1.Effects of dexamethasone on toll-like receptor 4 mediated signaling pathways in mice dendritic cells

AIM: To observe the effects of dexamethasone on the expression of toll-like receptor 4(TLR4)and the activation of NF-?B of dendritic cells(DC),and investigate the mechanism of dexamethasone suppressing differentiation,maturation,and function of DC.METHODS: DC generated from C57BL/6 murine bone marrow cells were induced by GM-CSF and IL-4.Dexamethasone was added in the DC's culture system,the cells were analyzed by flow cytometry(FCM) to determine the expression of TLR4 & CD11c,and by electrophoretic mobility shift assay(EMSA) to verify the activation of transcription factor NF-?B of DC.RESULTS: Dexamethasone could suppress the expression of CD11c and the activation of NF-?B with a dose-effect relationship in DC,but it could not suppress the expression of TLR4 of DC.CONCLUSION: The mechanism of dexamethasone involved in inhibition of differentiation,maturation,and function of DC is perhaps related to suppression of NF-?B activation.
2.Biliary manometry in patients with cholelithiasis

Objective To explore the relationship between cholelithiasis and the function of sphincter of Oddi (SO). Methods To identify the existence of calculi, choledochoscopy was performed in patients 6 weeks after exploration of the common bile duct (CBD) and T tube drainage, in which 71 were patients with stones in gall bladder, CBD or intra-hepatic bile duct, and 9 with trauma of pancreas or liver.Biliary manometry was performed after choledochoscopy, and an additional manometry was applied after calculus removal if calculi were detected. The indices measured included SO basal pressure ( SOBP), amplitude of SO contractions (SOCA), frequency of SO contractions (SOF) and CBD pressure (CBDP). The patients with cholelithiasis were classified into cholecystolithiasis group, choledocholithiasis group, and hepatolithiasis group according to the position of calculi. Patients with trauma were assigned as the control group.Results All variables in 50 patients with choledocholithiasis were similar before and after the procedure.The variables in patients with cholecystolithiasis and choledocholithiasis had no difference from those of the control (P > 0. 05 ). The SOBP and SOCA of patients in hepatolithiasis group were lower than those of the control group ( P < 0. 05 ), while no difference in SOF was detected (P > 0. 05 ). Conclusion The function of SO in patients with hepatolithiasis is abnormal ( decrease in SOBP and SOCA). Biliary manometry cannot be the reliable evidence for the existence of calculi in bile duct.
3.The study of the relation between cholangiolithiasis and the pressure of sphincter of Oddi

Objective To explore the changes of the pressure of sphincter of Oddi(SO)in postoperative patients with cholangiolithiasis.Methods Sixty cases with multiple biliary calculi operation (recurrent cholangiolithiasis group),80 cases with biliary calculi operation at first time(cholangiolithiasis group)and 9 cases with external injuries of pancreas or liver(control group),who were examined in order to identify common bile duct pressure(CBDP),SO basal pressure(SOBP),amplitude of SO contractions (SOCA),frequency of SO contractions(SOF),then comparing the difference of statistics among the three groups.Results CBDP,SOBP,SOCA,SOF in recurrent cholangiolithiasis group and cholangiolithiasis group were significantly higher than those in control group[(13.78 ±9.91),(12.65 ±7.64)mm Hg(1mm Hg =0.133 kPa)vs.(12.54 ± 2.35)mm Hg,(15.27 ± 9.15),(14.89 ± 7.87)mm Hg vs.(13.63 ± 3.27)mm Hg,(106.30 ± 54.70),(98.39 ±38.29)mm Hg vs.(87.65 ±56.38)mm Hg,(6.91 ± 1.92),(6.25 ±2.17)times/min vs.(5.26 ± 2.11)times/min](P < 0.05),but there was no significant difference between recurrent cholangiolithiasis group and cholangiolithiasis group(P > 0.05).Conclusion SO dysfunction resides in the postoperative patients with cholangiolithiasis,and it is possible related with the formation and/or recurrence of cholangiolithiasis.
4.The diagnosis and surgical management of pancreatic duct stones

Objective To evaluate the diagnosis and surgical management for pancreatic duct stones (PDS). Methods The clinical data of 19 PDS patients admitted to our hospital from Jan. 1985 to Sep. 2003 were analyzed retrospectively. Results Epigastric pain was the first symptom in all patients,and chronic pancreatitis was associated, pancreatic carcinoma coexisted in 6 cases. Fifteen cases were found to have PDS or pancreatic duct dilation by B-type ultrasonography. Twelve patients underwent pancreatolithotomy plus side to side Roux-en-Y pancreaticojejunostomy. Pancreatoduodenectomy was performed on four patients ,caudal pancreatectomy and pancreatolithotomy on three patients. Six pancreatic carcinoma patients died during follow-up of 9~26 months. Abdominal pain was relieved completely in 11 cases and partially in 1,stones and pain recurred in one patient. Conclusions B-type ultrasonography、CT and endoscopic retrograde cholangiopancreatography(ERCP) are most useful diagnostic methods;Surgical procedures should be selected according to the following factors: (1) location of the pancreatic duct stones. (2) stenosis of the main pancreatic duct.(3) pancreatic duct stones associated with or without pancreatic carcinoma.
5.Related factors associated with pelvic adhesion and its influence on fallopian tube recanalization in infertile patients

Objective To investigate factors with pelvic adhesions and the effect of different degrees pelvic adhesions on fallopian tube recanalization in infertile patients.Methods Total of 527 infertile patients undergoing hysteroscopy and laparoscopic surgery in Affiliated Hospital of Chinese People's Armed Police Forccs Logistics College were studied retrospectively.According to the extent of pelvic adhesions,tubal umbrella adhesions and atresia,377 cases were classified into adhesion groups,including 73 cases in grade Ⅰ,221 cases in grade Ⅱ,75 cases in grade Ⅲ and 8 cases in grade Ⅳ based on adhesion score.The 150 cases with no obvious pelvic adhesion were matched as control group.Among 8 cases with grade Ⅳ ahesion were exluded from ahesion group the relationship between pelvic adhesions and related history,abdominal lesions,tubal patency and the prognosis were studied.Results(1)Related factors:the frequency of pelvic adhesion and more than 7 years of infertility of 23.9%(88/369)in adhesion group were significantly higher than 12.0%(18/150)in control groups.(2)History:compared with the control group(12.7 %,19/150;28.7%,43/150;11.3%,17/150;12.0%,18/150;17.3%,26/150),patients with pelvic adhesions present more incidence abortion(23.6%,87/369),uterine cavity operation(38.2%,141/369),ectopic pregnancy(20.9%,77/369),pelvic inflammatory disease(25.5%,94/369)and abdominopelvic surgery (31.4%,116/369).(3)Endoscopy exploration:the incidence of hydrosalpinx(24.7%,91/369),tube distorted(15.7%,58/369)and salpingostomy(72.9%,269/369)in adhesion group were higher than those in control group(2.0%,3/150;4.0%,6/150;12.0%,18/150),but relatively lower incidence of pelvic endometriosis lesions(5.7%,21/369)and mesosalpinx cysts(16.3%,60/369)than those in control group(16.0%,24/150;30.0%,45/150).The rate of proximal tubal recanalization(59.5%,91/153)in adhesion group was lower than 75.4%(52/69)in control group.However,the rate of distant tubal recanalization of 84.4%,(281/333)in adhesion group and;13/15 in control group didn't show statistical difference.(4)Prognosis:the rate of ectopic pregnancy of 9.7%(29/299)in adhesion group was significantly higher than 3.1%(4/128)in control group.Among cases with grade Ⅲ adhesion exhibited the highest rate of ectopic pregnancy(13.0%,7/54;OR =4.62,95% CI:1.29-16.50).(5)Multivariate analysis:it was found that more than two drug abortions(OR =3.29,95% CI:1.34-8.07),pelvic and (or)abdominal surgery history(OR =2.20,95% CI:1.35-3.57)and pelvic inflammatory disease history (OR =1.54,95% CI:1.21-1.97)were risk factors with pelvic adhesions.Conclusion More than or equal to two drug abortion history,pelvic inflammatory disease and pelvic and abdominal surgery damage were important factors for pelvic adhesions of infertility patients,which may decrease the possibility of proximal tubal recanalization and increase ectopic pregnancy risk.
7.Investigation on the application of venous access in very low birth weight infants

Objective:To explore the types of venous access, indwelling time and the correlation with catheter-related bloodstream infection (CRBSI) of very and extremely low birth weight infants (VLBW & ELBW) in NICU.Methods:The self-designed venous access data collection form was used to collect the venous access data of VLBW & ELBW infants who were admitted to NICU of Children′s Hospital of Fudan University from January to December 2019. SPSS 22.0 software was used for data analysis, description and binary Logistic regression analysis.Results:A total of 218 cases were collected, including 9 cases of peripheral intravenous (PIV), 30 cases of PIV+ umbilical vein catheter (UVC), 43 cases of PIV+PICC, 136 cases of PIV+UVC+PICC. The average indwelling time of UVC was 6 days, the average indwelling time of PICC was 22 days. There were 23 cases (10.55%) got CRBSI and 195 cases (89.45%) without CRBSI. Binary Logistic regression analysis showed that birth weight ( OR=1.003, 95% CI 1.000-1.006, P<0.05) and combination form of venous access ( OR=0.139, 95% CI 0.023-0.834, P<0.05) of VLBW & ELBW infants were associated with CRBSI. Conclusions:In NICU, PIV, UVC and PICC are the three main ways to indwell venous access for VLBW & ELBW infants. The occurrence of CRBSI is closely related to the type and combination of indwelling venous access.
8.Association between β-arrestin2 genetic polymorphism and response to methadone maintenance treatment in heroin-dependent patients in Han population in Hunan province

Objective To study the distributions of three single nucleotide polymorphisms (SNPs) in β-arrestin2 (ARRB2) which including rs3786047,rs1045280 and rs2036657 and to elucidate the relationship between these SNPs and response to methadone maintenance treatment (MMT) among heroin-dependent patients of Han ethnicity population in Hunan.Methods Han MMT patients were recruited in four random-chosen MMT clinics from Hunan province.Demographics,history of drug-use and MMT were recorded.ARRB2 SNPs were genotyped to determine the association between SNPs and response to MMT.Results Distributions of the three SNPs were in Hardy-Weinberg equilibrium in both groups (responders vs.non-responders).There was no statistical significance in the distribution frequency of genotype on rs3786047 (x2=0.486 2,P=0.784),rs1045280 (x2=1.591 9,P=0.451) and rs2036657 (x2=1.061 5,P=0.588) in ARRB2 among the responders or the non-responders.Conclusion Associations between the ARRB2 genotypes,rs3786047,rs1045280 and rs2036657,and MMT response in Han MMT patients in Hunan province did not appear.
9.Efficacy of "tennis racket"-like flap with retrograde medial plantar on repairment of frontal plantar tissue defects

Objective To explore the clinical experience and surgical method of the repairment of frontal plantar tissue defects by using "tennis racket"-like flap with the medial plantar retrograde,and to study the reliability in the clinical application of the medial plantar retrograde flap.Methods From June 2011 to June 2016,"tennis racket"-like flap with the medial plantar retrograde was used to repair the frontal plantar tissue defects in 10 cases.The cutting range of flap was from 3.5 cm × 2.0 cm to 8.0 cm x 4.0 cm in size;in all patients the donor area was covered by skin grafts.Results All flaps survived and wounds healed by first intention.In 10 patients the donor sites healed primarily with a straight scar,and the appearance and texture of the flaps were satisfactory.All patients were followed up from 6 to 24 months (mean 12 months).According to the Chinese foot function evaluation standard trial evaluation,the outcomes were excellent in 9 cases,good in 7 cases,and medium in 2 cases.Conclusions "Tennis racket"-like flap with the medial plantar retrograde is less anatomic variation with reliable blood supply,and sensory recovery is quick;the donor site is a small crater and cicatrial contractures are light;the cost is low.All patients are treated on one session and therefore it is an ideal method for the repairment of frontal plantar tissue defects.
10. Influence of psychological capital on occupational stress of employees in labor-intensive electronic companies

OBJECTIVE: To explore the effects of psychological capital on occupational stress of employees in labor-intensive electronic enterprises. METHODS: A total of 1 723 employees in 5 labor-intensive electronic enterprises were selected as study subjects by cluster sampling method. The Psychological Capital Questionnaire and Simple Job Stress Questionnaire were used to investigate their psychological capital and job demand-control( JDC) occupational stress. The effect of psychological capital on occupational stress was analyzed. RESULTS: Total average score of psychological capital of the study subjects was( 4. 4 ± 0. 7). The average scores of self-efficacy,hope,resilience,optimism dimensions were( 4. 3 ± 0. 9),( 4. 5 ± 0. 8),( 4. 5 ± 0. 8) and( 4. 4 ± 0. 7),respectively. The proportions of active,relaxed,nervous and passive occupational stress accounted for 24. 2%,24. 7%,23. 5% and 27. 6% respectively. The detection rate of JDC mode high occupational stress was 68. 7%( 1 184/1 723). The proportion of active occupational stress of the low-psychologicalcapital group was lower( 28. 3% vs 20. 2%,P < 0. 001),the proportion of passive occupational stress was higher( 23. 4% vs 31. 7%,P < 0. 001),the positive rate of JDC high occupational stress was higher( 63. 3% vs 74. 1%,P <0. 01),compared with the high-psychological-capital group. The multivariate logistic analysis showed that the risk of JDC high occupational stress in the low-psychological-capital group was higher than that in the high-psychological-capital group( P < 0. 01),after eliminating the confounding factors such as length of service,education level and personal monthly income. CONCLUSION: The psychological capital of employees in labor-intensive electronics enterprises was associated with the occupational stress of the JDC model. The occupational stress can be reduced by improving the psychological capital of workers.
