3.Clinical observation of diabetic retinopathy after transplantation of autologous stem cell in diabetic patients

To observe the effect of autologous stem cell transplantation in diabetic retinopathy.
●METHODS:Totally 58 cases (116 eyes) who underwent autologous stem cell transplantation were confirmed as no diabetic retinopathy (18 eyes), mild non-proliferative diabetic retinopathy (NPDR) (41 eyes), mid-level NPDR (51 eyes); severe NPDR (6 eyes) by ophthalmoscope directly or indirectly and fluorescence fundus angiography (FFA ). Follow - up was 6 - 12mo, the changes of retinopathy were observed.
●RESULTS: The total effective rate of vision and retinopathy was 84. 4%, 76. 7%. The results of severe NPDR was statistically worse than the other groups ( P <0. 05).
●CONCLUSlON: The stable blood glucose level and improved pancreatic function after autologous stem cell transplantation might be helpful in diabetic retinopathy, the long effects need to be researched further.
4.Survey on the non-psychiatric physician's ability of diagnosis and treatment of depression/anxiety disorders in general hospital

Objective To investigate the status of the non-psychiatrist' s diagnostic and therapeutic ability of depression/anxiety disorders in 57 General Hospitals.Methods 1 152 non psychiatric clinicians in 57 general hospitals were surveyed.A custom-made questionnaire included the training of mental health-related knowledge which the general hospital physicians received and typical anxiety/depression case analysis.Results Among 1 521 non-psychiatric clinicians,596 (51.74%) clinicians participated the training of psychiatry,562 (48.78%) participated the training of medical psychology and 230(20.97%) clinicians participated the training of Healthy Psychology.In professional setting,59 (5.12%) clinicians participated the training of psychotherapy,255 (22.14%) clinicians had attended related academic symposiums.80(6.94%) clinicians believed that they understand the clinical display of anxiety/depression disorders,52 (4.51%) clinicians expressed the understanding of diagnostic criteria of anxiety/depression disorders and its treatments,while 44(3.82%) clinicians only possessed the knowledge of anxiety/depression disorder treatment.In the typical case analysis,it revealed that 794 (68.89%) clinicians made accurate diagnosis,458(57.68%)clinicians made a choice of medical treatment,764(96.22%) clinicians chose psychotherapy,29 (3.65%) clinicians applied physical therapy,while 438 (55.16%) clinicians combined drug therapy with one or more other therapeutic methods.Binary logistic regression analysis showed that the age(P=-0.093,Exp(B)=0.911)and work experience(P=-0.002,Exp(B) =1.080)significantly contribute to the diagnostic accuracy of non-psychiatrists.Conclusion The non-therapeutic psychiatric clinicians in general hospitals have certain basic knowledge of depression/anxiety disorders with lower level of diagnosis and treatment diagnosis and treatment.And there is bigger difference among different hospitals.
5.Effect of total flavonoids of acanthopanax senticosus on erythrocyte membrane fluidity in mice

Objective To observe contents and effect of total flavonoids of acanthopanax senticosus extract on erythrocyte membrane fluidity.Methods The contents of total flavonoids of acanthopanax sentieosus were deter- mined by spectrophotometry,and erythrocyte membrane fluidity and microviscosity were measured by fluorescence polarization technique.Results The total flavonoids contents of acanthopanax senticosus extract and injection were alike.Both of them increased erythrocyte membrane fluidity and lowered microviscosity,and the effect of acan- thopanax senticosus injection was more significant.Conclusion Total flavonoids of acanthopanax senticosus can in- crease erythrocyte membrane fluidity,so that blood viscosity is lowered and hemorheology factors are improved.
6.Effect of whole-body hypothermia on cardiac hemodynamics neonatal piglet after hypoxia-ischemia

Objective To determine the effect of mild hypothermia on neonatal piglet cardiac hemodynamic function after hypoxia-ischemia (HI).Method Twenty five 7-day-old piglets were used for hypoxic ischemic brain damage (HIBD) model by the method of temporary occlusion of the bilateral carotid arteries and followed by mechanical ventilation with low concentration of oxygen (FiO_2=6%) for 30 minutes.The piglets were randomly divided into three groups:group A (normothermia with body temperature to 39℃,n=9),group B (body temperature to 36℃for 72 hours,n=8),and group C (body temperature to 34℃for 72 hours,n=8).Mild hypothermia was initiated at 4 hours after HI,the systolic and diastole function were evaluated by Doppler echocardiography at pre-HI,post-Hi 4 hours and post-HI 72 hours.Results There were no significant differences in left ventrieular ejection time/left ventrieular ejection time (LPEP/LVEF),right ventricular ejection acceleration time/right ventricular ejection time (RACT/RVET) and CO at post-HI with hypothermia 72 hours in three groups,but the heart rate decreased in B and group C group.Compared with nonnothermia,mild hypothermia treatment showed no significant differences in MAP,LPEP/LVET,RACT/RVET,CO,SV at post-HI with hypothermia 72 hours.Conclusions Body temperature decreased by 3~5℃for 72 hours will not aggravate hemodynamic abnormity.
7.Biological security evaluation of cross-linked hyaluronic acid for ophthalmic implant material

Background Hyaluronic acid is a mucopolysaccharide existing in extracellular matrix and having good biocompatibility.Using chemical crosslinking method can improve the physical properties of the material,so cross-linked hyaluronic has potential clinical application value.Objective The present study was to evaluate the histocompatibility and biological security of cross-linked hyaluronic acid as ophthalmic implant material.Methods Cross-linked hyaluronic acid implant material was prepared according to the criteria of Biological Evaluation of Medical Devices (GB/T16886.5-2003).Eighteen 8-week-old male New Zealand white rabbits were randomized into the experimental group and the control group.Cross-linked hyaluronic acid material with 5.0 mm diameter was implanted into corneal stroma interlaminationally in the experimental group,and only corneal stromal interlaminational pocket was made without any implanting material in the control group.Biological response of cornea was assessed in vivo from 1 week through 3 months after operation by slit lamp microscope.The corneas were obtained 2 weeks,1 month and 3 months respectively for histopathological examination.Mouse embryonic fibroblasts were cultured in cross-linked hyaluronic acid film plate,medical silicone material culture plate and regular culture plate respectively for 24 hours,and the cell growth state and morphology were observed under the inverted microscope and scan electron microscope.MTT assay was used to test the relative growth rate of the cultured cells 48 hours after cultured using extracted liquid of hyaluronic acid implant material.Results Cross-linked hyaluronic acid implant material showed a well healing to the corneas of rabbits during the observation duration,without obvious inflammatory response and neovascularization.The arrangement of stromal fibers was uniform in order,and no infiltration of inflammatory cells was seen under the light microscope.The cells grew well after cultured with cross-linked hyaluronic acid film and regular medium for 24 hours,but in the silicone culture group,fewer of adherent cells and more floating cells were found.The relative growth rate of the cells was 87.50% 48 hours after cultured with extracted liquid of hyaluronic acid implant material.Conclusions The cross-linked hyaluronic acid material has good histocompatibility and biological security in rabbit cornea tissue.
8.Expression and clinical significance of angiopoietin-1 in multiple myeloma.

OBJECTIVETo study the expression of angiopoietin-1 (Ang-1) and its receptor Tie-2 in multiple myeloma (MM) patients and RPMI8226 cells, and analyze the significance of Ang-1 expression and its relevance to the tumorigenes and development of MM.
METHODSRT-PCR and Western blot were used to detect the expression of Ang-1 and Tie-2 in bone marrow (BM) samples from 112 MM patients and 24 control subjects, and in RPMI8226 cells. The expression levels of Ang-1 in different groups and disease stages were analyzed.
RESULTSThe positive rate and expression level of Ang-1 were significantly higher in MM group than in control group (P < 0.05). The positive rates of Ang-1 were not significantly different between newly diagnosed and relapsed/refractory MM groups, but its expression level was significantly higher in the latter group than in the former group (P < 0.05). Tie-2 was detected only in 12 MM patients and did not in control group and RPMI8226 cells. Microvessel density in BM samples were significantly higher in MM group than in control group (25.21 ± 0.80 vs 5.23 ± 0.20, P < 0.01), and were higher in Ang-1-positive MM group than in Ang-1-negative MM group (32.98 ± 1.70 vs 16.55 ± 1.30, P < 0.05). The positive rates of Ang-1 protein were not significantly different between stage II and stage III MM (52.1% vs 60.9%, P > 0.05), but the expression level of Ang-1 protein was higher in stage III than that in stage II MM (0.40 ± 0.07 vs 0.22 ± 0.04, P < 0.05). In the newly diagnosed MM patients, the positive rate of Ang-1 protein in PD patients was significantly higher than in PR and MR patients (70.0% vs 19.1%, P < 0.01).
CONCLUSIONHigh expression of Ang-1 is found in MM patients and RPMI8226 cells, and its expression is associated with the disease stage, prognosis and targeted therapy of MM.

9.Comparison of Galectin-3, NT-proBNP and Echocardiography Parameters for Assessing Cardiac Function in Heart Failure Patients

Objective: To compare the effect of galectin-3 (Gal-3), NT-proBNP and echocardiography paramerters on assessing cardiac function in patients with chronic heart failure (HF). Methods: A total of 144 patients treated in our hospital from 2016-03 to 2016-11 were enrolled. According to the NYHA classification, the patients were divided into 2 groups: HF group and Normal cardiac function group. n=72 in each group. Basic clinical information was collected, blood levels of Gal-3 and NT-proBNP were examined, echocardiography was conducted to measure left ventricular ejection fraction (LVEF) and left ventricular end diastolic diameter (LVEDD). Correlations between Gal-3, NT-proBNP and echocardiography parameters were studied, the abilities of Gal-3, NT-proBNP and echocardiography for estimating HF were compared. Results: Compared with Normal cardiac function group, HF group had increased blood levels of NT-proBNP [3499.5 (1431.3-9088.0) ng/L] vs [384.1 (122.1-1540.5) ng/L] and Gal-3 [3.0 (1.71-5.8) pg/ml] vs [1.9 (1.4-2.6) pg/ml], decreased LVEF [49.5% (42%-58%)] vs [62.5% (59%-67%)], enlarged LVEDD [52.0 (46.3-57.8) mm] vs [46.0 (42.0-49.0) mm] and elevated serum creatinine [113.6 (90.5-152.7) umol/L] vs 82.4 (69.1-97.4) umol/L], all P<0.05. Correlation analysis showed that NT-proBNP and Galectin-3 were positively related to LVEF and LVEDD; Gal-3 and NT-ProBNP had the strongest correlation (r=0.57, P<0.01). The AUC of ROC for Gal-3 was 0.674 (0.584-0.763), for NT-proBNP was 0.837 (0.771-0.902) and for LVEF was 0.806, (0.735-0.878) which implied that NT-proBNP was the most powerful parameter for estimating HF. Conclusion: Gal-3 had the ability to estimate HF and could be used as a biomarker, while its ability was lower than NT-proBNP in clinical practice.
10.Predictive factors of hepatitis B virus DNA negative conversion among patients with chronic hepatitis B during lamivudine therapy.

OBJECTIVETo study the occurrence and predictive factors of hepatitis B virus DAN negative conversion among patients with chronic hepatitis B and liver cirrhosis during lamivudine therapy.
METHODSClinical data of 202 patients with chronic hepatitis B and 55 patients with post-hepatitis B cirrhosis treated with lamivudine therapy were analyzed and PCR as well as mispairing PCR-restricted fragment length polymorphism (RFLP) assay were applied. The data including ALT, AST, total bilirubin (TBIL), albumin, etc. were analyzed by SPSS software.
RESULTSThe HBV DNA negative conversion occurred in 87.62%, 95.04%, 86.63%, 82.65%, 75.43%, and 77.27% of the chronic hepatitis B patients and in 94.54%, 98.18%, 81.81%, 74.35%, 73.91%, and 63.63% of liver cirrhosis patients, respectively, after 3, 6, 12, 18, 24, 30 months. The results of log-rank test and Cox proportional hazard model analysis indicated that combination of lamivudine and interferon, high ALT level,low HBV DNA level, and low albumin level at baseline were significantly related to HBV DNA negative conversion.
CONCLUSIONCombination treatment with lamivudine and interferon, high ALT level, low HBV DNA level, and low albumin level at baseline were significant predicting predicting factors for HBV DNA negative conversion in chronic hepatitis B patients.
