatment of mild and moderate cognitive dysfunction after cerebral infarction .Methods Infarction were enrolled after 70 cases of mild and moderate cognitive im-pairment patients were randomly divided into nicergoline treatment group (observation group) and aniracetam treatment group(con-trol group) ,all 35 cases .After 12 weeks of treatment were compared before and after treatment mini mental state examination (MMSE) ,activities of daily living scale(ADL) scores and transcranial Doppler(TCD) to improve the situation .Results After 12 weeks ,both groups after treatment compared with treatment MMSE score was significantly higher (P< 0 .05) ,and nicergoline group than aniracetam group(P< 0 .01);ADL score before treatment were significantly reduced (P< 0 .05) ,TCD improvement nicergoline group than aniracetam group (P<0 .01) .Conclusion Nicergoline mild and moderate cognitive dysfunction after cerebral in-farction have significant effects ,and better than aniracetam ,no significant adverse reactions occur for a wide range of clinical applications .
2.Expression and antitumor activity of fusion protein RGD-TRAIL in Pichia pastoris.

To compare the activity of RGD-TRAIL in different expression systems, RGD-TRAIL in both Escherichia coli (E.coli) and Pichia pastoris was constructed and expressed. In vitro activity of RGD-TRAIL from Pichia pastoris expression system was also analyzed. Genetic engineering techniques were used to construct recombinant plasmid pET30-rgd-trail and pHBM-rgd-trail. The recombinant protein RGD-TRAIL was purified with Ni ion affinity chromatography after induction. MTT assay, ELISA, scratch wound healing, transwell migration assay and Hoechst 33342 staining were performed to detect the effects of RGD-TRAIL on proliferation, binding activity, migration and apoptosis. The expression of apoptosis-associated proteins was detected by Western blotting. Recombinant protein RGD-TRAIL was successfully expressed in a form of inclusion body in E.coli, while expressed secretorily in Pichia pastoris. It possessed more potent cytotoxicity than RGD-TRAIL in E.coli by MTT assay. The RGD-TRAIL expressed by Pichia pastoris showed powerful binding affinity with cancer cells expressing α(v), DR4, DR5 and highly potent cytotoxicity through inducing apoptosis of cancer cells. Nuclear fragmentation was examined by Hoechst 33342 staining. Cleaved PARP and caspase-3 were also detected after incubation with RGD-TRAIL. Additionally, RGD-TRAIL inhibited migration significantly in A549 and HT1080 cells. The results demonstrate that Pichia pastoris expression system is more suitable for the recombinant protein RGD-TRAIL. Its binding affinity and antitumor activity might make RGD-TRAIL a promising candidate for cancer therapy.

Objective: To study the expression of the proliferating c el l nuclear antigen (PCNA) and cell nuclear area in the different layers of oral d ysplasia epithelium. Methods: S-P immunohistochemical method an d morphometric technique(computer image measurement method) were used to examine the PCNA and cell nuclear area in the different layers of the normal oral epith elium(NOR) in 8 cases and abnormal oral epithelium in 46 cases including mild d ysplasia(LD), moderate dysplasia(MD), severe dysplasia(SD) and well-differentia ted squamous cell carcinoma(SCC). Results: The positive PCNA sta ining cells in normal epithelium was found mainly in the basal layer. The prolif erating index(PI) in the whole layer of abnormal oral epithelium were increased (P05). Objective To study the clinical and pathologic features,histological criteria and pathologic factors contributing to diagnosis of mixed epithelial and mesenchymal tumors(mixed m?llerian tumors,MMT)of the uterus.Methods A retrospective study of 102 cases of MMT of the uterus (74 adenofibromas including 9 recurrent cases,3 atypical polypoid adenomyomas,2 carcinofibromas,10 adenosareomas and 13 carcinosarcomas)was undertaken.Clinical records,gross features and tissue slices were reviewed.The follow-up data were analysed.Results The most common symptom was vaginal bleeding.Clinical signs included pelvic mass,uterine polyps,and enlarged uterus.Benign MMT usually presented as exophytic polypoid masses extending into the uterine cavity or protruding through the external os,often broad-based,lobulated and papillary.It was hard to distinguish low-grade malignant MMT from the benign ones by gross appearance.High-grade malignant MMT had the common gross features of carcinoma and sarcoma.Histologically,MMT showed a biphasic differentiation of mesenchymal and epithelial components.MMT were classified according to whether these elements were benign or malignant.Nine cases of adenofibroma without unique features for the diagnosis of adenosarcoma recurred at postoperative intervals of 3 to 96 months.Recurrent tumors were almost always confined to the original site.Conclusions Uterine MMT tumors according to WHO diagnostic criteria are not rare.The differential diagnosis depends on a multifactorial analysis.The recurrent adenofibromas may be a kind of borderline tumors with benign appearances and malignant behavior.
7.A comparative assessment of the efficacy of carbomer gel and carboxymethyl cellulose containing artificial tears in dry eyes.

The present study aimed to compare the clinical efficacy of a 0.4% carbomer gel and 1% carboxymethyl cellulose (CMC) containing artificial tears in treatment of dry eye patients. Sixty subjects with mean age of 45.89 years who had symptoms and signs of dry eye were enrolled in this prospective, investigator-masked and stratified random sampling study. The subjects were divided into two parallel groups with 30 subjects (60 eyes) in each group. One group received carbomer gel, and the other group received 1% CMC containing artificial tears. Subjects received the drops 3 to 4 times or more per day for 3 months. At the first visit time, the precorneal residence time of these two drops was measured. The efficacy was assessed by comparing the subjective symptoms (ocular dryness, foreign body sensation, burning sensation and pain), and the objective test results of tears breakup time, Schirmer's test and corneal fluorescein staining prior to the study and after the treatment. As a result, the ocular residence time of carbomer gel was significantly longer than that of 1% CMC (P<0.001). Most of the primary subjective symptoms and objective test results were improved after treatment in both carbomer gel group and 1% CMC group. As to the improvement of each symptom and objective test result, carbomer gel was more effective than 1% CMC group (P<0.01). In conclusion, carbomer gel had longer precorneal residence time and was more effective than 1% CMC in the treatment of patients with dry eyes.

Age-related macular degeneration(AMD), the most common cause of irreversible blindness over 50 years in developed countries, is a degenerative disease occurs with age. The prevalence rate in our country is also increasing by year and the pathogenesis is still undiscovered. In this review, we focus on the associated risk factors reported in recent years, aim to further understand the pathogenesis of AMD.
9.Advances in research on silent cerebral infarct

The silent cerebral infarct is an clinic symptom that is so slight or transitory as to be easily neglected. There are only neural symptoms and signs,but irresponsible infarct focus can be seen on image. The risk factors of silent cerebral infarct may be the same as those of symptomatic cerebral infarct. Such infarct is likely to influence the course, clinic symptom and prognosis of acute cerebral infarct and may foretell symptomatic cerebral infarct and deteriorate into pseudo-global paralysis or multi-infarcted dementia. Therefore elder who suffer from hypertension or diabetes and experience vertigo, headache, language barrier but without apparent signs and symptoms should receive cerebral CT or MRI. Minor or third-degree precautionary measures can be taken as a chief therapy for cerebral infarct. Alternatively vitamines B_6,B_(12) and folic acid can be supplied to reduce the chance of hyperhomocysteinemia. Headache is the initial symptom in silent brain infarct. Magnesium is effective when dehydration does not work.
