ObjectiveTo investigate the expression of nucleolin in osteosarcoma and its clinicalsignificance.MethodsThe expression of nucleolin was evaluated by immunohistochemistry in paraffin-em bedded tissues from 35 cases of osteosarcoma and 15 cases of osteochondroma.ResultsAmong the osteo sarcoma cases, 8.6％ were negative for nucleolin, 20％ weakly positive, 34.3％ moderate positive, 37.1％ strong positive.The positive rate of nucleolin in osteochondroma was 40％, 46.7％, 13.3％, 0％, respectively.The nucleolin expression showed significant difference in the osteosarcoma and osteochondroma(P ＜0.01).Among the conventional ostcosarcoma cases, 6.9％ were negative for nucleolin, 13.8％ weakly positive, 34.5％ moderate positive, 44.8％ strong positive.The positive rate of nucleolin in paros teal osteochondroma was 16.7％, 50％, 33.3％, 0％, respectively.The nucleolin expression showed re markable correlation with pathological and genetical classification (P ＜ 0.05).The positive rate of nucleo 1in in osteoblastic osteochondroma was 5.9％, 5.9％, 29.4％, 58.8％, while in chondroblastic osteochon droma 0％, 33.3％, 33.3％, 33.3％ and 16.7％, 16.7％, 50％, 16.7％ in fibroblastic osteochondro ma, respectively.The positive expressions of nucleolin were almost the same in each subtype of osteosarco ma (P ＞ 0.05).ConclusionsThe nucleolin expression had correlation with property of tumors in bone, and the positive expressions of nucleolin had correlation with pathological and genetic classification of osteo

OBJECTIVE To investigate the distribution and characteristics of drug-resistance of Staphylococcus aureus (SAU) causing lower respiratory infection, for rational using of antibiotics in clinical practice. METHODS A retrospective analysis on S. aureus isolates and their drug-resistance characteristics were carried out. These strains were isolated from lower respiratory specimens in clinical laboratory of Renmin Hospital of Wuhan University from Jan 2007 to Dec 2007. RESULTS Among 94 strains, 69 were meticillin-resistant S. aureus(MRSA), accounting for 73.40%. All MRSA strains were resistant to penicillin G, while sensitive to vancomycin and teicoplanin. Resistant rate to chloramphenicol was 7.24 %. The average resistance rate of MRSA to quinolones, macrolides and aminoglycosides were relatively high (56.52-98.55%). And resistant rate of MRSA was higher than the meticillin-sensitive S. aureus (MSSA) in average level. CONCLUSIONS Hospitals at all levels are proposed to strengthen drug resistance supervising so as to prevent the infection breaks.

OBJECTIVE To study the distribution and drug resistance of nosocomial infection pathogens isolated from of lower respiratory tract in senile patients and preventive control measures.METHODS The sputum and lower respiratory tract secretion of 594 senile patients were cultured to detect pathogens which were confirmed by routine methods.Disk diffusion test was used to analyze drug resistance.RESULTS Pseudomonas aeruginosa was the top one(21.2%).The drug resistance of Gram-negative organisms were the highest.Extensive-spectrum beta-lactamase(ESBLs) were tested out with 38.2% occurrence.CONCLUSIONS Strictly control in every step of procedures could reduce nosocomial infection.

OBJECTIVE Through detecting to drug susceptibility results of 12 commonly used antibiotics in Pseudomonas aeruginosa (PAE),we provide the scientific basis for the prevention of infection and reasonable choice of antibiotics. METHODS Twelve antibiotics′ susceptibility tests in vitro were carried out in PAE isolated from lower respiratory tract in Renming Hospital,Wuhan University from Jan 2007 to Dec 2008. RESULTS In the anti-infective drugs,the resistance rate to ceftazidime was the lowest (36.8%),followed by amikacin (39.6%),piperacillin/tazobactam (43.8%),cefepime (47.8%) and cefoperazone/sulbactam (48.5%).The resistant status of PAE was serious and multi-drug resistance existed. CONCLUSIONS The lower respiratory tract infection caused by PAE which possesses single and multiple drug-resistance. We should more think of it and strengthen preventive measures to reduce the rate of infection. Reasonably prudent use of antibiotics is still the best way of delaying its rapid increase of drug-resistance strains.

BACKGROUND: It is still controversial that interspinous dynamic stabilization system Wallis and Coflex which one can provide better clinical effects for lumbar degenerative disease.OBJECTIVE: To systematically assess the clinical effectiveness and safety of Wallis and Coflex for lumbar degenerative disease.METHODS: According to the computer-based online search of PubMed, Embase, Medline, Cochrane Library, CBM,CNKI, Wanfang Database, and VIP, articles published before August 1st, 2016 were searched. Articles about Wallis comparing with Coflex for lumbar degenerative disease were included; the quality score of methodology was assessed by MINORS. Research data abstracted and synthesized by Review Manager 5.3 were used for meta-analysis.RESULTS AND CONCLUSION: (1) Six studies were included, and all studies were designed for non-randomized controlled trial. (2) There were no significant statistical differences in Japanese Orthopedic Association, Oswestry disability index, visual analogue scale score, Prolo functional score, segmental lordosis angle, and segment movement degree. Incidence of adverse events was significantlue scale less in the Wallis group than in the Conflex group (P < 0.05).(3) There was no significant difference in clinical efficacy between Wallis and Coflex in the early and mid-term follow-up.We can conclude that Wallis may provide better clinical safety than Coflex.

Objective To study the influence of traction on the blood circulation of femur head and its evaluation by DSA. Methods Using micro-catheter, transfemoral selective femoral circumflex arteriography in 22 healthy dogs was performed in unilateral hip before (Group A,n =22) and immediately (Group B,n=22) ,30( Group D,n =22) ,60(Group E,n=20) ,90( Group F,n = 10),120 (Group G,n=10) minutes during 2 kg skin hip traction, and immediately after traction removal (Group H,I,J,L and 0), and 30 minutes after traction removal with 60,90 and 120 minutes continuous traction(Group K,M and P),and 60 minutes after traction removal with 90 and 120 minutes continuous traction(Group N and Q). DSA was also performed immediately during 4 kg weight traction before continuous traction in 12 hips( Group C).Blood circulation of the femoral head was evaluated mainly by observing its perfusion and time of circulation. Femur head perfusion was assessed as good scoring 3,poor scoring 2 and extremely poor scoring 1. Femur head circulation time was assessed as normal scoring 3 .prolonged scoring 2 and remarkably prolonged scoring 1. Analysis of variance was employed for analysis of the angiographic findings between different groups.Results Good femoral head perfusion in Group A to Q was 22,0,0,0,0,0,0,22,22,1,18,0,0,8,0,0 and 1 hips respectively, poor one was 0,22,8,22,15,4,1,0,0,15,2,4,6,2,1,3 and 8 hips, respectively,extremely poor one was 0,0,4,0,5,6,9,0,0,4,0,6,4,0,9,7 and 1 hips, respectively; and normal femoral head blood circulation time was 22,0,0,0,0,0,0,22,22,1,18,0,0,8,0,0 and 1 hips, respectively, prolonged one was 0,22,9,22,15,4,2,0,0, 15,2,5,7,2,2,4 and 8 hips, respectively, remarkably prolonged one was 0,0,3,0,5,6,8,0,0,4,0,5,3,0,8,6 and 1 hips, respectively. Conclusions Traction could lead to ischemic response and circulation disorder of canine femoral head. The longer the traction time or the bigger the traction weight was, the poorer the femoral head perfusion and the longer the femoral head circulation time were, and the slower they recovered. DSA could directly reflect these changes in the femoral head with these angioarchitectural and hemodynamic indexes.

OBJECTIVE: To compare the clinical features and outcomes of benign paroxysmal positional vertigo (BPPV) associated with Meniere's disease and idiopathic BPPV. METHOD: Reviewing the clinical records of 372 patients with BPPV, 289 patients with idiopathic BPPV and 36 patients with BPPV accompanied by Meniere's disease and were enrolled in this study. All patients were diagnosed by using the Dix-Hallpike test or roll test and treated with the canalith repositioning procedure. The outcomes were compared between the two groups. RESULT: The patients with BPPV associated with Meniere's disease presented the following features, in which they differed from the patients with idiopathic BPPV (P < 0.05): (1) a higher percentage of female patients; (2) a longer duration of symptoms; (3) frequent involvement of the horizontal semicircular canal; (4) a greater incidence of canal paresis; (5) more therapeutic sessions needed for cure and a higher rate of recurrence. CONCLUSION The BPPV associated with Meniere's disease differs from idiopathic BPPV in clinical features, treatment response recurrence tendency.

Objective To study the characteristics of DWI in nude mice models of hepatic Bel7402 tumors after treatment with adenovirus-mediated cytosine diaminase-thymidine kinase ( Ad.CD-TK) double suicide gene therapy, and then to identify whether DWI can be used for assessing curative effect of postoperative tumors.Methods Thirty nude mice models of hepatic Be17402 tumors were successfully created using cell suspension method,after the tumor grew to more than 1 cm in diameter,20 tumor models were treated by intratumoral administration of Ad.CD-TK for 3 days plus intraperitonea( i.p.) treatment with 5-Fc and GCV for the duration of the study.Then they were randomly divided into three groups during 5-Fc and GCV treatment.The remaining 10 tumor models were used as controls.MR scanning were performed in 10th day before and after tumor implantation in all models by using EPI-SE series and SENSE technology for treatment group. Tumor volumes and ADC values were calculated pretreatment and posttreatment. Cell apoptosis were determined by using TUNEL method.Analyze the change of ADC and apoptosis index (AI) in different times,t test was used for comparison the difference of AI and ADC values respectively. Results After 10 days,the tumor volumes of the treatment groups and controls were respectively (724.16 ±57.45 ) mm3,( 754.57 ± 66.84 ) mm3,with no significant difference ( t =0.488,P ＞ 0.05 ).The ADC values of the treatment groups were (0.98 ±0.11 ) × 10-3 mm2/s,the ones of the control groups were (0.68 ±0.04) × 10 -3mm2/s;AI of the treatment groups were(23.25 ±6.57)％,the ones of the control groups were (2.57 ± 0.58) ％.There were difference in both groups ( t =4.473,5.874 ; P ＜ 0.01 ).Conclusion DWI can be effectively to monitor the early pathological changes of hepatic Bel7402 tumors after Ad.CD-TK double suicide gene therapy,and provide experimental evidences for clinical application.

Aim To investigate the effect of magnetic nanoparticleencapsulated epirubicin(MNPE) on inducing apoptosis of human liverHep-6 tumor cell line in vitro and provide new method for local ablation ofliver in order to improve survival period of patients and quality oflife. Methods Inductive apoptosis of nano-magnetic pharmoparticle toHep-6 tumor cell of primonary hepatic cell caner was investigated by DNAelectrophoresis, electron nicroscopy , and flow cytometry analysis. Theseitems were divided into three groups, control, drug-control, and grouptreated with magnetic nauoparticle encapsulated epirubicin. The changes ofhuman liver Hep-6 apoptosis induced by magnetic nanoparticle encapsu-lated epirubicin were observed on different time-point and with differentnanoparticle encapsulated epirubicin and control group of biaoroubixingwere divided into high-dosage and low-dosage group. And the ultimateconcentration of 10 mg/L and 100 mg/L were given respectively on hu-group was iucreased from 25% to 54% after 24 hours. The apoptosis ratein the experimental group, biaoroubixing group and control group was78%, 53% and 2% respectively after 36 hours. There was significantdifference( t = 3.05. P ＜ 0.05) between the results of each group. Theapoptosis rate and quantity of medicine presented positive relativity withtime ( r = 0.96, P ＜ 0.05 ) .Conclusion Magnetic nanopartiele encap-sulated epirubicin presents the advantages of slow degradation, release ofmagnetic nanoparticle system and better target and can induce apoptosis ofliver tumor Hep cell.

Objective To investigate the guidance value of TOI classification in treating traumatic T-type atlantoaxial dislocation (ADD).Methods A retrospective case series study was made on 32 cases of traumatic TOI T-type ADD treated between January 2012 and December 2015.There were 19 males and 13 females,aged (38.4 ± 14.7) years.Fifteen cases of T1-type underwent external fixation or internal fixation without fusion,while 17 cases of T2-type underwent internal fixation with fusion.Symon-Lavender clinical standard,Japanese orthopedic association score (JOA),visual analogue scale (VAS),atlas-dens interval (ADI) and space available for the cord (SAC) were used to evaluate the therapeutic effect.Results Patients were followed up for 6-54 months (mean,32.4 months).At final follow-up,ADI was decreased to (2.3 ± 1.4) mm from preoperative (5.6 ± 1.6) mm,but SAC was increased to (15.4 ± 1.9) mm from preoperative (12.0 ± 2.9) mm(P ＜ 0.01).At final follow-up,cervical axial rotation range of motion was 102°-154° in T1-type cases and 57°-93° in T2-type cases.Range of motion for atlantoaxial joint was preserved in T1-type cases,but lost in T2-type cases.According to the Symon-Lavender clinical standard,there were 14 cases of mild disability,nine moderate disability,eight severe disability and one extremely severe disability before operation;there were 21 cases of mild disability,nine moderate disability and two severe disability at last follow-up.Significant difference was observed in the grades according to the Symon-Lavender clinical standard before operation and at last follow-up (P ＜0.05).At last follow-up,JOA score was increased to (14.6 ± 2.9) points from preoperative (9.9± 3.2) points,and VAS was decreased to (2.7 ± 1.3)points from preoperative (6.0 ± 1.6)points (P ＜ 0.01).Conclusions By using TOI classification,reconstruction of stability and improved neurological function can be achieved in treatment of traumatic T-type atlantoaxial dislocation.Non-fusion treatment of T1-type atlantoaxial dislocation can preserve range of motion for atlantoaxial joint.
