1.The application of tympanic membrane plus adenoidectomy in children with secretoryotitis media

Objective To explore the application effect of grommet insertion and adenoidectomy application effect in the treatment of children with secretory otitis media.Methods 64 children with secretory otitis media were randomly divided into two groups,32 cases in the observation group,32 cases in the control group.The control group used a simple tympanic membrane in the treatment of catheter,the observation group was treated with grommet insertion and adenoidectomy,follow-up 6 months curative effect were observed.Results The control group,the observation group after treatment backbone transmission difference were (12.1 ± 1.9)dB,(4.5 ± 1.2)db,two groups of backbone transmission errors were significantly lower than that before treatment (t =4.432,5.357,all P < 0.05),compared with the control group,the observation group was significantly lower (t =10.299,P < 0.05).The control group,the observation group of eustachian tube function recovery rate were 25.0%,68.8%,there was significant difference between two groups (x2 =3.96,P < 0.05).Total effective rate in the observation group was 93.8 %,which was higher than 71.9% in the control group(x2 =5.42,P < 0.05).Conclusion Tympanic membrane and adenoidectomy in the treatment of children with secretoryotitis media has significant effect.
2.Risk Factors of Hospital-acquired Infection among Uremia Patients and Nursing Strategy

OBJECTIVE To observe the clinical characteristics and risk factors of the hospital-acquired infection among uremia patients.METHODS The clinical data of 187 cases of uremia were retrospectivety analyzed during from Apr 2003 to Dec 2006.RESULTS There were 61 cases with hospital-acquired infection.The infection rate was 32.6%.The repiratory(54.1%) and urinary(21.3%) systems were the main infection sites.The main pathogens were Escherichia coli,Klebsiella pneumoniae,Staphylococcus epidermidis,S.aureus and fungi.The related factors were renal function,serum albumin,hemoglobin,duration in hospital,age and invasive medical procedures.CONCLUSIONS The hospital-acquired infection with uremia is with higher rate in respiratory system.The risk factors are renal failure,decrease in serum albumin and hemoglobin,abuse use of antibiotics and invasive medical procedures and deserve corresponding nursing strategy.
3.Clinical Observation of the Effects of Mirtazapine on Life Quality Improvement in Malignant Tumor Patients with Moderately Severe Depression

Objective To observe the effects of mirtazapine to improve the life quality of malignant tumor patients with moderate or severe depres-sion. Methods Patients with mood disorders screened by the physician were then graded by the Hamilton Depression Scale(HAMD)under the tu-telage of ward psychological consultant. Fifty patients with moderate or severe depression were selected by psychological doctors and recruited for the study. After obtaining consent,the patients received 15-60 mg/d mirtazapine every night for 9 weeks. According to the Quality of Life Rating Scale (QOL),the scores of these patients were evaluated at the pretreatment,the 3rd week and the 9th week of the post-treatment. Then,the life quality conditions before and after treatment were revaluated. Results The scores of HAMD were 33.16±7.25,28.74±6.24,and 22.58±6.71 at the pretreat-ment,the 3rd week and the 9th week of the post-treatment,respectively. There were significantly different among the HAMD scores at the pretreat-ment,the 3rd week and the 9th week of the post-treatment.(F=30.99,P<0.01). The scores of HAMD were improved significantly between the pre-treatment and the 3rd week of the post-treatment(P<0.05). The scores of HAMD were also improved significantly between the 3rd week and the 9th week of the post-treatment(P<0.05). After treatment by mirtazapine,the appetite,spirit,sleep,fatigue,pain,the reorganization of cancer,the attitude towards treatment,the side effect of treatment and the facial expression were significantly improved(P<0.01). Conclusion Mirtazapine could improve the life quality of malignant tumor patients with moderate or severe depression.
4.Air Pollution and Risk of Stroke Occurrence in Shanghai City:a Time-series Study

Objective To research the acute adverse effects of air pollution on the risk of stroke in the air pollution exposed people.Methods Daily counts of strokes occurrence (2004-2007) were obtained from the population-based stroke registry in one district of Shanghai.Air pollution data was obtained from the Shanghai Environmental Monitoring Center.A semi-parametric generalized additive model was used to study the acute effects of air pollution on stroke occurrence after controlling for long-term and seasonal trends,weather variables,and day of the week.Results The relative risks of stroke occurrence for per 10 ?g/m3 increase of PM10,SO2 and NO2 were 1.02 (95%CI:1.01-1.03),1.05 (95%CI:1.04-1.06) and 1.09 (95%CI:1.08-1.10),respectively.Conclusion The study has provided a new evidence for the association between air pollution and risk of stroke occurrence.
5.Effects of erythropoietin on nestin expression in neural stem cells of neonatal rats with hypoxia-ischemia brain damage

BACKGROUND: Nestin is a specific antigen of neural stem cells which widely expressed in lesion of nervous system and brain regeneration.Thus,nestin expression is commonly used to assess whether lesion or damage of the nervous system can promote neural regeneration.OBJECTIVE: To investigate the effects of erythropoietin(EPO)on nestin expression in neural stem cells after hypoxia-ischemia brain damage(HIBD)in neonatal rats from the angles of neural regeneration and activation of neural stem cells.METHODS: HIBD model was established by ligation of the right common carotid artery along with 2-hour 8% hypoxia exposure in neonatal rats.The control group was not subjected to hypoxia-ischemia,and the right common carotid artery was dissociated.The treatment group received an intraperitoneal injection of recombinant human erythropoietin(rh-Epo,5 000 IU/kg)once a day for three days after hypoxia/ischemia,while the two other groups intraperitoneally received normal saline at the same time.In each group,rats were randomly executed immediately,at 4,7,14 days after operation(n = 8).The nestin expression in hippocampal dentate gyrus region was examined by immunohistochemical staining and image quantitative analysis respectively.RESULTS AND CONCLUSION: The number of nestin-positive cells was significantly increased in HIBD group compared to control group at all time points(P < 0.05),and it was also significantly increased in treatment group than the other two groups at all time points(P < 0.05).The numbers of nestin-positive cells in hippocampal dentate gyrus region were significantly increased,and peaked on day 7 after operation in the three groups.The results showed that exogenous rh-Epo could enhance the expression of nestin in hippocampal dentate gyrus region of neonatal rats with HIBD,and promote the proliferation of neural stem cells,rh-Epo plays an important role in the regeneration and repair of neurocytes damaged by hypoxia-ischemia.
6.Cloning, expression and bioactivity analysis of human granulysin

Objective To obtain recombinant human granulysin using prokaryotic expression system. Methods Total RNA was extracted from cultured PBMC. Granulysin gene segments were obtained with granulysin-specific primers by RT-PCR and then inserted into pET32a(+) plasmid. After identification by DNA sequence, pET-GN-LY9K and pET-GNLY15K were transferred to E. Coli Rosetta (DE3). The fusion protein was identified by SDS-PAGE and Western blot. The bioactivity of granulysin fusion protein was measured by MTT assay. Results The prokaryotic expression vectors pET-GNLY9K and pET-GNLY15K were successfully constructed. The corresponding protein was highly expressed in E. Coli. Recombinant protein was specifically bound by anti-granulysin antibody. GNLY9K fusion protein significantly inhibited the growth of A549 cells in a dose-dependent manner, while GN-LY15K had little effect on the growth of A549. Conclusion Granulysins with different mw were successfully expressed using prokaryotic expression system, which might be helpful for the further study of granulysin.
7.Information Controlling Evaluation of Concentration Exceeding Standard of Positive Ions in Parenteral Nutrition Orders

Objective: To evaluate the self-developed intelligent assist order system for parenteral nutrition on the control of concentration exceeding standard of positive ions.Methods: The special comments, pharmacist audit and information control were used to intervene the concentration exceeding standard of positive ions in parenteral nutrition orders and the effect of the three intervention methods were evaluated.Results: For the intervention of concentration exceeding standard of positive ions in parenteral nutrition orders, the information control was better than the special comments and pharmacists audit.Conclusion: The intelligent assist order system for parenteral nutrition can solve the problem of concentration exceeding standard and promote the rational application of parenteral nutrition in hospitals.

9.Increased enrichment of fetal nuclear red blood cells via polyvinylidene difluoride by combined interfering membrane ionic channels

Objective The effectiveness on the enrichment of nuclear red blood cells(NRBC) in umbilical cord blood by polyvinylidene difluoride membrane was investigated with intervention on membrane ionic channels.Methods NRBC was isolated by the isolation solution with a density of 1.067 g/ml.The rates of the cell enrichment using verapamil and furosemide intervention by discontinuous density centrifugation were examined by flow cytometry.The NRBC that past through PVDF membrane with pore size of 5 ?m were checked and quantified by nested-PCR.Results The optimal centrifugation density was 1.067 g/ml.The mean purity of NRBC after centrifugation was 2.54%,yet the purity after intervention increased to 9.36%(3.7-fold increment).The NRBC passing through PVDF membrane was from 0.83% to 6.15%,with enrichment of 7.4 times.When NRBC was 120 cells/ml,the density of the nested PCR products increased by nearly 4 times after intervention(4.48 vs 17.78).Conclusions Verapamil and furosemide could affect the morphology of cells and promote the rate of NRBC enrichment.
10.Study on Assessment of Absorbed Radiation Dose of Astronaut in Super-large Solar Particle Events

Objective To develope energic spectrum of super-large solar particle events (SPE),human body model and a Monte Carlo method for estimating the organ dose of astronauts in SPE without the shielding of geomagnetic field. Methods A voxel-based human phantom and shielding model of spacecraft were developed and the Monte Carlo program was compiled and applied to calculate the doses of astronaut's organs with the transport protons of the spectra of super-large SPEs in 1956 and 1972, normalized by the proton fluence for energy greater than 30 MeV. Results The Monte Carlo program was developed successfully and the organ dose under different shielding thickness were also calculated with the program in the normalized spectra of SPEs in 1956 and 1972 without the effects of geomagnetic field. They were similar with prior studies. Conclusion The results are reliable by the comparison with other calculations of prior researchers. Exposure to super-large SPE without the shielding of geomagnetic field would result in a very large organ absorbed dose and may be very danger for manned spaceflight.
