1.Effects of Different Preparation Technologies on the Content Determination of Alkaloids in Aconitum carmichaeli by RP-HPLC

OBJECTIVE:To compare the different sample preparation and optimize the isolation of three kinds of alkaloids in Aconitum carmichaeli.METHODS:RP-HPLC was applied and performed on Zorbax extend-C18 column (150 mm?4.6 mm,5 ?m) column.The differences among the content of alkaloids prepared by preparation technologies were compared.RESULTS:The best preparation was aether ultrasonic extraction.The separation condition was as follows:mobile phase consisted of acetonitrile-0.1% ethylenediamine (gradient elution) with flow rate of 1 mL?min-1.The detection wavelength was set at 240 nm and column temperature was 30 ℃.CONCLUSION:Aether ultrasonic extraction is simple with ideal separation,extraction and re-covery to provide scientific basis for the quality control of A.carmichaeli.
2.Clinical significance of combined detection of P53 and Ki67 in patients with esophageal carcinoma

Objective To investigate the clinical significance of expression of P53,Ki67 in proliferation,invasion and metastasis of cancer cells of patients with esophageal carcinoma.Methods Eighty-six patients with esophageal carcinoma were selected as tumor group from January 2006 to May 2008.And another 86 patients without tumor were as control group.ABC-immunohistochemistry was used to detect the expression of P53 and Ki67 in 172 cases of patients with esophageal carcinoma.Results The expression of P53 in patients with esophageal carcinoma was different from that in control subjects (x2 =4.045,P < 0.05),and the P53 expression rate in two group were 66.3% (57/86) and 51.1% (44/86) respectively.The rate of P53 expression reaching to + + in two groups was also significant (x2 =8.592,P < 0.01).Ki67 expression in patients with esophageal carcinoma was higher than that of control patients (76.7% (66/86) vs.60.5% (82/86),x2 =5.291,P < 0.05).Additionally,there was significant difference regarding of Ki6 expression reaching to + + positive(x2 =13.661,P < 0.01).Moreover,the expression of P53 and Ki67 were found to be a positively correlation(r =0.400,P =0.00).The expression of P53 and Ki67 were related with invasion and differentiation of cancer cells (x2 =3.945,5.794 respectively,all P < 0.05) and lymph node metastasis (x2 =5.570,4.354 respectively,all P < 0.05).Conclusion Over-expression of P53 and Ki67 were involved in the process of proliferation,invasion and metastasis of cancer cells in esophageal carcinoma,which coherently impacted the occurrence and development of esophageal cancer.The combined detection of P53 and Ki67 seems to help the early diagnosis of esophageal cancer and evaluation of the degree of malignancy,invasion,metastasis and prognosis.Over-expression of P53,Ki67 might be served as a reference marker in screening for chemotherapy for esophageal cancer.
3.Meta-analysis on interspinous dynamic stabilization system Wallis versus Coflex for lumbar degenerative disease

BACKGROUND: It is still controversial that interspinous dynamic stabilization system Wallis and Coflex which one can provide better clinical effects for lumbar degenerative disease.OBJECTIVE: To systematically assess the clinical effectiveness and safety of Wallis and Coflex for lumbar degenerative disease.METHODS: According to the computer-based online search of PubMed, Embase, Medline, Cochrane Library, CBM,CNKI, Wanfang Database, and VIP, articles published before August 1st, 2016 were searched. Articles about Wallis comparing with Coflex for lumbar degenerative disease were included; the quality score of methodology was assessed by MINORS. Research data abstracted and synthesized by Review Manager 5.3 were used for meta-analysis.RESULTS AND CONCLUSION: (1) Six studies were included, and all studies were designed for non-randomized controlled trial. (2) There were no significant statistical differences in Japanese Orthopedic Association, Oswestry disability index, visual analogue scale score, Prolo functional score, segmental lordosis angle, and segment movement degree. Incidence of adverse events was significantlue scale less in the Wallis group than in the Conflex group (P < 0.05).(3) There was no significant difference in clinical efficacy between Wallis and Coflex in the early and mid-term follow-up.We can conclude that Wallis may provide better clinical safety than Coflex.
4.Efficacy of carbamazepine combined with flunarizine hydrochloride for treating tinnitus.

OBJECTIVE:
To determine whether carbamazepine is effective in treating the subjective tinnitus.
METHOD:
Randomized, prospective,double-blind,controlled trial was used in our study. The study group consisted of 100 adult patients who consulted our outpatient clinic complaining of subjective tinnitus, excluded objective tinnitus and the patients who had tinnitus caused by obvious diseases, such as outer and middle ear diseases, 50 patients were given carbamazepine and Flunarizine Hydrochloride, 50 patients were given Vitamin B6 and Flunarizine Hydrochloride. After a week the effect of the different group of medicines was observed. Tinnitus questionnaire was performed before the treatment, and pure tone audiogram, tinnitus pitch and loudness matching were performed at the end of the treatment.
RESULT:
Completion of treatment, tinnitus loudness matching assessment showed that the efficacy of the carbamazepine group was similar to that of the control group. The efficacy of treatment was respectively 26% by intend to treat (ITT) and 28.3% by per protocol (PP) in the carbamazepine group and 26% by ITT and 27.7% by PP in the control group. The efficacy of treatment has no statistically significance for tinnitus loudness of the experimental group and the control group. The subjective tinnitus improvement rate showed no difference between two groups. Pure tone thresholds fluctuated within 10 dB in the beginning and at the end of the treatment. There were serious side effects in the carbamazepine group. The side effects rates were respectively 55.3% and 16.7% in the carbamazepine group and the control group, respectively. The difference had statistical significance.
CONCLUSION
Our results showed that the efficacy of carbamazepine combined with Flunarizine Hydrochloride is similar to that of the control group. There was no improvement in listening. But the side effects of it were more serious than that of the control group. It should not be recommended for the treatment of tinnitus.

5.The clinical significance of sentinel lymph node biopsy in breast cancer surgery

Objective To evaluate the accuracy of sentinel lymph node biopsy (SLNB) in predicting the axillary lymph node status in breast cancer patients undergoing a breastectomy. Methods From Mar 2002 to Jun 2003,patent blue dye and/or 99m Tc-Sc were used to detect sentinel lymph nodes,samples were sent for fast pathology.Three patients in which the SLN were judged as negative by both the two tests were freed of futher axillary lymph node dissection (ALND). Results SLN were successfully identified in 78 of 81 (96.3%) patients. The accuracy rate of SLNB to predict axillary lymph node status was 97.5%,with 9.7% false negative rate. The success rate,accuracy rate and false negative rate by blue dye only and by a combination of two above methods were 92.5%,94.2%,15.8%,and 100%,100%,0, respectively . Conclusions SLNB accurately predict the axillary status in clinically node negative breast cancer patients .
6.Micro RNA -433 inhibits proliferation and invasion of glioma cells by targeting histone deacetylase 6

Objective To detect the expression of micro RNA (miR)-433 and histone deacetylase 6 (HDAC6) in glioma tissues and investigate the effect of miR-433 on cell proliferation and invasion of human glioma cell line U251. Methods (1) Forty-two glioma samples, collected from patients accepted surgical resection and conformed by pathology in our hospital from January 2010 to December 2014, and 13 healthy brain tissues, collected from patients accepted surgery for craniocerebral trauma at the same time period, were used in our study; reverse transcription (RT)-PCR was used to detect the mRNA expression levels of miRNA-433 and HDAC6 in the glioma samples and brain tissues. (2) Human glioma cell line was routinely cultured and divided into blank control group, nonsense sequence control group and miRNA-433 mimics group;cells in the later two groups were transfected with nonsense sequences or miRNA-433 mimics, and cells in the blank control group did not give any treatment;the mRNA expression levels of miRNA-433, P21 and HDA C6 in these 3 groups were detected by RT-PCR;the cellular viability was measured by CCK-8 assay;flow cytometry was used to monitor the changes of cell cycle and apoptosis; cell invasion was evaluated by Transwell assay; HDAC6 protein expression was detected by Western blotting. (3) Wide-type (WT)HDAC63'-UTR and mutant type (MUT)HDAC63'-UTR luciferase report vectors were established; miR-433 mimics+WT HDAC63′-UTR and nonsense sequences+WT HDAC63'-UTR were transfected into the U251 cells, and dual-luciferase experiment was used to detect the fluorescence intensity of the cells; miR-433 mimics+MUT HDAC63'-UTR and nonsense sequences+MUT HDAC63'-UTR were transfected into the U251 cells, and dual-luciferase experiment was used to detect the fluorescence intensity of the cells. (4) U251 cells were divided into nonsense sequence control group, HDAC6 expression plasmids group and HDAC6 siRNA group, and nonsense sequences, HDAC6 expression plasmids or HDAC6 siRNA were transfected respectively; RT-PCR was used to detect the P21 and HDAC6 mRNA expressions and miRNA-433 expression; U251 cells were divided into miR-433 mimics group and miR-433 mimics+HDAC6 expression plasmids group, and miR-433 mimics or miR-433 mimics+HDAC6 expression plasmids were transfected, respectively, and one-5 d after that, CCK-8 was used to detect the cellular viability. Results (1) The miRNA-433 expressions gradually increased and HDA C6 mRNA expressions gradually decreased in the high-grade gliomas, low-grade gliomas and normal brain tissues, and significant differences were noted among each two groups (P<0.05);the miRNA-433 expression was negatively correlated with HDA C6 mRNA expression in the glioma tissues (r=0.829, P=0.000). (2) As compared with blank control group and nonsense sequence control group, miRNA-433 mimics group had significantly higher miRNA-433 and P21 mRNA expressions, cell percentage at G0/G1 stage, and apoptotic rate (P<0.05), and had statistically lower HDAC6 mRNA expression, cellular viability on 2-5 d of culture, number of transmembrane cells and HDAC6 protein expression (P<0.05). (3) The luciferase activity in cells from miR-433 mimics+WT HDAC63'-UTR group was significantly lower as compared with that in the nonsense sequences+WT HDAC63'-UTR group (P<0.05);the luciferase activity in cells from miR-433 mimics+MUT HDAC63'-UTR group and nonsense sequences+MUT HDAC63'-UTR group showed no significant differences (P>0.05). (4) The HDA C6 mRNA expressions were gradually increased, and P21 mRNA expressions were gradually decreased in the HDAC6 siRNA group, nonsense sequence control group, and HDAC6 expression plasmids group, with significant differences (P<0.05);on 2-5 d of culture, the cellular viability in the miR-433 mimics+HDAC6 expression plasmids group was significantly higher than that in the miR-433 mimics group (P<0.05). Conclusions The miRNA-433 expression level is low in human glioma tissues;miRNA-433 over-expression may inhibit the cell activity and promote cell apoptosis of glioma cell line U251 in vitro via inhibiting the HDAC6 expression.
7.Preoperative high-dose chemotherapy with peripheral blood stem cell support in breast cancer: report of 3 cases.

OBJECTIVETo investigate the possibility and short-term effect of high dose chemotherapy with peripheral blood stem cell support in the preoperative therapy of breast cancer, and-its influence on the following operation and would healing.
METHODSThree patients with T(3)N(1)M(0) (III(a)), T(4)N(1)M(0) (III(b)), T(4)N(1)M(1) (IV) of breast cancer were diagnosed histopathologically. After receiving HDC/APBSCT, the 3 patients were operated on. HDC/APBSCT process included 2 cycles of FEC induction chemotherapy; PBSC mobilization, APBSC collection and cryopreservation and PBSC infusion; and high-dose chemotherapy, APBSC infusion and supportive therapy. The therapy consisted of CTX2.5 g/m(2), VP-16 600 mg/m(2), and cerboplatin 600 mg/m(2) delivered on day 1, APBSC infusion 48 h later, rhG-CSF (150 microg, BID) was administered 4 h after infusion of APBSC until WBC was higher than 10 x 10(9)/L. During HDC/APBSCT, the patients were protected in the air laminar flow room with supportive therapy of antibiotics, anti-virus and anti-fungus drugs. They left the air laminar flow room after their WBC was greater than 2 x 10(9)/L. Case 1 was treated by radical mastectomy, Case 2 by improved radical mastectomy, Case 3 by improved radical mastectomy and transplantation of skin for the large area.
RESULTSRapid recovery of bone marrow function was observed in all 3 patients. Operation was performed 4 weeks after HDC/PBSCT in Cases 1, 2 and 33 days in Case 3. No influence was seen on operative procedure and would healing, especially in Case 3 with a large area of skin transplantation. Two patients with stage III(a) and III(b) have been alive since the treatment for 30 months and the other with stage IV died of brain metastasis 16 months later.
CONCLUSIONSHDC/APBSCT as a preoperative therapy for breast cancer has no influence on the coming surgery and would healing, even on skin transplantation for a large area. It has a practical response in stage III(a) and III(b), but it is still controversial in stage IV. This method as a salvage therapy for patients with breast cancer of intemuediate or stage.

8.Isolation and identification of mosquito-borne arboviruses in Yuncheng city, Shanxi province, 2012.

OBJECTIVETo investigate the species and distribution of mosquitoes and mosquito-borne arboviruses in Yuncheng city of Shanxi province, China.
METHODSMosquito samples were collected in 19 collection sites from Linyi county and Yongji city in Yuncheng city, in August, 2012. After identification and classification, all the specimens were homogenized and centrifuged to acquire supernatant before being inoculated to both C6/36 and BHK21 cells for viral isolation. Positive isolates were identified with arbovirus species-specific primers under RT-PCR, for further sequencing and phylogenetic analysis.
RESULTSA total of 10 455 mosquitoes of 7 species in 4 genuese were collected. The predominant mosquito species in Linyi county was Culex pipens pallens (91.96%, 3 911/4 253), but the one in Yongji city was Culex tritaeniorhynchus (72.85%, 4 518/6 202). A total of 23 strains of viruses were isolated from the mosquito pools. 15 strains from Culex tritaeniorhynchus and Culex pipens pallens were identified as genotype I Japanese encephalitis virus (JEV). Four strains from Culex pipens pallens were identified as Culex flavivirus (CxFV). Three strains from Culex pipens pallens were identified as Culex pipiens pallens densovirus (CppDNV). One strain from Armigeres subalbatus and Aedes albopictus was identified as Getah virus (GETV).
CONCLUSIONFour kinds of arboviruses were isolated from the mosquito pools, including GETV and CxFV, which were isolated and documented in Shanxi province for the first time. In the city of Yuncheng, Culex tritaeniorhynchus had been the predominant species and major vector for transmitting JEV. Genotype I JEV remained the major JEV circulating in the local natural environment.
