1.Relative Analysis in Displaying Nasopharyngeal Carcinoma Lesions with MR Imaging Protocols

Objective To study the relationship in displaying nasopharyngeal carcinoma(NPC)lesions with different MR imagingprotocols.Methods 67cases of NPC proved by pathology were reviewed. Each patient was scanned with six MR imaging protocols (Tra T_1WI, TraT_2WI , SagT_1WI , CorFSIR , TraCE-T_1WI , CorCE-T_1WI ).Results All cases were displayed as mucosal thickening and /or soft tissue masses of nasopharynx. The involved parts were as follows: parapharyngeal spaces in 49 cases(73.1%) ,carotid sheaths in 33 cases ( 49.3% ) , prevertebral muscles in 32 cases ( 47.8% ) , medial pterygoids in 15 cases ( 22.4% ) , lateral pterygoids in 7 cases ( 10.4% ) , pterygoid plates in 9 cases ( 13.4% ) ,pterygopalatine fossae in 5 cases (7.5%), sphenoidal sinuses in 16 cases(23.9%), ethmoidal sinuses in 6 cases(9.0%), maxillary sinuses in 3 cases(4.5%),orbital cavity in 1 case (1.5% ), sphenoid bones in 12 cases(17.9%), petrous apices in 19 cases(28.4%), clivuses in 41 cases(61.2%), cavernous sinuses in 7 cases(10.4%), temporal lobes in 3 cases(4.5%) and cervical lymphnode mestases in 45 cases(67.2%). The lesions displayed by combination of TraT_2WI, SagT_1WI , CorFSIR and CorCE-T_1WI were corresponded with those displayed by the all six MR imaging protocols . Conclusion One or more MR imaging protocols can be optimized for displaying each lesion of NPC. The combination of Tra T_2WI , Sag T_1WI , CorFS IR and Cor CE -T_1WI can display NPC lesions completely.
2.Clinical Outcome of Extraperitoneal Laparoscopic Radical Prostatectomy for Prostate Cancer

Objective To evaluate the clinical efficacy of extraperitoneal laparoscopic radical prostatectomy(ELRP)for prostate cancer patients, and to summarize the experience of surgical treatment. Methods The clinical data of 50 prostate cancer patients who underwent ELRP by the same performer from January 2010 to June 2015 were retrospectively reviewed. Results All cases were all successfully completed ,no case was converted to open surgery. The average operation time was 238.8 min,average operative blood loss was 409.1 mL,and intraoperative or postopera?tive blood transfusion was 6(12%). The mean postoperative catheterization time was 23.7(17?38)d. The mean postoperative hospital stay was 15 (10?34)d. The postoperative recovery time of eating was 2?4 d,and the ambulation time was 1?3 d. Totally 3 cases(6%)had lymph node metasta?sis,and 7 cases(14%)had positive surgical margin. Totally 9 cases(18%)had surgery?related complication. Patients were followed up for 6 to 58 months,with an average of 12.5 months. One case(2%)had biochemical recurrence,and the tumor?free survival rate was 84%. At the end of fol?low?up,all of the patients were continent. Conclusion ELRP is safe and effective for the treatment of prostate cancer. With the development of minimally invasive techniques,the applications of RP are increasingly widespread. However,large?scale and long?term follow?up studies are still needed for high?risk prostate cancer patients.
3.Practice of Drug Storehouse Management Work Based on Intelligent System in Our Hospital

OBJECTIVE:To change traditional working mode of drug storehouse by applying intelligent system,improve drug storage management,improve work efficiency and quality of drug storehouse,and to provide reference for hospital drug storehouse information management. METHODS:The changes of drug storehouse working mode and management in our hospital were compared before and after the application of intelligent drug storehouse management system. The time taken to complete individual work and the times of error occurrence 12 weeks before and after the application of the system were adopted as indexes to evaluate the effects of the system. RESULTS:Through the combination of Internet+Internet of things bar code,radio frequency identification,electronic tags,etc.,intelligent drug storehouse management system,which included temperature and humidity management,drug storehouse management,supply chain management three modules,was established in our hospital. Its application improved the drug storehouse working mode and process,strengthened the drug storehouse management in respect of staff,supplier,cold chain drugs and special drugs. Before application,average weekly time-consuming of drug shelfing,bill warehousing,outgoing and deliverying and inventory were(11.92 ± 0.701)h,(13.96 ± 0.752)h,(14.96 ± 0.542)h,(4.58 ± 0.376)h;the times of errors within 12 weeks were 10,8,9,2,respectively. After application of the system,average weekly time-consuming were(9.83±0.718)h,(11.29±0.753)h,(9.91±0.557)h,(3.00±0.316)h,respectively(vs. before application, all P<0.05). The times of errors within 12 weeks were 1,1,2,0. CONCLUSIONS:The application of intelligent drug storehouse management system optimizes workflow of drug storehouse in our hospital,strengthens the management of drug storehouse,and improves working efficiency and quality of the drug storehouse.
4.Digital anatomical analysis of transiliac-transsacral screw insertion pathway in Chinese adults

Objective To investigate the transiliac-transsacral screw insertion pathway (TSIP) so as to provide an anatomical basis for clinical surgical practice.Methods CT scanning data of normal pelvis in 90 Chinese adults were selected by random number table.There were 45 males and 45 females,aged from 21 to 82 years (mean,53.88 years).After computed tomography scan,pelvic data of the 90 subjects were entered into Mimics software 16.0 for three-dimensional reconstruction.The outer frames of sacrum and ilium at sagittal plane were depicted.Maximum inscribed circle method was applied to determine the screw insertion pathways which traversed 1st (S1) and 2nd (S2) sacral segments,respectively.The diameters and lengths of screw pathways,the distances from screw insertion points to both anterior superior iliac spine and posterior superior iliac spine,as well as the angles between pathways and anatomic surface were measured.Results Most subjects had the condition of a trans-S1 TSIP (male 78%,female 76%),and all the tested pelvis had the condition of trans-S2 TSIP.There was no statistically significant difference between the left and right side parameters in male group,and so was in female group (P >0.05).TSIP radius:there was no statistically significant difference among male S1 [(5.52 ± 1.91)mm],male S2 [(5.35 ± 1.05)mm],and female S1 [(5.49± 1.34)mm] (P>0.05),but each of them was greater than female S2 [(4.79 ± 1.40) mm] (P < 0.05).TSIP length:male S1 [(158.25 ±9.84) mm] was larger than male S2 [(138.94 ± 9.75) mm],and female S1 [(154.91 ± 9.40) mm] was larger than female S2 [(141.01 ±8.60)mm].The screw insertion point was located at the outer ilium side.The distances from S1 to anterior superior iliac spine and to posterior superior iliac spine were (96.49 ±6.91)mm and (68.22 ±6.35)mm in males but (100.48 ±8.15)mm and (61.57 ±6.84) mm in females.The distances from S2 to anterior superior iliac spine and to posterior superior iliac spine were (114.43 ±8.77)mm and (49.62 ±8.54)mm in males but (114.75 ± 10.19)mm and (44.52 ±8.36)mm in females.Compared with those with a condition of TSIP in S1,a pelvis without a condition of an S1 TSIP had larger S2 TSIP radius.The S2 TSIP radius in one with such condition in male was (5.10 ±0.84)mm and that in one without such condition in male was (6.22 ± 1.27) mm.The corresponding female data was (4.37 ± 0.92)mm and (6.11 ± 1.84)mm (P < 0.05).Both S1 and S2 TSIP were almost vertical to sagittal plane and parallel to coronal plane.Conclusions Anatomically,a S1 or S2 transiliactranssacral screw is available in most Chinese adults for sacroiliac joint fixation on both sides.Mimics software can be helpful to ensure the pathway of screw fixation,which provides reference for transiliactranssacral screw fixation technique.
