1.Evaluation of the chemiluminescence immunoassay for diagnosis of syphilis in the clinical screening test

Objective To evaluate the specificity of Architect chemiluminescence immunoassay ( CLIA) for diagnosis of syphilis in the clinical screening test.Methods 65 774 syphilis specific antibody results in the Beijing Tongren Hospital were retrospectively analyzed.In this study, Architect CLIA for diagnosis of syphilis was used as a screening test for the pre-operative patients from August 2011 to July 2012.All the repeatedly reactive samples were tested by the tolulized red unheated serum test ( TRUST) and Treponema pallidum particle agglutination ( TPPA) assay.Samples with with discordant results were verified by western blot ( WB) Finally, all the results were statistically analyzed by software SPSS 17.0.Results Among 65 774 samples, 940 (1.43%) were found to be repeatedly reactive using the Architect CLIA.Of these sera, 330 ( 35.11%) were reactive and 610 ( 64.89%) were nonreactive by TRUST.Because a TRUST titer of 1:1was not always sufficient to confirm a TPPA reactive result, all the samples with CLIA reactive results were finally performed by TPPA assays.The results showed that 843 ( 89.68%) were positive and 97 (10.32%) were negative.After arbitrated by the WB, out of 97 TPPA negative sera, 18 (18.56%) were positive;7(7.22%) were indeterminate and 72(74.23%) were negative.Conclusions The specificity of Architect CLIA for diagnosis of syphilis was 99.89%.The results showed that confirmation by TPPA on sera with screening test for syphilis by Architect CLIA effectively decreased the false-positive results and could be suitable for a routine supplementary for syphilis.However, if necessary, the WB should be further utilized.
2.The application of digital simulation positioning machine in radiation therapy oncology

Objective To discuss the application of digital simulation positioning machine in radiation oncology. Methods We used digital simulation positioning machine to shoot isocenter reset digital simulator images of 0 degrees direction and 90 degrees direction, 10 × 10 cm2 rectangular field and actual time radiation field with any field number. Compared the images taken with the TPS corresponding DRR image through bone marker matching to measure the error of isocenter and actual time radiation field , the linear accelerator Iview-GT system with the same method was shot in rectangular field simulator the same field number EPID image , and the TPS corresponding DRR image. Finally, we compared the measured error of the digital simulator positioning machine with the error of Iview-GT system image. Results The result of measured isocenter error of the digital simulator positioning machine was better than EPID image error (P=0.000). Conclusion The application of digital simulation positioning machine in radiation oncology can be more accurate to measure the error and correct the position deviation, however, it can also make the radiotherapy and the treatment more accurate.
3.Coronary CTA combined CT myocardial perfusion imaging in diagnosis of coronary artery disease:Meta-analysis

Objective To assess the diagnostic value of coronary CTA (CCTA) combined myocardial CT perfusion imaging (CTP) in coronary artery disease (CAD) with Meta-analysis.Methods Chinese and English literatures evaluating CCTA combined CTP in diagnosis of CAD in PubMed,Wed of Science,Cochrane Library,China National Knowledge Infrastructure (CNKI) and Wanfang Periodical Database from inception to December 2016 were searched.The extraction was conducted according to the criteria for diagnostic recommended by Cochrane Method Group.The quality of included studies was assessed with quality assessment of diagnostic accuracy studies-2 instrument.The statistical analysis was performed by Meta-disc 1.4 software,the diagnosis efficacy was calculated and the symmetric receiver operating characteristic curves (SROC) was drawn to obtain the area under curve (AUC).Results Five studies involving a total of 611 vessels were included.The pooled sensitivity (SEN),specificity (SPE),diagnostic odds ratio (DOR),positive likelihood ratio (PLR),negative likelihood ratio (NLR) were 0.96 (95%CI [0.92,0.98]),0.91 (95%CI [0.88,0.94],206.14 (95%CI [100,424.94]),9.33 (95%CI [5.57,15.61]),0.06 (95%CI [0.03,0.13]),respectively.The AUC of SROC was 0.977.Conclusion CCTA combined CTP has significant value in diagnosis of CAD.
4.Biological characteristics of a human specifically targeted antimicrobial peptide C16LL-37 against Streptococcus mutans.

OBJECTIVEThis study aimed to evaluate the biological characteristics of a human specifically targeted antimi- crobial peptide C16LL-37 against Streptococcus mutans (S. mutans).
METHODSIn this study, an antimicrobial peptide LL-37, a peptide derived from CSP(C16) (S. mutans competence stimulating peptide), and recombinant peptide C16LL-37 were synthesized by Fmoc-chemistry-based strategy. The selectivity and antibacterial activity of C16LL-37 were identified by the colony counting method on microbial culture plates. After treatment of C16LL-37 at 32 µmol · L⁻¹, the morphological changes in S. mutans were observed by using scanning electron microscopy (SEM). In addition, enzyme-linked immunosorbent assay was used to evaluate the hemolytic activity and antibacterial activity of C16LL-37 under different conditions.
RESULTS1) The minimum inhibitory concentration of C16LL-37 was 16 µmol · L⁻¹, and the minimum bactericidal concentration was 64 μmol ·L⁻¹. 2) The survival rate of S. mutans was 3.46% after C16LL-37 treatment at 64 µmo-L⁻¹ for 30 min, whereas it was 0% at 64 µmol · L⁻¹ for 60 min. The survival rates of four other kinds of bacteria were more than 60% at any time (P < 0.05). 3) The morphological change in S. mutans was observed after C16LL-37 treatment at 32 µmol · L⁻¹ by using SEM. S. mutans presented an irregular shape, rough surface, and evident splitting. 4) The hemolysis rate of C16LL-37 (≤ 64 µmol · L⁻¹) was less than 0.33%. 5) This study showed no significant in- fluence on the antibacterial activity of C16LL-37 under different conditions, such as temperature, pH, salinity, and trypsin at low concentration (P > 0.05).
CONCLUSIONC16LL-37 exhibited obvious specificity for S. mutans, strong antibacterial activity, low toxicity, and high stability. Thus, C16LL-37 has good potential in caries research and clinical application.

5.Radioprotective effect of Ganoderma lucidum polysaccharides on irradiated mice

Objective Radiation can cause multiple damages to tissues and organs.This study aimed to explore the protec-tive effect of Ganoderma lucidum polysaccharides ( GLPs) against the 60 Co-γray radiation injury in mice and provide an experimental basis for the clinical use of GLPs. Methods One hundred mice were randomly divided into five groups of equal number: normal control, gavage control, radiation control, high-dose GLPs, and low-dose GLPs.Models of radiation injury were made in the mice by whole-body exposure to 60 Co-γrays.Three days before and after mod-eling, the animals in the high-dose and low-dose GLPs groups were given GLPs intragastrically at the dose of 100 and 50 mg/kg respec-tively, once daily for 14 days.Then the 30 day survival rate and sur-vival time of the model mice were recorded and the changes in the pe-ripheral blood index, spleen index, and serum superoxide dismutase( SOD) activity were observed. Results GLPs significantly increased the 30-day survival rate and the mean survival time of the mouse models (P<0.05), decreased the reduction of WBC count in the peripheral blood, and shortened the time of WBC restoration ( P<0.05 ) .Furthermore, GLPs obviously improved the spleen index and SOD activity of the Co-γray irradiated animals. Conclusion GLPs, with a significant anti-radiation effect, can effectively raise the survival rate of the mice exposed to a lethal dose of 60 Co-γrays, reduce radiation injury to WBC and platelets, and increase the activity of SOD in irradiated mice.
6.Basic and clinical research progress of diffuse intrinsic pontine glioma

Diffuse intrinsic pontine glioma( DIPG)is a highly invasive tumor located in the pons (middle)of the brain stem. They are usually diagnosed during childhood and account for 10% -15% of primary brain tumors in children. DIPG has a very poor prognosis. Fewer than 10% of DIPG patients survive more than 2 years after diagnosis. The imaging manifestations of DIPG are typical,and biopsy is only performed in atypi-cal cases. The tissue specimens of newly diagnosed DIPG are very few and limit its molecular biological research. Recent advances in surgical and molecular-analytic techniques have increased the safety of biopsy which has already been used in many clinical trials step by step. The research of DIPG′s molecular pathogenesis and treatment is sure to achieve new breakthroughs.
7.Clinical analysis of prostate cancer in young men under 50 years

Objective To analyze the clinical features of patients with prostate adenocarcinoma under 50 years.Methods Between January 2008 and April 2014,we reviewed 21 cases with prostate cancer under 50 years old.The mean age in those patients was 48 years old (ranged 42-49 years old).Their tPSA level was elevated,including>10 μg/L in 17 cases,4-10 μg/L in 4 cases.21 cases were all confirmed by pathology.The results Gleason score showed 6 scores in 3 cases,7 scores in 6 cases (3+4 scores in 5 cases and 4+3 scores in one case),8 scores in 7 cases and 9 scores in 5 cases.In the study,clinical stage was for T2N0M0 in 16 cases,T3N0M0 in 1 case,T4NxM0 in 1 case and T3-4N1M1 in 3 cases.The treatments were hormonal therapy was chosen in 5 cases and radical prostatectomy was performed in 16 cases,including 16 cases with T2N0M0 stage and one case with T3N0M0.Results In those patients who accepted the radical prostatectomy,the duration of follow-up ranged from 3 to 65 months (mean 23 months).During the follow-up,14 patients had a lower incidence of biochemical recurrence.1 patient (T2,PSA 82.8 μg/L,GS 9) had external beam radiotherapy one month after the radical prostatectomy because of tumor invasion into the prostatic capsule.Then his PSA level returned to the 0.2 μg/L.1 patient (T3,PSA 38.9 μg/L,GS 8) had external beam radiotherapy 18 months after the radical prostatectomy because of biochemical recurrence and the tPSA level returned to the 4.0 μg/L.All patients who underwent radical prostatectomy had favourable recovery of urinary continence.In 5 patients who had androgen deprivation therapy,2 patients died after 63 or 65 months and one patient was lost to follow-up.The PSA level in one patient decreased from 71.8 μg/L to 2 μg/L after four months treatment.One patient had castration resistant prostate cancer and the adjuvant external beam radiotherapy was considered,subsequently.Conclusions Men under the 50 years old,who are diagnosed with localised prostate cancer,usually demonstrated the early clinic stage and high Gleason scores.It should not be discouraged from RP.Young men with metastatic prostate cancer have a poor prognosis.
8.Application of Bone Anchor and AO/ASIF Hollow Screw in Treatment of Trans-scaphoid Perilunar Dislocation

Objective To investigate the outcome of trans-scaphoid perilunar dislocation treated with bone anchor and AO/ASIF hollow screw.Methods Eight patients with trans-scaphoid perilunar dislocation were treated by Depuy Mitek bone anchor and AO/ASIF hollow screw.The postoperative follow-up period was 6~24 months(average 11.1±5.3 months).All patients were assessed by the Cooney's grading system.Results The outcomes of 8 patients were execellent in 5 cases,good in 2 cases,general in 1 case.6 cases got pain relief with an increase of range of motor and grip strength at certain drgree.Postoperative roentgenogram displayed congruity among wrist bones,with no fracture disunion,no loose internal fixation and no traumatic arthritis.Conclusion The bone anchor and hollow screw have advantages as simple-manipulated and reliable for treating trans-scaphoid perilunar dislocation.

In the western world, adenocarcinoma of the prostate is the most common malignant neoplasm of human males. In recent years, the incidence of the disease has increased dramatically in China. Surgery is an important treatment for prostate cancer. This article reviews the progress in radical retropubic and perineal prostatectomy, standard laparoscopic and robot-assisted laparoscopic radical prostatectomy. It covers the necessity, techniques and experience of surgery, nerve preserving techniques and erectile dysfunction, complications and outcomes, advantages and disadvantages of and comparison between various surgical approaches.

10.Superomedial pedicle reduction mammaplasty in treatment of severe gynecomastia

Objective To investigate the effect of superomedial pedicle reduction mammaplasty in the treatment of severe gynecomastia.Methods 24 males with severe gynecomastia were involved in this study,who underwent superomedial pedicle reduction mammaplasty.Results All the patients were followed up for one to two years,and there were no complications such as skin necrosis,nippleareolar complex (N A C) hypaesthesia,hematoma or infection occurred.Minimal wound dehiscence occurred in two cases,and the rest cases were healed well.Conclusions Superomedial pedicle reduction mammaplasty in the treatment of severe gynecomastia could achieve satisfactory results,and good breast shapes are produced.
