1.Early surgical treatment for injury of posteromedial structure of the knee

[Objective]To discuss the postoperative effect of the posteromedial structure injury (medial collateral ligament,MCL,posterior oblique ligament,POL) of the knee.[Method]Fourteen cases of complex posteromedial structure injury were treated from January 2002 to October 2007,twelve of which combined with ACL injury and two combined with PCL injury.Classification of posteromedial structure injury:typeⅠ,8 cases,ruptured from the femur with small bone fragment;type Ⅱ,2 cases,ruptured from the femur without bone fragment;typeⅢ,2 cases,the structure were torn from the body area,and type Ⅳ,2 cases,ruptured from the tibia.The injury were repaired by different techniques according to the classification.Eight cases applied star-shaped steel plate,four with GⅡ anchor screws(2 attached to the femur and 2 to the tibia adhesion respectively),and 2 cases with end-to-end suture.The ruptured ACL and PCL were reconstructed by arthroscopy.Autologous semitendinosus and gracilis were used for 8 ACL reconstruction and B-T-B technique for the other two ACL reconstruction.Two ruptured PCL were reconstructed with allograft of Achilles tendon.[Result]All cases were followed up for twelve months at average(6-18 months) except two cases only for four months.The functional score of the knees were improved from 56.7(50-60) preoperatively to 81.2(74-94) postoperatively in average valued by Lysholm system.Ten cases received normal knee flexion and extension range after repair of the posteromedial structure while 2 cases with restricted 5 degree extension.Nine cases received excellent result in lateral stress test while 2 cases with "+" and 1 case with "++" at zero angle.[Conclusion]Early repaire for the posteromedial structure injury will deliver excellent knee stability.
2.Methylation and aberrant expression of Wnt antagonist SFRP2 in gastric cancer

AIM:To determine the methylation and expression of Wnt antagonist secreted Frizzled-related protein-2(SFRP2) in gastric cancer and to explore the role of SFRP2 in gastric carcinogenesis.METHODS:Methylation status of SFRP2 was detected by methylation-specific PCR(MSP).Real-time PCR was used to determine the expression of SFRP2 in gastric cancers and matched cancer adjacent normal tissues.SFRP2 expression in gastric cancer cell lines was determined by reverse-transcriptional PCR(RT-PCR) and Western blotting.RESULTS:SFRP2 methylation was found in 26(65%) gastric cancers and in 3(7.5%) matched cancer adjacent normal tissues.The frequency of methylation for SFRP2 was significantly higher in gastric cancers than that in matched normal tissues(x2=28.614,P
3.Clinical study of continuous video EEG monitoring in patients with acute cerebral infarction

Objective To investigate the effect of continuous video EEG monitoring on acute cerebral infarction comorbid epileptiform discharges and clinical seizures,and the relationship between cerebral infarction site and seizures.Methods Of the 337 patients continuously video EEG monitored in NICU at our department,259 were included for this study according to our inclusion and exclusion criteria.We recorded patients' epileptiform discharges,type and time of clinical onset,detailed CT and (or) MRI display,cerebral infarction site,and demographic data including admission age,gender,disease history and NIHSS score.Results There were 227/259 (88%) cases showed abnormal discharge in EEG,including 94 with side hemispheric slow wave,99 with local changes,34 with slow wave combined with epileptiform discharges.Early seizures was found in 12 cases,including 3 cases with seizure-induced cerebral infarction,9 cases with attack within one week after cerebral infarction.Ten in the 12 patients had tonic-clonic seizures,and the other 2 cases hadlocal seizures.Epileptiform discharges in EEG was found in 9 cases of the 12 patients.Clinical seizures were not observed in 25 patients with epileptiform discharges in EEG.Results on the relationship between cerebral infarction site and epileptiform discharges showed that incidence of epilepsy in patients with watershed infarction was significantly higher than other types of cerebral infarction (29.0% vs 13.4% and 9.0%,P =0.03,0.01 ).Conclusion Continuous video EEG monitoring is capable of detecting early epilepsy and epileptiform discharges.It can provide theoretical basis for the prevention and treatment of epilepsy.
4.Protective effects of aldioxa tablets on aspirin-induced gastric mucosal lesions in rats

Objective To study the protective effects of aldioxa tablets on aspirin-induced gastric mucosal lesions in rat model. Methods Sixty healthy male Wistar rats were randomly divided into five groups: Ⅰ (injury group), Ⅱ (control group), Ⅲ (sucralfate protective group), Ⅳ (aluminium hydroxide protective group), and Ⅴ (aldioxa protective group). The three protective groups were treated with sucralfate, aluminium hydroxide and aldioxa tablets respectively before gastric mucosal injury was induced. Then the ulcer index (UI), epithelial damage scoring (EDS) were measured, and the pathological changes on histological sections and ultrastructural sections of gastric mucosa were assessed under microscope or electron microscope. Results The data of group Ⅰ, Ⅱ, Ⅲ, Ⅳ, Ⅴ obtained were as follow: ulcer index: 42.13±6.22, 3.13±1.46, 8.63±3.48, 18.00±6.16, 8.00±3.17, respectively; epithelial damage scoring: 3.67±0.49, 1.25±0.45, 1.41±0.51, 2.42±0.79, 1.50±0.52, respectively. In comparison with injury group, the ulcer index and epithelial damage scoring of gastric mucosa in aldioxa protective group were significantly decreased. Conclusions The results revealed that aldioxa tablets had a significant protection effect on rats with acute gastric mucosal injury induced by aspirin.
5.Simultaneous Determination of 6 Kinds of Components in Yindan Pinggan Capsules by HPLC

OBJECTIVE:To develop a method for simultaneous determination of chlorogenic acid,geniposide,gentiopicro-side,ferulic acid,baicalin and ammonium glycyrrhizinate in Yindan pinggan capsule. METHODS:HPLC method was adopted. The separation was performed on Wonda SilTM-C18 column with mobile phase consisting of acetonitrile-0.4% phosphoric acid solu-tion(gradient elution)at the flow rate of 0.8 mL/min. The detection wavelength were set at 325 nm(chlorogenic acid,ferulic ac-id),250 nm (geniposide,ammonium glycyrrhizinate) and 275 nm (gentiopicroside,baicalin). The column temperature was 30 ℃. RESULTS:The linear ranges were 0.087-3.480 μg for chlorogenic acid(r=0.9998),0.201-8.040 μg for geniposide(r=0.9997),0.200-8.000 μg for gentiopicroside(r=0.9995),0.016-0.640 μg for ferulic acid(r=0.9999),0.105-4.200 μg for ba-icalin (r=0.9999) and 0.028-1.120 μg for ammonium glycyrrhizinate (r=0.9995),respectively. The limits of quantitation were 1.31,0.75,1.14,1.25,0.94,0.98 ng,and the limits of detection were 0.87,0.67,0.96,0.93,0.60,0.88 ng,respectively. RSDs of precision,stability and reproducibility tests were lower than 2.0%;recoveries were 99.9%-101.9%(RSD=0.7%,n=6), 98.7%-100.9%(RSD=0.9%,n=6),98.1%-101.5%(RSD=1.4%,n=6),98.5%-101.3%(RSD=1.3%,n=6),98.5%-101.7%(RSD=1.2%,n=6),98.2%-101.4%(RSD=1.2%,n=6),respectively. CONCLUSIONS:The method is simple and accurate , can be used for simultaneous determination of chlorogenic acid,geniposide,gentiopicroside,ferulic acid,baicalin and ammonium glycyrrhizinate in Yindan pinggan capsule.
6.Study on Isolation and Purification Technology of Active Components of Senecio cannabifolius with Macro-porous Adsorption Resin

OBJECTIVE:To optimize the isolation and purification technology of active components of Senecio cannabifolius with macroporous adsorption resin. METHODS:The type of macroporous adsorption resin was selected with static adsorption using adsorption capacity and resolution rate of chlorogenic acid and hyperoside as index. The isolation and purification condition was op-timized with dynamic adsorption using the elution volume of chlorogenic acid and hyperoside as index,such as maximal sample size,rinse water quantity,volume fraction and collective volume of eluant ethanol. RESULTS:Among 7 kinds of resin,HPD100 had the best purification property;the optimal purification technology was as follows as mass concentration of sample 6 mg/ml,the speed of sample loading 2 ml/min,maximal sample size 3 times of column volume(BV), rinse water quantity of 2.5 BV,60%ethanol as eluting reagent. The contents of chlorogenic acid and hyperoside were increased from 0.90,0.18 mg/g to 7.26,1.04 mg/g after purification. RSD of each index were all ≤3.0%(n=3) in validation test. CONCLUSIONS:The isolation and purification technology of active components of S. cannabifolius with HPD100 macroporous adsorption resin is stable and effective.
7.Randomized Controlled Trail of Pingchuan Cataplasm Combined with Hyperthermia Therapy for Stable Chronic Obstructive Pulmonary Disease with Qi Deficiency Syndrome of Both Lung and Spleen

Objective To investigate the clinical efficacy of Pingchuan cataplasm combined with hyperthermia therapy for stable chronic obstructive pulmonary disease (COPD) with qi deficiency syndrome of both lung and spleen. Methods Sixty patients of stable COPD with TCM syndrome of qi deficiency of both lung and spleen were selected and randomized into treatment group and control group, 30 cases in each group. Based on conventional treatment, the treatment group was treated by Pingchuan cataplasm combined with hyperthermia, and the control group was given analog stick applicator without additional heat treatment. After 6 weeks of treatment, the TCM symptom score and the time of the first exacerbation after treatment were observed. Results Compared with the control group, Pingchuan cataplasm can reduce the patients’ TCM symptom score (F=9.843, P=0.003), and extend the interval of acute exacerbation after treatment (F=4.451, P=0.040). Conclusion Pingchuan cataplasm combined with hyperthermia applicator can improve symptoms and reduce the frequency of acute exacerbation, thus can treat and prevent the acute exacerbation of COPD.
8.Five Pathogen Mixed Infection in STDs of Female Genitourinary Tract:Analysis and Status

OBJECTIVE To study the status of five pathogen mixed infection in sexually transmitted diseases(STDs) and analyze the clinical meaning.METHODS We detected five common pathogens by fluorescence polymerase chain reaction,which were Neisseria gonorrhoeae(NG),Chlamydia trachomatis(CT),Ureaplasma urealyticum(Uu),human papilloma virus(HPV),Candida albicans(Ca),and herpes simplex virus.RESULTS We analyzed the status of infection among 4 601 patients,got 279 mixed infection cases,accounted for 6.1% in all cases;and the population with ages from 21 to 40 years was accounted for 89.6%. CONCLUSIONS Fluorescence polymerase chain reaction is a simple,rapid,high sensibility technique for quantitation testing of STDs pathogens,and we should pay great heed to its effective control.
9.The treatment of the fissuration of pancreatojejunal stoma by the bridge-crossing internal drainage

Objective The purpose of this study was to discuss the therapies for hemorrage caused by the fissuration of pancreatojejunal stoma and pancreatic leakage after pancreatoduodenectomy.Methods After three cases of pancreatoduodenectomy,the disruptions of pancreatojejunal stoma resulted in serious pancreatic leakage and the hemorrage in abdominal cavity.During all the second operations,the drainage-tube insertions into the main pancreatic ducts were used to lead the pancreatic juice into the neighboring loop of jejunum.Results Afer the operations,the supportive treatment,continuous irrigation of peritoneal cavity and pancreatic enzyme inhabition were given to the patients of these cases and all of the patients were successfully cured.Conclusions The bridge-crossing internal drainage which inserts drainage-tube into the main pancreatic duct was a convenient and effective therapy and method to rescue the hemorrage caused by the fissuration of pancreatojejunal stoma and pancreatic leakage after pancreatoduodenectomy.While the patients' lives were saved,their functions of pancreas were preserved and the qualities of life were improved after the operations.
10.Study of Reversing Invasion of Human Gastric Cancer Cell Line BGC-823 by Targeting Angiopoietin-1 Using siRNA

Objective To knock down angiopoietin-1 expression in human gastric cancer cell line BGC-823 and to observe its effect of reversing tumor invasion. Methods siRNA sequence fragments was designed to target angiopoietin-1 and transferred into human gastric cancer cell line BGC-823. RT-PCR was used to assess the transcription level of angio-poietin-1 mRNA, then western blot and immunofluorescence were used to examine the expression level of three invasion-as-sociated proteins include integrinβ1, CD44V6 and Ang-1. Cell adhesion ability was evaluated by cell adhesion assay and cell invation was determined by matrigel and transwell plastic dual-chamber culture system. Results Ang-1 mRNA was knocked down by siRNA showed by RT-PCR. The expression of integrinβ1, CD44V6 and Ang-1 were significantly lower than control group(P<0.05), so did the cellular adhesion and invasion abilities(P<0.05). Conclusion Knocking down angiopoietin-1 by siRNA can reverse invasion of human gastric cancer cell line BGC-823 and may provide new ideas and reference for gene therapy of gastric cancer in the future.
