1.Clinical study on the effect of oral and acupoint application of traditional chinese medicine combined with azithromycin for the children with mycoplasma pneumonia

Objective To investigate the clinical curative effect of oral and acupoint application of traditional chinese medicine combined with Azithromycin for the children with Mycoplasma Pneumonia. Methods A total of 160 children with mycoplasma pneumonia were divided into two groups according to digital random table method, with 80 cases in each group. The patients of the two groups were given conventional treatment to relieve cough and reduce phlegm; On the basic treatment of conventional treatment, the control group were treated with azithromycin, wile the treatment group were treated with oral traditional chinese medicine Tinghuang runfei decotion and acupointion application of Tinglizi, Baijiezi, Shengnanxing, Dahuang, and Bingpian. The treatment of both groups last 3 weeks. The time of defervescence, the disappearance time of cough and pulmonaryrales of the two groups were compared, and the serum levels of inter leukin 8 (IL-8), tumor necrosis factor-a (TNF-a), thrombin regulatory proteins (TM), D-dimer were compared before and after treatment. The total effect rate and the incidence of adverse events were compared.Results The defervesce time (3.79 ± 1.68 dvs. 4.88 ± 1.61 d,t=3.846), disappearance time of cough (8.21 ± 2.42 dvs. 10.35 ± 2.60 d,t=5.389) and disappearance time of pulmonary rales (6.21 ± 1.89 dvs. 7.78 ± 2.08 d,t=4.997) in the treatment group were significantly less than those in the control group (P<0.01). The levels of IL-8 (9.98 ± 4.26 ng/Lvs.14.18 ± 4.82 ng/L, t=5.840), TNF-a (20.78 ± 5.93ng/Lvs. 26.07 ± 6.42 ng/L,t=5.414), TM (9.63 ± 2.88μg/Lvs. 13.08 ± 3.37μg/L,t=6.961), D-dimer levels (0.09 ± 0.04 ng/Lvs. 0.15 ± 0.06 ng/L,t=7.442) the treatment group were significantly better than those in the control group (P<0.01). The total effect rate in the treatment gruop was significantly higher than that of the control group (97.5%vs. 88.8%;χ2=4.783,P=0.029), but there was no significant difference in the incidence of adverse events between the two groups (16.3%vs. 10.0%;χ2=1.370,P=0.272).Conclusions Oral and acupointion of traditional chinese medicine Tinghuang runfei decotion combined with azithromycin could relieve the symptoms, improve the clincal effect and show its safety for the children with mycoplasma pneumonia.
2.Analysis of IL-17 and IL-10 expressions of patients with hepatic cystic echinococcosis in cyst stages

Objective To explore the significance of IL-17 and IL-10 in patients with different types of hepatic cystic echinococcosis ( CE ) in cyst stages , to investigate the values of evaluation of hydatid cyst activity . Methods 33 patients with hepatic hydatid disease and 21 healthy people (control group) were enrolled, divided into 4 groups, according to WHO ultrasonic classification for CE:Control group (n=21), CEⅠ,Ⅱgroup (active, n=14), CEⅢgroup (transitional, n=10), and CEⅣ,Ⅴgroup (inactive, n=9). The serum concentrations of IL-17 and IL-10 were measured by enzyme-linked immunosorbent assay. Results Serum levels of IL-17 in CEⅢgroup were significantly higher than other groups (P<0.05). There were no significant difference in serum IL-17 levels between CEⅠ,Ⅱgroup [(38.57 ± 3.83) pg/mL] and CEⅣ,Ⅴgroup [(40.22 ± 5.15) pg/mL]. Serum levels of IL-10 were significantly higher in CEⅢgroup [(14.65 ± 2.15) pg/mL] than CEⅠ,Ⅱgroup [(11.38 ± 0.71) pg/mL] (P<0.05), while CEⅠ,Ⅱgroup were significantly higher than CEⅣ,Ⅴgroup [(7.60 ± 0.46) pg/mL] (P<0.05). There were no significant difference of IL-17/IL-10 ratio between CEⅠ, Ⅱgroup (3.44 ± 1.04) and CEⅢgroup (4.60 ± 2.40), and there were no significant difference of IL-17/IL-10 ratio between CEⅢgroup and CEⅣ,Ⅴgroup (5.39 ± 1.95). IL-17/IL-10 ratio in CEⅠ, Ⅱgroup was significantly lower than CEⅣ, Ⅴgroup (P<0.05). The serum concentrations of IL-17 has no correlation with IL-10 in different cyst stages. Conclusions The synergy effect of IL-17 and IL-10 may play the certain role to promote the parasite progress. Neither IL-17 nor IL-10 level could be as an independent marker of echinococcal cysts activity, while IL-17/IL-10 ratio reflect the hydatid cyst activity in certain degree, but as a echinococcal cysts activity marker, further evidence should be needed.
3.The effects of rhubarb on expression of glucocorticoids receptor and peripheral blood lymphocytes in burning-induced septic rats

Objective To investigate the effects of rhubarb on the expression of glucocorticoids receptor (GR)and peripheral blood lymphocytes in burning-induced septic rats. Methods Sixty-six male healthy Sprague-Dawley(SD)rats were randomly divided into sham operated control group(n=18),sepsis model group(n=24) and rhubarb treatment group(n=24),each group was further randomly divided into 12,24 and 72 hours subgroups according to different time points. The model of scald sepsis was replicated by scald injury induced by boiling water at the rat back accounting for 30% total body surface area(Ⅲ grade of scald),and administration of endotoxin (5 mg/kg)into the peritoneal cavity 12 hours after scald injury. After the successful establishment of septic models, the rats in the rhubarb treatment group were immediately infused with 50 mg/kg rhubarb powder dissolved in 1 mL saline through a gastric tube,while the rats in sham operated control group and sepsis model group received saline by the same way as a substitute for rhubarb. The the binding capacity of GR of peripheral blood leucocyte and binding activity of GR of hepatocyte were analyzed by radiation ligands binding assay. The CD4+,CD8+as well as CD4+/CD8+ ratio in peripheral blood lymphocytes were detected by flow cytometer. Results The binding capacity of GR of peripheral blood leucocyte and binding activity of GR of hepatocyte were significantly decreased in a time-dependent manner in sepsis model group compared to those of the sham operated control group,while in the rhubarb treatment group they were increased in a time-dependent manner after interference of rhubarb, and they were higher than those in the model group at the same time points〔leukocyte GR binding capacity (locus/cell)at 12,24,72 hours :1 515.38±300.44,1 859.63±258.26,1 890.50±307.88 vs. 1 122.63±225.39, 1 008.88±150.41,724.38±91.19;hepatocyte GR binding capacity(fmol/mg):210.19±26.26,258.01±20.98, 283.38±38.21 vs. 153.11±30.07, 129.83±26.89, 94.08±14.30, all P<0.01〕. Compared with the sham operated control group,the CD4+ and CD8+ were decreased in various degrees at 12 hours and 24 hours in the septic group, at 24 hours the differences being statistically significant (P<0.01 and P<0.05). CD4+/CD8+ratios were decreased significantly at all time points,the differences were statistically significant at 24 hours and 72 hours(both P<0.01). The CD4+ T cell and CD4+/CD8+ ratio at all the time points were increased at various degrees in the rhubarb treatment group,and the differences from those in the sepsis model group at 24 hours and 72 hours were statistically significant (1.58±0.69, 1.56±0.49 vs. 1.02±0.41, 1.01±1.68, both P<0.01). Conclusion Rhubarb can modulate the binding capacity of GR of peripheral blood leucocyte and the binding activity of GR of hepatocyte,and via its influence on the number of peripheral leucocytes,the immune dysfunction in the sepsis processes is improved.
4.Effects of inhalation-intravenous general anesthesia on stress response of patients with liver hydatidosis for pericystectomy

Objective To investigate the changes of stress response on inhalation-intravenous general anesthesia during pericystectomy for Liver hydatid cyst. Methods Thirty ASA I-II patients with liver hydatidosis for pericystectomy were studied. The blood from right jugular vein were collected for measurement of serum cortisol (Cor) concentrations and glucose (BG) levels, and MAP, HR and BIS were observed and recorded at different point-times: pre-intubation for 1 min, post-intubation for 3 min, pre-incision for 1 min, post-incision for 3 min, post-incision for 10 min, post-surgical exploration 10 min. The data of pre-intubation and pre-incision served as controls. Results The levels of Cor concentration of post-incision for 10 min were decreased than those of pre-intubation and pre-incision (P<0.05); MAP of post-incision for 10 min and post-surgical exploration for 10 min were increased than those of pre-intubation and pre-incision (P < 0.01); For HR, the data of post-surgical exploration 10 min was much quicker compared with pre-incision (P < 0.01), which is slower than that of pre-intubation(P<0.01). Conclusion Inhalation-intravenous general anesthesia may inhibit the stress response during intubation,incision and surgical exploration for liver hydatidosis pericystectomy. Hemodynamic changes during incision may reflect the trend of stress response in advance.
5.The influence of whole nursing model on health-related knowledge awareness and satisfaction rate of patients with liver hydatidosis receiving pericystectomy

Objective To observe the influence of whole nursing model on health-related knowledge awareness and satisfaction rate of patients with liver hydatidosis receiving pericystectomy.Methods 180 patients in hospital receiving resection of liver hepatic cyst were randomly divided into the observation group and the control group with 90 patients in each group.The control group was given usual care,while the observation group received whole treatment model.The patients' health-related knowledge awareness and satisfaction rate were compared between two groups.Results The rate of health-related knowledge awareness and satisfaction about nursing work in the observation group preceded the control group.Conclusions This whole treatment model can boost the awareness rate of health-related knowledge and nursing satisfaction,which not only provides the care with full,comfortable and efficient services,but also advances the self-care capacity and cognition of disease of patients.
6.Effects of small-dose glucocorticoid on glucocorticoid receptor and cellular immune function in critical patients

ObjectiveTo study the effects of small-dose glucocorticoid (GC) on glucocorticoid receptor (GR) and cellular immune function in critical patients.MethodsForty ICU critical patients admitted in Shanghai Changzheng Hospital from March 2007 to March 2009 were enrolled in the study and were divided into GC group and non-GC group according to the use or absence of GC.Blood samples were collected at days 1,7 and 10 after GC treatment to detect GR binding affinity of mononuclear leukocytes (MNLs) and polymorphonuclear leukocytes (PMLs) in the peripheral blood and the CD4/CD8 ratio in the T lymphocytes.The method of GC use was that the hydrocortisone was given intravenously at a dose of 100 mg every eight hours.ResultsGR binding capacity of MNLs at day 1 and 7 showed no statistical difference between the GC and non-GC groups.GR binding capacity of MNLs in the GC group was lower at day 1 and was much lower at day 7 (P < 0.05 ).However,in the non-GC group,it was lower at day 1,but showed significant improvement at day 7 ( P < 0.05 ).The change of GR binding capacity of PMLs was similar to that of MNLs.There was no significant difference of CD4/CD8 ratio between the GC and non-GC group at day 1.The ratio of CD4/CD8 in the non-GC group was significantly higher than that in the GC group at day 10 (P <0.05).CD4/CD8 ratio in the GC group showed a slight reduction at day 10,with no significant difference from that at day 1.While,the non-GC group showed a significant increase of CD4/CD8 ratio at day 10 as compared with that at day 1 (P < 0.05 ).ConclusionLow-dose GC plays some role in the negative feedback regulation of GR binding capacity of peripheral blood leukocytes and in the inhibition of cellular immune function.
7.Surgical treatment for compression of inferior vena cava by hepatic hydatid cyst

Objective:To investigate the surgical treatment for compression of inferior vena cava (IVC) by hepatic hydatid cyst.Methods:The retrospective and descriptive study was conducted. The clinical data of 10 patients with hepatic hydatid cyst compressing inferior vena cava who were admitted to the First Affiliated Hospital of School of Medicine of Shihezi University from May 2013 to May 2018 were collected. There were 7 males and 3 females, aged (46±5)years, with a range from 38 to 51 years. Patients underwent subadventitial close total pericystectomy, subadven-titial partial pericystectomy or subadventitial close total pericystectomy with partial IVC resection according to the relative distance between hepatic hydatid cyst and IVC, and the feasibility of total pericystectomy. Observation indicators: (1) surgical situations; (2) postoperative situations; (3) follow-up. Follow-up was conducted using outpatient examination or telephone interview. Patients were followed up at the outpatient department at postoperative 3, 6, 12, 24 months using color Doppler ultrasound examination to detect the recurrence of hepatic hydatid cyst and inferior vena cava occlusion. The endpoint of follow-up was 2 years after operation, and the secondary endpoint of follow-up was recurrence of hepatic hydatid cyst. The follow-up time was up to May 2020. Measure-ment data with normal distribution were represent as Mean±SD, and count data were described as absolute numbers. Results:(1) Surgical situations: 10 patients underwent surgery successfully without massive hemorrhage and septic shock during the operation, including 7 patients undergoing subadventitial close total pericystectomy, 2 patients undergoing subadventitial partial pericys-tectomy, and 1 patient undergoing subadventitial close total pericystectomy with partial IVC resection. The operation time and volume of intraoperative blood loss of 10 patients were (99±27)minutes and (99±48)mL, respectively. (2) Postoperative situations: the time to drainage tube removal and duration of postoperative hospital stay of 10 patients were 2-5 days and (7.1±1.8)days. None of patients underwent postoperative bleeding, residual cavity infection, biliary fistula, or perioperative death. (3) Follow-up: of 10 patients, 9 patients completed the endpoint of follow-up and no recurrence of hepatic hydatid cyst or inferior vena cava occlusion formation was detected. The other 1 patient was followed up at postoperative 3, 6 months but failed to be followed up at postoperative 12 months, and no recurrence of hepatic hydatid cyst or inferior vena cava occlusion formation was detected during the follow-up time.Conclusion:Complete resection of hepatic hydatid cyst can be achieved by subadventitial close total pericystectomy, subadventitial partial pericystectomy or subadventitial close total pericystectomy with partial IVC resection.
8.Effect of protoscolex on T subsets in mice spleen cells in vitro

Objective:To observe the effect of protoscolex on Th subsets and correlative cytokine in mice spleen cells in vitro.Methods:Co-culture spleen cells from BALB/c mice with protoscolices,then IL-4,IFN-γand TGF-βproduction in cell culture supernatants were analyzed by ELISA.The percentage of Th subsets were detected by Flow Cytometry analysis.Results:Secretion levels of IL-4 and TGF-βwere significantly increased in spleen cells at different time point in co-culture system with protoscolices.Ratios of Th2 and Treg cells were also significantly increased in co-culture system at different time points than the control groups.However,there was no statistical significance for ratio of Th1 cells at different time points.Conclusion:The protoscolex can increase the ratios of Th2 cells and Treg cells from spleen cells.Secretion levels of IL-4 and TGF-βwere also increased in spleen cells co-cultured with protosco-lices.The results suggest that these Th cell subsets play a role in the immune escape of the hydatid disease.
9.Cost effectiveness evaluation of four kinds of surgical methods for hepatic echinococcosis

We analyzed the cost effectiveness of four different surgical methods (A,B,C and D) in treatment of hepatic echinococcosis.Totally 757 cases of hepatic echinococcosis surgery clinical data and cost information of hospitalized cases were collected from nine hospitals in Xinjiang during 2005-2013.The clinical effects,cost effectiveness ratio and incremental costeffectiveness ratio were analyzed and compared.The total complication rates were 15.8%,9.2%,0% and 2.9%;the recurrence rates were 7.3%,6.2%,0% and 0%;the cure rates were 77.8%,84.6%,100.0% and 97.1%;the cost (RMB) were 11 947.3,18 543.6,25 510.7 and 18 877.4,cost-effectiveness ratio were 153.6,219.1,255.1 and 194.3,incremental cost-effectiveness ratio /C//E were 964.4,610.4 and 358.0 respectively for group A,B,C and D.The results after adjusting of price factor were consistent with original cost effectiveness analysis.The complete resection of inner and outer capsule (D) is the most effective and economical way of clinical operation and may worthy be promoted in treatment of hepatic echinococcosis.
10.Expression and clinical significance of aldehyde dehydrogenase 1 in nasopharyngeal squamous carcinoma

Objective To investigate the significance of aldehyde dehydrogenase 1 (ALDH1)expression in appraising the prognosis of nasopharyngeal squamous carcinoma (NPC).Methods The expression of ALDH1 protein was detected by immunohistochemistry in human specimens obtained from 120 NPC patients without the history of radiotherapy or chemothetapy.Results A relatively high expression of ALDH1 were observed in 40.0 % (48/120) patients of NPC,the expression of ALDH1 in the edge of the cancer nests and stroma,particularly spindle cancer cells were strongly positive.The positive expression of ALDH1 was closely associated with patients' nasopharyngeal tumor size (P =0.011),lymphatic metastasis (P =0.005) and clinic stage (P =0.001),but not associated with their gender and age (both P > 0.05).The Kaplan-Merier and Cox regression analysis indicated that ALDH 1 closely correlated with clinical progrosis of the patients.It showed that ALDH1 was an independent risk factor which may affect the prognosis of NPC patients.Conclusion The expression of ALDH1 protein closely correlate with clinic outcome of NPC,suggesting that ALDH1 is a risk factor for clinic progrosis of NPC patients.
