1.Evaluation of the effect of evidence-based nursing model in improvement of the quality of life and emotional state of parturients with natural childbirth during perinatal period

Objective To investigate the clinical effect of evidence-based nursing model on improvement of the quality of life and emotional state of parturients with natural childbirth during perinatal period. Methods 120 cases of parturients with natural childbirth from November 2009 to November 2010 in our hospital were chosen.They were randomly divided into the control group and the observation group with 60 cases in each group.The control group was taken conventional methods of nursing care,and the observation group was given evidence-based nursing.The anxiety and depression scores and quality of life score,pain score of the first labor stage of the two groups were compared.The clinical effect of evidence-based nursing on improvement of the quality of life and emotional state of parturients with natural childbirth was evaluated comprehensively. Results After observation,the quality of life prenatal and postnatal for the two groups were compared,the observation group had obvious advantages than the control group,the difference between the two groups was significant.The anxiety and depression scores antepartum and postpartum for the two groups were compared,the observation group was lower than the control group,and the difference between the two groups was significant.The labor pain of the first stage was compared,the rate of the third-grade pain in the observation group was significantly lower than the control group. Conclusions Evidence-based nursing has been applied effectively into the clinic.It has been proven that effective measures can prevent and improve the questions that have arisen or prone to arise for perinatal stage.It can improve the quality of life and improve the emotional state for maternal perinatal period.It is conducive to maternal and child health,and is worthy of clinical application.
2.Influence of humanized management mode on infection consciousness and infection control effect of medical staff

ObjectiveTo study the influence of humanized management mode for infection consciousness and control effect of infection of medical staff. Methods90 medical staff from June 2010 to March 2011 were selected as research object,and they were randomly divided into the control group and the observation group with 45 cases in each group,then the infection consciousness,grasping situation of related knowledge and infection control effect of two groups before and after the management on the third and sixth month were analyzed and compared.Results The infection consciousness,grasping situation of related knowledge and infection control effect of the observation group after the management on the third and sixth month were all better than those of the control group,there were significant differences. ConclusionsThe humanized management mode plays an active role in improving infection related evaluation indices of hospital staff,and is also effective in control of infection.
3.Retinal Changes of Intravitreal Triamcinolone Acetonide: a Pathohistological Study

Objective To investigate the effect of triamcinolone acetonide(TA) with different dosage and place of production on retina.Methods Twenty-four rabbits were divided into 6 groups(A:untreated as a blank control;B: normal sodium;C:10mg imported TA;D:10 mg domestic-made TA;E:25 mg imported TA;F:25 mg domestic-made TA).Excepting group A every right eyeball underwent vitrectomy before intravitreal injection triamcinolone acetonide.All the eyeballs were extirpated on the 21st day after the intravitreal injection,and then pathological examinations were performed.Results Ganglion cell's light edemata had been seen in group B,C,D,E,F.Cell hyperplasia and vacuolar degeneration had been seen in the internal granular layer or external granular layer in some animals of group E,F.Two samples(1 in group E,1 in group F)observed there were a great amount of fibrinoid materials in cavum vitreum and cavum between amphiblestroid membrane and choroid membrane.Pathological changes in group E,F were severer than in group C,D.Conclusions Intravitreous injection TA at 25 mg can cause photoreceptor cells hyperplasia in rabbits,while there are no toxic effects at 10 mg.No obvious difference is observed between the groups of imported TA and domestic-made TA.
4.Application of the Integrated Teaching Method of PBL and TBL Based on Cases in Pharmacology Teaching

Objective To explore the apphcation of the integrated teaching method of PBL and TBL based on cases in pharmacology teaching.Method Ninety-two students majored in Pharmaceutical Preparations were selected into the control group,and 184 students majored in pharmacy were enrolled into the experimental group.The teaching effect was comprehensively evaluated by using the combination of theoretical examination and student feedback.Results In the experimental group,the average score of students in Pharmacology (73.68 ± 9.40) was significantly higher than that of the control group (67.34 ± 7.18),and the feedback of the students in the experimental group was significantly better than that in the control group.Conclusion Compared with the traditional teaching method,the integrated teaching method can significantly improve the students' theoretical examination results and students' learning interest.
5.Inhibitory effects of Huoxue Jiedu decoction on cellapoptosis via down-regulating 8-OhdG in obstructive nephropathy rats

Aim To observe the effect of eplerenone(EPL) and Chinese decoction on cell apoptosis in obstructive nephropathy rats.Methods Sixty male Wistar rats were randomly divided into sham group, UUO group, EPL group and ZY group(n=15).Except sham group, the rats in the other groups were ligated with unilateral ureteral obstruction(UUO) for renal interstitial fibrosis model.The rats were treated with eplerenone at 100 mg·kg-1·d-1 added to diet in EPL group, and orally 13.7 g·kg-1·d-1 decoction of Chinese medicine in ZY group.The kidneys were harvested on 14th day, the number of renal cell apoptosis were detected by TUNEL, and serum aldosterone and 8-OhdG were detected with radioimmunoassay and ELISA.Caspase-12, caspase-9, Bax and Bcl-2 were examined by immunohistochemistry and Western blot.Results The levels of serum aldosterone, serum and urine 8-OhdG and the number of positive apoptotic cells increased significantly in UUO rats compared with Sham group.The overexpression of caspase-9, caspase-12 and Bax and down-regulated Bcl-2 were obvious in UUO group(P<0.01).The level of 8-OhdG, expression of caspase-9, caspase-12 and Bax were down-regulated, and Bcl-2 expression was up-regulated in eplerenone and Chinese decoction treated rats(P<0.01).Conclusion Eplerenone and Chinese decoction could inhibit cell apoptosis induced by oxidative damage after UUO via caspases and(or) Bax pathway.
6.Qualitative study on hospice care needs of patients with end-stage cancer

Objective:To understand the needs of end-stage cancer patients for hospice care, so as to provide a basis for the development of human-centered quality nursing services in hospice care.Methods:13 patients with end-stage cancer in tumor ward were interviewed by personal in-depth interview, and the data were sorted and analyzed by Colaizzi's seven-step analysis method.Results:A total of four themes of hospice care needs of patients with end-stage cancer were extracted. The need to maintain self-subject: the desire to maintain self-efficacy and self-image of dignity in life; the need to relieve the symptoms of self-discomfort. The need to relieve the physical symptoms and psychological discomfort. Meet the needs of self-determination: participate in medical care decisions and choose comfortable social relationships. Express their own emotional needs: the past farewell and remembrance and future wish to achieve the expectationsConclusions:Patients with end-stage cancer have diverse needs for hospice care. In clinical work, medical staff should adhere to the people-oriented concept, explore patients' needs and preferences, and implement individualized quality nursing measures to meet patients' needs for hospice care.
7.Advances of functionalized carbon nanotubes in diagnosis and treatment of tumor

Carbon nanotubes have become a kind of superior antitumor drug and gene carrier due to their special cylindrical structure with high loading capacity and easy penetration into the cell membrane. With the gradual development of researches on carbon nanotubes, their applications in tumor photothermal therapy and diagnosis have also become current research hotspots. However, chemical inertness and aggregation of carbon nanotubes have limited their clinical application. To overcome their drawbacks, researchers would functionalize the carbon nanotubes before further researches. In this article, applications of functionalized carbon nanotubes in cancer chemotherapy and gene therapy, photothermal therapy and cancer diagnosis are summarized.
8.Therapeutic effect of allisartan isoproxil tablet on carotid atherosclerosis lesion in patients with essen‐tial hypertension/

Objective :To evaluate therapeutic effect of allisartan isoproxil tablet on carotid atherosclerosis (CAS) le‐sion in patients with mild to moderate essential hypertension (EH).Methods :A total of 120 patients with mild to moderate EH complicated CAS treated in our hospital were enrolled .They were randomly divided into valsartan group (n=60 ,received valsartan 80mg/d) and allisartan isoproxil group (n=60 ,received allisartan isoproxil tablet 240mg/d) ,both groups received measure by carotid vascular ultrasound before treatment ,24 weeks and 48 weeks af‐ter treatment and its result were compared between two groups .Results :Compared with before treatment ,there were significant reductions in IMT ,size ,thickness and number of carotid atherosclerotic plaques on 24 weeks and 48 weeks after treatment in two groups ,and above indexes of 48 weeks were significantly lower than those of 24 weeks , P<0.05 or <0.01. The decreased value of IMT in allisartan isoproxil group was significantly higher than that of valsartan group ,and there were no significant difference between two groups in time point of size ,thickness and number of carotid atherosclerotic plaques , P>0.05 all.Conclusion :Therapeutic effect of two drugs on size ,thick‐ness and number of carotid atherosclerotic plaques are similar ;but therapeutic effect of allisartan isoproxil tablet on IMT is significantly better than that of valsartan .
9.Case-control study on relationship between diet quality and thyroid nodules

Background Both domestic and foreign studies on the diet quality of patients with thyroid nodules are rare at present, and the relationship between thyroid nodules and diet quality is still unclear. Objective This study aims to evaluate the diet quality of thyroid nodules with the China Healthy Diet Index (CHDI) and to explore the relationship between diet quality and thyroid nodules. Methods We conducted a hospital-based case-control study in which 387 patients with thyroid nodules were matched individually (1∶1) by gender and age (±3 years) to the controls who ordered a routine physical examine and with negative thyroid nodules reported by B-ultrasound in the same hospital. A structured questionnaire was applied to collect data on general characteristics and diet. CHDI was employed to evaluate the diet quality of the two groups. CHDI is based on the Dietary Guidelines for Chinese Residents and the recommended food intake in the balanced diet pagoda, combined with the dietary nutritional status of Chinese population. The index contains 13 scoring components, and the highest total score is 100, with a higher score representing better dietary quality. CHDI scores < 60, 60-<80, and ≥80 are recognized as unqualified, qualified, and good diet quality, respectively. Results The scores of the thyroid nodule cases were lower than the scores of the controls in total vegetables, dark vegetables, fruits, and fish/shrimps (all Ps<0.05). The nonparametric test results found that there were significant differences in the distributions of scores in total vegetables, dark vegetables, fruits, soybeans, and fish/shrimps between the case and the control groups (all Ps<0.05). The percentages reaching the full scores in food variety, meat and eggs, and empty calories in both groups exceeded 60%, while the percentages reaching the full scores in whole grains/beans/tubers, dairy, soybeans, and sodium were lower than 30% in the two groups. The median CHDI scores of the thyroid nodule cases and the controls were 69.1 and 72.9, respectively; the cases' CHDI score was lower than the controls' (P<0.05). The proportions of unqualified diet in the case and the control groups accounted for 23.0% and 13.7% respectively (P=0.001). After being adjusted for education level, marital status, occupation, monthly household income per capita, body mass index, hypertension/diabetes, energy, and iodine intake, the results of logistic regression analysis showed that the risk for thyroid nodules was negatively associated with CHDI score (CHDI qualified group vs. CHDI unqualified group, OR=0.484, 95%CI: 0.291-0.804; CHDI good dietary quality group vs. CHDI unqualified group, OR=0.414, 95%CI: 0.230−0.746). Conclusion The diet quality of patients with thyroid nodules is lower than that of the control group, as the former reported lower consumptions of total vegetables, dark vegetables, fruits, and fish/shrimps. Those who have good or qualified diet quality show a reduced risk of thyroid nodules than those with unqualified dietary quality.
10.Psilocybin facilitates fear extinction in mice by promoting hippocampal neuroplasticity.

BACKGROUND:
Posttraumatic stress disorder (PTSD) and depression are highly comorbid. Psilocybin exerts substantial therapeutic effects on depression by promoting neuroplasticity. Fear extinction is a key process in the mechanism of first-line exposure-based therapies for PTSD. We hypothesized that psilocybin would facilitate fear extinction by promoting hippocampal neuroplasticity.
METHODS:
First, we assessed the effects of psilocybin on percentage of freezing time in an auditory cued fear conditioning (FC) and fear extinction paradigm in mice. Psilocybin was administered 30 min before extinction training. Fear extinction testing was performed on the first day; fear extinction retrieval and fear renewal were tested on the sixth and seventh days, respectively. Furthermore, we verified the effect of psilocybin on hippocampal neuroplasticity using Golgi staining for the dendritic complexity and spine density, Western blotting for the protein levels of brain derived neurotrophic factor (BDNF) and mechanistic target of rapamycin (mTOR), and immunofluorescence staining for the numbers of doublecortin (DCX)- and bromodeoxyuridine (BrdU)-positive cells.
RESULTS:
A single dose of psilocybin (2.5 mg/kg, i.p.) reduced the increase in the percentage of freezing time induced by FC at 24 h, 6th day and 7th day after administration. In terms of structural neuroplasticity, psilocybin rescued the decrease in hippocampal dendritic complexity and spine density induced by FC; in terms of neuroplasticity related proteins, psilocybin rescued the decrease in the protein levels of hippocampal BDNF and mTOR induced by FC; in terms of neurogenesis, psilocybin rescued the decrease in the numbers of DCX- and BrdU-positive cells in the hippocampal dentate gyrus induced by FC.
CONCLUSIONS
A single dose of psilocybin facilitated rapid and sustained fear extinction; this effect might be partially mediated by the promotion of hippocampal neuroplasticity. This study indicates that psilocybin may be a useful adjunct to exposure-based therapies for PTSD and other mental disorders characterized by failure of fear extinction.
