1.Application of Peptide Self-assembled Multilayers on Detection of Thrombin

Peptide self-assembled multilayers ( SAMs ) was coated onto the quartz surface. The assembly conditions, such as the assembly agent concentration and assembly time, were examined. The SAMs was characterized via UV-vis absorption spectrometry and scanning electron microscope. Our results showed that the optimal concentration and assembly time for the 3-aminopropyl-triethoxysilane aqueous were 1% ( V/V ) and 3 h respectively, and those of gold nanoparticles were 2. 4 × 10-4 mol/L and 12 h, respectively, while those of peptide solution were 1 × 10-4 mol/L and 12 h, respectively. A new sensitive method, based on the theory that peptide could be cleaved at the site of Arg-Gly by thrombin, was established to detect thrombin. In addition, a good linear relationship was obtained in the range from 2. 8×10-12 mol/L to 9. 9×10-10 mol/L, and the detection limit was 1. 4×10-12 mol/L. The peptide self-assembled multilayers were also used in the analysis of blood serum samples, and the recovery rate was within the range from 91. 6% to 107. 6%.

OBJECTIVETo observe the clinical results and the changes of gingival crevicular fluid prostaglandins E(2) (GCF-PGE(2)) levels three months after indomethcin rinsing.
METHODSNineteen periodontal patients who had received periodontal treatment before were chosen and divided into two groups randomly: indomethcin test group and placebo control group. The clinical parameters and gingival crevicular fluid (GCF) samples were obtained respectively at 0, 1, 3 month. The levels of GCF-PGE(2) were measured by radioimmunoassay (RIA).
RESULTSAfter 3 months' rinse treatment, the bleeding index and the amounts of GCF in test group decreased significantly than those of the control group. Attachment level improved in test group as well. The levels of GCF-PGE(2) significantly decreased in test group, which not changed in control group. The percentage value of plaque reduced considerably in both test group and control group after rinsing, but no significant difference was found between these two groups.
CONCLUSIONGingival inflammation and the levels of GCF-PGE(2) are reduced after topical indomethcin administration. The effect is related to the decrease of local PGE(2) levels.

4.Clinical analysis of 155 young female patients with breast cancer

Objective To analyze the clinic characteristics, lifetime and prognostic factors of young female breast cancer patients. MethodsClinical data of 155 patients under 35 years of age with breast cancer were retrospectively reviewed and followed up.ResultsThe positive rate of hormone receptors was 61.6 % (77/125) in all cases who had been detected receptor status. The median survival time in hormone receptors positive and negative group were 119.0 and 51.3 months (P<0.01), and 5-year survival rates were 68 % and 33 %, respectively. For patients who had been treated with adjuvant tamoxifen (47.1%), the median survival time was 182 months which longer than without tamoxifen (P <0.05). The median disease-free survival time and median survival time were 24 and 91 months in all cases. The overall 3-, 5- and 10-year survival rates were 79 %, 60 % and 51%, respectively. Multifactor analysis with the COX model indicated that tumor size, axillary metastatic status, tamoxifen treatment and overexpression of Her-2 were independent prognostic factors. While clinic stage and hormone receptors status might be referenced prognostic factors. ConclusionYoung women breast cancer patient may have good prognosis if multimodality treatment is conducted. Tumor size, axillary metastatic status, adjuvant endocrine therapy and overexpression of Her-2 are independent prognostic factors.
5.Changes in Neuropeptide Y Y1 Receptor mRNA Level in the Infarcted Myoc ardial Tissues of Rats

Aim To investigate the change in neuropeptide Y(NPY) Y1 receptor level in infarcted myocardium tissues of (MI) rats. Method MI was induced by ligating the left descending anterior coronary artery (LAD) in the heart of rats. The techniques of reverse transcription polymerase chain reaction (RT-PCR) were used to identify the exist of NPY Y1 receptor mRNA in myocardial tissues. The technique of semi-quantitative PCR wa s used to observe the change of NPY Y1 receptor mRNA level. Results NPY Y1 receptor mRNA distributed in the heart tissues of rat. Compared with sham operated rats ,the NPY Y1 receptor mRNA level both in infarction area and non-infarction area was increased significantly after MI fo r 1d and 3d. The NPY Y1 receptor mRNA level in the heart tissues of sham-op erated rats was also significantly increased compared with that in control rats . Conclusion These results suggest that MI may result in increase NPY Y1 receptor level in the heart tissues of rats. Stre ss stimulation such as surgery may also increase the NPY Y1 receptor level in the heart.

Betatrophin is a newly found factor that affects the metabolisms of sugar and fat , which is mainly ex-pressed in the liver and adipose tissue .This factor not only has an influence on the glucolipid metabolism , but also has regulating effect on replicating pancreatic βcell.Glucose and lipid metabolic disorder and the decrease in the number of pancreatic βcell are main risk factors of diabetes , diabetic great vascular complications and other com-plications.Therefore, betatrophin level has close relationship with diabetes .
7. Evaluation of menopause and treatment choice of breast cancer patients before endocrine therapy

Aromatase inhibitors (AIs) directly applies to postmenopausal breast cancer patients. Patients underwent bilateral ovariectomy or ≥60 years were acknowledged as postmenopausal.Alternatively, for <60 years breast cancer patients, sex hormone detection to evaluate menopause is recommended by National Comprehensive Cancer Network (NCCN) guideline, textbooks, and AIs clinical trials.However, series of clinical trial found that, a broad overlap region of follicle stimulating hormone and estradiol appeared between premenopausal and postmenopausal patients, which unable to determine the menopause even with sensitivity promotion of detection equipment or manners.We have abandon this detection in clinical treatment, and decision making was only according to the relapse risk and disease status. We recommend bilateral ovariectomy resection accompanied with AIs for breast cancer patients with high recurrence risk (e.g. T3-4 or LNM≥4) or patients with advanced metastatic disease.However, patients with low or moderate recurrence risk can be treated with tamoxifen.
8.Analysis of radiotherapy strategy for 110 breast cancer patients after R0 resection of local recurrence after radical mastectomy

Objective To explore the effect of prognosis of consolidation radiotherapy for patients after R0 resection of local recurrence after radical mastectomy. Methods Totally 110 breast cancer patients with local recurrence receiving R0 resection were admitted and treated in our hospital from January 1st, 2003 to November 30th, 2015 were retrospectively analyzed. Results The median local progression time of 74 patients receiving consolidation radiotherapy ( 67.3%) was remarkably better than that of those without radiotherapy(36 patients, 32.7%), and the difference was statistically significant (χ2 =8. 526, P<0.05). Meanwhile, there was no statistically significant difference (P>0.05) of distance disease-free survival and overall survival between the radiotherapy group and the non-radiotherapy group. Multifactor analysis indicated that pseudo-adjuvant endocrine therapy (χ2 =7.541,95%CI:27.1% -80.4%, P <0.05), DDFS(≥2 years vs. <2 years,χ2 =4.068,95%CI:101.4% -267%,P<0. 05) and pseudo-adjuvant radiotherapy(χ2 =14.126, 95%CI:21.7% -80.4%, P <0. 05 ) were the independent risk factors affecting the OS of patients with local recurrence after R0 resection. Conclusions For the patients with local recurrence after R0 resection of local recurrence, it is recommended that consolidation radiotherapy should be done and the radiation field should include the same side of the chest wall and clavicle area lymphatic drainage area.

10.Application of queuing theory model combined with the dynamic allocation of nursing staff in the management of blood collection room in outpatient

Objective To explore the use of queuing theory model combined with the dynamic allocation of nursing staff in the management of blood collection room in outpatient. Methods Queuing theory model combined with dynamic allocation of nursing staff were applied to scientifically allocate nurses in blood collection room. Three months before and after allocation, nursing staff, inspection personnel and patients were investigated through satisfaction questionnaire and the management effect was evaluated. Results Patients′satisfaction degree toward blood collection staff was increased from 80% to 95%; inspection personnel′s satisfaction degree toward blood collection staff was increased from 62.5% to 87.5%;nurses′satisfaction degree toward themselves was increased from 70% to 95% (χ2=20.571,13.333,5.22;P<0.05) . Patients′waiting time before blood collection was shorten to 20 min from 30 min. Conclusions The application of queuing theory model combined with dynamic allocation of nursing personnel can improve the rational use of blood collection personnel in outpatient and nursing quality, and it also can improve the satisfaction of inspection personnel to nurses, of patients and nurses to themselves.
