Objective To investigate the correlated clinical factors of upper gastrointestinal hemorrhage induced by acute organophosphorus pesticide poisoning(AOPP) and look for effective method for the prevention and treatment of upper gastrointestinal tract bleeding(UGTB).Methods 49 patients with severe AOPP were divided into UGTB group(group A,26 cases)and non-UGTB group(group B,23 cases).The time,frequency and causes of UGTB were observed.We also observed the differences in acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ),blood glucose and cholinesterase in serum.Results The causes of UGTB induced by AOPP were poison damage of stomach mucous membrane,gastric lavage damage and irritability ulcer;APACHE Ⅱ scores of group A were more than those of group B obviously;the level of cholinesterase was lower in group A.Correlation analysis showed that the APACHE Ⅱ scores had positive correlation and the level of cholinesterase had negative correlation with UGTB induced by AOPP.Conclusion Both the APACHE Ⅱ scores and level of cholinesterase can reflect the UGTB induced by AOPP.Active treatment of the primary disease and stress status and often the operation of gastric lavage are important ways to prevent and treat UGTB induced by AOPP.

Objective To observe the effect of valsartan on brian ultrastructure, Klotho gene and micro-inflammatory factor [intercellular adhesion molecule-1(ICAM-1) and vascular cell adhesion molecule-l(VCAM-1)] expression in spontaneously hypertensive rat models. Methods Ten male spontaneously hypertensive rats of 22 weeks age were selected and randomly divided into a hypertension group and a valsartan intervention group, while another 5 Wistar-kyoto rats were set as a normal contrast group. The brain ultrastructure of the 2 groups was observed by electron microscope. The expression of micro-inflammatory factor (ICAM-1 and VCAM-1)and Klotho gene was detected with RT-PCR, immunohistochemistry, and Western blot, respectively. Results The cerebral neuron damage of spontaneously hypertensive rats whose ultrastructure showed cell-pyknosis, chromatin margination and typical apoptotic body formation were alleviated after the intervention of valsartan. RT-PCR showed that the gene expression of Klotho increased while ICAM-1 and VCAM-1 decreased after valsartan intervention. Immunohistochemistry and Western blot also showed that the protein expression of Klotho increased, while ICAM-1 and VCAM-1 decreased after valsartan intervention. ConclusionValsartan can improve the brain ultrastructure of spontaneously hypertensive rats by increasing the expression of Klotho.

Objective To evaluate the CT findings of the parotid basal cell adenoma and it's diagnostic value.Methods The CT appearances of 10 patients with parotid basal cell adenoma confirmed by histopathology were reviewed retrospectively.The plain and the contrast-enhanced CT scan were performed in all cases.Results The well-defined mass in varied size with marked enhancement in contrast-enhanced CT imagines,in which the CT value increased to 200Hu maximally(average,84Hu),were the pathognomonic CT characteristics of the parotid basal cell adenoma.Conclusion The qualitative diagnosis of the parotid basal cell adenoma could be made based on the CT findings combined with some valuable clinical information before operation.

Objective To investigate the diagnostic value of the combination of exercise electrocardiography test(EET) and coronary artery calcification(CAC) determined by electron beam computed tomography(EBCT) in coronary artery disease.Methods One hundred and thirty-five patients underwent coronary angiography,EET and EBCT.The value of calcium score(CS) and EET in diagnosing coronary artery disease were separately and unitedly evaluated.Results According to calcification score cut points by age and gender,the sensitivity,specificity,positive and negative predictive value of CAC for diagnosing coronary artery disease were 77%,81%,74% and 83%,respectively.The corresponding parameters of EET were 73%,76%,68% and 80%.After combining CAC and EET,the sensitivity and negative predictive value of parallel test were 93% and 92%,and the specificity and positive predictive value were 96% and 91%,respectively.Conclusions Exercise electrocardiography test combined with coronary artery calcification determined by electron beam computed tomography can significantly improve their diagnostic value for coronary artery disease.

Objective To observe the analgesic effects of patient-controlled intravenous analgesia (PCIA) and continuous infusion of morphine into epidural space(CIEA) after suprapubic transvesical prostatectomy. Methods Seventy-five patients undergoing suprapubic transvesical prostatectomy were randomly assigned to PCIA group,CIEA group and control group. In PCIA group, 2 0mg/h morphine as a recommended dose would be injected intravenously by the patient through a self-controlled analgesic delivery system in which another 1 mg morphine could be injected whenever patient felt pain until the pain relieved. The lockout time was 20 minutes. In CIEA group, 0 08 mg/h morphine was injected into epidural space. 50～75mg of pethidine was injected whenever the control group patients felt pain. VAS(vision simulate score), frequency and duration of bladder spasm, discontinuance time of bladder irrigation and BP were observed. Methods The pregnant golden hamsters were randomly divided into the treated group and the control groups. In the chamber made by ourself for passive smoking experiment, the animals of the treated group were exposed to smoke from embryonic day 4 to 7, 3 times per day for 4 consecutive days.
