1.Application of flow cytometry in the judgment of apoptosis and prognosis in patients with acute leukemia

Objective To investigate the clinical significance of flow cytometry(FCM)in cell apoptosis and prognosis in patients with acute leukemia.Methods FCMpropidium iodide(PI)DNA quantitative stain was used in 55 patients with acute leukemia(acute lymphoblastic leukemia treatment in 20 cases,alleviating period of 25 cases, 10 cases of acute non lymphocytic leukemia)to detect cell apoptosis and DNA cycle content changes,and compared with the control group.Results Acute leukemia cell apoptosis percentage Apo% in remission period was the highest, and it was (26.70 ±4.11)%,which of the initial treatment group was (10.23 ±1.05)%,remission with early treat-ment group and control group,the differences were statistically significant(t =3.970,2.341,all P <0.05).Leukemia DNA index (DI),aneuploid (AN rate),S phase cell percentage (SPF)value were (2.03 ±0.66 ),40% and (15.68 ±2.17)respectively,which were significantly increased.Conclusion The content of DNA in patients with acute leukemia is abnormal,the amount of apoptosis,SPF and other indicators have certain reference value for the prognosis of patients with acute leukemia.
2.Detection of cerebrospinal fluid leukemia cells in the diagnosis of leukemia by flow cytometry

Objective To investigate the application and clinical significance of flow cytometry(FCM)in cerebrospinal fluid(CSF)detection in the diagnosis of central nervous system leukemia(CNSL).Methods By the CD34,CD45 based different monoclonal antibody combined FCM detection method,CSF of 100 patients with CNSL was detected multi parameter of minimal residual disease(MRD).And with the natural sedimentation of cerebrospinal fluid after smear method,CSF cytology results were compared and analyzed.Results Of the 100 patients,CSF exami-nation was positive in 42 patients.CSF cytology was positive in 16 cases,the difference was statistically significant (χ2 =4.19,P <0.05).10 cases of nervous system symptoms of CSF examination were positive,and only 5 cases of CSF cytology positive(χ2 =6.81,P =0.018).In 100 cases of MRD detection,18 cases increased in white blood cell count,FMC detected in 17 cases,cytological diagnosis of 11 cases (χ2 =5.89,P =0.024).CSF with elevated pres-sure in 11 patients,FCMpositive in 11 cases,cytology detected in 4 cases(χ2 =5.12,P =0.031).CSF protein was elevated in 19 cases,FCMdetected 17 cases,cytology detected 7 cases(χ2 =7.14,P =0.001).Conclusion FCM combined with routine CSF detection can improve the positive rate of CNSL diagnosis,and it has important significance for early detection and treatment of CNSL,and to prolong the disease -free survival of patients with leukemia.
3.Nosocomial Lower Respiratory Infection and General Anesthesia: A Risk Factor Analysis and Prophylaxis

OBJECTIVE To explore the factors and effective preventive method of nosocomial lower respiratory(infection) after general anesthesia through trachea intubation.METHODS Using bacterial culture and identification technique to detect the samples collected from pipe and the sides of anesthetic machine,from disposable virus/(bacteri)a respiratory filter and buccal and bronchial secretion of patients with general anesthesia through trachea(intubation.) RESULTS Eight of 15 anesthetic machines without disinfection were with positive bacterial culture.All sides of virus/bacteria respiratory filter,and buccal and bronchial secretion of patients during operation had same(bacteria.) No bacteria growth was found in anesthetic machine and disposable virus/bacteria respiratory filter used after operation.The same bacteria as from buccal secretion were isolated from anesthetic machine in patients(without) using virus/bacteris respiratory filter.CONCLUSIONS The(results) showed the anesthetic machine is more easy to be contaminated by patients,and the reuse of anesthetic machine is an important factor for nosocomial(lower) respiratory infection after general anesthesia through trachea intubation.Intensifying periodic disinfection of(anesthetic) machine and using virus/bacteria respiratory filter can prevent contamination between anesthetic(machine) and patients and decrease nosocomial lower respiratory(infection) after operation.
4.Prevent effects of ex vivo oxygenated blood continuous perfusion on donor-lung injury

Objective To study the protective effect of normothermic ex vivo perfusion to prevent donor-lung injury.Method After being flushed with perfadex solution through the pulmonary artery and vein,the donor lungs were divided to control group (preserved with 4℃ cold static preservation,n =8) and experimental group (preserved with ex vivo oxygenated blood continuous perfusion,n =8).Pulmonary function indexes,including pulmonary vascular resistance (PVR),lung ventilation resistance(LVR),the left inferior pulmonary vein(LIPV) blood PO2,and lung water content,were evaluated.Lung injury were studied by light microscopic observation and detecting the expression of proimflammatory factor IL-1β was detected.Result (1) Lung function evaluation:in both group,PVR,LVR and lung water content gradually increased,LIPV PO2 gradually declined;But in the same time point PVR and LVR in group were were significantly less than in control group (P<0.05),LIPV PO2 in experimental group were were significantly more than in control group(P<0.05).(2)Lung injury evaluation:Lung tissue edema classification in both group were significantly higher than in normal lung(P<0.05),but classification in experimental group were significantly lower than in control group(P<0.05).The expression of IL-1β in experimental group (0.422 ± 0.132) were significantly lower than in control group(0.578 ± 0.163,P<0.05).Conclusion Ex vivo oxygenated blood continuous perfusion without ventilator is a feasible technique for lung preservation.It ameliorates hypothermic time-dependent ischemic injury.But further systems evaluation in complete physiologic condition is necessary.
5.The effect of heart valve replacement in the same period of radiofrequency ablation on atrial fibrillation

Objective To investigate the effectiveness of heart valve replacement in the same period of radiofrequency ablation in the treatment of atrial fibrillation.Methods Eighty-six patients with atrial fibrillation underwent heart valve replacement in the same period of bipolar radiofrequency ablation system according to the maze heart valve replacement operation principle.And optimized the operation method.Results No death occurred during operation.The average ablation time was (23 ± 8) minutes with a range of 16-56 minutes.Sinus rhythm of the heart was restored after surgery in all patients (100%).The long-term success rate was 98.8% (85/86).Conclusion Our optimization of heart valve replacement in the same period of radiofrequency ablation enhanced the success rate of the operation,simplified the procedures,meanwhile it might need widely application clinically.
6.A preliminary study of the CaO/ZnO nanocore-shell structured antibacterial material

Objective:To study the antimicrobial properties of CaO/ZnO core-shell nanoparticles.Methods:The CaO/ZnO core-shell nanoparticles were prepared via precipitation method.The pH and calcium ion release from the samples which composed of eugenol and nanoparticles were examined respectively.The form of the particles was observed under electron microscope,the ions were analysed by inductively coupled plasma(ICP).The antibacterial activities against Streptococcus mutans,Enterococcus faecalis,Escherichia coli and Staphylococcus aureus were evaluated by agar diffusion test (ADT).Results:CaO/ZnO core-shell nanoparticles were spherical with core-shell structure and with the diameter of 80-90 nm.The calcium ion release and pH were gradually increasing from the nanoparticles in PBS.The antibacterial activity of CaO/ZnO core-shell nanoparticles-eugenol was significantly greater than that of iRoot SP and zinc oxide-eugenol sealer(P<0.01).Conclusion:CaO/ZnO core-shell nanoparticles possess antibacterial activity.
7.Role of growth arrest specific protein 6 in regulating macrophage polarization in wound healing in mice

Objective:To investigate the role of growth arrest specific protein 6 (Gas6) in regulating macrophage polarization in wound healing.Methods:Clean male B6 mice were randomly(random number) divided into the normal group, skin defect group, skin defect group + normal saline group (PBS group), skin defect + Gas6 (1 μg) group, skin defect + Gas6 (5 μg) group, and skin defect + Gas6 (10 μg) group. Ten mice in each group were used to observe the healing of skin wounds. Macrophages were isolated from the wound tissues of the remaining 6 mice on the fifth day after modeling. The levels of IL-6 and IL-10 were detected by enzyme-linked immunosorbent assay (ELISA), the mRNA expression levels of arginase-1 (Arg-1) and inducible nitric oxide synthase (iNOS) were detected by RT-PCR, and flow cytometry was used to detect the expression of M1 marker CD197, M2 marker CD163 and F4/80. HE staining was used to detect the pathological changes of skin wounds. Masson staining was used to analyze the granulation tissue and collagen deposition.Results:Scab began to form on the surface of the wound on the third day after the skin defect model was established. The wound area of the Gas6 treatment group was smaller than that of the PBS group, and the wound healing was better than that of the PBS group. Compared with the normal group, the proportion of CD197 in macrophages of the skin defect group was significantly increased ( P=0.00 49), the proportion of CD163 and F4/80 double positive was significantly decreased ( P=0.00 86), the level of IL-6 was significantly increased ( P=0.00 13), the level of IL-10 was significantly increased ( P=0.00 14), the level of iNOS mRNA was significantly increased ( P=0.00 8), and Arg-1 was significantly increased in the skin defect group The mRNA level was significantly decreased ( P=0.01 21), and the inflammatory infiltration was aggravated. Compared with the PBS group, the proportion of CD197 in the Gas6 treatment group was significantly decreased ( P=0.00 0), the double positive rates of CD163 and F4/80 were significantly increased ( P = 0.00 0), the level of IL-6 was significantly decreased (P = 0.00 0), the level of IL-10 was significantly increased ( P=0.00 03), the level of iNOS mRNA was significantly decreased ( P=0.00 18), the level of Arg-1 mRNA was significantly increased ( P=0.00 1), and the number of inflammatory cells and the number of collagen fibers were increased. Conclusions:Gas6 can promote the transformation of macrophages from M1 to M2 in mice with skin defect, which is beneficial to the wound healing of skin defect.
8.Weighted index of comorbidities evaluates the impact of underlying diseases on hospital mortality of pneumonia patients

Objective To estimate the validity of Charlson' s weighted index of comorbidities (WIC) used to predicting 28-day mortality among ICU pneumonia patients with underlying diseases.Methods Aretrospective analysis of 160 adult patients with pneumonia admitted to a multi-discipline ICU of Shanghai Changzheng hospital between October 2010 and February 2012 was carried out.Clinical data were collected including age,gender,community acquired pneumonia (CAP) or hospital acquired pneumonia (HAP),underlying diseases,severity-of-sepsis,and 28-day mortality.WIC scores,acute physiology and chronic health evaluation (APACHE) Ⅱ,and sepsis related organ failure assessment (SOFA) were assessed within the first 24 hours after admission.Logistic regression analyses were used to evaluate the predictors for outcome.The receiver operating characteristic curve (ROC) was used to compare the performance of these scores between different methods.Results Of 160 enrolled patients,76 (48.8%) were CAP,82 (51.2%) HAP,and 106 (66.3%) male,54 (33.7%) female,and 99 (61.9%) patients survived and 61 (38.1%) died.The average age was (62.4 ± 17.3) years old.Compared with survivors,WIC,APACHE Ⅱ and SOFA scores were significantly higher in death group (P < 0.05).The multivariate logistic regression revealed that risk of death depends predominantly on age (OR =1.049,95% CI:1.011-1.088,P =0.011),WIC (OR =1.725,95% CI:1.194-2.492,P =0.004),APACHE Ⅱ score (OR =1.175,95%CI:1.058-1.305,P =0.003),SOFA score (OR =1.277,95% CI:1.048-1.556,P =0.015),presence of ARDS (OR =0.081,95% CI:0.008-0.829,P =0.034),and complicated with severe sepsis (OR =0.149,95% CI:0.232-0.622,P =0.004).The area under the receiver operating characteristics curve in predicting mortality was 0.639 (0.547-0.730) for the WIC,0.782 (0.709-0.856) for APACHE Ⅱ score,0.79 (0.714-0.866) for SOFA score and 0.842 (0.777-0.907) for the merger of three.Conclusions In pneumonia patients of ICU,WIC is a useful approach to predicting 28-day mortality,and the risk of death significantly depends on co-morbidities.
9.Study of heart and lung protection in thoracoscopic mitral valve replacement operation on beating-heart

Objective To investigate the clinical significance of thoracoscopy combined with beating-heart technique to provide a certain reference for selecting clinical operation mode.Methods Forty patients with chronic rheumatic mitral stenosis receiving thoracoscopic mitral valve replacement surgery in the cardiothoracic surgery department of our hospital from September 2012 to September 2015 were included in this study and divided into the experimental group(beating-heart group,20 cases) and control group(arrested heart group,20 cases).The operative time,CPB time,enzymology indicators,RI and operative complications were analyzed and compared between the two groups.Results One case in each group died of postoperative low cardiac output syndrome.Other cases successfully completed the operation without serious operative complications such as low cardiac output syndrome,serious cardiac arrhythmia and air embolism.Two groups had no statistically significant difference in the operation time and time of CPB (P>0.05),but the ICU staying duration had statistical difference between the two groups(P<0.05).The levels of CK-MB,cTnI,LDH,TNF-α,IL-6,IL-8 and RI were significantly increased in two groups(P<0.05),while the levels in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion Thoracoscopic mitral valve replacement operation with beating-heart may have better protection effect on heart and lung than thoracoscopic arrested-heart mitral valve replacement operation.
10.Clinical application of upper arm central venous port in cancer patients

Objective:To investigate the clinical effect of upper arm central venous port in cancer patients.Methods:A total of 500 patients with tumor were selected as the study subjects from March 29, 2018 to January 19 2020. Complications such as catheter-related thrombosis and catheter-related bloodstream infection during the indwelling period were recorded.Results:One patient with severe superior vena cava syndrome failed to be intubated. The other 499 patients were successfully intubated with a success rate of 99.8% (499/500). The rate of complications was 2.6% (13/499), and the rate of catheter-related bloodstream infection was 0.029‰ (4/139 614) in 4 cases, the incidence of catheter-related thrombus was 0.057‰ (8/139 614) in 8 cases. There were 1 case of body turnover, 1 case of median nerve injury, 1 case of local tissue necrosis and 1 case of lymphatic leakage. Extubation was planned in 15 patients, and unplanned extubation in 6 patients due to complications.Conclusions:The upper arm port has less complications and is suitable for patients with tumor chemotherapy and long-term need of infusion. It is worthy of clinical promotion.
