2.Clinical significance of the changes of plasma homocysteine,endothelin,mononitrogen monoxide,folic acid and vitamine B_(12) in acute cerebral infarction

Objective To detect the fluctuations of homoeysteine(Hcy),endothelin(ET),mononitrogen monoxide(NO),folic acid(FA) and vitamine Bi2 (VitB_(12)) in the plasma of patients suffered from acute cerebral infarction and to investigate the clinical significance.Methods 140 blood specimens from cerebral infarction patients and 61 specimens from healthy individuals were obtained in this study and a series of items of Hey,ET,NO,FA and VitB_(12) were detected.Results The Hey [(17.15±4.63) μmol/L]and ET [(32.62±18.76) μmol/L]were increased in acute cerebral infarction(ACI) group compared with that in control group[(10.12±2.62) μmol/L,(20.08±13.38) μmol/L respectively]and the difference was significant (P<0.01).The levels of NO [(39.82±7.26) μmol/L],FA [(7.54±4.01) ng/ml]and VitB_(12) [(247.70±119.85) pg/ml]were decreased compared with control group [(60.22±6.28) μmol/L,(10.08±3.86) ng/ml,(411.52±194.58) pg/ml respectively]and the differences significant (P<0.01).Gender and hypertension affected the values of Hey.Conclusions Hcy and ET were increased while NO,FA and VitB_(12) were decreased in acute cerebral infarction.Hcy had positive correlation with ET and negative correlation with FA and VitB_(12).
3.Clinical study of high-frequency oscillatory ventilation in the treatment of pulmonary hemorrhage in newborn infants

Objective To investigate the effects of high frequency oscillatory ventilation in the treatment of neonatal pulmonary hemorrhage curative.Methods 56 neonates with pulmonary hemorrhage were selected,28 cases in high frequency mechanical ventilation(HFOV) group and 28 cases in conventional mechanical ventilation(CMV)group.On the basis of conventional treatment,the HFOV group received high-frequency mechanical ventilation therapy,the CMV group treated with conventional ventilation.The breathing machine parameters of the two groups were observed and recorded,calculated oxygenation index (OI) and arterial/alveolar oxygen partial pressure ratio (a/APaO2) The duration of pulmonary hemorrhage,the duration of mechanical ventilation,the length of hospital stay,and the duration of oxygen therapy were compared between the two groups.Results Before treatment,there were no statistically differences of in OI and a/APaO2 between the two groups (t1 =0.28,P1 =0.78;t2 =0.47,P2 =0.64),1h treatment later,declining value of the OI and the rising of a/APaO2 in surviving children,and the therapeutic effect of the HFOV group were more obvious than those of the CMV group (t1'=2.14,P1'=0.04;t2'=2.26,P2'=0.03).In the HFOV group,23 cases cured(82.1%),5 cases died(17.9%).And in CMV group,15 cases cured(53.6%),13 cases died (46.4%).The duration of pulmonary hemorrhage,the duration of mechanical ventilation,the length of the hospital stay,and the duration of oxygen therapy of the HFOV group were significantly lower than those of CMV group,the differences were statistically significant (P =0.046,0.044,0.048,0.035).Conclusion HFOV can better improve oxygenation function in infants with pulmonary hemorrhage,shorten the course of the disease and improve the cure rate,recommend the application and clinical treatment.
4.Study on the Influence of the Reimbursement Mechanism on the Output Efficiency: Based on County-Level Traditional Chinese Medicine Hospitals

Objective To evaluate the traditional Chinese medicine(TCM) output efficiency of the coUnty-level TCM hospitals in Jiangsu Province before and after the reimbursement mechanism reform,and to analyze the factors which affect the efficiency of county-level TCM hospitals.Methods Using DEA to analyze the efficiency of county TCM hospitals,and using the Tobit regression to analyze the influence factors.Results The output efficiency of county-level TCM hospitals in Jiangsu Province is low,and the reimbursement mechanism reform in county-level hospitals of TCM efficiency further decreases;payment model,cost model and revenue model of regression analysis indicate that different factors has different influence on output efficiency of county hospitals of TCM.Conclusion The reimbursement strategy of county-level TCM hospitals should be optimized,the balance of revenue and expenditure structure be adjusted,and the intensity of TCM services reimbursement be increased.
5.Study on the Elderly’s Economic Burden and Risks Due to Non-communicable Chronic Diseases

Objective:To analyze different age group elderly’s financial burden and risk due to non-communicable chronic diseases (NCDs). Methods:Based on CHARLS baseline investigation data, disease economic burden, disease economic risk for family and disease relative risk are used to analyze the economic risk and economic burden of the different age groups of urban and rural NCDs elderly. Results: The diseases financial burdens of urban NCDs elderly are greater than rural; for diseases family risk and relative risk, NCDs elderly are greater than non-NCDs elderly, urban NCDs elderly are greater than rural, high-age group NCDs elderly are greater than low-age group NCDs elderly. Conclusion: It is needed to improve the NCDs elderly’s medical security ability with basic health insurance, especially to improve medical security ability of the high-age group for rural elderly.
6.Curriculum and teaching practice of general education course of ‘ basic medicine introduction'in Sun Yat-sen University

7.Glycolysis and tumor

Tumor cells obtain energy mainly by glycolysis to maintain growth and proliferation. It is related to the DNA mutations of mitochondria and anti-oncogene mutations. Inhibition of glycolysis can inhibit proliferation and kill tumor cells. The rate-limiting enzyme of glycolysis and hypoxia inducible factor may be the new targets for cancer therapy.

Cytochrome P450 monoxygenase converts arachionic acid to four epoxyeicosatrienoic acid regiosomes: 5,6-EET(epoxyeicosatrienoic acid);8,9-EET; 11,12-EET and 14,15-EET.Recent studies show that EETs are involved in signal transduction. EETs open Ca 2+ -sensitive K + channel and inhibit Na + channel,Ca 2+ -sensitive Cl - channel and so on. What is more ,EETs have been demonstrated to activate PP60 c-src and initiate a tyrosine kinase cascade that mediates mitogenic effects.

10.Periareolar Incision in the Treatment of Multiple or Giant Fibroadenoma of the Breast

7 cm in diameter)of the breast were treated in our hospital by lumpectomy via a periareolar incision.The length of the incision was less than half of the perimeter of the areola.For multiple fibroadenomas,circumareolar incision(not longer than half of the nipple-areolar perimeter)was made in different quadrants according to the position of the main tumor,so that the multiple benign tumors could be integrally removed through the approach between the hypodermic fat and glands via radial incisions.Similarly,giant benign tumors were treated through the same approach,cut into pieces,and then removed.Results Totally 165 tumors were removed from the 46 patients with multiple fibroadenomas.In the resected masses,19 were from one patient,who had bilateral multiple breast tumors.The 2 patients with giant breast fibroadenoma were also cured by lumpectomy,the diameter of the largest tumor in the two was 8 cm,while the circumareolar incisions were shorter than 3.5 cm.No nipple necrosis occurred after the operation in our series.Follow-up was achieved in all of the patients for 1 month to 2 years(mean,14.4 months),during which,11 cases showed no incision scar,26 had non-notable scars,9 cases developed observable incision scars,and 2 cases had apparent scars owing to their susceptibility to scar.Conclusions Lumpectomy via minor circumareolar incision is convenient and effective for multiple or giant benign fibroadenoma of the breast with good cosmetic outcomes.
