1.Clinical significance of ambulatory blood pressure monitoring

Ambulatory blood pressure monitoring(ABPM)system can record patients blood pressure of 24 h on the predetermined time. The primary data forms the graph,the curve and the general report by operation of the related software after the analysis and statistics processing. Patients keep daily life condition,so the blood pressure data is more accuracy. ABPM can provide the dynamic change of the patients 24 h blood pressure,including the 24 h blood pressure survey data,the undulation situation and the change tendency. ABPM has become an useful element in the evaluation and follow - up of hypertension in adults. And ABPM is increasingly used to evaluate the blood pressure of children and adolescents in recent years. The ABPM has been shown to differ significantly values.
2.Preventive and therapeutic effects of shenfu injection in rats with renal ischemia reperfusion

AIM: To explore protective and therapeutic effects of the shenfu injection (SF) against renal ischemia reperfusionand and its possible mechanisms by studying the impact of SF on P38 mitogen activated protein kinase (P38MAPK), tumor necrosis factor alpha (TNF-?) and intercellular adhensive molecule-1 (ICAM-1). METHODS: 36 male SD rats were randomly divided into control group, ischemia reperfusion (IR) group and SF group. Rats were subjected to left renal pedicle occlusion followed by reperfusion with contralateral nephrectomy. The expression of P38MAPK and ICAM-1 were evaluated by Imunohistochemistry. The expression of TNF-? in the kidney tissue and plasma were analyzed by enzyme-linked immunoadsordent assay (ELISA). 3.Clinical Observation of Shenmai Injection Combined with Insulin in the Intensive Treatment of Stress Hy-perglycemia in the Septicopyemia Patients

OBJECTIVE:To observe the efficacy of Shenmai injection combined with insulin in the intensive treatment of stress hyperglycemia in the septicopyemia patients. METHODS:The data of 156 septicopyemia patients with stress hyperglycemia was collected Retrospectively,according to the different medication divided into control group (78 cases) and observation group (78 cases).All patients determined the pathogenic bacteria and received related anti-infection treatment,improving ventilation state, adequate fluid resuscitation support,immune therapy,nutritional support and other conventional treatment;based on it,control group received different dosages of insulin [1 U/ml,intravenous micro pump,with speed of 0.1 U/(kg·h)] based on different blood glucose levels;observation group additionally received 40 ml Shenmai injection,adding into insulin mixed solution,intrave-nous micro pump. Endpoint events(mortality),recovering time of blood glucose,total dosage of insulin,basic indicators [Acute physiology and chronic health(24 APACHE-Ⅱ)score,mechanical ventilation time,hospitalization days in ICU and frequency of hypoglycaemia] and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total mortality rate in observa-tion group was significantly lower than control group,recovering time of blood glucose was significantly shorter than control group,the total dosage of insulin was significnatly less than control group,mechanical ventilation time was significantly shorter than control group,hospitalization days in ICU was significantly less than control group,the differences were statistically significant (P<0.05).before treatment,there were no significant differences in the 24 APACHE-Ⅱ score in 2 groups (P>0.05);after treat-ment,the 24 APACHE-Ⅱ score in 2 groups were significantly reduced,and observation group was lower than control group,the differences were statistically significant(P<0.05). And there were no obvious adverse reactions in 2 groups during treatment. CON-CLUSIONS:Based on conventional treatment,Shenmai injection combined with insulin shows good efficacy in the intensive treat-ment of stress hyperglycemia in the septicopyemia patients,it can decrease total dosage of insulin,reduce mortality rate and 24 APACHE-Ⅱscore,shorten mechanical ventilation time and decrease hospitalization days in ICU,with good safety.
4.The relationship of peripheral circulating endothelial cell level and the coronary artery lesion in children with Kawasaki disease

ObjectiveTo investigate the relationship between the change of circulating endothelial cell (CEC) level and coronary artery lesion (CAL) of Kawasaki disease (KD),and to further explore the method for early diagnosis of KD.MethodsThirty KD children were recruited for study,including 23 children with complete type of KD and seven children with incomplete KD.According to the results of echocardiography,the KD group was divided into CAL group (9 cases) and non-coronary artery lesion (NCAL)group (21 cases).Ten healthy children were enrolled as control group.Double-blind and controlled trial was conducted,and Hladovec method was applied for CEC counting.Results The CEC level was ( 1.09 ±0.60) × 107/L in KD group,which was higher than that of control group [ (0.38 ±0.14) × 107/L],and the difference was statistically significant ( t =2.85,P < 0.01 ).The CEC level in the CAL group [ ( 1.84 ± 0.24) × 107/L] was higher than that of the NCAL group[ (2.01 ±0.38) × 107/L],and the difference was statistically significant ( t =2.24,P < 0.05 ).The CEC level was ( 1.16 ± 0.63 ) × 107/L in the complete type of KD group and (0.83 ± 0.45 ) × 107/L in the incomplete KD group,which showed no significant difference between the two groups ( t =1.86,P > 0.05 ).CondusionCEC level was elevated significantly in the acute phase of KD.The CEC level in CAL group was higher than that of NCAL group in acute phase.CEC level detection may be helpful for the early diagnosis of KD.
5.DSA in digital replantations

ObjectiveTo assess revascularization and vessel anastomosis in digital replantations with DSA.MethodsTwelve cases of digital replantations underwent digital subtract angiography during 2 to 4 days after fingers reattachment. The vessel anastomosis,hemodynamics,stenosis and discontinuation were investigated.The unobstructed and smooth anastomosis was suggested as early stage survival of the reattached fingers,the spasm and stenosis of the reattached vessels were considered as mild vascular crisis,and the discontinuation of hemodynamics were indicated as severe vascular crisis.ResultsThe total 27 vessels were clearly displayed on DSA.Of these vessels,23 vessels were unobstructed and smooth,all digits were survived.Diagnosis coincidence of early stage survival was 100%(23/23). Two vessels were obstructed,which were testified having thrombus by operation research.The other 2 vessels were spasm,the digits were also survived ultimately by expectant treatment.All 4 abnormal vessel anatomosis were found by DSA.Conclusion DSA is important modality in assessing revascularization and blood circulation for digital replantations,guiding in dealing with the vascular crisis,and in predicting early stage survival of the reattached digits.
6.Post market trial of hepatitis B immunoglobulin for intravenous administration

Objective To observe the efficacy and adverse reaction of the test drug hepatitis B immunoglobin on the post market.Method Employed by the methods of multiple center's clinical trials and using the recommended dosage of hepatitis B immunoglobulin for intravenous administration,the clinical efficacy of either prevention or treatment for hepatitis B recurrence and drug related adverse reactions were observed.This consisted of 22.1 months,13 hospitals,and 525 patients with hepatitis B related liver transplantation.Result The results showed a contrasting probability of adverse reactions for different doses among the observation period.Within 6 months postoperatively with a greater or equal to recommended drug dose,the infection rate was less than 4%.In contrast,the infection rate was greater than 12% in the group with less than the recommended drug dose.Conclusion There was an obvious dose effect relationship,and the drug safety and recommended treatment rationality were verified.
7.Effect of polylactic acid-polyglycolic acid copolymer/collagen type Ⅰ microspheres combined with BMSCs on bone defects in osteoporotic rats

Objective To evaluate the bone repair capacity ofpolylactic acid-polyglycolic acid copolymer (PLGA)/collagen type Ⅰ (CoI) microspheres combined with BMSCs after being injected in intertrochanteric bone defect of osteoporotic female rats.Methods Prepared PLGA microspheres.The microspheres were coated with Col.BMSCs of the third passage were cultured with PLGA/CoI microspheres.Forty 3-month-old female SD rats were ovariectomized to establish osteoporotic animal models.The osteoporotic rats were randomly divided into 5 groups,including SHAM group,PBS group,Cell group,MS group and Cell+ MS group.There were 8 rats in each group.Different material was injected into the intertrochanteric bone defect site which was made with electric drill.Four rats of each group were sacrificed at 1 month and 3 months post-operation.The fenora were taken to measure the intertrochanteric bone mineral density (BMD) with DEXA and evaluate trabecular stucture with Micro CT.Results After 7 days of coculture,BMSCs seeded on PLGA/CoI microspheres had nice adherance and proliferation.There was no difference of BMC and BMD among all groups at 1 month post-operation.Tb.Th of Cell+MS group was higher than that of PBS group and MS group at 1 month post-operation.%Tb.Ar of Cell+MS group was higher than that of Cell group and MS group at 1 month post-operation.Tb.Sp of Cell+MS group had a tendence to decrease compared with other groups but there was no statistical difference at 1 month post-operation.After 3 months of operation,the BMC of Cell+MS group had a tendence to increase compared with that of PBS group and MS group but showed no statistical difference.BMD and Tb.Th of Cell+MS group was higher than those of other groups.%Tb.Ar of Cell+MS group was higher than that of SHAM group and PBS group.Tb.Sp of Cell+MS group had a tendence to reduce compared with other groups but showed no statistical difference.Conclusion The bone defect of osteoporotic site can be repaired 1 month after the injection of the PLGA/CoI microspheres combined with BMSCs.The trabecular reconstruction and bone quality of osteoporotic site can be improved 3 months after the injection.
8.Relationship between paravertebral vascular leakage and pulmonary cement embolism during percutaneous vertebroplasty

BACKGROUND:Percutaneous injection of bone cement into the vertebral body can enhance the vertebral strength and vertebral stability, and obtain pain relief. But the bone cement may leakage into the paravertebral vessels during injection, and may back to the right ventricle and flow into the pulmonary artery through vena cave and thus causing pulmonary embolism.
OBJECTIVE:To evaluate the relationship between cement paravertebral vascular leakage and pulmonary cement embolism during percutaneous vertebroplasty.
METHODS:Total y 134 cases of osteoporotic compression fractures treated with vertebroplasty were retrospectively analyzed. Among them, 23 cases of cement paravertebral vascular leakage were considered as the experimental group, and the 43 cases without cement paravertebral vascular leakage and had the complete imaging data were considered as the control group. The spine and chest X-ray films were taken before and after vertebroplasty to detect whether there were cement paravertebral vascular leakage and pulmonary cement embolism in the patients of two groups.
RESULTS AND CONCLUSION:Among the 134 patients, 23 patients had cement paravertebral vascular leakage, and accounted for 17.2%. Among the 23 patients with cement paravertebral vascular leakage, three cases had pulmonary cement embolism without chest symptoms, and accounted for 13%. No pulmonary cement embolism occurred in the 43 patients without paravertebral vascular leakage of the control group, and there was no significant difference between two groups (P=0.039). Although the patients with cement pulmonary embolism remained asymptomatic, pulmonary cement embolism remained possible if cement paravertebral vascular leakage was detected during vertebroplasty.
9.Influencing factors for radial shortening after surgical operation for intra-articular fractures of the distal radius

ObjectiveTo analyze the influencing factors and clinical results of radial shortening after operation of intra-articular fractures. MethodsA total of 54 patients with intra-articular fractures of the distal radius treated surgically from January 2003 to January 2008 were followed up and divided into radial shortening group ( > 4 mm) and control group ( < 4 mm) by whether radial shortening was more than 4 mm. Fracture types, comminution and defection degree, age, surgical approach and internal fixation method, whether bone implant in operation, post-operative volar tilting angle and ulnar inclination angle, and wrist function score were investigated and compared between the two groups. ResultsThe incidence of radial shortening in post-operation of intra-articular fractures of the distal radius was 30%.Age and whether bone implant in operation were significantly different between the two groups. Type C fractures were more liable to radial shortening than type B fractures. Operation approach and internal fixation method have no significant influence on the radial shortening. Volar tilting angle and ulnar inclination angle were not significantly different between the two groups and the excellent and good rate of function score was lower in the radial shortening group. ConclusionThere is a high incidence of radial shortening after operation for the intra-articular fractures of the distal radius, which affects the clinical results of operation. Therefore, we should pay more attention to the influencing factors and take corresponding precaution methods during operation.
10.One case of adult Kawasaki disease.

Kawasaki disease is far more frequent in children than in adults. The pathogenesis of Kawasaki disease is unknown, but it involves changes to the coronary artery and other diverse clinical manifestations. There are currently no specific laboratory diagnostic indexes, and especially since the disease is rare in adults, so it is extremely easy to misdiagnose or to overlook entirely. Our retrospective analysis of an diagnosis of and treatment for Kawasaki disease in an adult provides a guide to clinical doctors in terms of understanding Kawasaki disease, early diagnosis of it, and improved prognosis.
