2. Role of extracellular signal-regulated kinase 1/2 in pathogenesis of stree-induced gastric ulcer

Objective: To investigate the role of extracellular signal-regulated kinase 1/2 (ERK1/2) signal pathway in the pathogenesis of stress-induced gastric ulcer. Methods: Animal model of stress-induced gastric ulcer was established in rats with water-immersion restraint (WIR) stress. The mucosal activation of ERK1/2 was observed before and 5, 15 and 30 min, and 1, 2 and 3.5 h after WIR stress. Some animals were also treated with an intravenous injection of PD98059 (1 mg/kg), a specific ERK1/2 inhibitor, 1 h prior to WIR stress. Expression of total ERK1/2 and caspase-3 were detected by Western blot analysis; ERK1/2 activity was measured by kinase activity assay using myelin basic protein as a specific substrate. DNA-binding activities of the transcription factors activator protein-1 (AP-1) and nuclear factor-κB (NF-κB) were determined by electrophoretic mobility shift assays (EMSA). Mucosal TNF-α and IL-1β mRNA expression was analyzed by Northern blot analysis. The degrees of the gastric mucosal lesions were expressed as ulcer index (UI) and pathological evaluation. Apoptosis in the gastric mucosa was examined by an in situ TdT-mediated dUTP nick-end labeling (TUNEL) method. Results: Activated ERK1/2 was very weakly expressed in the gastric mucosa of normal rats. ERK1/2 was rapidly activated in the gastric mucosa of rats 15 min after WIR stress and the activity reached the maximal after 3.5 h. Pretreatment with PD98059 significantly inhibited ERK1/2 activation, decreased AP-1 and NF-κB activities and TNF-α and IL-β mRNA expression, and obviously relieved gastric mucosal lesions, accompanied by caspase-3 activation and increased apoptosis. Conclusion: The present results indicate that ERK1/2 activation plays an important role in the development of stress-induced gastric ulcer.

6.The value of combined application of CT perfusion imaging and CT angiography in the diagnosis of hyperacute ischemic cerebrovascular disease

Objective To evaluate the value of combined application of computed tomography perfusion imaging(CTPI)and CT angiography(CTA) in the diagnosis of hyperacute ischemic cerebrovascular disease.Methods 46 patients with hyperacute ischemic cerebrovascular disease(onset within 6 hours) were examined by conventional CT , CTPI and CTA.Results Conventional CT scan showed low density signal in 5 of 46 patients. CTPI revealed normal perfusion in 16 patients and hypoperfusion in 30 patients. 27 patients existed stenosis or occlusion of middle cerebral arteries(MCA) and anterior cerebral arteries(ACA) by CTA. CTA also showed obvious slight MCA in 2 cases and normal image in 17 cases. 25 cases with large or moderate cerebral infarction volume were confirmed by CTPI as having ischemic hypoperfusion lesions and stenosis or occlusion of arteries by CTA. In 9 cases with small cerebral infarction volume, CTPI revealed perfusion defect in 5 patients and CTA displayed stenosed arteries in 2 patients. CTPI of 12 patients with TIA were normal. But CTA showed remarkable slight MCA and more blood vessel network in its distal segment in 2 of 12 patients.Conclusion combined application of CTPI and CTA can detect hyperacute ischemic lesion at far more early stage. They are helpful in differentiation TIA from infarcts with different volumes.
7.Application of CT perfusion imaging in hyperacute ischemic cerebrovascular disease

Objective To investigate the application of computed tomography perfusion imaging(CTPI)in hyperacute ischemic cerebrovascular disease. Methods Conventional computed tomography (CT) and CTPI were performed within 6 hours of symptom onset in 46 patients with cerebrovascular disease. Results In 46 cases,5 cases diagnosed as cerebral infarction obtained regular CT positive results.In 41 having regular CT negative results,29 had cerebral infarction,and the other 12 had transient ischemic attack(TIA).In 16 cases with CTPI negtive,12 were diagnosed as having TIA and the others having small infarction volume.30 cases were confirmed by CTPI as having ischemic lesions.In 30 cases,5 had ischemic lesions larger in CTPI than in regular CT and 25 had regular CT negtive results.The regional cerebral blood flow(rCBF),time to peak(TP),mean transit time(MTT)in 30 cases with CTPI positive were found obviously changed.The side of ischemic lesion as compared with the opposite side,and the core of ischemic lesion as compared with peripheral zone were found changed significantly( P
8.Clinical application of antidepressant combined with α-blocker in the treatment of chronic prostatitis

Objective To investigate the clinical effect of antidepressants combined with α-blockers in the treatment of chronic prostatitis.Methods A retrospective analysis of 90 cases of chronic prostatitis treated in our hospital from January 2016 to December 2016 was divided into two groups according to the random number table.The control group was treated with α-blocker Combined with vitamin C treatment, the study group using antidepressants combined with α-blockers treatment.The NIH-CPSI score and SDS score were recorded before and after treatment, and the therapeutic effects were compared between the two groups.Results There were no significant differences in NIH-CPSI scores and SDS scores between the two groups.After treatment, the scores of NIH-CPSI and SDS were significantly better than those before treatment, and the study group was superior to the control(91.1%) was higher than that of the control group(71.1%), the total effective rate of depression(93.3%) was higher than that of the control group(73.3%), and the total effective rate was 91.1%, The difference was statistically significant(P<0.05),comparable.Conclusion Antidepressants combined with α-blockers are effective in the treatment of chronic prostatitis, which is beneficial to the alleviation of clinical symptoms and the improvement of their symptoms.Therefore, it has a good clinical value and worthy of further popularization.
9.Ultrasonic Features of Wilms′ Tumor in Children

Objective To study the ultrasonographic characteristics of Wilms′tumor (WT), aims at elevating the diagnostic level.Methods We retrospectively studied sonographic features in 30 cases of WT proved by operation and pathology.Results The tumors with heterogeneous pattern of solid and cystic tissue were shown in 22 cases.Seven cases demonstrate predominantly echogenic tumor. A huge cystic mass with numerous septas was shown in 1 case. The tumor extended into renal pelvis in 4, into renal vine in 1.The tumor metastasized to lymphnod in 1, to bilateral lungs in 1. Among 30 cases, the tumor was huge and grew beyond the renal profile in 26 cases, in 3 cases the tumors took up the renal sinus and made its structure turbulence, the renal shape was approximately normal.One case was extrarenal WT,the tumor located in retroperitonemu.Conclusions Sonographically, typical WT is a large heterogeneous mass within or without cystic areas, it also can be a pure cyst. It is easy to diagnose. When the tumor is limited to renal outline, it is easy to confusion with renal cancer. In addition, there are bilateral WT and extrarenal WT. The sonographic characteristics is similar among WT, clear cell sarcoma of kidney, malignant rhabdoid tumor and congenital mesoblastic nephroma. It is difficult to distinguish them from ultrasonography.
10.Comparison of two different nucleus delivery methods in small incision cataract surgery

AIM: To compare the effect of treating cataract by sodium hyaluronate injection and lens - loop nucleus extraction of small incision cataract excision and explore more suitable method to promote the use of surgery in primary hospital.
METHODS: All 146 cataract patients ( 166 eyes ) were allocated to two groups in random: the control group 78 patients ( 84 eyes ) received conventional lens loop for extracapsular cataract extraction;the experiment group 68 patients (82 eyes) underwent nucleus extraction in small incision with sodium hyaluronate injection for extracapsular cataract extraction. Postoperative visual acuity and intraoperative and postoperative complications were compared between two groups.
RESULTS: Incidence of posterior capsule rupture difference was statistically significant (P<0. 05) in small incision nucleus extraction with sodium hyaluronate injection compared with conventional lens loop for extracapsular cataract extraction. And there was no significant difference ( P> 0. 05 ) in postoperative visual acuity, astigmatism degree and postoperative complications compared with those of pre-operation.
CONCLUSION: Compared with the lens loop nucleus extraction, sodium hyaluronate injection for extracapsular cataract extraction can effectively protect the posterior lens capsule and can be promoted in small incision cataract surgery.
