1.In vitro biological characteristics and immune regulation defects of bone marrow-derived mesenchymal stem cells from patients with aplastic anemia

BACKGROUND: Previous research has indicated that bone marrow-derived mesenchymal stem cells had immunological regulation and hematopoiesis role. There were significant differences in immunological regulation and hemopoietic function between patients with aplastic anemia and normal cases. OBJECTIVE: To summarize the in vitro biological characteristics and its immune regulation defects of bone marrow-derived mesenchymal stem cells from patients with aplastic anemia.METHODS: A computer-based online search was conducted in PUMMED (1987-2009) and VIP database (1989-2009) with the key words of "mesenchymal stem cell, bone marrow" in both Chinese and English. There were 55 articles in total. Articles which were related to biological characteristics and its immune regulation defects of bone marrow-derived mesenchymal stem cells were included; however, duplicated articles were excluded. Finally, 32 articles were included, containing 3 reviews in English, 23 original articles in English, and 6 original articles in Chinese. RESULTS AND CONCLUSION: Bone marrow-derived mesenchymal stem cells had the capacities of high proliferation, self-renewal and multilineage differentiation; in addition they had the roles of supporting hematopoiesis, promoting implantation and hematopoietic reconstitution in vivo. Bone marrow-derived mesenchymal stem cells still had the effects of negative immune regulation and reducing the incidence of graft-versus-host disease (GVHD). Aplastic anemia correlated with hematopoietic stem cells in the pathogenesis of intrinsic defects in the proliferation or differentiation of hematopoietic microenvironment and immune system abnormalities and other related disorders. Bone marrow-derived mesenchymal stem cells of patients with aplastic anemia played an important role in the pathogenesis of this disease.
2.Clinical study on transjugular intrahepatic portosystemic shunt in the treatment of hepatorenal syndrome

Objective To explore the efficacy and safety of transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of hepatorenal syndrome (HRS).Methods From January 2009 to December 2012,the clinical data of three type 1 HRS cases and five type 2 HRS cases received TIPS operation were collected.The volume of 24 h urine,liver and renal function,and the occurrence of complications of portal hypertension were recorded and followed-up.Wilcoxon rank sum test was performed for non-normal distributed measurement data.Results TIPS of all the patients was successful and combined with devascularization in four patients.Before operation,the serum creatinine and volume of urine was 204.0 (180.8,229.8) μmol/L and 810 (600,925) mL,seven days after operation was 160.7 (120.2,184.0) μmol/L and 1 300 (975,1 500) mL,respectively,and the differences were statistically significant (Z=-2.1 and-2.2,both P<0.05).One month after operation,renal function of five patients recovered to normal.At the 7th day after operation,total bilirubin level of three patients increased and that of two patients decreased to the preoperative base line value at one month and three month.Mild or moderate hepatic encephalopathy occurred in three patients after operation and relieved after medicine treatment.At 6th month after operation,two of three type 1 HRS patients survived.Of five type 2 HRS patients,one received liver transplantation at 6th week after operation,three of the left four cases survived at the 6th month after operation.Conclusion After safety and efficacy were evaluated,TIPS may be one of the methods used for the treatment of HRS.
3.Analysis of clinical and pathological characteristics on henoch-schonlein purpura nephritis

Objective To analyze the clinical and pathological characteristics and laboratory outcomes in henoch-schonlein purpura nephritis(HSPN) patients.Methods Twenty patients with HSPN were respectively studied. The clinical characteristics and laboratory outcomes were compared between patients with different glomerulus and renal tubule pathologic classification respectively. Results Patients with grade Ⅲ had a higher proteinuria rate than those with grade Ⅱ,and the glomerulus and renal tubule pathologic markedly changed in grade Ⅲ than that in grade Ⅱ.The HSPN patients had a higher concentration of IgA,C 3 in plasma and of minimal protein in urine than that in normal groups.Conclusions There are both glomerulus and renal tubule pathologic changes in HSPN patients. The basic pathologic changes are parallel and correlated with clinical manifestation.

Nonalcoholic fatty liver disease (NAFLD) is a common liver disease with multiple factors involved,and its incidence is gradually increasing around the world.Recent studies have found that gut microbiota is involved in the development and progression of NAFLD.This article summarizes the association between gut microbiota and NAFLD from the aspects of influencing factors for gut microbiota and the roles of gut microbiota and its metabolites in the development and progression of NAFLD and points out that the intervention of gut microbiota and its metabolites may be a new target for the prevention and treatment of NAFLD.

7.Vitrectomy of rhegmatogenous retinal detachment in morning glory syndrome

We report a case of retinal detachment in morning glory syndrome(MGS). We think that identification of the retinal break, removal of the traction force by vitrectomy, the use of long-acting gas as endotamponade, all contributed to the successful treatment of the disease. The pathogenic mechanisms of the disease and the function of B-scan to the diagnosis are included.
9.Effect of compound trabeculectomy for primary angle - closure glaucoma with persistent high intraocular pressure

AIM: To evaluate the clinical effects of compound trabeculectomy for primary angle-closure glaucoma with persistent high intraocular pressure.
●METHODS:Thirty-three cases ( 34 eyes ) with primary angle - closure glaucoma were executed compound trabeculectomy. All patients were divided into two groups: group A: 18 cases ( 18 eyes ) were executed compound trabeculectomy under high intraocular pressure higher than 35mmHg after using lOP-lowering medicine for 48-72h; group B: 15 cases (16 eyes) were executed compound trabeculectomy, the intraocular pressure was normal after using lOP-lowering medicine.
●RESULTS:Thirty-four eyes were successfully completed surgery with follow-up of 6 to 18mo and there were no serious complications, including suprachoroidal hemorrhage. The visual acuity before operation in group A and group B were 0. 02 ± 0. 01, 0. 04 ± 0. 02, respectively. And the postoperative visual acuity in group A and group B increased 0.2±0. 06, 0. 3±0. 07, respectively (P<0. 01). All of them, intraocular pressure with 30 eyes were controlled in 9-23 mmHg, 3 eye was controlled by normal through adding in several kinds different lOP-lowering medicine. One eye was failure. Compared to preoperation, postoperative intraocular pressure of two groups were decreased by normal, which were 17. 9 ± 9. 1 mmHg and 15. 4±8. 4mmHg, respectively (P<0. 01). There was no significant difference between two groups for postoperative intraocular pressure.
●CONCLUSION: The primary angle-closure glaucoma with persistent high intraocular pressure should be executed compound trabeculectomy decisively, even under high intraocular pressure, which prevent further damage and loss of visual function. As long as to fully consideration the preoperation, intraoperative careful operation, careful nursing, it is safety and effective to have compound trabeculectomy under the continuous high intraocular pressure for primary angle-closure glaucoma.
