1.A case-control study of risk factors for bladder cancer in Taizhou

Objective:
To analyze the influencing factors for bladder cancer in Taizhou,and to provide evidence for strengthening the prevention and treatment of bladder cancer in Taizhou.
Methods :
A total of 500 cases of bladder cancer diagnosed in Taizhou Hospital from 2012 to 2017 were selected as a case group,and 504 patients without tumor or urinary system diseases during the same period were selected as a control group. A structured questionnaire was used to retrospectively investigate the demographic information,occupational exposure(whether they were exposed to aromatic amine,polycyclic aromatic hydrocarbons,tobacco,tobacco smoke or heavy metals at work),healthy behaviors and diets of the two groups one year before admission. A Logistic regression model was used to analyze the influencing factors for bladder cancer.
Results :
There was no significant differences in sex,age,ethnicity,education and marital status between the case group and the control group(P > 0.05). Patients with occupational exposure history accounting for 31.60% in the case group and 24.60% in the control group,for overweight/obesity were 37.60% and 31.74%,for smoking were 55.80% and 46.23%,for high vegetable intake frequency were 43.80% and 52.58%,for high fruit intake frequency were 55.40% and 62.70%,for physical activity were 24.60% and 31.75%,respectively. The results of multivariate logistic regression analysis showed that occupational exposure(OR=1.861,95%CI:1.229-2.836),overweight/obesity(OR=1.374,95%CI:1.021-1.863),current smoking(OR=1.664,95%CI:1.101-2.503)and previous smoking(OR=1.454,95%CI:1.016-2.066)were the risk factors for bladder cancer. High vegetable intake frequency(OR=0.731,95%CI:0.566-0.947),high fruit intake frequency(OR=0.659,95%CI:0.463-0.927)and vigorous physical activity(OR=0.566,95%CI:0.403-0.798)were the protective factors for bladder cancer.
Conclusion
Occupational exposure,overweight/obesity,current smoking,previous smoking were the risk factors for bladder cancer. High vegetables intake frequency,high fruit intake frequency and vigorous physical activity were the protective factors for bladder cancer.
2.Surveillance of schistosomiasis japonica in potential endemic areas in Hubei Province from 2008 to 2012

Objective To explore the potential key risk factors of schistosomiasis transmission in potential endemic areas so as to provide the evidence for setting up the prediction and surveillance systems of schistosomiasis outbreak epidemic. Methods From 2008 to 2012,fixed and mobile surveillance sites in potential endemic areas of 2 counties in Hubei Province were selected. The immunological assays and stool examinations were carried out to investigate the schistosome infection situation of local people, mobile population and livestock. The distribution of Oncomelania hupensis snails was investigated in risk areas and suspicious ar-eas,and spreading patterns of snails were observed in the rivers that directly connected with the Yangtze River. Results A total of 6 052 local people aged 6-65 years were screened by IHA immunological tests,and the positive rate of antibody was 1.19%(72/6 052). Totally 72 antibody positives were examined by Kato-Katz technique and there were no positives. A total of 5 004 mo-bile persons were tested by IHA immunological tests and the positive rate was 1.36%(68/5 004). Totally 68 antibody positives were examined by Kato-Katz technique and there were no positives. Totally 287.07 hm2 potential endemic areas were investigated for Oncomelania snail detection,and no snails were found. The investigation on snail spreading patterns and the surveillance on suspicious circumstances were carried out,with no snails found. Conclusions In the schistosomiasis potential endemic areas, some positives of IHA immunological tests are found. Therefore,monitoring is still needed to be strengthened.
3.Surveillance of schistosomiasis in areas along water transfer project from Yangzte River to Hanjiang River in Qianjiang basin

Objective To evaluate the impact of the water transfer project from the Yangtze River to the Hanjiang River on the transmission of schistosomiasis.so as to provide scientific basis for the development of control strategy.Methods A field investigation on snail status,the infection situation of human and cattle as well as related endemic factors was performed in the project areas of the Qianjiang basin.Results There were 33 endemic villages and 48 non-endemic villages along the water transfer project.In endemic villages,the infection rates of human and cattle were 0.42% and 0.48%,respectively.In the endemic area,the snail area was 17.28 hm~2,the mean density of living snails was 0.043 No./0.1 m~2,meanwhile,no snail Was found in the non.endemic areas.Conclusion The surveillance on epidemic situation should be carried out for a long time in order to prevent the spreading of snails.
4.Composition of urinary stones in Taizhou of Zhejiang province

Objective To analyze the composition of urinary stones in Taizhou of Zhejiang province.Methods Clinical data of 1 022 patients with urinary stones admitted in Taizhou Hospital of Zhejiang province were retrospectively reviewed.The samples of urinary stones were collected and analyzed by infrared spectrophotometry.Results There were 722 males and 300 females with a male to female ratio of 2.4:1 and with a mean age of (53.4±13.6) years (14-88 years).Among 1 022 patients,the stones with single composition were found in 299 cases (29.3%);the most common single-component was anhydrous uric acid (15.9%),followed by calcium oxalate monohydrate (12.0%).The mixed stones were found in 723 cases (70.7%);the most common mixture was calcium oxalate monohydrate,calcium oxalate dehydrate and carbonate apatite mixture (316 cases,30.9%).Calcium oxalate (58.9%,602/1 022) was the most common major component and frequently seen in upper urinary tract stones,followed by uric acid (21.8%,223/1 022).Uric acid calculi was predominant component in male patients(χ2=30.97,P=0.00),while the rate of infection stones was higher in women (χ2=60.69,P=0.00).The mean age of patients with uric acid stones was 59.4 years,which was older than that with other components (t=7.62,P=0.00).The uric acid stones were more common in upper urinary tract stones(χ2=42.97,P=0.00).The mean age of patients with infection stones was 49 years,which was younger than that with other types of stones(t=4.87,P=0.00).Conclusion Mixed stones with calcium oxalate monohydrate,calcium oxalate dehydrate and carbonate apatite are the predominant urinary stones in Taizhou of Zhejiang province,while the most common single-component stones are anhydrous uric acid stones.Location,age and sex are associated with the types of urinary stones.
5.Construction and evaluation of bone metastasis of breast cancer in nude mice model

Objective To establish nude mice model of bone metastasis of breast cancer,and study the best method to evaluate the model.Methods The human breast cancer cell,MDA-MB-231PEGFPN,was injected into left ventricule of nude mice to construct the bone metastasis of breast cancer animal model.Methods of X-ray,whole body optical imaging and pathology were used to evaluate the bone damage after six weeks.Results After six weeks,the result of pathology showed that breast cancer cells had already spread to brain,bone marrow and lung of nude mice.The degree of bone destruction was seen clearly in X-ray,and location of breast cancer cells was observed by whole body optical imaging system.Conclusions The results showed that combined application of X-ray,whole body optical imaging and pathology are the best modalities to evaluate bone metastasis of breast cancer in nude mice.
6.Retroperitoneoscopic adrenalectomy in semilateral supine position

Objective To discuss the semilateral supine position for retroperitoneoscopic adrenalectomy. Methods From Jan. 2006 to Dec. 2008, 36 patients (20 males and 16 females with mean age of 43 years) underwent retroperitoneoscopic adrenalectomy in 60° -70° semilateral supine position. There were adrenal cortex adenomas in 18 cases, pheochromocytoma in 6 cases, adrenal cysts in 3 cases, myelolipoma in 2 cases, gangliocytoma in 1 case, lymphangioma in 1 case, metastatic tumor in 1 case and corticohyporplasia in 4 cases. The mean diameter of the tumors was 2.6 cm( 0.5 - 7.7 cm ). The tumors were superior to the renal pole in 5 cases, anteromedial in 10 cases and superomedial in 17 cases. The three ports that were usually used in lateral position and were placed anteriorly to create retroperitoneal place: the first port was placed 2 -4 cm superior to the iliac crest along the anterior axillary line, the other two were placed just below the costal margin along the midaxillary line and at the same level along the midclavicular line, and dissected along the anterior surface of kidney to its superomedial aspect, so as to avoid the hampering of the kidney in the exposing of the diseased adrenal gland. Results The procedure was completed successfully in all of the cases with the operating time of 37 - 145 min ( mean 69 min) and intraoperative blood loss of 30 - 100 ml (mean 48 ml). Six cases had rupture of peritoneum, which were sutured and the procedure was continued to completion. The postoperative hospital stay was 3 -8 d (mean 5 d ). Thirty-five patients were available for follow-up of 3 - 28 months ( mean 14 months). The case of metastatic tumor died of the primary diseases in the 12th month postoperatively. No other complication was found. Conclusion With this alternative position and ports' location, the procedure of retroperitoneoscopic adrenalectomy could be easier and safer than the conventional position.
