1.Protective effect and mechanisms of penotoxifylline on ischemia-reperfusion injury in rat kidneys

Objective To investigate the protective effect of penotoxffylline (PTX) on renal iscbemia-reperfusion injury (IRI) and its mechanisns. Method Seventy-five male SD rats were randomly divided into three groups with 25 rats in each group: the sham operated controls, Iri group and FIX treated group. The rat model of renal IRI was established with occlusion of left and fight kidney pedicle for 45 minutes. Sham rats underwent laparotomy without IR.Treated mrs received FIX 20 mg/kg at 30 minutes before operation through their tail vein,tben PTX 6 mg/(kg·h) IV with pump. SO group and IRI group rots were IV normal saline equivalently. The pathological change of kidney, serum creatinine (sCr) values and the levels of MDA,SOD,TNF-α and ICAM-1 in homogenate of kidney tissue were measured before ischemia, 0, 1, 4, and 24 hours after reperfitsion. Results After IRI, renal tubular epithelial cells manifested swelling, degeneration, necrosis, and inflammatory ceils infiltration. After treated with PTX, the pathological change of the kidney was significantly alleviated, and inflammatory cells infiltration reduced. In FIX group,the kidney tissue pathological change was ameliorated, and the values of serum Cr, the scores of renal tubules were significantly lower than those in IRI group (P<0.05). Compared with IRI group, the levels of TNF-α in bemegenate of kidney in FIX group were significandy lower at 0 h, 1 h,4 h and 24 h after reperfiJsion (P<0.05). Compared with IRI group, the levels of MDA, ICAM-I in homogenate of kidney in FIX group were obviously lower at4 h, 24 h after reperfusion (P<0.05), while the level of SOD in homogenate of kidney was significantly higher (P<0.05). TNF-α correlated with levels of MDA, SOD, ICAM-1 ( r =0.801, -0.895,0.838,and P<0.01). Conclusions PTX had the protective effect on renal IRI by directly inhibited expression of TNF-α in kidney tissue,then decreased expression of MDA, ICAM-1 and ameliorated the inflammatory ceils infiltration in kidney.

Endothelial progenitor cells (EPCs) exist in bone marrow, umbilical cord blood and peripheral blood of adult mammals, including humans. Furthermore, the discovery of EPCs has led to the notion of adult vasculogenesis, in which bone marrow (BM)-derived EPCs home to and incorporate into sites of new blood vessel formation, where they differentiate into endothelial cells, which is consistent with postnatal vasculogenesis. It has become apparent that circulating BM-derived EPCs are involved in promoting physiologic and pathologic neovascularization, such as wound healing and tumor growth. They are of great clinical importance in pro- or anti-angiogenic therapies. [
3.Role of PI3K/Akt in endothelial progenitor cells differentiation in vitro

BACKGROUND:The molecule mechanism underlying endothelial progenitor cells(EPCs) differentiation into mature endothelial cells remains poorly understood.It requires for the regulation of the expression of various genes inside cells by various signal pathways,PI3K/Akt maybe included.OBJECTIVE:To investigate the role of PI3K/Akt in EPCs differentiation.DESIGN,TIME AND SETTING:A group control in vitro experiment was performed at the Pathology Laboratory in Shengjing Hospital of China Medical University from September 2007 to March 2008.MATERIALS:Wistar rats aged 4-6 weeks,of either gender.METHODS:EPCs were isolated from rat bone marrow with density gradient centrifugation.Then difference-speed adherence screening method was used to obtain the second time adherent cells,which then were inoculated in culture flask.After 5 days of culture,cells were collected to be indentified with laser confocal microscopy.The differentiating EPCs were those that have positive results of both AC133 and vWF stainings.MAIN OUTCOME MEASURES:The mRNA and the protein expression of AC133,vWF,PI3K and Akt in EPCs were detected with Western blot and reverse transcription-polymerase chain reaction(RT-PCR) after 0,3,7,10 and 14 days of inoculation respectively.RESULTS:According to the RT-PCR and Western blot detection,AC133 showed the strongest expression at day 0,the weaker at day 3 and none at day 7,10 and 14(P

N 6-methyladenine (m 6A) is the most abundant internal RNA modification found in a variety of eukaryotic messenger RNAs and long non-coding RNAs, which is dynamically regulated by m 6A methyltransferases and demethylases, and mainly works after recognized by m 6A binding proteins. m 6A is involved in almost every step of the RNA life cycle, including RNA transcription, splicing, localization, nuclear transport, translation, and degradation. Disorder of m 6A modification can lead to RNA dysfunction and abnormal gene expression. m 6A modification regulators express abnormally in various cancers, and play an important role in cancer formation, proliferation, differentiation, invasion and metastasis.
5.Effects of Inhibitor of Growth 4 on the Proliferation,Migration and Expression of Angiogenesis Related Factors in Endothelial Cells

Objective To investigate the effects of inhibitor of growth 4(ING4)on the proliferation,migration and the expression of angiogenesis related factors such as VEGF,MMP-2,MMP-9 in endothelial cells. Methods Human umbilical vein endothelial cells(HUVECs)were cultured in vitro;ING4 plasmid and siRNA were constructed and transfected to HUVECs;the proliferation of HUVECs was evaluated by MTT assay;the ability of migration was evaluated by Transwell assay;real-time PCR and Western blotting were used to determine the expression of mRNA and pro-tein of angiogenesis related factors such as VEGF,MMP-2,and MMP-9. Results MTT and Transwell assay showed that ING4 has the ability to inhibit the proliferation and migration of HUVECs,and the results of real-time PCR and Western blotting proved that ING4 can inhibit the expres-sion of angiogenesis related factors such as VEGF,MMP-2,and MMP-9. Conclusion ING4 can inhibit the proliferation and migration of HU-VECs,down-regulate the expression of angiogenesis related factors such as VEGF,MMP-2,MMP-9,and inhibit angiogenesis.
6.The multivariable analysis of prognostic factors of elderly patients with nosocomial Candidemia in intensive care unit

Objective To evaluate the prognostic factors of elderly patients with noscomial Candidemia in intensive care unit. Methods A retrospective study was carried out in 75 elderly patients with noscomial Candidemia from 2001 to 2008. Multivariate logistic regression analysis was performed to find the correlations of prognosis with clinical and biochemical parameters. Results A total of 75 Candida strains were isolated from blood. The proportion of non-albicans species reached to 70.7%. Fluconazole-resistant candidiasis was increased. Overall mortality rate was 48.0%.Multivariate logistic regression analysis showed that septic shock, comorbid bacteremia, higher serum creatinine and sequential organ failure assessment (SOFA) score more than 10 were independent predictors for in-hospital mortality (Ward=6.34, 5.15, 8.04, 6.82, all P>0.05). Conclusions Noscomical Candidemia in elderly critical illness patients leads a high mortality, the proportion of nonalbicans species and fluconazole-resistant candidiasis increase. The blood culture and drug sensitive test should be performed routinely to provide proper evidence for antifungal therapy. Monitoring the high risk factors, effective and reasonable therapeutics are the guarantee for reducing the mortality induced by Candidemia.
7.Preliminary study on ventricular-arterial coupling of patients with hypertension by ultrasonography

Objective To evaluate the cardiovascular stiffness and its coupling of patients with hypertension by ultrasound.Methods Fifty patients with essential hypertension and 30 age- and gendermatched subjects without hypertension,diabetes and other cardiovascular diseases as control group were enrolled in this study.The parameters of structure and function of left ventricle,blood flow were measured by echocardiography.The blood pressure and carotid-femoral pulse wave velocity (CFPWV) were also measured.The derived corresponding parameter:end-systolic pressure(ESP),effective arterial elastance (Ea),end-diastolic (Ed),end-systolic ventricular elastance (Ees) and Ea/ Ees were calculated respectively.Results Ees was correlated positively with ejection fraction (r =0.378,P =0.005),while Ea was correlated positively with CFPWV( r =0.289,P <0.001).Ed was correlated negatively with e/a ( r =- 0.333,P =0.027).Posterior wall of left ventricle was correlated positively with Ed and Ea( r =0.388,P =0.016; r =0.336,P =0.026).Ea and Ed in patients with hypertension were significantly higher than those in control group( P <0.05),but there was no significant difference of Ees and Ea/ Ees between two groups( P > 0.05).Conclusions Arterial stiffness is associated with ventricular stiffness,and their matching relation can be applied to evaluate ventricular-arterial coupling.
8.Role of elemene in Inhibiting tumor angiogenesis

Elemene is a novel universal non-cytotoxic antitumor drug. Based on downregulation of VEGF expression, induction of apoptosis of vascular endothelial cells and suppression of growth of tumor cell, elemene is becoming helpfully a promising inhibitor of angiogenesis, however, the detail mechanisms remains unclear and needs to be further elucidated.
9.Preliminary study on new parameter of myocardial elasticity

Objective To discuss the value of the new parameter of myocardial elasticity derived from circumferential strain.Methods Right ventrieular apical pacing was established on the beagle dogs.Before and after pacing,the peak of early diastolic flow velocity (E peak),peak of atrial flow velocity (A peak),and the ratio of A/E were measured.The peak pressure-rise rate (+ dp/dtmax),peak pressure-fall rate (-dp/dt max),left ventricular systolic pressure(LVSP),LV diastolic pressure(LVDP)were measured by cardiac catheterization.The circumferential strain at the levels of base (CSRA),papillary (CSPM) and cardiac apex (CSAP) were measured,and the parameter of myocardial clasticity at the levels of base (EBA),papillary (EPM) and cardiac apex (EAP) were measured and calculated,respectively.Results ①After pacing,the + dp/dt and LVDP were increased,but the-dp/dtmax was decreased (P <0.05).② After pacing,the CSPM and CSAP were all decreased,but EBA,EPM,EA were all increased(P <0.05).③ The areas under the receiver operating characteristic curves (ROC) of CSAP,CSPM,EAP,EPM and EBA were all 1,and the area under the curve of CSBA was 0.687.EAB,EPM and EAP were significant negative correlation with CSPM and CSAP,but are positive correlated with A/E and LVDP (P < 0.05).Conclusions The parameter of myocardial elasticity derived from circumferential strain includes useful information about myocardial deformation and function.The new parameter can be applied to assess the diastolic function of left ventricle.
10.Over the Counter Dispensing Model in Outpatient Dispensary

OBJECTIVE:To study the effect of over the counter dispensing model on hospital pharmaceutical work quality improvement and its social benefits. METHODS: The practice of over the counter dispensing model in the outpatient dispensary in our hospital was compared with the traditional model of drug dispensing through a small window. RESULTS: Over the counter dispensing service model can help improve the work efficiency in outpatient dispensary, strengthen pharmacists' consciousness on pharmaceutical care. CONCLUSION: Over the counter dispensing model can be applied in large and medium-sized hospitals.
