4.Analysis of risk factors and treatment of postoperative delirium in patients with benign prostatic hyperplasia

Objective To investigate the risk factors and treatment of postoperative delirium in patients with benign prostatic hyperplasia(BPH). Methods For 256 postoperrative BPH patients in our department,the clinical parameters including age,hypoxemia,preoperative infection,postoperative pain,and introvert were analyzed,and the treatment were summarized. Results The incidence of postoperative delirium was higher in patients aged≥75 years than<75 years[17.4%(19/109)vs.5.4%(8/147),X2=8.307,P=0.0043,higher in patients with preoperative infection (pulmonary or urinary)versus without the infection[15.0%(17/113)vs.7.0%(10/143),X2=4.337,P=0.037],higher in patients with hypoxia versus without hipoxia[16.5%(16/97).vs.6.9%(111159),X2=4.885,P=0.027]and higher in patients with post-operative pain versus without the pain(16.9%(10/59)vs.1.2%(1/85),X2=10.146,P=0.0014].Hematocrit and hemoglobin in patients with postoperative delirium were lower after operation versus pre-operation[(0.29±0.04)vs.(0.42±0.04),t=12.2314,P<0.001;(102.39±6.58)g/L vs.(114.58±8.36)g/L,t=5.9537,P<0.001].All delirium patients were recovered after sedation treatment,improving the quality of sleep,controlling the infection and correcting electrolyte turbulence. Conclusions Advanced age,hypoxemia,preoperative infection,postoperative pain are important risk factors for delirium.
5.Study of quality of life in patients with stroke at the acute stage and the suitability of SF-36

Objective To assess the quality of life(QOL)in patients with stroke at the acute stage,and to determine whether all the dimensions of the Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)are applicable to the stroke patients.Methods Fifty stroke patients at the acute stage were studied.The SF-36 was employed to assess the quality of life of the patients,and a self-designed questionnaire was used to record the general information of them.The data obtained from the patients was compared with the Chinese norm reported in literature. We also calculated the percentage of subjects who achieved the lowest possible score(score of 0,floor effect)and the percentage of subjects who achieved the highest possible score(score of 100,ceiling effect).Results The QOL of the acute stroke patients,as reflected by scores with all the dimensions of the SF-36 scale,was significantly reduced when compared with the norm data(P
6.Clinical analysis of stereotactic radiotherapy combined with Gemzar for treatment of advanced pancreatic cancer

Objective: To observe the efficacy of stereotactic radiotherapy combined with gemzar for treatment of unresectable pancreatic cancer.Methods: Gemzar at dose of 1000mg/m 2,28 days per cycle,drug was given at 1,8,15 days,for two cycles.After 24 hours all patients were treated with stereotactic radiotherapy.Per fraction was 5~7G Y,three times per week.The total dose was 40~48G Y.Results: 13 patients were followed up for one year,symptoms were reduced and median survival was 12 months.Conclusion: Stereotactic radiotherapy combined with gemzar has important valves for unresectabal pancreatic cancer.
7.Variance of Perinatal Asphyxia in Latest 10 Years and Analysis of Mortality and Risk Factors of Perinatal Asphyxia

Objective To analyze the incidence rate and mortality of perinatal asphyxia and effects of new resuscitation technique on asphyxia as well as the risk factors of asphyxia in latest 10 years.Methods A retrospective evaluation was done for all the newborns who were born in the provincial women and children′s health care hospital from 1995 to 2004.The morbidity,mortality and fatality rate were calculated for each observed year and different seasons.The influence of gender,body weight,gestational age as well as polyembryony and mode of delivery on the asphyxia was analyzed.Results The morbidity of mild birth asphyxia was decreased dramatically and maintained at about 1.5% after using new resuscitation technique,however,there were no obvious effects on the sever asphyxia.In the same time,no big influence on fatality rate of birth asphyxia was observed.The incidence rate was highest in April,but the mortality and fatality of asphyxia was highest in July.The incidence of asphyxia was also related with gender,polyembryony,birth weight,prematurity babies and aids to delivery from voginal.Conclusions The incidence of perinatal asphyxia is related with the gender,polyembryony,birth weight and gestation age as well as seasons.New resuscitation technique can reduce the morbidity of mild birth asphyxia,and no effect on the severe asphyxia as well as fatality rate.
8.Establishment of real time PCR for detecting plasma cell free DNA of rats and its significance.

OBJECTIVEOvertraining is a serious problem in sports, assessed by comprehensive multi-index evaluation, but so far there is still no sensitive, specific monitoring indicator or simple evaluation method to evaluate it. This research established a method for detecting plasma cell free DNA (cfDNA) of rats by real time PCR and discuss edits significance: a new molecular marker of overtraining?
METHODSTwelve male SD rats were randomly divided into control group and overtraining group. The overtraining group rats were undertaken overtraining on a motor-driven treadmill for 5 weeks, while the control group rats kept quiescent. All the rats were drawn blood at pre-and after-5 weeks to detect plasma levels of cfDNA, testosterone (T) and corticosterone (Cort) as well as peroxidation/antioxidation parameters (T-AOC, MDA, SOD, GSH-Px) and creatin kinase (CK).
RESULTS(1) Plasma cfDNA of rat was detected specifically by our real time PCR. (2) Compared with control group rats, the plasma cfDNA of overtraining rats increased obviously (about 5.43 fold). (3) Plasma cfDNA was related to plasma T, Cort, T/C ratio and MDA (correlation coefficent were -0.729, 0.854, -0.655 and 0.720, respectively) rather than plasma T-AOC, GSH-Px, SOD and CK.
CONCLUSION(1) A real time PCR method was established successfully to determine plasma cfDNA of rat. (2) A remarkable raises of plasma levels of cfDNA were found in overtraining rats which were associated with T, Cort and T/C, suggested that plasma cfDNA might be a new molecular marker of overtraining. (3) The increase of plasma cfDNA of overtraining rat might correlate with enhanced oxidative stress induced by overtraining instead of muscle damage.

10.Determination of Ginsenoside Rg1 and Ginsenoside Re in Jiangshen Capsules by HPLC Simultaneously

OBJECTIVE: To determine ginsenoside Rg1 and ginsenoside Re in Jiangshen Capsules by HPLC simultaneously. METHODS: The separation was performed on Kromasil-C18 column, the mobile phase consisted of acetonitrile-0. 05% H3PO4 solution ( 21∶ 79) with flow rate of 1. 0mL? min-1 and detection wavelength of 203nm. RESULTS: The linear ranges of ginsenoside Rg1 and ginsenoside Re were 0. 502~ 4. 016? g( r=0. 999 7) and 1. 090~ 8. 720? g( r=0. 999 8) , respectively, with average recovery at 98. 8% ( RSD=1. 24% ) and 99. 4% ( RSD=1. 68% ) , respectively. CONCLUSION: This method is simple, rapid, accurate and reliable, and suitable for the quality control of Jiangshen Capsules.
