Objective To investigate the effect of high-flux hemodialysis on mineral and bone metabo-lism( MBD) in diabetic kidney disease patients with maintenance hemodialysis .Methods 60 cases with diabetic kidney disease undergoing maintenance hemodialysis were studied and they were randomly divided into two groups according to the number table method .Patients in each group were treated using either high-flux dialyzer or low-flux dialyzer for 6 months.Blood calcium(Ca), blood phosphorus(P), calcium*phosphorus(Ca*P), alkaline phosphatase(AKP), parathyroid hormone(PTH), and 25 hydroxy vitamin D3(25-OH-D3),etc were deter-mined at the start and end of the study .Results In high flux dialysis group , serum P, serum Ca*P, and PTH levels was(1.52 ±0.50)mmol/L,(3.05 ±1.19)pg/L and(368.61 ±235.32)pg/L respectively, which was sig-nificantly decreased than before treatment ((1.78 ±0.55)mmol/L,(3.94 ±1.31),(427.45 ±288.93)pg/L) (P<0.05), while serum Ca, and 25-OH-D3 levels were(2.3 ±0.16)mmol/L and(21.64 ±8.51)nmol/L respectively, which were increased significantly than before treatment ((2.12 ±0.18)mmol/L,(16.77 ±7.69) nmol/L)(P<0.05), yet in low flux dialysis group serum Ca, P, AKP, PTH, and 25-OH-D3 didn't change sig-nificantly before and after treatment ( P>0.05) .Conclusion High-flux dialyser provides a better effect on MBD for patients with diabetic kidney disease undergoing maintenance hemodialysis .

Intestinal schistosomiasis is a kind of intravascular parasitic diseases, and chronic inflammation of colon is one of the basic pathological changes of the sickness. However, the mechanism of caveolin-1 and tight junction proteins in the pathogenesis of intestinal schistosomiasis is still unclear.Aims: To study the expressions and significances of caveolin-1 and tight junction protein occludin, claudin-1 in schistosomiasis colitis in mice. Methods: Forty BALB/c male mice were randomly divided into control group and infection group.Schistosomiasis colitis model was established by placing 40 Schistosoma Japonicum cercarie on the abdomen.Mice were sacrificed after 8 weeks.HE staining was performed. The permeability of intestinal vascular endothelium was detected by Evans blue method. The leukocyte counts in peritoneal lavage fluid were measured.qPCR was used to determine the mRNA expressions of caveolin-1, occludin, claudin-1 and eNOS in colon tissue.Western blotting and immunohistochemistry were used to detect the protein expressions of caveolin-1 and occludin. Results: Large number of egg granuloma was observed in colon submucosa and accompanied by extensive inflammatory cells infiltration in infection group.Compared with control group, content of Evans blue and leukocyte counts in peritoneal lavage fluid were significantly increased (P<0.05);mRNA expressions of caveolin-1, occludin, claudin-1 were significantly decreased (P<0.01);protein expressions and positivity rates of caveolin-1 and occludin were significantly decreased in infection group (P<0.05).Conclusions: Down-regulation of expressions of caveolin-1, occludin and claudin-1 can induce leukocyte accumulation via increasing the permeability of intestinal vascular endothelial cells, thereby involving in the development of schistosomiasis colitis.

Objective To study the normal macular thickness in children provide reference for the diagnosis of macular thickness abnormity in teenagers. Methods Databases including Wanfang,Pubmed and some others were searched for studies on macular thickness in Asian children.Data of macular thickness were extracted and were merged into only one by formulae. Results 12 studies were enrolled. The mean foveal minimum and central macular thickness were 143.89±17.32μm(n=2062 eyes) and 181.60±17.93μm(n=1974 eyes),respectively. The eight areas of inner circle (1~3mm diameter) and the outer circle (3~6mm diameter) macular thickness were also calculated. The central macular area seemed thicker in pre-school children than school children. Conclusion Chidren's macular thickness have different characteristic from adults',so it is important to take age, refraction, axial length, race and sex into account before making an abnormal macular thickness diagnosis.Meanwhile,a large sample of population-based cross-sectional study in children is necessary for a more effective standard.

To explore an easily executive and conveniently generalized method with effectiveness and low costing for myopia screening at the early stage.Research was performed to testify the reasonability of using axial length/corneal radius (AL/CR) for myopia screening in children by comparing the cost between AL/CR plus vision acuity (VA) and simple VA examination.Chinese school-aged children in Yangfang school district,Beijing (n=1427,aged 6~12 years old) were randomly grouped for either pure VA examination or for VA plus AL/CR examination. Those suspicious myopic children were informed for further refractive examination. Finally, the cost from screening to definite diagnosis of refractive error was calculated.Generally, compared with VA examination, the total cost was reduced by 15.12% and by 12.34%,respectively,in the elder group using VA plus AL/CR examination.VA plus AL/CR examination is an economical and reasonable method for screening suspicious myopia in Chinese school-aged children compared with pure VA examination.

Objective To investigate the clinical significance of BiPAP mechanical ventilation in the treatment of chronic obstructive pulmonary disease(COPD)with type Ⅱ respiratory failure. Methods A total 56 patients suffering from respiratory failure due to exacerbation of COPD were randomly divided into treatment group (BiPAP plus routine therapy)and control group(routine comprehensive therapy).Blood gases, respiratory rates, heart rates, scores of the activity of accessory respiratory muscles, incubation rate, fatality rate were observed. Results In the treatment group,PO2 greatly increased,PCO2 greatly decreased,pH improved significantly, and respiratory rate, heart rate and scores of the activity of accessory respiratory muscles greatly decreased. The incubation rate and the fatality rate were significantly lower in the treatment group than those in the control group(P<0.05).Conclusion BiPAP mechanical ventilation plus routine therapy on COPD with type Ⅱ respiratory failure can improve the curative effects, lessen respiratory muscle fatigue, and decrease the need for incubation and fatality rate.
