Objective To evaluate the feasibility of endoscopic resection and closure for non-intraluminal gastric stromal tumors originating from the muscularis propria layer. Methods Included in the study were 46 patients with gastric submucosal lesions originating from the muscularis propria layer, detected by gastroscopy and endoscopic ultrasonography. The lesions were removed by endoscopic resection and closure, which were further diagnosed as stromal tumor by means of pathologic and immunohistochemical examinations. The patients were followed up with endoscopy for evaluation of therapeutic effect and complications. Results All lesions were successfully removed, with serosa layer remained in 2 cases and full layer resection in other 44, which were all closed by endoscopic clips.Combination managements of acid suppression, gastrointestinal decompression and intravenous antibiotics were applied in all patients.Pathology reports confirmed complete resection of all lesions, with 0.5 to 3.7 cm in diameter.Normal diet was restored in 44 patients 48 ~ 72 h after the procedure.Pneumoperitoneum and focal peritonitis occurred in 2 cases, one of which underwent rupture and was clamped again. The 2 patients recovered after 10-12 days of conservative treatments.Follow-up endoscopy revealed white ulcerous scar in all cases. Conclusion Endoscopic resection and closure therapy is a safe, economic and less invasive treatment for non-intraluminal gastric stromal tumors originating from the muscularis propria layer.

Objective To analyze the CT and MR imaging features of intracranial teratomas to improve the diagnostic accuracy. Methods Twenty patients with intracranial teratomas, admitted to and performed surgery in our hospital from January 2010 and January 2014,were chosen;the teratoma was confirmed by histopathology. The CT and MR imaging features, clinical and pathology materials of these patients were retrospectively analyzed. Results Twelve immature teratomas,7 mature teratomas and one teratoma with malignant transformation were noted;18 patients were in the central line area and 2 were not.In the 9 patients performed CT scan,7 showed heterogeneous mass;peritumoral calcification was more common than intratumoral calcification, and it turned out to be round with diameter less than 1 cm,while intratumoral calcification was multifocally strip and patchy. In the 17 patients performed MR imaging,14 showed heterogeneous signal and fatty signal was detected in 3;enhanced MR imaging showed that 6 presented nodular enhancement,3 presented multilocular enhancement markedly, patchy enhancement showed in 2,and one showed marked enhancement in mural nodule and cystic wall.In 8 patients performed diffusion-weighted imaging,7 displayed hypointense to isointense and one demonstrated isointense to hyperintense. Conclusions Intracranial teratoma has some characteristic imaging features,so by combining clinical materials,it should be considered when fatty component is detected in a heterogeneous intensity tumor with multiple cystic change, calcification and a hypointense to isointense signal on DWI;however, pathology is needed to make a definite diagnosis.

Objective To investigate the association between KRAS gene mutations and clinicopathological characteristics and prognosis of colorectal cancer (CRC) patients. Methods The retrospective casecontrol study was conducted. The clinicophathological data of 315 patients who underwent radical resection of CRC in the Yangpu Hospital Affiliated to Tongji University between January 2007 and July 2011 were collected.Nextgeneration sequencing was performed to identify KRAS gene mutations from surgical specimens.Observation indicators:(1) detection of KRAS gene;(2) association between KRAS gene mutations and clinicopathological characteristics of CRC patients;(3) follow-up and survival situations;(4) multivariate analysis of KRAS gene mutations in the prognosis of CRC patients.Follow-up using outpatient examination and telephone interview was performed to detect postoperative overall survival up to August 2016.Comparisons of count data were analyzed using the chi-square test.Measurement data with skewed distribution were described as M (interquartile range),and comparison between groups was analyzed using the nonparametric test. The survival rate was calculated using the Kaplan-Meier method, and survival was compared using the Log-rank test. The multivariate analysis was done using the COX regression model. Results (1) Detection of KRAS gene:all the 315 patients finished gene detection of surgical specimens, including 172 in wide-type mutations and 143 in mutant-type mutations (mutations at codon 12 and 13 of KRAS exon 2 and other mutant points were respectively detected in 80,24 and 40 patients, and 1 patient had simultaneous mutations at codon 12 and 13 of KRAS exon 2;missense and nonsense mutations were respectively detected in 141 and 2 patients). The major point mutations were at p.G12D and p.G13D.(2) Association between KRAS gene mutations and clinicophathological characteristics of CRC patients:tumors located in the proximal colon, distal colon and rectum were respectively detected in 34,48,90 patients with wild-type mutation and in 44,27,72 patients with mutant-type mutation, with a statistically significant difference (x2 =0.038,P<0.05).(3) Follow-up and survival situation:315 patients were followed up for 3-115 months, with a median time of 78 months. The postoperative overall survival rate was 41.0% in 172 patients with wild-type KRAS mutations,27.4% in 80 patients with KRAS codon 12 mutations,26.3% in 24 patients with KRAS codon 13 mutations and 48.2% in 40 patients with other KRAS mutations, showing a statistically significant difference (x2=0.040,P<0.05).(4) Multivariate analysis of KRAS gene mutations in the prognosis of CRC patients:the results of multivariate analysis showed that mutations at codon 12 of KRAS exon 2 was an independent factor affecting poor prognosis of CRC patients (Hazard ratio=1.543,95% confidence interval:1.050-2.265,P<0.05).Conclusions Most KRAS mutations of CRC patients are at codon 12 and 13 of KRAS exon 2,and the major point mutations are at p.G12D and p.G13D.KRAS gene mutations may be associated with tumor location.Mutations at codon 12 of KRAS exon 2 is an independent factor affecting poor prognosis of CRC patients.

OBJECTIVETo observe the effect of Euphorbia kansui (E. KS) alcohol extracts on urination and kidney-related expressions of mice injected with normal saline and to discuss its impact on kidney.
METHODMice intraperitoneally injected with normal saline were observed for urination and changes in kidney-related histiocytic factors of after intragastrical administration of E. KS and compared with normal mice.
RESULTE. KS alcohol extracts can promote urination of mice injected with normal saline and enhance peripheral serum creatinine, with no obvious pathological change showed in tissue sections. It had a certain effect on reducing AQP2 expression and enhancing TNF-alpha expression.
CONCLUSIONEuphorbia kansui in large dose has a remarkable effect on kidney but may be accompanied with pathological reactions to some extent, especially the dose of 1.2 g x kg(-1). The pathological reactions may be related with increased serum creatinine and TNF-alpha expression.
