1. Application of novel nanoparticles carriers in drug or gene delivery

OBJECTIVE: To summary the application of new types of nanoparticles carriers emerged in recent years in drug or gene delivery. METHODS: By sorting, analyzing and summarizing domestic and foreign literatures, the characteristics, in vivo and in vitro properties and the applications in pharmacy of novel nanoparticles carriers such as nano cochleates, virus-like particles, hydrogel nanoparticles, gold nanoshells, carbon nanomaterials, quantum dots and dendrimers were reviewed and elaborated. RESULTS AND CONCLUSION: Nanoparticles possess special physical and chemical properties which could improve the stability and bioavailability of drugs and have a targeting and sustained release effect.
2.Influence of Different Types of Seizures on Peripheral White Blood Cell Counts

Objective To analyze the influence of different types of seizures and nonepileptic events on peripheral white blood cell (WBC)counts. Methods We prospectively collected blood samples from all patients and detered WBC count to evaluate the relation of each type of seizure,duration,frequencies,and ttme lapse between a seizure and collection of blood sample to peripheral WBC count. Results Peripheral WBC count was elevated in about 50% cases after a generalized seizure. The length of a seizures was positively associated, whereas its frequencies and the lapse time was negatively correlated with the increase in WBC counts. Conclusion Peripheral WBC counts increase significantly after a generalized seizure.
3. Advances on Characterization of Lyotropic Liquid Crystals as Drug Delivery System

Lyotropic liquid crystal(LLC) is generally a kind of micelle association formed by self-assembly of amphiphilic lipid polymers in polar solvents. It have been applied in the study of drug delivery of administration of oral, skin injection, ocular, and cavity channel routes(including buccal cavity, nasal cavity, vagina), et al. Medical liquid crystal mainly includes lamellar phase, inverse hexagonal phase and inverse cubic phase. Inner assembling order in different structures of liquid crystals will affect the drug localization, viscosity, molecular interactions, and further drug release in vitro and in vivo, pharmacokinetics and so on. Thus, it is very important to characterize their microstructures. In this article, the characterization methods of LLC microstructures are comprehensively described based on the research of LLC as drug delivery carrier and related literatures, which can provide reference for further study of LLC drug delivery system.
4.Clinical significance of recognizing Zuckerkandl tubercle during thyroid surgery

Objective To study the clinical significance of recognizing Zuckerkandl tubercle (ZT) during thyroid surgery. Methods One hundred and sixty-one patients (218 sides) having underwent thyroid lobectomy or thyroidectomy were retrospectively analyzed. The presence of ZT, the position and size of ZT, and the relationship between ZT and recurrent laryngeal nerve (RLN), superior parathyroid (SP) were observed. Results In 218 sides, 179 sides (82.1%) found ZT, with right side in 93 cases and left side in 86 cases. The main relationship between ZT and RLN was A type, accounting for 90.5%(162/179). There were no statistical differences in ZT grade and the relationship type between ZT and RLN between left side and right side (P>0.05). There was negative correlation in ZT grade and the relationship type between ZT and RLN (r=-0.269, P<0.01). In right side, 92.3%(72/78) of SP located on the top of ZT at 10-11 o′clock position;in left side, 94.6%(70/74) of SP located on the top of ZT at 1-2 o′clock position. Conclusions ZT is an important anatomical mark in the thyroid surgery, which helps to identify and protect the RLN and SP, so as to reduce surgical complications.
5.Prophylactic central lymph node dissection for patients of cN0 thyroid papillary carcinoma

Objective To explore central lymph node metastasis (CLNM) of cN0 papillary thyroid carcinoma(PTC).Methods We evaluate the clinical data of 245 patients with cN0 unilateral TPC undergoing total thyroidectomy and bilateral central lymph node dissection.Results Central lymph node metastasis of cN0 TPC was 36.8% (91/245),significantly related to gender(x2 =5.626,P =0.018),age (x2 =6.255,P =0.012),tumor size (x2 =20.063,P < 0.001) and capsule invasion (x2 =15.024,P < 0.001).The central lymph node metastasis was 51.7% (60/116) in 116 patients with tumor size > 1 cm,and the contralateral CLNM was 22.4% (26/116) which was related to capsule invasion (x2 =9.737,P =0.002) and delphian LNM(x2 =10.982,P =0.001) There were 6 cases (2.4%) suffering from temporary unilateral recurrent laryngeal nerve injury.96 cases (39.2%) suffered from temporary hypoparathyroidism and 3 cases (1.2%) from permanent hypoparathyroidism.The median follow-up time was 40 months.6 cases were found regional lymph node recurrence on the ipsilateral side.Tumor size > 1 cm was significantly related with recurrence (P =0.008),as CLNM with higher recurrence rate (P =0.001).Conclusions CLNM is common in cN0 PTC patients,and central lymph node metastases increase the risk of recurrence.Prophylactic bilateral central lymph node dissection is suggested for tumor size > 1 cm cases.
6.Research progress in promoting stem cell function and differentiation via electrical stimulation

Nowadays,stem cells are recognized as the most promising seed cells of tissue engineering due to their self replication ability and multilineage differentiation potential.In order to apply them to tissue engineering,a variety of physical and biochemical methods were tried to promote the proliferation,function and differentiation,and good effects were achieved.Recently,as a rising method of physical stimulation,electrical stimulation received more and more attention and had been proved to have good effects on stem cell proliferation and differentiation.This paper summarized the advances in promoting effects of electrical stimulation on stem cell proliferation and differentiation,and discussed the possible mechanisms of stem cell differentiation induced by electrical stimulation.

9.Changes of motoneuronal function on remote site after local injection of botulinum toxin type-A

BACKGROUND: After local injection of Botulinum toxin type-A (BoTX-A), not only the function of the neuromuscular conjunction was affected, but also the changes occurred remote from the injected site. F-waves result from the back fire of the motoneuron activation, which may indirectly reflect the functional state of the motoneurons.OBJECTIVE: To evaluate the remote effect of local BoTX-A injection by F-wave test.DESIGN: Self-control study based on patients with movement disorders.SETTING: Neruologic clinic in a university hospital.PARTICIPANTS: Twenty-six patients with movement disorders not received previous local BoTX-A were selected from Neurological Clinic in Renmin Hospital of Wuhan University between September 2002 and July 2003, including 19 cases with hemificiospasm, 5 Meige syndrome and 2 torticollis spasmodicus.INTERVENTIONS: F- and M-waves of ulnar and tibial nerves were recorded before 1, 12 - 24 weeks after local injection of BoTX-A in 26 patients.MAIN OUTCOME MEASURES: The following parameters were analyzed:latency(ML) and amplitude (Mamp) of M-wave, minimal (Fmin) and average latency (Fave), amplitude of negative peak(Famp), duration (Fdur), persistence (Fpcr) and chronodispersion (Fchr) of F-wave.RESULTS: No definite F-response of ulnar nerve stimulation was obtained 1 week after injection in 3 HFS patients (5 nerves) . Fave prolonged significantly on ulnar and tibal nerve and Fdur increased significantly on ulnar nerve 1 week after injection, but there was no significant difference 12 - 24 weeks later, compared with before injection. No significant correlation of the altered F-wave parameters was found with the dosage of BoTX-A.CONCLUSION: Fdur and Fave could sensitively assess the remote effect,which correlates with distance away from the injected muscle, rather than the dosage of BoTX-A.
10.The remote effect of local injection of botulinum toxin type-A: an evaluation with F-wave

Objective To evaluate the remote effect of local injection of botulinum toxin by use of the F-wave measures. Methods The F-wave responses as well as M-waves were recorded before injection, and at 1 week,12 to 24 weeks after local injection of botulinum toxin type-A (BTX-A) in 26 patients, including 19 with hemi-facial spasm (HFS),5 Meige syndrome and 2 torticollis spasmodicus (TS).The following parameters were analyzed: M-wave latency (ML) and amplitude (Mamp), F-wave minimal latency(Fmin) and average latency(Fave),amplitude (Famp), duration (Fdur), persistence (Fper) and chronodispersion (Fchr). The above parameters were obtained through the electric stimulation of ulnar and tibial nerves, and recorded from the abductor digiti minimi and extensor digitorum brevis, respectively. Results No definite F-wave was obtained by electric stimulation of ulnar nerve at 1 week after injection in 3 HFS patients (5 nerves). The Fave recorded from electric stimulation of ulnar and tibial nerves prolonged significantlyand Fdur from ulnar nerve increased significantly at 1 week after injection, but were not significantly different from those of pre-injection when recorded at 12 to 24 weeks after injection. No significant correlation of the altered F-wave parameters was found with the dosage of BTX-A. Conclusion Fdur and Fave could sensitively assess the remote effect of the local injection of BTX-A, the remote effect might be correlated with the distance between injected muscle and tested muscle, rather than the dosage of BTX-A.
