1.Clinical characteristics and prognosis of patients with liver metastasis from gastric cancer

Objective Liver is a common site of metastasis of gastric cancer and hitherto there has been no uniform treatment for liver metastasis of gastric cancer. This study was to investigate the clinical characteristics and prognostic factors of liver metastasis of gastric cancer in order to provide some evidence for the improvement of the diagnosis and treatment of the disease. Methods We ret-rospectively analyzed the clinical characteristics and survival data of 143 patients with liver metastasis of gastric cancer treated in our hospital between March 2007 and September 2012. We investigated the independent risk factors affecting the prognosis of the disease u-sing the Cox Regression Model. Results Seventy-five ( 52. 4%) of the 143 patients survived for 12 months, 41 ( 28. 7%) for 24 months, and 18 (12.6%) for 36 months, with a median survival time of 14 months. Univariate analysis revealed statistically significant differences in the survival time among the patients with different histological grades of primary gastric cancer, time of liver metastasis, types of metastatic liver cancer, extra-liver metastasis, and treatment methods ( P<0.05) . Multivariate Cox regression analysis showed the histological grade of gastric cancer to be low-or non-differentiation ( HR=2.67, 95% CI:1.51-4.72) and the type of liver meta-static cancer to be H2 ( HR=1.89, 95% CI:1.08-3.29) or H3 ( HR=2.13, 95% CI:1.32-3.44) , which were independent risk fac-tors affecting the prognosis of liver metastasis of gastric cancer, while palliative chemotherapy ( HR=0.55, 95% CI:0.32-0.95) was an independent protective factor of its prognosis. Conclusion Patients with liver metastasis from gastric cancer usually have a poor prognosis, especially those with histologically low-grade gastric cancer or type-H2 or -H3 liver metastatic cancer. The prognosis of thepatients undergoing palliative chemotherapy may have a better prognosis than those receiving best supportive care. A comprehensive therapy should be recommended to patients with liver metastasis from gastric cancer.
3.Effect of the regulation of adiponectin on the expression of nitric oxide synthase in the retinas of diabetic rats

Background Diabetic retinopathy (DR) is a common complication of diabetic mellitus,but its pathogenesis is still unclear.Adiponectin may restrain inflammatory reaction and reduce adhesion of vascular endothelial cell to influence diabetic microangiopathy.Relation between adiponectin and nitric oxide synthase (NOS)is less reported.Objective This study was to observe the effect of adiponectin on the expression of NOS in the retinas of diabetic rats.Methods Forty 8-10 weeks Sprague-Dawley (SD) female rats were collected.The rats were randomized into the normal control group (10 rats),adiponectin group (15 rats) and diabetic model control group (15 rats) using the random number table method.Tetraoxypyrimidine was intraperitoneally injected to establish the diabetic model in the rats of the adiponectin group and diabetic model control group,and 10 μg/kg of adiponectin was then injected into the rats of the adiponectin group.Western blot was used to detect the expression of the adiponectin protein in the rat retinas,and the expression of NOS in rat retina was located by immunochemistry.The use of the animals complied with the Regulations for the Administration of Affairs Concerning Experimental Animals by State Science and Technology Commission.Results The relative amount of adiponectin (adiponectin/β-actin)in the retinas was 0.85 ± 0.21,0.79 ± 0.17 and 0.42 ± 0.08,respectively,in the normal control group,adiponectin group and diabetic model control group,with significant differences among the 3 groups (F =4.236,P =0.000).The adiponectin/β-actin ratio in the retinas of the diabetic model control group was significantly declined in comparison with the normal control group and adiponectin group (q =6.615,P =0.000 ; q =6.026,P =0.000).The NOS levels (A value) in the retinas were 0.244 ± 0.035,0.262 ± 0.032 and 0.367 ± 0.066,respectively,in the normal control group,adiponectin group and diabetic model control group,showing a significant difference among them (F =3.752,P =0.001).The A value of NOS in the diabetic model control group was significantly increased in comparison with the normal control group and adiponectin group (q =3.488,P =0.002 ; q =3.079,P =0.005).NOS expression was localized to the inner nuclear layer and retinal ganglion cell layer.Conclusions Adiponectin reduces NO content in diabetic rat retinas by regulating NOS expression.

5.Application of surgical treatment for ventral hernia in the elderly

Objective To assess the characteristics of abdominal hernia and summarize the therapeutic efficacy of mesh plug tension-free hernioplasty in elderly ventral hernia. Methods All 207 elderly patients with abdominal hernia who were admitted to our hospital during January 1999 to December 2001 and received the mesh plug tension-free hernioplasty were analyzed retrospectively. Results There were 405 patients receiving the mesh plug tension-free hernioplasty from 1999 to 2001, in whom 207 had elderly ventral hermia. Among the 207 elderly patients, 144 had inguinal oblique hernia, 38 direct hernia, 11 direct and indirect hernia, 6 sliding hernia, 6 incisional hernia, 1 femoral hernia, and 1 femoral and inguinal direct hernia. Postoperative complications were urinary retention(8 cases), hypoincisional hematoma(4), scrotal hydroceles(1), and incisional infection (1). One hundred eithty-seven patients were followed-up for 4-40 months and no recurrence was found. Conclusions The mesh plug tension-free hernioplasty is of advantages for elderly abdominal hernia, such as simpleness in performance and a good therapeutic effect. It is especially suitable for the elderly patients due to less trauma and low recurrence rate. It is also important to pay attention to the postoperative complications to the elderly.
6.A comparative assessment of the efficacy of carbomer gel and carboxymethyl cellulose containing artificial tears in dry eyes.

The present study aimed to compare the clinical efficacy of a 0.4% carbomer gel and 1% carboxymethyl cellulose (CMC) containing artificial tears in treatment of dry eye patients. Sixty subjects with mean age of 45.89 years who had symptoms and signs of dry eye were enrolled in this prospective, investigator-masked and stratified random sampling study. The subjects were divided into two parallel groups with 30 subjects (60 eyes) in each group. One group received carbomer gel, and the other group received 1% CMC containing artificial tears. Subjects received the drops 3 to 4 times or more per day for 3 months. At the first visit time, the precorneal residence time of these two drops was measured. The efficacy was assessed by comparing the subjective symptoms (ocular dryness, foreign body sensation, burning sensation and pain), and the objective test results of tears breakup time, Schirmer's test and corneal fluorescein staining prior to the study and after the treatment. As a result, the ocular residence time of carbomer gel was significantly longer than that of 1% CMC (P<0.001). Most of the primary subjective symptoms and objective test results were improved after treatment in both carbomer gel group and 1% CMC group. As to the improvement of each symptom and objective test result, carbomer gel was more effective than 1% CMC group (P<0.01). In conclusion, carbomer gel had longer precorneal residence time and was more effective than 1% CMC in the treatment of patients with dry eyes.

8.Duration for silicon gastric tube to indwell in stomach of elderly patients

Objective To investigate the duration for silicon gastric tube to indwell in stomach of aged patients.Methods One hundred elderly patients needing long-term nutrition support were equally randomized into two groups: control group and observation group.The former had the gastric tubes indwelled in the stomach for 4 weeks and the latter for 3 weeks.The two groups were compared about the used tubes in terms of the color of tube at the length of the first 10cm,the indentation by reflection for 10 seconds at the tube of first 10 cm,the hardness of the tube at the length of the first 10 cm and the springback angle of tube at the first 10cm after reflection.Results The tubes in the observation group were significantly better than those in the control group in terms of color,indentation,hardness and springback angle(all P<0?01).Conclusion The duration of indwelling silicon gastric tubes in stomach can be three weeks for old patients needing long-term nutrition support via indwelling silicon tubes in their stomach.
9.Sacral Primary Tumors:X-ray and CT Manifestations

Objective To analyse the X-ray and CT manifestations of sacral primary tumor.Methods 10 cases with sacral primary tumors verified by pathology,including sacral chordoma(n=5),giant cell tumor(n=2),chondrosarcoma(n=2),neurilemmomas(n=1),were analysed retrospectively.Results (1)The manifestations of CT and X-ray of sacral chordoma presented as a midline destructive lesion,involving prominently at sacrum 3~5,with flocculent calcification and irregular bone crests.(2)Giant cell tumor involved predominately sacrum 1~3 with expansile destructive area and commonly in young adults.(3)Chondrosarcoma and neurilemmomas had difference characteristic on X-ray film and CT.One case with chondrosarcoma had bone destruction in sacrum 1~3 and giant mass grow into pelvis.Conclusion CT combined with X-ray film is helpful in diagnosis and differential diagnosis of sacral primary tumors.
10.Optimization of Raman spectra acquisition conditions and its application in a comparison study on mesenchymal stem cells and embryonic stem cells

BACKGROUND: Raman spectrum, compared with conventional detection technologies, is a rapid, non-invasive and label-free optical method. Its application has become an issue of concern in biomedical research. However, further studies are warranted to optimize the acquisition condition of Raman spectra from different stem cells. OBJECTIVE: To explore the effect of the wavelength of laser and the groove frequency of gratings to obtain the optimized parameter combination for Raman spectrum collection in human stem cells. METHODS: Using human mesenchymal stem cells as samples, the effects of different laser wavelengths (532, 38,785 nm) and different grating groove frequency (600, 1200, 1800 gr/mm) on Raman spectra were compared respectively. Then the different combinations of the wavelength and groove frequency were used and compared in terms of the spectra resolution and acquisition time, and the best acquisition condition was selected and applied in a comparison study on the Raman spectra from human mesenchymal stem cells and human embryonic stem cells. RESULTS AND CONCLUSION: The wavelengths of lasers and groove frequencies of gratings showed compound impacts on both the spikes at different wavenumbers and the ratio between spikes; the combination of 785 nm and 1200 gr/mm was confirmed to be the best spectrum features for human mesenchymal stem cells. The comparison of Raman spectra from human mesenchymal stem cells and human embryonic stem cells implies that the embryonic stem cells contain higher nucleic acids than the mesenchymal stem cells, while the mesenchymal stem cells appear to contain more proteins and lipids.
