1.Evaluation of pedicle screws applied in the affected vertebra for the treatment of thoracolumbar burst fracture

[Objective]To investigate curative effect for thoracolumbar burst fractures by the method of vertebral pedicle screw fixation applied in the with transpedicular bone graft,and consider its indications.[Method]Thirty cases of thoracolumbar burst fractures were treated by vertebral pedicle screw fixation combining transpedicular bone graft.Among them 22 were male and 8 were famale,age ranged 18-64.All of cases were single segment fractures.The average height of anterior border was 35% before the operation,the average of Cobb's angle was 26?,the percentage of midsagital diameter was 55%.According to Frankel's neurological function classification,preoperative neurological function was Grade A in 4 cases,B in 8,C in 5,D in 6,E in 7.[Result]All cases were followed up for 6 to 30 months.All patients' pain relieved distinctly,the average height anterior border was increased to 97%,the average of Cobb's angle was 3.5?,the percentage of midsagital diameter was 92% postoperatively,showing significant difference(P

4.ACD-A solution as anticoagulant during continuous renal replacement therapy in high risk bleeding patients

Objective To assess the efficacy and safety of ACD-A solution as anticoagulant during continuous renal replacement therapy (CRRT) in high risk of bleeding patients.Methods Forty high risk bleeding patients on continuous veno-venous hemofiltration (CVVH) were randomly divided into two groups:ACD-A group (22 patients,61 cases) and heparin-free group (18 patients,47 cases).Serum creatinine,function of the coagulation system,electrolyte and acid-base were monitored pre-and post-CVVH.The vital signs of the patients during treatment,dialyser clotting and the incidence of bleeding episodes were recorded.Results (1) The serum level of creatinine decreased significantly after treatment in both groups,but the rate of decrease was obviously higher in ACD-A group than that in heparin-free group[(55.4± 10.2)% vs (42.0±5.2)%,P=0.031].(2) The average duration of CVVH treatment was (17.3±3.8) h in ACD-A group and (9.7±4.5) h in heparin-free group.There was significant difference between them (P =0.019).The frequency of dialyzer clotting was much higher in heparin-free group than that in ACD-A group (88% vs 4.9%,P < 0.001).(3) There was no significant difference in the function of the coagulation system between pre-and post-CVVH in either group (P > 0.05).(4) Electrolyte,acid-base and glucose tended to be stable during the treatment in ACD -A group.(5) The vital signs were kept stable and no bleeding episodes were found in all patients of two groups.Conclusions Anticoagulation with ACD-A is safe,effective and convenient for CRRT in critically ill patients at high risk of bleeding.The occurrence of complications can be reduced by configurating appropriate replacement fluid and close laboratory monitoring.

Biotechnological pharmaceutics is an important course for pharmacy undergraduates , however there are many problems in current curriculum design and teaching methods. With the advantage of our platform National Engineering Research Center of Immunological Products, we tried to carry out the teaching reformation based on Excellent Engineer Education and Training Plan. We optimized the curriculum standards and teaching design, highlighted the combination ofBiotechnological pharmaceuticsand Engineering, strengthened the experiment teaching, tried the reformation of teaching method such as flipped classroom and PBL, strengthened the cultivation of innovative thinking and scientific research ability. Our teaching reformation is beneficial to cultivating the compound talents in the field of biotechnological pharmaceutics.
6.Anterolateral ligament of the knee:anatomy, biomechanics and functional recovery

BACKGROUND: Anterior cruciate ligament injury accompanied by unstable rotation is a hotspot in sports medicine. Further understanding of the anatomy and biomechanics of the anterolateral ligament can play a guiding significance for the recovery of knee joint rotational stability.
OBJECTIVE:To systematicaly review current literature on the anterolateral ligament of the knee and to understand the incidence, anatomy, morphology and histology of the anterolateral ligament as wel as mechanism of anterolateral ligament injury.
METHODS: The first author searched the PubMed, Medline, CNKI, Wanfang, VIP databases for articles specificaly addressing the anterolateral ligament. Data extraction related to the incidence, anatomy, morphometry, biomechanics, and histology of the anterolateral ligament and its relation to the “Segond” fracture was performed. The retrieve time ranged from 1878 to 2015. Totaly 362 literatures were retrieved, including 342 articles in English and 20 in Chinese. According to inclusive and exclusive criteria, 42 articles were included in result analysis.
RESULTS AND CONCLUSION:The occurrence rate of the anterolateral ligament ranges from 83% to 100%, and this range occurs because of smal discrepancies in the definition of the bony insertions of the anterolateral ligament. The anterolateral ligament originates anterior and distal to the femoral attachment of the lateral colateral ligament. It spans the joint in an oblique fashion and inserts between the fibular head and Gerdy tubercle of the tibia. Exact anatomic and morphometric descriptions vary in the literature, and there are discrepancies regarding the anterolateral ligament attachment to the capsule and lateral meniscus. The anterolaterial ligament is a contributor to the stability of tibial internal rotation, and histologicaly, it exhibits paralel, crimped fibers consistent with a ligamentous microstructure. The footprint of the anterolateral ligament has been shown to be at the exact location of the Segond fracture. The anterolateral ligament is a distinct ligamentous structure at the anterolateral plane of the knee, and it is likely involved in the control of excessive tibial internal rotation that can cause the Segond fracture.
7.Integration of quality education into physiology teaching reform

According to the requirements of the ‘three guidance’ talent-cultivation model pro-posed by our university, humanistic spirit was permeated from multi angels during the process of phys-iology. Students'occupation quality was cultivated; physiology knowledge and clinical practice was combined for students; early contacting with clinical knowledge was conducted and teacher-student role reversal method was applied to train students' communication skills. Teaching practice showed that the reformed teaching model not only can ensure the quality of teaching but also can improve the comprehensive quality of students.
8.Effect of different ocular spherical aberration on visual performance after phacoemulsification

AIM: To compare the visual acuity and contrast sensitivity of eyes with different corneal spherical aberration implanted with the same aspherical IOL and evaluate the effect of different ocular spherical aberration on visual performance after phacoemulsification.
METHODS:It was a prospective case series study. Forty-six eyes of thirty-nine age-related cataract patients in our department were included. The patients were divided into 3 groups according to the value of preoperative corneal spherical aberration. Eyes with corneal spherical aberration≤0. 2μm were assigned to group A, those with corneal spherical aberration >0. 2μm and ≤0. 3μm to group B, and those with corneal spherical aberration≥ 0. 3μm to group C. All patients underwent phacoemulcification and recieved AcrySof IQ aspheric IOL. Uncorrected visual acuity ( UCVA ) , best-corrected visual acuity( BCVA) , contrast sensitivity, and total ocular higher - order aberrations for a 6. 0mm pupil were recorded 3mo postoperatively. ANOVA were used to analyze the data.
RESULTS: There were no significant differences in UCVA and BCVA between the 3 groups (P=0. 287, 0. 115). Contrast sensitivity was no statistically significant difference between the 3 groups at any spatial frequency under photopic、 mesopic, and mesopic with glare conditions (P>0. 05). With a 6. 0mm pupil diameter, root mean square values for total ocular higher - order aberrations were lower in groups A and B than that in group C (P=0. 000). The difference of total ocular spherical aberration was statistically significant between the 3 groups (P=0. 000). Coma and trefoil were similar between the groups (P=0. 788,0. 590), with no statistically significant differences.CONCLUSION:Implantation of the same aspherical IOL in eyes with different corneal spherical aberration results in similar visual acuity and contrast sensitivity. Small differences of ocular spherical aberration after phacoemulsification have no effect on visual performance.
9.Outcomes of combined wavefront-guided and aspheric LASIK for myopia and myopic astigmatism

AIM: To evaluate the clinical outcomes of combined wavefront - guided and aspheric laser in situ keratomileusis ( LASIK ) to correct myopia and myopic astigmatism.
METHODS: Prospective study. Forty-five patients ( 62 eyes ) with myopia and myopic astigmatism were randomly dividied into two groups:the wavefront-guided and aspheric LASIK group and the wavefront- guided LASIK group. Safety, efficacy, predictability, ocular higher order aberrations(HOAs), and contrast sensitivity under mesopic condition were compared at 6mo postoperatively.
RESULTS: Both platforms had equal safety, efficacy and predictability. At 6mo after operation, total HOAs, spherical aberration and coma increased in both groups(P<0. 01 ). The changes of total HOAs and spherical aberration in wavefront-guided and aspheric group were significantly less than those in wavefront-guided group ( P<0.05 ) , while the change of coma between two groups was not statistically significant ( P = 0. 657 ). Contrast sensitivity in the wavefront-guided and aspheric group recovered to preoperative levels under mesopic conditions at all spatial frequencies ( P > 0. 05 ), while contrast sensitivity in the wavefront-guided group recovered to preoperative levels at all spatial frequencies(P>0. 05).
CONCLUSION: Wavefront-guided and aspheric LASIK induced less HOAs and associated with better mesopic contrast sensitivity compared with wavefront - guided LASIK.
10.Clinical decision on solitary brain metastasis in patients with non-small cell lung cancer

Solitary brain metastasis in non-small cell lung cancer (NSCLC) patients was previously considered as Stage IV. Gen-erally, only chemotherapy or radiotherapy rather than surgery was considered the treatment for these patients;hence, achieving the de-sired effect was difficult. In recent years, the treatment benefit for these patients significantly increased. Several patients were even pro-vided the chance of being cured through standardized and individualized treatment by a multiple disciplinary team (MDT). This article introduces two solitary brain metastasis patients with NSCLC who were treated by MDT in Tianjin Medical University Cancer Institute and Hospital. This article aims to explore a complete set of diagnostic and treatment practices for the benefit of more patients.
