1.Oral erythromycin for the prevention and treatment of feeding intolerance in preterm infants-meta analysis of randomized controlled trials

Objective To evaluate the efficacy and safety of oral erythromycin in the prevention and treatment of feeding intolerance in preterm infants. Methods The Cochrane Library,PubMed,EMBASE,CBMdise,VIP,WartFang and CNKl were searched up to the year of 2008.Randomized controlled trials (RCT) of erythmmycin for feeding intolerance in preterm infants were included.According to the dosage (low-dose,3-15 mg/kg and high dose,>15 mg/kg)and gestational age(≤32 weeks vs>32 weeks),all infants were divided into several subgroups.Meta-analysis was performed with the Cochrane Collaboration's software RevMan. Results Nine RCrs involving 542 premature infants were included.(1)In preventive studies,low-dose erythromycin could significantly decrease the duration of total parenteral nutrition compared with the high-dose erythmmycin (WMD=-2.99,95%CI:-3.99--1.98).(2)Intreatment studies,highdose erythromycin could significantly decrease the duration of total parenteral nutrition (WMD=-7.06.95%CI:-7.91--6.20,P<0.01)and hospital stay (WMD=-8.10,95%CI:-14.02--2.18,P=0.007)compared with the placebo when gestational age≤32 weeks. Erythromydn could decrease the incidence of iaundice(RR=0.36,95%CI:0.21-0.63,P=0.0003)which might be the effect of bigh-dose erytromycin. Conclusions Compared with placebo,oral erythromycin of different dosage might help to achieve full enteral feeding,shorten the duration of parenteral nutrition and hospital stay,and reduce the associated cholestasis jaundice of preterm infants with different gestational age respectively.
2.Study on effect and mechanism of Shenshaoruangan Decoction on liver fibrosis in rats

Objective To study the effect and mechanism of Shenshaoruangan Decoction on liver fibrosis in rats.Methods Seventy-two male Wistar rats were selected and normally fed for 1 week.Then they divided into the control group(n=12) and model constructing group(n=60).The model constructing group was subcutaneously and intraperitoneally injected by 40% CCl4 olive oil for constructing fibrosis rat model.After successfully constructing model,which was divided into the model group(equivalent normal saline gavage),positive control group(colchicines 0.154 mg · kg-1 · d-1),low-dose group(2 mL per 100 g body mass,containing Shenshaoruangan Decoction crude drug 0.23 g/mL),medium-dose group(2 mL per 100 g body mass,containing Shenshaoruangan Decoction crude drug 0.46 g/mL) and high-dose group(100 g/2 mL,containing Shenshaoruangan Decoction crude drug 0.69 g/mL),12 cases in each group.The differences of related indicators rat hepatic fibrosis after 8-weeks treatment were observed.Results The control group was the stage 0 standard,the liver fibrosis degree in the positive control group was significantly better than that in the model group(P<0.05);the liver fibrosis degree in the low-dose group,medium-dose group and high-dose group was gradually alleviated;the liver fibrosis degree in the high-dose group was significantly slighter than that in the model group and-dose group(P<0.05).The detection values of PC-Ⅲ,LN,Ⅳ-C and HA in the model group were significantly higher than those in the control group(P<0.05),and the detection values of PC-Ⅲ,LN,Ⅳ-C and HA in the positive control group,lowdose group,medium-dose group and high-dose group were significantly lower than those in the model group(P<0.05).Serum SOD detection value of the model group was significantly lower than that in the control group(P<0.05),the serum MDA level in the model group was significantly higher than that in the control group(P<0.05);serum SOD level in the positive control group,low dose group,medium-dose group and high-dose group was significantly higher than that in the model group(P<0.05),while serum MDA level in those group was significantly lower than that in the model group(P<0.05).Conclusion Shenshaoruangan Decoction can effectively alleviate the liver fibrosis degree,and its mechanism is to reduce the levels of liver fibrosis indicators,inhibit the prodmotion of peroxides and alleviate the damage of free radicals on liver cells.
3.Clinical application of ERA resilient attachment in restoration of Kennedy Ⅰ dentition defect

Objective To investigate the effect of clinical application of ERA resilient attachment on Kennedy Ⅰ dentition defect. Methods Twenty-eight Kennedy Ⅰ dentition defect cases were restored by ERA resilient attachment dentures. After the examination of the function of the dentures, oral tissue, X-ray, the satisfaction to the dentures was evaluated. Results After the follow-up of 6 to 32 months, all the patients were satisfied with the dentures in esthetic, retention, normal function and healthy peri-odontium. Conclusion ERA resilient attachment is an esthetieal, economical and reliable approach to Kennedy Ⅰ dentition defect restoration.
4.Influence of four kinds of thread design on the primary stability of immediately loaded dental implants

Objective To discuss the effects of implants of different thread face angles on the primary stability of immediately loaded implant using the 3-dimensional finite element analysis with the models of the immediately loaded implants. Methods Using the commercial code of Pro/E software, Hypermesh software, and ABAQUS software we created 3-dimensional finite element models. The micromotions of the finite element models with different screw face angles (V-shape, buttress, square-shape and inverse buttress) were computed with the ABAQUS software. Results Concerning different thread face angles, the micromotion of buttress implant was the minimum and the micromotion of inverse buttress implant was the maximum with vertically loading; the micromotion of inverse buttress implant was the minimum and the micromotion of buttress implant was the maximum with horizontal loading. Conclusions Different screw-types have great influence on vertical interfacial micromotions but little influence on horizontal interfacial micromotions. There are two angles which are formed by top /bottom edge and the implants. The larger are the angles, the smaller are the vertical interfacial micromotions, but the weaker of the strength. Thus in designing the screw-type implants, we should consider the angles of thread faces and the strength.
5.Relationship Between Serum Resistin and Carotid Intima Media Thickness in Patients With Essential Hypertension

Objective:To determine the relationship between serum resistin levels and carotid intima media(IMT)thickness in patients with essential hypertension. Methods:This study consisted of 272 patients with essential hypertension. The patients were divided into three groups according to their serum resistin levels. Group 1,n=91,serum resistin level 1.233-3.701 ng/ml;Group 2,n=91,serum resistin level 3.728-8.777 ng/ml;and Group 3,n=90,serum resistin level 8.809-28.658 ng/ml. Results:The carotid IMT and maximum carotid IMT of Group 3 were the highest in three groups.(P<0.05).As shown in multivariate analysis for factors affecting carotid IMT,serum resistin level(β=0.220,t=5.793,P=0.000)was independently associated with the carotid IMT after controlling the age,blood glucose,uric acid,low-density lipoprotein cholesterol,systolic blood pressure and diastolic blood pressure. Serum resistin level(β=0.189,t=4.733,P=0.000)was independently associated with the maximum carotid IMT after controlling the age,blood glucose,body mass index,diastolic blood pressure,diabetes mellitus,high sensitivity C reactive protein,total cholesterol and triglyceride. Conclusion:Serum resistin was independently associated with the increased carotid IMT in essential hypertension patients.
6.Cloning, expression and functional analysis of the dhbC gene from the siderophore producing bacterium Bacillus subtilis CAS15.

We amplified dhbC gene from the siderophore producing bacterium CAS15 by PCR. After ligated the PCR product to pMD18-T vector and then sequenced, we obtained a 1197 bp fragment. The blast result showed that the nucleotide acids of dhbC gene (Accession No. FJ194456) of CAS15 shared 99.7% identity with that of dhbC gene of Bacillus subtilis (GenBank Accession No. Z99120), and was predicted to encode a 43.8 kD polypeptide with 398 amino acid residues. We cloned the dhbC gene into expression vector pET-30a(+) and then transformed into Escherichia coli BL21(DE3) via calcium chloride transformation method, and obtained the recombinant E. coli BL21(DE3)/pET-30a-dhbC. Induced by 1 mmol/L IPTG the fusion protein 6His-DhbC, a 48.8 kD polypeptide was successfully expressed mainly in soluble form in E. coli BL21(DE3), and the amount reached highest at 30 degrees C for 4 h. According to the N-terminal fusion 6 His-tag, we purified the recombinant polypeptide by Ni2+ metal affinity chromatography and finally identified it by Western blotting. The result indicated that the recombinant DhbC had the antigenicity to rabbit anti-his-tag polyclonal antibody, which provides the basis for the study of practical utilization in production and the biocontrol mechanism of B. subtilis. Finally, we deleted dhbC gene by gene knockout and then retransformed it into the dhbC gene-delected mutant, which confirmed that dhbC gene play an important role in siderophore biosynthesis.

7.Quality of Care for People with Disabilities and Related Factors in Hubei, China

Objective To investigate the current situation of quality of care for people with disabilities and the related factors. Methods In November, 2015, 399 disabled persons from five special service institutions in Hubei, China were conveniently sampled, and investigated with the Chinese version of Quality of Care and Support (QOCS) for people with disability scale and demographic questionnaire. Results The total score of QOCS was (38.11±6.24), and the proportion of total score in the dimensions of caring provision, caring environment and caring information were more than 70%. The score of QOCS was various with the age, domicile, employment, medical insurance, monthly household expenditure and expenditure for food of the people with disabilities, and the age (β=0.06, P<0.01) and medical insurance (β=-0.850, P<0.001) were the independent factors related with the score of QOCS. Conclusion People with disabilities self-reportedly satis-fied in the quality of care in Hubei, and it can be improved from the increase of medical insurance level.
8.The levels of NO decrease induced apoptosis in human placental trophoblast cells through oxidative stress

Aim To investigate the possible mecha-nisms of the levels of NO decrease induced apoptosis in human placental trophoblast cells. Methods Human placental trophoblast cells ( HTR-8 ) were cultured in 5 ml DMEM-F12 culture medium with 37℃ 5% CO2 . Then, the old culture medium was discarded and re-placed with 10,100,500,1 000 μmol·L-1 L-NAME, and the group without L-NAME was set as the control group, cultured for 48h. The effects of L-NAME on the survival of cells were detected by methylthiazolyldiphe-nyl tetrazolium bromide ( MTT); the content of NO in cells was tested by nitrate reductive enzymatic;trans-mission electron microscopy, flow cytometry analysis and Annexin-V FITC dyeing were used to test the effects of L-NAME on apoptosis in HTR-8 cells;restore Fe3+ colorimetric assay was applied for detection of to-tal antioxidant capacity ( T-AOC ) , xanthine oxidase for detection of superoxide dismutase ( SOD) activity, and thiobarbituric acid colorimetry for determination of content of MDA. Results Compared with the control group, the survival rate of HTR-8 cells and the levels of NO in 100,500,1 000 μmol·L-1 L-NAME group were significantly reduced(P<0.05,P<0.01). Flow analysis and Annexin-V FITC staining showed that L-NAME could induce cell apoptosis in a dose-dependent manner. The number of cell apoptosis was negatively correlated with the content of NO ( r = -0.5210 ) in HTR-8 cells. Transmission electron microscopy results showed that compared with the control group, the ex-perimental group's cell nucleus shape was irregular, nuclear pyknosis in irregular shape, the chromatin ag-glutination or side the collection, mitochondrial swell-ing or enrichment, crest fracture or dissolved, even vanished, forming the vacuole, especially in 100 μmol ·L-1 L-NAME group, the apoptotic bodies obviously appeared. At the same time, T-AOC, SOD levels in HTR-8 cells decreased ( P <0.05 ) , and the MDA content increased ( P<0.05 ) . The number of cell ap-optosis was negatively correlated with the level of T-AOC ( r= -0.3212 ) , SOD ( r= -0.2779 ) in HTR-8 cells , while positively correlated with the content of MDA(r=0.2807). Conclusion Oxidative stress may play an important role in the levels of NO decrease in-duced apoptosis in human placental trophoblast cells.
9.Expression of serum platelet factor 4 in high-grade intraepithelial neoplasia of gastric cardia and its clinical significance

Objective To investigate the expression of serum platelet factor 4 (CXCL4) in high-grade intraepithelial neoplasia (HGIN) of gastric cardia and its clinical significance. Methods Sixty-four HGIN patients and 29 patients with advanced cardiac cancer (ACC) from January 2010 to December 2011 in the Fourth Hospital of Hebei Medical University were selected as the research objects. Forty healthy people treated with endoscope screening were chosen as the control group. Enzyme linked immunosorbent assay (ELISA) was used to detect the expression of CXCL4 in serum. The expression differences were compared by using variance analysis and LSD-t. Results The CXCL4 detection showed that the expression levels of CXCL4 were(0.75±0.37)ng/ml,(0.87±0.33)ng/ml,(1.13±0.51)ng/ml respectively in HGIN group,ACC group and the control group (F = 10.77, P < 0.05). The CXCL4 expression levels in HGIN and ACC group were lower than that in the control group (t = 4.80, t = 2.67, both P < 0.05). There was no difference between the CXCL4 expression levels in HGIN and ACC group(t =1.27,P >0.05).Conclusions The expression level of CXCL4 in HGIN patients with gastric cardia is significantly decreased. CXCL4 may be related with the occurrence and angiogenesis of gastric cardia cancer, which can be considered as an important serum marker for early screening of gastric cardia cancer.
10.Role of ERO1αand its DNA methylation in homocysteine-induced inhibition of hepatocyte proliferation

Aim To explore the role of ERO1 αand its DNA methylation in homocysteine (Hcy)-induced in-hibition of hepatocytes proliferation.Methods The hepatocytes stimulated with 0 μmol·L -1 Hcy were set as the normal group (NC group)and the hepatocytes stimulated with 1 00 μmol·L -1 Hcy as the experimen-tal group (Hcy group).Methyl thiazolyl tetrazolium (MTT)reduction assay was used to reflect the prolifer-ation of the hepatocytes;qRT-PCR and Western blot were used to detect the mRNA and protein levels of ERO1 α;the expression of green fluorescence protein was observed in hepatocytes after the recombinant plas-mid of ERO1 α was constructed,which was used to confirm if the recombinant plasmid into hepatocytes was successful,then the mRNA and protein levels of ERO1 αwere assayed and the proliferation of the hepa-tocytes was also detected;ntMSP was used to detect the change of ERO1 αDNA methylation.Results The mRNA and protein levels of ERO1 αwere decreased in Hcy group compared with NC group,and the prolifera-tion activity of hepatocytes in Hcy group was de-creased.Sequencing result showed that the recombi-nant plasmid of ERO1 αwas constructed successfully. QRT-PCR and Western blot revealed that ERO1 αwas overexpressed. The result of MTT suggested that ERO1 αoverexpression restored hepatocyte proliferation inhibited by Hcy.Hcy caused ERO1 αDNA hyperm-ethylation.Conclusions Hcy inhibits hepatocyte pro-liferation by downregulating the expression of ERO1 α, and methylation of ERO1 αpromoter may play a role in this process.
