1.Experimental Studies on Anxin Granules in Preventing andTreating Hyperlipidemia

[Objective] To observe the preventive and therapeutic effect of Anxin Granules in the treatment of hyperlipidemia [Methods] Thirty - two New Zealand male rabbits were randomized into 4 groups. Groups A, B, C and D were given with common forage, high fat forage, high fat forage with Zhibituo Tablets, high fat forage with Anxin Granules respectively. After 10 weeks of treatment, serum levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol ( LDL-C), high-density lipoprotein cholesterol (HDL-C) and malondialdehyde ( MDA) and activity of superoxide dismustase (SOD) were detected. [ Results ] Anxin Granules decreased the serum levels of TC, TG, LDL-C and MDA. [Conclusion] Anxin Granules has a good effect in preventing and treating hyperlipidemia and atherosclerosis by regulating blood lipid metabolism.
2.Conservative treatment for patients with plasma cell mastitis

Objective To investigate the etiology and novel treatment of plasma cell mastitis (PCM).Methods 145 cases of PCM undergoing massage dredge,ductal lavage and local amalgesic injection from Jan.2011 to Dec.2012 were retrospectively analyzed.Results 145 cases of PCM underwent conservative treatment for 2 to 8 weeks,among whom 128 cases were cured and the other 17 cases underwent surgical treatment for poor outcome.Among the 128 cases undergoing conservative treatment,9 cases had relapse during the follow-up of 12 to 36 months.Conclusions The combined treatment of massage dredge,ductal lavage and partial closed treatment is effective for patients with PCM,and it can keep patients from having mastectomy and reduce the recurrent rate.
3.Clinical study on the left ventricular function in hypertrophic cardiomyopathy patients

Objective To explore the left ventricular overall systolic and diastolic function in hypertrophic cardiomyopathy (HCM) patients with echocardiography.Methods 30 normal people and 30 hypertrophic cardiomyopathy patients were examined by routine echocardiography and examine the results of LVDd,IVSd,LVPW,LVFE and E/A.Results The results of IVSd [(16.47 ± 2.08) cm],LVPW [(10.28 ± 0.56) cm] and LVEF [(62.18 ±6.74) %] in HCM patients were superior than control group [(9.56 ± 0.45) cm,(9.30 ± 0.98) cm,(57.66 ±5.22) %] (t =-17.809,-4.756,-2.91,all P < 0.05) while the results of LVDd [(40.28 ± 3.80) cm] and E/A[(0.99 ±0.17)] of HCM patients were obviously lower than control group [(45.15 ±3.84) cm,(1.10 ±0.24)](t =4.899,2.132,all P < 0.05) and LVEF was obviously larger than control group (P < 0.05).Conclusion Echocardiography can exactly evaluate left ventricular overall systolic and diastolic function in hypertrophic cardiomyopathy (HCM) patients and guide the diagnosis and treatment.
4.Resection of mammary intraductal tumors under ultra-fine fiberoptic ductoscopy

Objective To assess the value of the resection of mammary intraductal tumors under ultra-fine fiberoptic ductoscopy.Methods Ductoscopic examinauions were performed in 312 patients with nipple discharge,and those with positive findings of intraductal tumors were given further surgical treatment.The location of lesions was determined according to the position of skin projection of the cold light source,the passage of the mammary ducts was determined depending on the direction of intraductal dilatator,and the distribution of the distal ends of mammary ducts was determined with methylene blue staining.Results Intraductal tumors were found in 78 cases,including solitary papilloma in 68 cases,papillomatosis in 5 cases,and intraductal carcinoma in 5 cases.The lesions were located at the major mammary ducts in 22 cases(28.2%,22/78),at the first branches of segmental ducts in 38 cases(48.7%,38/78),at the second branches in 14 cases(17.9%,14/78),at the third branches in 3 cases(3.8%,3/78),and at the fourth branches in 1 case(1.3%,1/78).The distance between the lesions and the lacteal outlet was 1~5 cm(mean,2.5 cm).All the 78 cases were surgically treated.Postoperative pathological findings revealed 65 cases of papilloma,8 cases of papillomatosis,and 5 cases of intraductal carcinoma.The diagnostic accordance rate of ductoscopy was 96%(65/68) for papilloma and 100% for papillomatosis(5/5) and intraductal carcinoma(5/5).There were 65 cases of ductectomy,8 cases of segmentectomy,3 cases of breast-conserving radical resection,and 2 cases of modified radical resection.Conclusions On the basis of the position of skin projection of the cold light source,the direction of intraductal dilatator,and the methylene blue staining,the location of lesion can be accurately identified for precise resection,which carries out a minimally invasive treatment for mammary intraductal tumors.
5.The value of 320-detector row computed tomography angiography in diagnosing diabetic dorsalis pedis arteriosclerotic disease

Objective To investigate the value of 320-detector row CT angiography for evaluating diabetic dorsalis pedis arterio-sclerotic disease.Methods 18 patients with diabetes and 20 healthy volunteers underwent 320-detector row CT angiography.The obtained thin-slice images were reformatted using maximum intensity projection,volume rendering,curved planar reformation and multiplanar reconstruction to evaluate the features of dorsalis pedis artery.Results 320-detector row CT angiography could exactly show the course,morphological features of the dorsalis pedis artery and the anatomical detail of arteriosclerotic disease.Arterioscle-rostic was found in variant degree in 18 cases.Among all,6 and 7 sides were slight arteriostenosis in proximal and distal segment,re-spectively.Moreover,2 and 3 sides were medium arteriostenosis in proximal and distal segment,respectively.The most common type of plaque property were mixed plaque.Conclusion 320-detector row CT angiography is of significant value in the assessment of arte-riosclerotic disease of dorsalis pedis arteries in the patients with diabetes.
6.Treatment of comminuted clavicular fracture with steelwrie perforating and annular Ligating method: report of 32 cases

Objective To study the application value of steelwire perforation and cerclage in treatment of comminuted clavicular fracture. Methods 32 patients with comminuted clavicular fracture were treated with steelwire perforation and cerclage. Two holes were made at the lateral margin at the ends of fractured clavicle, then a steel wire of 0.6mm in diameter passed through them and fixed the fracture. The bone fragments were fixed either with perforation or with cerclage according to their size and shape. Results The mean follow up time was 59 months. All the 32 cases of fracture healed within expected time. The function of the shoulder joints recovered well, with the excellent and good rate being 90.6% . Conclusion The steelwire perforation and cerclage bears advantages of being less traumatic, providing stable fixation, promoting fracture healing and making the secondary operation easy.
7.Expression of iodothyronine deiodinases on human placenta in pregnant women with different thyroid disease

Objective To investigate the expression of type Ⅱ and type Ⅲ iodothyronine deiodinases (D2 and D3 ) of human placenta on pregnant women with different thyroid diseases and different autoimmunity.Methods Pregnant women with different thyroid diseases and autoimmunity were selected into the experiment group,and pregnant women who had no individual or family history of thyroid diseases with normal thyroid function and negative thyroid peroxidase antibody (TPOAb) were selected into the control group.Expression level of iodothyronine deiodinase( D2 and D3 )on placenta was measured by RT-PCR.Results D2 and D3 were both expressed on human placenta.D2 showed significantly higher expression level in hypothyroidism uncontrolled group than that in control group ( 0.916 ± 0.035 vs 0.833 ± 0.029,P < 0.05 ),however,D3 showed a lower expression(0.766 ±0.038 vs 0.848 ±0.052),on placenta and the difference was not significant(P > 0.05 ).In those who had history of hyperthyroidism or hypothyroidism but whose thyroid function become normal by effective treatment,D2 and D3 expression exhibited no difference from that of the control(P > 0.05).In those pregnant women with normal thyroid function and TPOAb-positive,D2 and D3 expression was (0.842 ± 0.032 ) and ( 0.837 ± 0.053 ) respectively and there was also no difference from that of the control( P > 0.05 ).Conclusion D2 and D3 were simultaneously expressed on human placenta.In those pregnant women with hypothyroidism,the level of D2 expression is high and the level of D3 expression is low.Those changes might be important and helpful for the stabilization of thyroid hormone transportation between mother and fetus.
8.Inhibition of amikacin on platelet aggregation and blood coagulation

Objective To observe the inhibition of amikacin in vitro on platelet aggregation and blood coagulation tests, and to study its effects on hemostasis and the related mechanisms.Methods Plateletrich plasma and platelet-poor plasma from donors were mixed with different concentration of amikacin, which was divided into four groups:0 mg/L, 30 mg/L, 91 mg/L and 910 mg/L group.The maximial ratio of platelet aggregation induced by ADP were measured with Platelet Aggregation Analyzer.The expression levels of P-selectin, GP Ⅱ b/Ⅲ a and Fg-R were determined with Flow Cytometer.The PT, APTT, TT and Fg of platelet-poor plasma were detected with Blood Coagulation Analyzer. The four concentration of amikacin mentioned above and two anticoagulants (62.5 U/ml of sodium heparin and 109 mmol/L of sodium citrate)were interacted with fresh whole blood, in which the blood CT and plasma Ca2+ were detected. Blood samples were collected from 10 patients with acute lower respiratory tract infection before and 30 minutes after routine amikcin treatment respectively.The maximial ratio of platelet aggregation, the expression levels of P-selectin, GPⅡ b/Ⅲa and Fg-R induced by ADP were measured; while PT, APTT, CT and plasma Ca2+ were determined.Results At 30 mg/L of amikacin group, the maximal ratios of platelet aggregation (65.8±3.9)%, the expression levels of P-selectin (9.2 ± 1.0)% and Fg-R (12.6 ± 1.7)% were statistically lower than those [(88.0 ±4.6%, (16.1 ± 1.3)% and (31.0 ±2.5)%]at 0 mg/L of amikacin group ( t = 9.442,8.432,9.993,P < 0.01 ).At 30 mg/L of amikacin group, the APTT (80.5 ±6.8) s and CT ( 857 ± 66) s were significantly higher than those [(33.0 ± 3.6) s and (447 ± 35 ) s] at 0 mg/L of amikacin group ( t = 11.312, 13.211, P < 0.01 ). There was a negative correlation between amikacin concentration and maximial ratio of platelet aggregation ( r = - 0.832, P < 0.05 ), but a positive correlation between amikacin concentration and inhibitory rates of platelet aggregation ( r = 0.939, P <0.05) was observed, as well as APTT (r >0.870, P<0.05).At 30 mg/L, 91 mg/L, and 910 mg/L of amikacin groups, the P-selectin and Fg-R expression were remarkably inhibited with a dose-dependent manner, the CT was notably enhanced [Fwithin subjects =21.44, 26.24, ( >29.81 ), P <0.01].At 0 mg/L,30 mg/L, 91 mg/L and 910 mg/L of amikacin groups, the PT values were ( 14.7 ± 1.9) s, ( 15.2 ± 1.7) s,(15.6±1.5) s and (22.1 ±2.1) s, respectively (F=8.21,P<0.05), but there was no markeddifference for the levels of GP Ⅱ b/Ⅲ a, TT, Fg and plasma Ca2+ among the four groups ( P > 0.05 ).After 30 minutes of amikacin treatment, the maximial ratio of platelet aggregation (51.6 ± 10.1)%, the expression levels of P-selectin (6.8 ± 1.8) % and Fg-R ( 20.1 ± 5.8 ) % were significantly lower than those [(66.8 ± 11.4)%, ( 10.9 ±3.1 )% and (28.5 ±7.4)%] before amikacin treatment, but APTT (49.8 ±5.9) s and CT (660 ±59) s were remarkably higher than those [(26.9 ±3.8) s and (410 ±45) s] before amikacin treatment, respectively ( t = 5.456,8.875,7.423,10.012,11.322, P < 0.01 ), while the GP Ⅱ b/Ⅲ a expression, PT and Ca2+ concentration had no significant changes ( P > 0.05).Conclusions There are inhibitory effects of amikacin on platelet aggregation mainly through the inhibition of both fibrinogen receptor activation and secretion reaction of activated platelet. Amikacin may also inhibit pathway of coagulation system factor to prevent blood coagulation.Therefore, risk of hemorrhage may be investigated in the patients with amikacin for anti-infection treatment.
9.Effects of Anxin Granules on blood lipid and ultrastructure of aorta in rabbits with dyslipidemia

OBJECTIVE: To study the effects of Anxin Granules on dyslipidemia in rabbits caused by high fat plus high cholesterol diet. METHODS: Thirty-two healthy New Zealand male white rabbits were randomly divided into 4 groups: normal control group, untreated group, Zhibituo Tablet-treated group and Anxin Granule-treated group. Rabbits in the normal control group were fed with regular chow, while rabbits in the other three groups were fed with high fat plus high cholesterol diet. Zhibituo Tablets and Anxin Granules were administered to the rabbits in Zhibituo Tablet-treated group and Anxin Granule-treated group at a daily oral dose respectively. At the end of the 10th week, the levels of serum total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), apolipoprotein A(1) (ApoA1) and apolipoprotein B (ApoB) were tested in each group, and the ultrastructures of the aorta were also observed by an electron microscope. RESULTS: Anxin Granules could reduce the levels of TC, TG, LDL-C and ApoB. The results observed by electron microscope showed that, as compared with the untreated group and the Zhibituo Tablet-treated group, the atherosclerosis of aorta in the Anxin Granule-treated group was lighter. And it was found that there were few lipid droplet vacuoles in cytoplasm of the endothelial cells, and various cell organs and elastic membrane were existed, but no lipid droplet vacuoles in cytoplasm of the medial smooth muscle cells. CONCLUSION: Anxin Granules can regulate the metabolism of blood lipid and inhibit the formation of atherosclerosis caused by hyperlipidemia in rabbits.
10.Umbilical cord blood mesenchymal stem cell transplantation for the treatment of renal anemia:Hot spots and issues

BACKGROUND:Umbilical cord blood as a source of hematopoietic stem cel s has become a hot topic. Adjuvant therapy of umbilical cord blood transfusion has been used in China to correct renal anemia of uremia patients. OBJECTIVE:To evaluate the efficacy and safety of umbilical cord blood mesenchymal stem cel s in the treatment of renal anemia, and to compare with the effects of different sources of mesenchymal stem cel s with different transplantation methods on the treatment of kidney diseases. METHODS:A retrospective analysis was conducted on six renal anemia patients in the Department of Outpatient, Henan Red Cross Blood Center between January 2010 and December 2012. The experiment was approved by Medical Ethics Committee, six patients were informed consent for treatment programs, and parturients and their families have signed the informed consent. The newborns umbilical cord blood of 80-140 mL were col ected with closed sterile plastic blood bags, and then the separated umbilical cord blood mesenchymal stem cel s were transfused into the renal anemia patients through the superficial vein in the back of the hand with the number of≥1×108/copy, two copies per time, and re-transfused after 4 days, a total of three times. The blood hematocrit, hemoglobin, red blood cel s in urine, renal blood flow changes were observed before and after treatment. RESULTS AND CONCLUSION:The hemoglobin, blood hematocrit, red blood cel s in urine and renal blood flow were significantly increased before and after treatment (P<0.05). Transfusion of multiple copies of umbilical cord blood mesenchymal stem cel s through the superficial vein in the back of the hand is convenient and safe, which is considered as a new method for the treatment of renal anemia. But there are certain limitation in the data and the conclusion as the clinical research was designed as self-control, so further confirm is needed.
