Acta Pharmaceutica Sinica 2014;49(6):832-836
Hypoxia occurs in chronic and acute vascular diseases and tumor formation. The ability of tumor cells to maintain a balance between an adaptation to hypoxia and cell death is regulated by a family of transcription factors called hypoxia-inducible factor 1 (HIF-1). Tumor hypoxia mediated by HIF-1 would facilitate the likelihood of resistance to chemotherapy and radiotherapy, proliferation, metastasis and the invasive potential; all of which culminate in a decrease in patient survival. And HIF-1 alpha subunit decides the activity of HIF-1, which is regulated by oxygen. So understanding the role of HIF in signal pathway, drug resistance mechanism and its feature is crucial for developing novel anticancer therapies. In recent years, more attentions have focused on HIF-1 alpha inhibitors. It is expected that development of more potent and selective HIF inhibitors will provide an effective treatment of cancer and other HIF-related diseases. So we will focus on the biological characteristics and mechanism of HIF-1 to review currently studied HIF-1 inhibitors.

8. Comparison of crude and prepared Polygonum multiflorum-induced idiosyncratic hepatotoxicity based on lipopolysaccharide model
Chinese Traditional and Herbal Drugs 2015;46(10):1481-1486
Objective: To investigate the effects of crude and prepared Polygonum multiflorum (PM) to liver injury based on idiosyncratic hepatotoxicity model co-treated with lipopolysaccharide (LPS) at a non-hepatotoxic dose, aiming to compare the difference between them. Methods: The Sprague-Dawley (SD) rats were used by ig administration, respectively with 50% alcohol extracts from crude or prepared PM alone or co-treated noninjurious dose of bacterial lipopolysaccharide (LPS, 2.8 mg/kg) via tail vein injection. Blood was collected from the inferior vena cava into a sodium citrate-anticoagulant tube, and the liver was removed. The plasma samples separated from the collected blood were used for analysis on ALT and AST activities and the isolated livers were used for histopathological assessment. Results: Both crude and prepared PM were nontoxic on normal SD rats under the dose of 17.28 g/kg, when co-treated with LPS, the liver injury was found at the dose of 1.08 g/kg of crude PM, however, 8.64 g/kg of prepared PM. Conclusion: When processed, the maximum safe therapeutic dosage of PM would be enlarged from 1.08 g/kg of crude PM that is close to clinical dosage to 8.64 g/kg of prepared PM, which is four times larger than the crude one in LPS model. It shows that toxicity of PM could be reduced by processing.

Objective:To explore the efficacy and safety of 192Ir three-dimensional brachytherapy combined with intensity-modulated radiotherapy in patients with unresectable advanced obstructive extrahepatic cholangiocarcinoma. Methods:A retrospective analysis of the efficacy and safety of 18 patients with unresectable advanced malignant obstructive extrahepatic cholangiocarcinoma admitted to the Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei province from January 2014 to January 2018.All patients were pathologically confirmed and received biliary stent implantation, followed by sequential intensity-modulated radiotherapy and 192Ir three-dimensional brachytherapy. The clinical data of all patients were collected, including gender, age, clinical stage, adverse reactions, brachytherapy dose-volume parameters, changes in bilirubin levels before and after treatment, evaluation of efficacy, local control rate and survival rate. Results:All patients successfully completed the treatment.Re-examination was conducted one month after the combined treatment, of the 18 patients, 16 had partial remission (PR) and 2 were stable(SD). Among them, 14 patients had tumor shrinkage after brachytherapy compared with intensity-modulated radiotherapy, and 4 patients showed no significant change. The 6-month local control rate (LC) was 94.4% (17/18). The 1-year survival rate was 55.6% (10/18), and the 2-year survival rate was 38.9% (7/18). Statistical analysis showed that after combined treatment, total bilirubin and direct bilirubin were significantly reduced, and jaundice symptoms were significantly relieved. The adverse reactions of the patient were nausea, vomiting, fever, biliary tract infection, etc. There were no adverse reactions of grade 3 or above.Conclusions:192Ir three-dimensional brachytherapy combined with intensity-modulated radiotherapy in patients with inoperable advanced extrahepatic cholangiocarcinoma has a good local control rate and tolerable adverse reactions, but the impact on long-term survival requires a large sample of controlled studies.

China Pharmacy 2017;28(16):2208-2211
OBJECTIVE:To study the effects of nimesulide combined with oxaliplatin on transplanted tumor growth and im-mune function of rats with esophageal cancer. METHODS:Rats were randomly divided into model group(intragastrically given So-dium carboxymethyl cellulose solution+intravenously given 5% Glucose injection in tail),nimesulide group(intragastrically given 20 mg/kg),oxaliplatin group(intravenously given 13.6 mg/kg in tail)and combination group,10 in each group. Esophageal can-cer Eca109 cells were subcutaneously injected to develop transplanted tumor model. After modeling, rats in each group received rel-evant medicines by corresponding ways, once a day for ig,once every 4 d for iv in tail. Rats were sacrificed after 8 weeks, tumor volume and quality of rats were measured, tumor inhibition rate was calculated, and contents of tumor markers(CEA,CYFRA21-1,SCCAg),percentages of immune cells(CD3+,CD4+,CD8+T cells and NK cell)in peripheral blood were detected. RESULTS:Compared with model group, tumor volume and quality in other 3 groups were decreased (P<0.05);contents of tumor markers were decreased (P<0.05). Percentages of CD3+,CD4+ T cells and NK cell in nimesulide group were increased, percentages of CD8+T cell was decreased(P<0.05). Percentages of CD3+,CD4+T cells and NK cell in oxaliplatin group and combination group were decreased, percentages of CD8+ T cell was increased(P<0.05). Compared with nimesulide group and oxaliplatin group,tu-mor inhibition rate in combination group was increased(P<0.05);contents of tumor markers were decreased(P<0.05);percent-ages of immune cells were lower than nimesulide group and higher than oxaliplatin group(P<0.05). CONCLUSIONS:Nimesulide can enhance the oxaliplatin's antitumor effect on esophageal cancer, and decrease its inhibition degree on immune functions.
