1.Expectations and needs of home rehabilitation resource integration for patients with chronic diseases: a qualitative research

Objective:To understand the expectations and needs of patients with chronic diseases for the home rehabilitation resource integration and analyze the available resources and integration methods of chronic disease home rehabilitation projects so as to provide a reference for establishing a complete chronic disease home rehabilitation service system.Methods:The research adopted the phenomenological research method in qualitative research. From October to December 2019, the purpose sampling was used to select 16 chronic disease patients with home rehabilitation who were followed up by the Chronic Disease Management Center of a comprehensive Class Ⅲ Grade A hospital in Shanghai for semi-structured in-depth interviews. The Colaizzi 7-step analysis method was used to organize the interview results and refine the theme.Results:A total of 4 themes of the expectations and needs of patients with chronic diseases for the home rehabilitation resource integration were extracted including that the effective resource for home rehabilitation of chronic diseases was limited; the level of home rehabilitation of chronic diseases needed to be improved; resources related to home rehabilitation of chronic diseases were obviously fragmented; the government and primary medical institutions should strengthen the importance of home rehabilitation of chronic diseases.Conclusions:At present, patients with chronic diseases have high expectations and needs for the home rehabilitation resource integration. The medical resources available in the region should be effectively integrated, and a resource sharing platform should be formed to improve the home rehabilitation effect of patients with chronic diseases so as to promote the overall development of chronic disease home rehabilitation projects.

The Health Environment Promotion Campaigns (HEPCs) focus on the major environmental health issues and relevant factors of concern among the general public, and promote the achievement of the national health goal. Based on the summary and analysis of the background, key indicators, specific actions in different domains of the HEPCs, this paper proposes suggestions for scientifically implementing HEPCs from five aspects, namely, formulating implementation plans, establishing pilot areas, building comprehensive service platforms, improving the health literacy of residents and strengthening the development of protection technologies and standards.

6.Usage condition of transitional care quality indicators for chronic disease in 55 hospitals in China

Objective? To investigate the application status of transitional care quality indicators for chronic disease in hospitals in China so as to provide a reference for improving transitional care quality evaluation for chronic disease. Methods? We selected nursing managers from 55 hospitals participating a national nursing conference in Shanghai in 2018 as subjects by convenience sampling. The self-designed hospital transitional care quality indicators questionnaire for chronic disease was used to investigate the usage condition of transitional care quality indicators for chronic disease in 55 hospitals. χ2 test was used to compare the utilization ratio of third-level indexes of transitional care of Class Ⅱand Class Ⅲ hospitals. Results? Among 55 hospitals, the highest usage of first-level indexes was structure index (0.813±0.164). The top 3 usage of second-level indexes included hospital readiness (0.932±0.047), patient satisfaction (0.927±0.262) and adverse event rate (0.888±0.143); the bottom 3 usage included health status of patients (0.470±0.112), external hospital extension (0.418±0.280) and connect inside and outside of hospital (0.309±0.149). In importance of second-level indexes, the top 3 usage included the adverse event rate, hospital readiness and material resources; the bottom 3 usage involved the utilization ratio of medical resources, connect inside and outside of hospital and external hospital extension. There were statistical differences in the usage rates of medical resource indexes in ClassⅡand ClassⅢ hospitals, such as readmission rate, visiting rate in emergency department, hospital stay and mortality (χ2=9.406, 5.430, 6.339, 6.339;P< 0.05). Conclusions? Among 55 hospitals of China, the transitional care quality evaluation for chronic disease is generally insufficient which is needed to be paid attention to. Hospitals needs to optimize the proportion of indexes, improve and pay more attention to the evaluation for process indicators and outcome indicators, and perfect management and monitor for transitional care.
7.Correlation between coaching leadership and psychological capital of nursing managers

Objective? To investigate the current condition and correlation of coaching leadership ability and psychological capital of nursing managers. Methods? A cross-sectional study was conducted at the National Nursing Conference on "Efficient Nursing Management and Clinical Practice in the Context of Great Health" in June 2018. Totally 217 nursing managers from 107 hospitals and 38 regions were selected by convenience sampling and investigated by the General Information Questionnaire, Coaching Leadership Perception Assessment Scale (CLPAS), and Psychological Capital Questionnaire(PCQ). Rank-based non-parametric statistical method was used to compare the scores of coaching leadership of nursing managers with different general information. Spearman correlation analysis was used to analyze the correlation between coaching leadership and psychological capital, and stratified regression analysis was used to analyze the influencing factors of coaching leadership. A total of 222 questionnaires were distributed with 217 valid ones retrieved, yielding an effective recovery rate of 98%. Results? The 217 nursing managers achieved a total score of (64.15±6.19)points from CLPAS, (90.00±9.31) points from PCQ, there was positive correlation among each dimensions in CLPAS and PCQ (P<0.01); there was good effect of the psychological capital in predicting the coaching leadership ability (P<0.01). Conclusions? The coaching leadership ability of nursing managers needs to be improved. The psychological capital of nursing managers will affect their coaching leadership ability and should be paid attention to; effective measures should be taken to improve the psychological capital of nursing managers so that it can play a greater role in the nursing team and improve the efficiency of nursing management.

This article summarizes the research progress of continuing care of cardiac rehabilitation for patients with coronary heart disease after receiving percutaneous coronary intervention, including the meaning and significance, implementation status and application effects, and provides suggestions for the development of continuing care of cardiac rehabilitation in China with attempts to facilitate the promotion of continuing care of cardiac rehabilitation.
9.Status and risk factors of postoperative hemorrhage in patients undergoing surgery

Objective To investigate the incidence and related factors of postoperative hemorrhage, so as to provide data support for preventing its occurrence and developing reasonable risk assessment tools. Methods The retrospective analysis of clinical data of 50 patients with postoperative hemorrhage after surgical operation in a 3A hospital in Shanghai from January to December 2016 was conducted. A 1:2 matching case-control study was conducted to explore the risk factors of postoperative bleeding in surgical patients by univariate and multivariate regression analysis.Results The total number of surgical operations (excluding obstetrics and gynecology, five senses) was 18942. Postoperative hemorrhage occurred in 50 cases, including 6 deaths. The incidence of postoperative hemorrhage was 0.26% and the mortality rate was 12%. The incidence of postoperative bleeding in the top four departments were gastrointestinal surgery (13 cases), hepatobiliary and pancreatic surgery (11 cases), cardiac surgery (10 cases), neurosurgery (9 cases). The statistical analysis of binary Logistic in the case group and the control group showed that postoperative albumin<35g/L, postoperative AST abnormality, postoperative ALT abnormality, postoperative hypertension were independent risk factors of postoperative bleeding.Conclusions The incidence of postoperative bleeding in this hospital is relativelylow, but the mortality is relatively high. Medical staff should pay more attention to the patients undergoing gastrointestinal, liver, pancreas, heart and brain surgery. It is also necessary to pay more attention to the blood pressure, albumin and liver function of the patients after operation,and establish the evaluation of prevention of hemorrhage, which has a certain early warning effect on the prevention of postoperative hemorrhage.

This paper shows the concept and current research situation of heart failure readmission, and transitional care providers. It focuses on eight transitional care models which can be applied to decrease the readmission for heart failure patients, and puts forward current challenges, so as to provide references for applying transitional care to reduce heart failure readmission in China.
