1.Incidence of metabolic syndrome in incipient female patients with rheumatoid arthritis

Objective To examine the incidence of metabolic syndrome (MS) in incipient female patients with rheumatoid arthritis (RA), and study the relationship between MS and disease activity of RA. Methods The clinical data of 102 incipient female RA patients (RA group) and 125 healthy controls (control group) were retrospectively analyzed. The incidence of MS was compared between 2 groups, and the clinical and laboratory data were analyzed between patients with MS and patients without MS. Results There were no statistical differences in age, body mess index (BMI), waist circumference, systolic blood pressure, diastolic blood pressure, fasting blood glucose (FBG), triglyceride (TG) and high-density lipoprotein cholesterol (HDL-C) between 2 groups (P>0.05). The incidences of waist circumference ≥ 80 cm, TG ≥ 1.70 mmol/L, HDL-C ≤ 1.30 mmol/L, MS according to the National Cholesterol Education Program Adult Treatment Panel Ⅲ (NCEP-ATPⅢ) standard and MS according to the Chinese Diabetes Society (CDS) standard in RA group were significantly higher than those in control group: 39.2% (40/102) vs. 25.6% (32/125), 37.3% (38/102) vs. 24.0%(30/125), 34.3%(35/102) vs. 21.6%(27/125), 34.3%(35/102) vs. 22.4%(28/125) and 27.5%(28/102) vs. 16.0% (20/125)%, and there were statistical differences (P<0.05). There were no statistical differences in the incidences of BMI≥25 kg/m2, systolic blood pressure ≥130 mmHg (1 mmHg = 0.133 kPa), diastolic blood pressure ≥85 mmHg and FBG≥5.6 mmol/L between 2 groups (P>0.05). The RA patients were divided into RA with MS (35 cases) and RA without MS (67 cases) according to the MS diagnostic standard of NCEP-ATPⅢ. The erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and blood uric acid in RA with MS patients were significantly higher than those in RA without MS patients:(68.1 ± 26.3) mm/h vs. (40.1 ± 30.6) mm/h, 4.260 mg/L vs. 1.505 mg/L and (330.5 ± 69.1)μmol/L vs. (269.0 ± 67.5)μmol/L, and there were statistical differences (P<0.05). There were no statistical differences in age, involved joint number, rheumatoid factor (RF), hemoglobin and incidence of anti-cyclic citrullinated peptide antibody (CCP antibody) between RA with MS patients and RA without MS patients (P>0.05). Conclusions The incidence of MS in incipient female patients with RA is higher than that in healthy controls, and RA patients are more prone to abdominal obesity, TG increasing and HDL-C decreasing. The inflammatory index and blood uric acid levels in RA with MS patients are significantly higher than those in RA without MS patients.
2.A modified liver cirrhosis portal hypertension model in canines induced by subcutaneous injection of carbon tetrachloride

Objective To find an ideal liver cirrhosis portal hypertension model in big animals suitable for surgical study.Methods Twenty canines were treated by subcutaneous injection of 60% CCl4 colza oil emulsion into the back,at a dose of 1.0-1.3 mL/kg every 10 days for a total of 6-8 times.At the same time,it was combined with the control of food intake.All canines were fed with rice mixed with 10% hog fat.The amount of rice was 15 g/kg per day from the first day to the forth day,but the amount of food was not controlled from the fifth day to the tenth day.During the process,the canines′ general condition was observed and the changes of hepatic functions such as ALT,TP,ALB,G and A/G were measured.The changes of liver morphology,the diameter and blood flow of portal vein were monitored with color Doppler.Every two weeks,a piece of hepatic tissue to observe the pathological change of liver was excised operatively.Results During the process,the body weight of the canines decreased continually;ALT increased during early and middle stages and decreased during advanced stage;TP and ALB always decreased,and A/G continuously decreased,but G did not change significamtly.Eight weeks after injection,liver became progressively smaller.its density was not well-distributed,and liver particles gradually became coarse,and liver capsule became rough.The diameter of portal veins became larger and the velocity of portal vein became slower.Liver cirrhosis with psudolobules was found on light microscopy from the tenth week to the twelfth week.Three canines died,with mortality of 15.0%.Conclusions It took 10-12 weeks to establish this liver cirrhosis portal hypertension models in canines by subcutaneous injection of CCl4.This method is convenient,has high success rate,and is suitable for use in surgieal research.
3.Acetabular revision using an anti-protrusion cage in patients with periprosthetic acetabular bone loss

Objective To investigate the risk factors related to the survival time of anti-protrusion cage in acetabular revision.Methods Data of 40 patients who had received acetabular revision using antiprotrusion cages between January 2002 and June 2010 were retrospectively analyzed.There were 16 males and 24 females,aged from 22 to 77 years (average,60.3 years).All patients were followed up for 12 to 82 months (average,39.2 months).According to the AAOS classification,there were 29 cases of type C and 11 cases of type D; while according to the Paprosky classification,there were 29 cases of type ⅢA and 11 cases of type Ⅲ B.The multiple regression analysis was utilized to investigate the relationships between different factors and anti-protrusion cage failure.The corresponding factors included gender,age,bone loss classification,superior or lateral migration of acetabular center of rotation,abduction angle,fixation manners (simple flange fixation or flange fixation plus transacetabular screw fixation),bone grafting techniques and cup type.Results At final follow-up,the average Harris score was 74.5±15.4.Two patients received rerevision due to prosthetic loosening.Obvious radiological loosening of prosthesis combined with pain was found in 2 cases.The statistical analysis indicated that superior migration of acetabular center of rotation,abduction angle and fixation manners were correlated with cup failure,especially when the superior migration was more than 8.5 mm or the abduction angle was larger than 53.5°.Flange fixation plus transacetabular screw fixation could reduce risk of cup failure.Conclusion There is a higher cup failure risk in acetabular revision using an anti-protrusion cage for patients with serious bone defect.However,implanting cup at the level of the true acetabulum as far as possible,decreasing abduction angle properly and using flange fixation plus transacetabular screw fixation can reduce cup failure risk.
4.Construction and application of a telemedicine-based home care system of palliative care for end-of-life cancer patients

Objective To construct a telemedicine-based home care system of palliative care for end-of-life cancer patients,and to evaluate its application effect.Methods The construction of the system was based on the literature research,interview results of the patients and their caregivers,and expert group meeting discussions.From May to August 2023,a total of 88 patients,who were about to be discharged from the palliative care ward of a cancer hospital in Hunan Province,were selected as the study subjects using a convenient sampling method,and then they were divided into an experimental group and a control group according to the order of admission.The experimental group was given home care services based on the system,while the control group was given routine discharge guidance and follow-up visits.The duration of the intervention was 8 weeks.The Chinese version of the Edmonton Symptom Assessment Scale and the Chinese version of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 15 palliative were used to evaluate the symptom burden and quality of life between 2 groups before and after intervention;the Telemedicine Satisfaction Questionnaire was used to evaluate the care satisfaction of the experimental group.Results The system of patient side includes 8 main features,namely personal health record establishment,doctor-patient communication,palliative care knowledge library,medication assistance,smart reporting and monitoring,intelligent Q&A,intelligent follow-up,and intelligent forms.The healthcare professional side includes 7 main functions,namely workbench,case management,follow-up management,warning reminder,popularization push,statistical analysis and user management.A total of 67 patients completed the intervention,with 33 in the experimental group and 34 in the control group.The symptom burden of the experimental group was lower than that of the control group(P<0.05);the quality of life in the experimental group was better than that in the control group(P<0.05);the score of care satisfaction was(59.00±6.66),which indicated a high level of satisfaction.Conclusion The system can reduce the symptom burden and improve the quality of life of end-of-life cancer patients with good practicality
