Objective To investigate the preventive effect of octreotide combined with netilmicin sulfate on postoperative infection in suppurative appendicitis and its effect on helper T cell 17(Th17)/regulatory T cells(Treg)imbalance.Methods 148 patients with acute suppurative appendicitis in Xiangshan Hospital of Traditional Chinese Medicine from August 2014 to May 2016 were randomly divided into observation group(n=74)and control group(n=74).All were underwent laparoscopic appendectomy surgery,the control group took netilmicin sulfate combined with ornidazole for anti-infection after operative,while the observation group added octreotide acetate subcutaneously.The levels of serum interleukin(IL)-4,IL-6,IL-10,IL-17,IL-23,transforming growth factor β(TGF-β),interferon γ(IFN-γ)and Th17,Treg cell ratio and expression levels of related transcription factorbefore and after treatment were detected,Th17/Treg change were analyzed.Results The levels of IL-4,IL-6,IL-10,IL-17,TGF-β were decreased,IFN-γ were increased after treatment(P<0.01),the improvement of those indicators in the observation group were supior than control group(P<0.05).Th17,Treg cell ratio and expression levels of related transcription factor orphan nuclear receptorγt(RORγt),Forkhead box protein P3(Foxp3)were decrease,Th17/Treg was reduced after treatment(P<0.01),those indexes in observation group were lower than those in control group(P<0.05).The wound infection,intra-abdominal infections and other adverse events had no significant difference in two groups.Conclusions Octreotide combined with netilmicin sulfate can prevent postoperative infection in suppurative appendicitis,regulate Th17/Treg imbalance,restore immune function,and no significant adverse reactions.

Objective To summarize the characteristics of pediatric abdominal hernia ultrasonic image and review the misdiagnosis of cases,to explore the values of high frequency color doppler ultrasound in the diagnosis of internal abdominal hernia. Methods Retrospective analysis the cases which diagnosed of internal hernia by abdomen colour ultrasound in January 2012 to December 2015 at our medical center,as well as the patients had surgery in our medical center for surgical treatment of 35 children (part results with pathological diagnosis), Retrospectively analyzed the characteristics of preoperative ultrasound image,and compare the results of abdominal operation results,and the pathological diagnosis. Results 30 cases confirmed by surgery for internal hernias. Conclusion ultrasound have higher value to exceed the diagnosis of internal hernia in childen,which most of the following intestinal obstruction in the diagnosis.

Objective:To analyze the equity of health resource allocation in village-level medical institution in China,thus to provide the basis for formulating relevant health resources allocation policy.Methods:Using Lorenz curve and Gini coefficient,the rationality and equity of allocation of village health resources in China were evalua-ted by the equity of population distribution and the equity of geographical distribution.Results:The Gini coefficient of village health workers and health institutions was 0.19 and 0.20 respectively according to population distribu-tion,and was 0.65 and 0.63 respectively according to geographical distribution.The Gini coefficient's difference of village-level health resources in the eastern,central and western regions was small according to population distri-bution,but the difference was larger according to geographical distribution among different areas.Conclusion:The equity of village-level health resource allocation in China was preferable according to population distribution,but the equity according to geographic distribution was poorer.The formulation of health policy should pay more atten-tion to the equity of geographic distribution and allocate rationally.

Objective: To systematically evaluate the optimal duration of adjuvant androgen deprivation therapy (ADT) after radiotherapy for prostate cancer. Methods: The CBM, CNKI, Wanfang, PubMed, EMbase, Cochrane Library, Clinical Trials.gov databases were searched by computer. The randomized controlled trial (RCT) on long-term and shortterm ADT for intermediate and high risk non-metastatic prostate cancer after radiotherapy were collected. The retrieval time was from the establishment of these databases to November 5, 2018. After independent screening literature, extracting data and evaluating the quality of the included studies, two researchers used RevMan 5.3 software for Meta-analysis. Results: A total of eight RCTs were selected, including 6 165 patients. Meta-analysis showed that the overall survival (OS) rate in longterm ADT group was higher than that in short-term treatment group [hazard ratio (HR) = 0.75, 95% confidence interval (CI): 0.63-0.89, P = 0.000 7]; the disease-free survival (DFS) rate in long-term ADT group was higher than that in short-term treatment group (HR = 0.81, 95% CI : 0.70-0.94, P = 0.007); the disease-specific survival (DSS) in long-term ADT group was higher than that in short-term treatment group (HR = 0.83, 95% CI : 0.71-0.96, P = 0.01); the local progress (LP) in long-term ADT group was lower than that in short-term treatment group [relative risk (RR) = 0.76, 95% CI : 0.59-0.97, P = 0.03]; the biochemical failure (BF) in long-term ADT group was lower than that in short-term treatment group (RR = 0.66, 95% CI : 0.52-0.83, P = 0.0004); the distant metastasis (DM) in long-term ADT group was lower than that in short-term treatment group (RR = 0.85, 95% CI : 0.74-0.96, P = 0.01). Conclusion: The long-term ADT treatment is superior to the shortterm treatment in OS rate, DFS rate, DSS rate, LP rate, BF rate and DM rate after radiotherapy for moderate and high risk prostate cancer. Due to the limitation of the quantity and quality of the included literatures, the above conclusion need to be verified by higher quality research.

OBJECTIVETo explore the effectiveness and prognosis on surgical treatment of posterior hip dislocations complicated with fractures of femoral head and acetabulum.

METHODSSeventeen patients with posterior hip dislocations complicated with fractures of femoral head and acetabulum were reviewed in the study. All the patients were treated with manual reduction within 12 hours after injury. CT location was used before operation. Absorbable screw fixation was performed for the femoral head fractures, while plate fixation or resection was performed for acetabular fractures.

RESULTSAll the patients got bony union within 6 months after operation without femoral head necrosis. Evaluate joint function according to Modifie Daobigne and Postal clinical classification criteria, 8 patients got an excellent result, 7 good and 2 fair. The excellent and good rate was 88.2%.

CONCLUSIONSurgical treatment can have a satisfactory prognosis for patient with posterior femoral head and acetabulum.

To investigate the effect of MMP-26 on human glioma angiogenesis and the possible mechanism.Methods The MMP-26 plasmid and empty plasmid pcDNA3.1 were stably transfected into U251 cells to establish a nude mice xenograft model,and then an in vitro human tumor tissue-based three-dimensional angiagenic model.Tissue disks were visually assessed over time to determine the percentage of wells that developed an angiogenic response(I％) and the density and length of neovessel growth were graded at intervals using a semiquantitative visual growth-rating scheme (angiogenic index,AI,0-16scale) in groups of MMP-26 transfected U251 cells (U251-MMP-26),pcDNA3.1 vector-transfected U251 cells (U251-pcDNA3.1) and non-transfected U251 cells (U251).RT-PCR and immunohistochemistry were used to detect the expression of mRNA and protein of MMP-26 and VEGF in groups of U251-MMP-26,U251-pcDNA3.1 and U251.Immunohistochemical localization of CD31 was determined in the endothelial tubes invading the fibrin-thrombin clot matrix.Results Immunohistochemical endothelial cell markers CD31 was positive in the vascular tubes invading the fibrin-thrombin clot matrix,confirming their endothelial origin.The angiogenesis results showed that difference of length of micro capillaries,density of branches,and the area occupied between U251-MMP-26 groups and control groups were significant.The percentage of tumor implants that developed invasion (I％) and the angiogenic index AI in U251-MMP-26 group on day 14 were higher than those of U251-pcDNA3.1 group and U251 group (P ＜ 0.05).The trends of I％ and AI in 14 days were significant compared with those in control groups.The expression of mRNA and protein of MMP-26and VEGF in U251-MMP-26 group was significantly higher in U251-MMP-26 group than those in U251-pcDNA3.1 group and U251 group(P ＜0.01).Conclusion The effect of MMP-26 on promoting glioma angiogenesis may be related to the increased expression of VEGF,which can be used as targets for anti-tumor therapy.

AIM: To investigate the effect of selective silencing of SLC7a8 on uptaking L-dopa in renal tubular epithelial cells of rat (NRK-52E). METHODS: The three siRNAs targeting SLC7a8 (siRNA-1, siRNA-2, siRNA-3) were designed and synthesized. A siRNA with nonspecific coding sequence (siRNA-con) was used for control. All siRNAs were transfected into NRK-52E cells. The siRNA-con transfected group, blank control group and gene-specific silencing SLC7a8 group were set up. The efficiency of transfection was estimated by flow cytometry. The efficiency of RNA interference was detected and screened by RT-PCR preliminarily, and was followed by Western blotting at protein level. The concentrations of L-dopa uptake into the NRK-52E cells were detected by ultraviolet spectrophotometry at different time points (6, 12, 24, 36, 48, 60, 72 and 120 min). RESULTS: The transfection efficiency was 94% detected by flow cytometry. The initial screening of RT-PCR showed that the efficiencies of RNA interference of siRNA-1 and siRNA-3 were higher, and siRNA-3 was the highest at protein level determined by Western blotting. No distinctive change was found between siRNA-con treated NRK-52E and blank control cells. The L-dopa uptake at different time points (6, 12, 24, 36, 48, 60, 72 and 120min) in siRNA-interference group was lower than that in siRNA-con transfected group and blank control group. No significant difference of L-dopa uptake between siRNA-con group and blank control group was observed. CONCLUSION: RNA interference technology selectively down-regulates SLC7a8 expression in rat renal tubular epithelial cells. The L-dopa uptake is also decreased after specifically silencing the slc7a8 expression.

OBJECTIVEThe automatic generation of planning targets and auxiliary contours have achieved in Eclipse TPS 11.0.

METHODSThe scripting language autohotkey was used to develop a software for automatically generated contours in Eclipse TPS. This software is named Contour Auto Margin (CAM), which is composed of operational functions of contours, script generated visualization and script file operations. RESULTS Ten cases in different cancers have separately selected, in Eclipse TPS 11.0 scripts generated by the software could not only automatically generate contours but also do contour post-processing. For different cancers, there was no difference between automatically generated contours and manually created contours.

CONCLUSIONThe CAM is a user-friendly and powerful software, and can automatically generated contours fast in Eclipse TPS 11.0. With the help of CAM, it greatly save plan preparation time and improve working efficiency of radiation therapy physicists.

Objective:To investigate the current status of social support and mental health among residents in Weifang and analyze the impact of social support on mental health,thus to provide reference for improving the mental health status of residents.Methods:A sample of 6174 residents in Weifang was investigated using Social Support Rate Scale (SSRS) and Kessler Psychological Distress Scale (K10).The data was analyzed statistically with Chi-square test,Kruskal-Wallis test and Multiple Logistic regression analysis.Results:About 0.3％ residents in Weifang reported low social support and 93 ％ residents showed good mental health.Gender,age,social support level and 3 dimensions of social support were associated with mental health respectively in the correlation analysis.In the multiple analyses,social support,subjective support,availability of support and mental health were positively correlated in the model of Good mental health and General mental health.Conclusion:The mental health status of residents in Weifang is not optimistic.The social support level has beneficial effect on mental health starus.Subjective support,objective support and the utilization of these supports imposes different degrees of inflnence on mental health status.

Objective:To understand cognition and satisfaction degree of mental health service among residents in Weifang,and provide reference for further development of mental health work.Methods:6174 residents were selected with random sampling method and a cognition survey was conducted using a self-designed questionnaire.Among them,553 residents were selected to investigate their satisfaction with mental health service.Results:The residents have unscientific cognition on mental health service,and they did not understand the delivery of community mental health services.Residents who have received mental health service were more satisfied with informed consent and patient privacy protection,and had low satisfaction with the clinic environment and the charge of inspection items.Conclusions:To improve residents' cognition and satisfaction of mental health service,we should further perfect the professional mental health institutions and personnel recruiting,and forcefully carry out mental health education.
