1.Values of blood sedimentation and procalcitonin in early infection after hip arthroplasty

Objective To investigate the diagnostic values of blood sedimentation and serum pro-calcitonin(PCT)in early infection after hip arthroplasty.Methods The clinical data of 15 patients with early infection after hip arthroplasty(infection group)and 17 patients without it(control group)in recent years were retrospectively reviewed.Preoperative and postoperative levels of blood sedimentation and serum PCT were analyzed for comparison.Results No significant differences in preoperative blood sedimentation were observed between the groups.Postoperative blood sedimentation increased in both groups,and level of the infection group was significantly higher(P <0.05).Preoperative serum PCT levels were similar be-tween the groups.Postoperative serum PCT level in the infection group elevated significantly,but the level of control group was not(P <0.05).Conclusion For early infection after hip arthroplasty,blood sedi-mentation and serum PCT are helpful indicators with certain sensitivity and specificity,and they may be commonly used for the early diagnosis of postoperative infection.
2.The application and observation of nitrous oxide inhalation in maxillofacial surgery

Objective:To observe the clinical efficacy of nitrous oxide inhalation in oral and maxillofacial surgery.Methods:1 80 pa-tients underwent maxillofacial surgery were randomly divided into 2 groups(n =90).The patients in group A were given nitrous oxide inhalation before surgery,those in group B were served as the controls.The blood pressure(BP),heart rate(HR)and oxygen saturation of blood(SpO2 )were monitored during surgery,the feelings of the patients for surgical procedures,pain VAS score and Houpt behavior rating scale were surveyed after surgery.Results:In the whole operation process,BP,HR and SpO2 of group A changed smoothly(P >0.05),BP and HR of group B increased during operation(P <0.05),SpO2 had no obvious change(P >0.05);Frankl treatment adher-ence score in group A and B was 3.66 ±0.05 and 2.91 ±0.05 respectively(P <0.05);Houpt behavior rating scale were 5.06 ±0.1 1 and 3.29 ±0.06 respectively(P <0.05).VAS score in group A was lower than that in group B(P <0.05).Mild adverse reaction was observed in 6 cases in group A.Conclusion:In the process of maxillofacial surgery,nitrous oxide inhalation has sedation,analgesia and anti-anxiety effects without severe adverse effect.
3.Clinical analysis of Frey syndrome incidence of 202 cases after parotidectomy

Clinical data of Frey syndrome of 202 cases after excision of parotid gland were analysed.Frey syndrom was observe in 22 out off 1 49 cases(1 4.7%)received block processing of sympathetic (SN)and parasympathetic (PN)nerve anastomosis and in 1 4 out off 53 (26.4%)without the block processing(P≤0.01 ).The block processing of SN and PN anastomosis is effective for the prevention of postop-erative Frey syndrome after parotidectomy.
4.Randomized controlled clinical study of modified seton therapy with counterparts drainage on high anal fistula

Objective To investigate the clinical effect of modified seton therapy with counterparts drainage on high anal fistula.Methods One hundred and forty-four patients with high anal fistula were randomly assigned to the observed group (modified seton therapy with counterparts drainage,74 cases) and the control group (the traditional high hanging line therapy,70 cases).The clinical efficacy,visual analogue scale (VAS),Wexner incontinence score,fecal incontinence quality of life index (FIQLI) score were compared.Results Observed group were cured with 40 cases,improved with 27 cases,ineffective with 7 cases,control group were cured with 25 cases,improved with 43 cases,ineffective with 2 cases,difference between two groups was statistically significant (P < 0.01).The healing time in observed group was significantly shorter than that in control group(P< 0.01).The VAS after therapy 1,7 d in observed group was significantly lower than that in control group (P < 0.05).The Wexner incontinence score,FIQLI score before therapy was no significant difference between two groups (P >0.05).The Wexner incontinence score,FIQLI-lifestyle score,FIQLI-depression/self-perception score,FIQLI-distress score after therapy 3 months in observed group was significantly higher than that before therapy and control group after therapy 3 months (P < 0.05),there was no significant difference Wexner incontinence score and FIQLI score before therapy compared with therapy 3 months in control group (P > 0.05).Conclusions Modified seton therapy with counterparts drainage may be a valid treatment for high anal fistula.The successful outcome is associated with significant improvement in quality of life.
5.Analysis of Medicine Utilization Variation of Inpatient Participated Insurance after Implementing the 2009 Edition of National Reimbursement Drug List

Objective:To understand the impact of adjusting national drug reimbursement list(NDRL) have on the benefits of patient covered by Basic Health Insurance ( BHI ) , medical insurance fund expenditure and specific drug marketing . Methods: Based on the survey data of medical service utilization situation of national inpatients participated BHI, to summarize and analyze the change of drug usage from 2009 to 2011. Results: In 2011, the drug expense of the national inpatient covered by BHI is approximately 132.7 billion yuan, increased by 37% compared to that of 2009; among which, the expense of entry drug accounts for about 76% out of the total drug use. In 2011, new increased expense on drug use is 13.5 billion, which increased to 98% compared to that of 2009. Conclusion:Adjustment of NDRL had little effect on total drug expenses increasing, but lead changes of the drug expense structure; and expense on some new entry drugs increased obviously. The focus of future adjustment of NRDL could gradually transfer from guaranteeing on basic medicine use to the promotion on clinical rational drug use.

8.Treatment of proximal and middle one-third of humeral fractures with tibial helical plate

Objective To explore the fixation method of tibial helical plate for treatment of proximal and middle one-third of humeral fractures and evaluate the clinical outcome. Methods A review was performed on 15 patients with proximal and middle one-third of humeral fractures treated by tibial helical plate from May 2004 to February 2009. There were 10 males and five females, at age range of 19-65 years ( average 38.0 years). The surgical method was summarized and the shoulder function evaluated.Results The follow-up lasted for 7-24 months (mean 13 months) , which showed bony union in all patients, with mean union time of 4.7 months. There was no implant loosening or failure. According to Neer 's criteria, the result was excellent in seven patients and good in six, with excellence rate of 86.7%.Conclusions Open reduction and internal fixation using tibial helical plate is simple and effective for treatment of proximal and middle one-third of humeral fractures.
9.Vicissitudes of Resistance to Imipenem in Gram-negative Bacilli

OBJECTIVE To investigate the vicissitudes of resistance to imipenem in Gram-negative bacilli isolated from clinical specimens.METHODS Gram-negative bacilli were isolated from various clinical specimens in our hosptial from Jan 2004 to Jan 2008.The identification and antimicrobiol susceptiblity test was determined by VITEK-AMS full automated microbiology analyzer.RESULTS A total of 6983 strains of Gram-negative bacilli were isolated,including 92 species and 2986 isolates of Enterobacteriaceae and 3961 isolates of non-fermenters,accounted for 42.8% and 56.7%,respectively.The resistant rate of Enterobacteriaceae to imipenem was lower than 1.0%.The resistant rates of Stenotrophomonas maltophilia,Burkholderia cepacia,Chryseobacterium meningosepticum,Pseudomonas aeruginosaand Acinetobacter baumannii to imipenem were 98.4%,97.0%,98.2%,70.1% and 47.5%,respectively.CONCLUSIONS Non-fermenters are predominant pathogens.Imipenem has high antimicrobial activity to Enterobacteriaceae in invro,but not to non-fermenters.The resistant rates of P.aeruginosa and A.baumannii to imipenem are increasing gradually.
10.Effects of hydrogen rich water on the expression of Nrf 2 and the oxidative stress in rats with traumatic brain injury

Objective To investigate the effects of hydrogen rich water on the expression of nuclear factor erythroid 2-related factor 2 (Nrf2) and oxidative stress in rats with traumatic brain injury (TBI).Methods Ninety healthy male Sprague-Dawley (SD) rats were randomly divided into sham operation group, TBI group and hydrogen rich water treatment group (HW group), with 30 rats in each group.TBI model was reproduced by the modified Feeney weight dropping method.The skulls of rats in sham operation group underwent only craniotomy without direct hit.The rats in HW group received brain injury by hitting after craniotomy, followed by injection of hydrogen rich water (5 mL/kg) intraperitoneally once a day for 5 days after successful reproduction of the model.The rats in sham operation group and TBI group were given an equal amount of normal saline in same manner.Six rats from each group were sacrificed at 6, 12, 24, 48 hours and 5 days after evaluating neurological severity scores (NSS).The brain tissue in injured ipsilateral cortex was harvested.The activity of catalase (CAT), glutathione peroxidase (GSH-Px), and content of malondialdehyde (MDA) were determined by spectrophotometry.The expressions of mRNA and nucleoprotein of Nrf2 were determined by quantitative real-time polymerase chain reaction (RT-qPCR) and Western Blot.The pathological changes were observed with microscopy after hematoxylin and eosin (HE) staining.Results ① NSS score:compared with TBI group, NSS in HW group at 12, 24, 48 hours and 5 days were significantly decreased (12 hours: 9.83±2.32 vs.13.17±2.71, 24 hours: 9.83±2.79 vs.13.50±2.43, 48 hours: 7.50±2.07 vs.11.83±2.14, 5 days:5.50 ± 1.87 vs.10.50 ± 2.43, all P < 0.05).② Compared with sham operation group, the activity of GSH-Px and CAT in TBI group were markedly declined after operation, while the MDA content was elevated significantly, especially at 24 hours [CAT (kU/g): 1.080±0.312 vs.3.571 ±0.758, GSH-Px (kU/g): 9.195±3.173 vs.32.385± 10.619, MDA (μmol/g): 12.282±2.896 vs.4.349± 1.511, all P < 0.01].Compared with TBI group, the parameters in HW group were improved, and they were similar as sham operation group.③ RT-qPCR: no significant difference was found in the expression of Nrf2 mRNA at each time point in three groups.④ Western Blot: the expression of Nrf2 nucleoprotein (gray value) in TBI group was apparently higher than that in sham operation group, and peaked at 24 hours (0.703 ± 0.262 vs.0.238 ± 0.120, P < 0.05), and the expression in HW group was obviously higher than that in TBI group, especially at 24 hours (1.110 ± 0.372 vs.0.703 ± 0.262, P < 0.05).⑤ HE staining: the brain structure in sham operation group was found to be intact.However, there were different degrees of pathological changes at each time in TBI group, especially at 24 hours.The pathological damage of brain tissue in HW group was significantly milder.Conclusions Hydrogen rich water can up-regulate the expression of Nrf2, and reduce oxidative damage of traumatic brain injury in rats.Nrf2 can up-regulate the expression of its downstream antioxidant enzymes, which may be the mechanism of the upregulation expression of Nrf2 in the study.
