1.Hippocampal ? oscillation involved in antiepilepsy effects of vagus nerve stimulation

Objective To study the effects of acute weak vagus nerve stimulation on hippocampal CA1 unit discharges and field potentials of normal and epileptiform discharging rats.Methods The experiments were performed with 55 male Sprague-Dawley rats weighing 180~250 g,SPF grade.Animals were divided into normal(n=45)and regularly epileptiform discharging(n=10)groups.The normal rat group:Separate the left neck vagus nerve and ligate the peripheral end.Acute weak electrical stimulation(10 Hz,0.5 ms,1.5~5.0 V,15~20 unit /train)were administered to the left neck vagus nerve central end.20 trains were performed with a regular interval of 5 min.Record the unit discharges of right hippocampal CA1 and field potentials of bilateral hippocampal CA1.The epileptiform discharging group:Gelatin spongia was put onto the left cortex to induce epileptiform discharges.After 30 min stable regularly discharging,continue the procedure in the first group.Results Cyclical theta oscillation(about 3~6.5 Hz)appeared in bilateral hippocampal CA1 of normal rats after the stimulation(strain m=5~7,38/45,84.4%).With oscillation,there is unit discharging.It has two kinds:theta-on(n=30)and theta-off(n=5).The sharp wave amplitude was inhibited while the sharp wave interval increased following the acute weak vagus nerve stimulation(n=10)in epileptiform discharging rats.Theta oscillation was induced during the sharp wave.With oscillation,there is tonic unit discharging(n=10).The number of action potential spikes positively correlate with the sharp wave intervals(P
2.THE INDUCEMENT AND DIFFERENTIATION OF THE RATS' BONE MARROW STROMAL CELLS TRANSFECTED WITH NURRL GENE

Objective To study the effect of Nurrl gene on the differentiation of rats' marrow stromal cells(MSCs) into neurous under the co-inducement of total panax notoginserg saponins(tPNS) and all-trans-retineic acid(ATRA) by coloning the Nurrl gene and transfecting it into MSCs. Methods Expressing plasmids pcDNA3.1-hygro-Nurrl were cloned,then transfected into MSCs with lipofectamine 2000.To begin with,MSCs were subcultured into 6-wells cultured plate at about 5?10~5 cells/well density and the wells were divided into four groups randomly which were Nurrl+tPNS/ATRA group,tPNS/ATRA group,Nurrl group and control group.Secondly,the plasmids were introduced to the MSCs in Nurrl+tPNS/ATRA group and Nurrl group,then protein expression of Nurrl was identified with immunocytochemistry.Thirdly,after the MSCs and plasmids had been co-cultured for 48 hours,cells in Nurrl+tPNS/ATRA group and tPNS/ATRA group were induced with BME in advance then with tPNS/ATRA in due form.For cells in Nurrl and control group,the only difference was that tPNS/ATRA was replaced with the culture.Finally we compared the different percentage of positive cells in four groups with TH,AChE and GABA antibodies by immunocytochemistry method. Results The immunocytochemical test showed that the MSCs transfected with Nurrl gene expressed Nurrl protein.The percentage of positive cells of TH antibody in Nurrl+tPNS/ATRA group was(38.4?4.6)% distinctly higher than that of tPNS/ATRA group,which was(5.9?3.4)%.Conclusion With tPNS/ATRA induced and immunocytochemistry of TH,positive cells percentage in Nurrl+tPNS/ATRA group was higher than that in tPNS/ATRA group,which showed a statistic difference.And the inducing function of tPNS and ATRA in MSCs differentiating into neurons was definite.
3.Notch signaling pathway,articular cartilage development and osteoarthritis

BACKGROUND:Osteoarthritis,often accompanied by cartilage abnormal signal transduction,suggesting that intracellular signal transduction may play an important role in osteoarthritis genesis and development.OBJECTIVE:To review recent advances in the research of Notch signaling pathway,and explore the mechanisms of chondrogenic differentiation of bone marrow mesenchymal stem cells and its role in cartilage development and chondrogenesis.To analysis the role of dysregulated Notch signaling in the development of osteoarthritis.METHODS:Pubmed was undertaken to identify the relevant articles published from March 1919 to February 2009 with the key words of"Notch signaling pathway,osteoarthritis,chondrocytes,chondrogenesis,bone marrow stem cells"in English.Simultaneously,CNKI Database was searched for relevant articles published from February 2004 to September 2008 with the key words of"Notch signaling pathway,osteoarthritis,chondrocytes,chondrogenesis,bone marrow stem cells"in Chinese Articles with the Notch signaling pathway-related research,Notch signaling pathway of bone marrow-derived mesenchymal stem cells,differentiation of cartilage relative to the mechanism studies,and the Notch signaling pathway disorders and osteoarthritis research were included,Reproducibility articles were excluded Development regulation of articular cartilage and cartilage cell proliferation were considered as evaluation indexes.RESULTS AND CONCLUSION:Of 632 articles retrieved,30 were included in our study according to the inclusive and exclusive criteria.The Notch pathway is a highly consewed signaling mechanism involved in many processes determining cell fate during development and maintenance of homeostasis of mature tissues in the adult organism.Activation of Notch occurs following Notch receptor/ligand engagement upon cell-cell contact,which initiates translocation of the Notch intracellular domain to the nucleus and activation of target genes.It has been demonstrated that the Notch signaling plays an important role in regulating articular cartilage development and chondrocyte proliferation and differentiation In recent studies abnormality of notch signaling was also found in disease,which suggests that notch signaling is involved in osteoarthritis initiation and development.Further study of the specific mechanism of notch signaling in the initiation and development of osteoarthritis will enable us to develop targeted approaches in the treatment of osteoarthritis,thus bring more strategies in the future treatment of osteoarthritis.
4.Analysis of the demographic and blood donation behavior of repeat blood donors and first-time donors——based on the data of blood donors in Zhejiang province from 2006 to 2015

Objective To compare with the characteristics of different demographic and blood donation behaviors of the first blood donors and the repeated blood donors,to analyze the related factors influencing the repeated blood donation behavior,to provide the evidence to develop the recall strategy for the retention of the first-time donors strategies.Methods Use methods such as the composition ratio of descriptive analysis,and logistic regression analysis,Retrospectively analyzed the data of 3 226 571 cases of the whole blood donors in Zhejiang province from 2006 to 2015.from BIS2.0 Results ZheJiang repeated blood donors in 2006-2015 is accounted for 30.8%,men (57.8%),the proportion of aged 25 above is higher than the first blood donors;71.7% of men in the repeated blood donors are 60-79 kg,52.2% of women repeated blood donors are 50 to 59 kg;40% of the repeat donors blood for the first time donate 400 mL;71.6% of the repeated blood donors to donate again in 0.5-2 years,and of these,40.8% back in 0.5-1 year.Conclusion The main factors on the demographic aspects that influence the repeated blood donation is occupation,cultural degree,the quantity of blood donation for the first time.The characteristics of the precise recall people are as follows:Age 26 to 45 years old,stable career,donate 400 mL for the first-time,weight 70-89 kg of male,weight 55 kg above of women.The better recall intervention Interval is preferred to 0.5-2 years,and 0.5-1 year is the best.
5.Effect of sevoflurane postconditioning on expression of CHOP in a rat model of hemorrhagic shock and resuscitation

Objective To evaluate the effect of sevoflurane postconditioning on the expression of CCAAT/enhancer-binding protein homologous protein (CHOP) in a rat model of hemorrhagic shock and resuscitation.Methods Thirty-six healthy adult male Sprague-Dawley rats,weighing 300-350 g,were divided into 3 groups (n=12 each) using a random number table:sham operation group (group S),hemorrhagic shock and resuscitation group (group HSR) and sevoflurane postconditioning group (group SP).Hemorrhagic shock was induced by withdrawing 40% of the total blood volume from the right carotid artery over an interval of 30 min,and 1 h later the removed blood was reinfused via the left jugular vein for resuscitation.Group SP inhaled 2.4% sevoflurane for 30 min starting from the onset of reinfusion.Mean arterial pressure was monitored and recorded at a 10 min interval.Before withdrawing blood (T0),immediately after the end of withdrawing blood(T1), at 1 h after the end of withdrawing blood(T2) and immediately after the end of reinfusion (T3),blood samples were collected from the common carotid artery for blood gas analysis.At 4 days after reinfusion,6 rats of each group were selected to detect spatial learning and memory ability by using Morris water maze test.The animals were then sacrificed,brains were removed for determination of neuronal apoptosis in hippocampal CA1 area using TUNEL.The rest 6 rats in each group were sacrificed at 72 h after reinfusion,and the hippocampus was isolated to detect the expression of CHOP by Western blot.Results Compared with group S,mean arterial pressure was significantly decreased,and lactic acid concentrations were increased at T1,2 in HSR and SP groups,and the escape latency was significantly prolonged,the percentage of time staying at the target quadrant was decreased,the number of apoptotic neurons in hippocampal CA1 area was increased,and the expression of CHOP was up-regulated in group HSR (P<0.05).Compared with group HSR,the escape latency was significantly shortened,the percentage of time staying at the target quadrant was increased,the number of apoptotic neurons in hippocampal CA1 area was decreased,and the expression of CHOP was down-regulated in group SP (P<0.05).Conclusion The mechanism by which sevoflurane postconditioning improves cognitive function is related to down-regulation of CHOP expression and inhibition of apoptosis in hippocampal neurons in a rat model of hemorrhagic shock and resuscitation.
6.Effects of parecoxib and morphine on remifentanil-induced postoperative hyperalgesia in patients undergoing orthopedic operation

Objective To investigate the effects of parecoxib and morphine on remifentanil-induced postoperative hyperalgesia in patients undergoing orthopedic operation. Methods Sixty ASA Ⅰ or Ⅱ patients,aged 20-62 yr, weighing 45-100 kg, undergoing orthopedic surgery, were randomly divided into 3 groups ( n = 20 each). Anesthesia was induced with midazolam, propofol, remifentanil and rocuronium. The patients were mechanically ventilated after tracheal intubation. Group Ⅰ received iv injection of morphine 0.15 mg/kg, group Ⅱ received iv injection of parecoxib 20 mg and morphine 0.075 mg/kg and group Ⅲ received iv injection of parecoxib 40 mg and morphine 0.075 mg/kg. Anesthesia was maintained with infusion of propofol and remifentanil and intermittent iv boluses of vecuronium. The emergence time, consciousness recovery time, extubation time,incidence of agitation and shivering, and VRS score at 5 min after recovery of consciousness were recorded. Pain at rest and at movement was evaluated using VAS score at 1,2, 4, 8, 12 and 24 h (T1-6) after surgery and MAP andHR were recorded simultaneously. The incidence of nausea and vomiting during 24 h after surgery was also recorded. Blood samples were taken before induction of anesthesia, at the end of operation and 24 h after operation for determination of plasma concentrations of PGE2 and TNF-α. Results There was no significant difference in emergence time, consciousness recovery time, extubation time, VRS scores, MAP, HR, incidence of agitation,shivering, nausea and vomiting among the 3 groups. Compared with group Ⅰ , VAS scores at rest at T1-2 and at movement at T1-6 were significantly increased in group Ⅱ , while VAS scores at rest and at movement decreased at T1-5 in group Ⅲ (P<0.05). VAS scores at rest at T1-6 and at movement at T1-5 were significantly lower in group Ⅲ than in group Ⅱ (P< 0.05). There was no significant difference in the plasma concentrations of PGE2 and TNF-α at different time points between group Ⅰ and Ⅱ (P>0.05). The plasma concentrations of PGE2 and TNF-α were significantly lower at the end of surgery in group Ⅲ than in group Ⅰ and Ⅱ (P<0.05). Conclusion Preoperative iv parecoxib 40 mg and morphine 0.075 mg/kg can reduce remifentanil-induced postoperative hyperalgesia in patients undergoing orthopedic operation, and the efficacy is better than that of morphine alone.
7.Efficacy of PCV-VG mode for lung protective ventilation in patients requiring one-lung ventilation during thoracoscopic surgery

Objective To evaluate the efficacy of pressure-controlled volume-guaranteed (PCVVG) mode for lung protective ventilation in patients requiring one-lung ventilation (OLV) during thoracoscopic surgery.Methods Sixty patients,aged 50-70 yr,with body mass index of 18-26 kg/m2,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective radical resection of esophageal cancer performed via video-assisted thoracoscope under general anesthesia,were divided into 2 groups (n=30 each) using a random number table:volume-controlled ventilation group (group V) and PCV-VG group (group P).The ventilator settings were adjusted,with a tidal volume 10 ml/kg and respiratory rate 10-12 breaths/min during two-lung ventilation,and with a tidal volume 6 ml/kg and respiratory rate 12-16 breaths/min during OLV.The inspiratory/expiratory ratio was 1 ∶ 2,pressure restriction was 35 cmH2O,and 33% oxygen was inhaled at 2 L/min.The end-tidal pressure of carbon dioxide was maintained at 35-40 mmHg.Visual analog scale score was maintained ≤ 3 after operation.After admission to the operation room (T0) and at 1,3 and 7 days after operation (T1-3),forced vital capacity (FVC),forced expiratory volume in first second (FEV1),and maximal mid-expiratory flow (MMEF) were measured,arterial blood samples were collected for blood gas analysis,arterial carbon dioxide partial pressure and arterial oxygen partial pressure (PaO2) were recorded,and alveolar-arterial oxygen tension difference (PA-a O2) was calculated.Clinical Pulmonary Infection Score was assessed at T1,T2 and T3.The chest tube removal time and length of postoperative hospital stay were recorded.Results Compared with the baseline at T0,FVC,FEV1,MMEF and PaO2 were significantly decreased,and PA-aO2 was increased at T1-3 in the two groups (P<0.05).Compared with group V,FVC,FEV1,MMEF and PaO2 were significantly increased,PA-aO2 and Clinical Pulmonary Infection Score were decreased,and the chest tube removal time and length of postoperative hospital stay were shortened at T1-3 in group P (P<0.05).Conclusion PCV-VG mode can achieve lung protective ventilation,which is helpful in improving outcomes in the patients requiring OLV during thoracoscopic surgery.
8.Effects of dopaminergic medication on decision-making under ambiguity in patients with early Par-kinson's disease

Objective To investigate the effects of dopaminergic medication on decision-making un-der ambiguity in patients with early Parkinson's disease( PD) . Methods Using Iowa Gambling Task ( IGT) for 24 early non-medication idiopathic PD patients( Hoehn and Yahr Scale≤Ⅱlevel) ,24 early idiopathic PD patients with regular dopaminergic medication and also for 24 healthy controls( HC) whose age,gender,and education match to PD patients to test their ability of decision-making under ambiguity. Results The results showed non-medication PD group showed impairments on digtal span and verbal fluency and decision-making task. There was significant difference in IGT task scores among the three groups(F=6.024, P=0.004) . The total net scores of advantageous choices in IGT were significantly lower in non-medication PD group( (-4.50 ±22.19) scores) than medication PD group((8.83±23.24)scores) and healthy group((15.92±15.77) scores) . The difference of net scores in block1 to block5 between non-medication PD group and medication PD group was gradually increased,and the difference of net scores in block5 was significant(P<0.05) . There was no significant difference in total net scores and net scores in block1 to block5 between medication PD group and healthy group(P>0.05) . As the game processing,medication PD group gradually shifted their se-lections toward the advantageous choices. But non-medication group did not exhibit this shift pattern and the performance was much poorer. Meanwhile, the study also indicated the total net scores of advantageous choices for non-medication PD group was positive correlation to the MoCA scores ( r=0.614, P=0.001). Conclusion The present study has shown that non-medication PD group has impairment in decision-making under ambiguity risk condition and prefer to choose risky options. when exogenous complement dopaminergic medication,the risk decision-making ability of medication PD group has been improved.
9.Effects of volume-guaranteed pressure-regulated ventilation on the pulmonary function during percutaneous nephrolithotomy

Objective To observe the effects of pressure control ventilation with volume guar-antee (PCV-VG)on the pulmonary function during percutaneous nephrolithotomy procedures in pa-tients with general anesthesia.Methods Forty patients scheduled for percutaneous nephrolithotomy were selected and randomly allocated into PCV-VG group (n =20)and volume controlled ventilation (VCV)group (n =20).For two modes of ventilation,the goal tidal volume was 6-8 ml/kg,and the respiratory rate was contralled to 12-20 bpm.PA-a O 2 ,OI,RI,Ppk,Pmean,Cst,Hct,Lac were re-corded at intubation (T0 ),1 5 min (T1 ),30 min (T2 ),60 min (T3 ),and 120 min (L4 )after intuba-tion.Results PCV-VG resulted in significantly lower PA-a O 2 ,RI,Ppk,Pmean compared with VC ventilation (P < 0.05 or P < 0.01 ),and significantly higher OI,Cst versus VC ventilation (P <0.05 or P < 0.01).Conclusion In general anesthesia patients undergoing percutaneous nephrolithoto-my,PCV-VG is superior to VCV in terms of lower airway pressure and more stable hemodynamics, thus protects pulmonary function.
10.Effect of goal-directed fluid management on hemodynamics in patients with orthopaedic arthroscopic shoulder surgery in beach chair position

Objective To evaluate the effect of conventional or goal-directed fluid management on hemodynamics in patients undergoing orthopaedic arthroscopic shoulder surgery in beach chair po-sition.Methods Thirty healthy adult patients,male 1 7 cases,female 13 cases,aged 18-65 years, weight 49-68 kg,ASA Ⅰor Ⅱ,undergoing elective arthroscopic shoulder surgery,were enrolled.Pa-tients were randomly assigned to the group R(Routine group,n = 1 5 )and the group S(SVV/CI/MAP-directed,n =1 5).All patients received 10 ml/kg of hydroxyethyl starch rapidly in group R;while in group S,if SVV > 13%,patients would receive 3 ml/kg of hydroxyethyl starch in 5 min, then the changes of each index were observed;if SVV <13% and CI< 2.5 L·min-1 ·m-2 ,given dopamine 0.5~1 μg·kg-1 ·min-1 ,until CI>2.5 L·min-1 ·m-2 .At 5 min after anesthesia induc-tion,patients were placed in a 60° upright position.The hemodynamic changes were monitored by FloTrac/Vigileo system.Heart rate (HR),mean artery pressure(MAP),cardiac index(CI),stroke volume variation(SVV),stroke volume index (SVI),were recorded on pre-induction (T1 ),post-induc-tion (T2 ),immediately after in beach chair position (T3 ),5 min after in beach chair position(T4 ),30 min after in beach chair position(T5 ),and at the end of surgery(T6 ).The duration of surgery,crys-talloid requirements,colloid requirements,urinary output,the dose of vasoactive drugs and the inci-dence of hypotension were recorded.Results Compared with T1 ,MAP,CI and SVI at T3-T5 point (after in BCP to the end of the surgery)were higher in both group(P <0.05 ).Compared with T2 , SVV in group R at T3-T5 were significantly increased (P <0.05),while SVV in group S only at T3 was slightly increased (P <0.05).Compared with group R,MAP,CI and SVI at T3-T5 were signif-icantly higher respectively,while SVV were higher at T3-T5 in group R (P <0.05).Compared with group R,the colloid requirements and total requirements in group S were significantly increased(P <0.05).Compared with group R,the doses of dopamine and ephedrine,the urinary output,the inci-dence of hypotension in group S were significantly reduced(P <0.05).Conclusion SVV/CI/MAP-di-rected fluid management is safer,more effective and renders much more stable hemodynamic than the routine fluid management.
