Objective To develop a portable jet-flow device for preliminary debridement of wound mainly in battlefield, which can also be used for the treatment of nuclear or chemical weapon injury at wartime and peacetime. Methods The device was composed of a dynamic system,a servo system and consumable accessories, and developed based on a pneumaticpower scheme.Composite power unit free of dependence on electric power transferred the pneumatic power to three kinds of disinfectants through the four-way pipes. Results The device with easy operation consumed a few disinfectant and completed debridement within 30 minutes, with infection avoided effectively. Conclusion The device gains advantages in low weight, compact structure, function and low cost, and can be used for early debridement in field conditions.

Objective To discuss the frequency of ventilator pipe replacement in ICU ward for reducing the frequency of VAP occurrence caused by bacterial colonization in ventilator-line. Methods 30 patients hospitalized to ICU ward from Janurary,2008 to June, 2009 who had mechanical-ventilation above 48h were randomly divided into group A, B and C with 30 patients in each group, then bacteria culture and strain analysis were employed for the pipeline sample of ventilator inspiratory side at the time point to be use and used mechanical ventilation for 1 day, 3 days and 7 days. Results There was no bacteria grew after ventilator-line sterilization, and positive ratio of bacterial culture in 1d, 3d and 7d were 46.66%,53.33% and 100.00%, respectively. Conclusions The frequency of ventilator pipe replacement should be shortened in patients with abdominal infection, to replace every 3 days is suitable in ICU where infectious patients gathered.

Objective To investigate the efficacy and safety of transrectal ultrasound guided transurethral balloon dilation of the prostate (TUDP) for the treatment of benign prostatic hyperplasia (BPH).Methods A total of 23 men with BPH who had undergone TUDP were retrospectively analyzed, including 16 men with indwelling urinary catheters before the operation.During the TUDP,the prostatic apex and membranous urethra were dilated by inner balloon, and the prostatic urethra and bladder neck were dilated by outer balloon. The patients were followed up at the 1st,3rd,6th and 12th month after the operation, and the observation parameters included subjective symptoms, such as international prostatic symptom score (IPSS) and quality of life (QOL) score, and objective parameters, such as maximum flow rate (Qmax) and post-void residual (PVR).Results The operation time was 30-165 min,and the intraoperative hemorrhage volume was 5-50 ml.The IPSS scores at 1st,3rd,6th and 12th month after the operation were 10.4± 4.2,8.7±3.2,9.5±4.6 and 8.3±1.5 respectively, which were significantly decreased in comparison to the IPSS score (22.0±7.2) before the operation (P<0.05).The QOL scores at 1st,3rd,6th and 12th month after the operation were 2.1±1.1,1.6±1.0,1.8±1.1 and 1.6±1.0 respectively, which were significantly improved in comparison to the QOL score (4.9±0.9) before the operation (P<0.05).The Qmax at 1st,3rd,6th and 12th month after the operation were (10.5±3.4),(12.4±4.2),(10.9±3.9) and (12.7±4.6) ml/s respectively, which were significantly increased in comparison to the Qmax(1.9± 1.9 ml/s) before the operation (P<0.05).There were 5,4,2 cases of urinary incontinence at the 1st,2nd and 3rd month after the operation, but they recovered at the 4th month follow-up.Besides,there were 3 cases suffered from epididymitis. Conclusion Transrectal ultrasound guided TUDP is a safe, effective and cheap option for the treatment of BPH,especially for the primary hospitals.

Objective To evaluate the feasibility, security and efficiency of general anaesthesia compared with regional anaesthesia for minimally-invasive thyroid surgery(MITS)with short-stay discharge. Methods 103 cases undergoing MITS with short-stay discharge from Jan.2013 to Jun.2013 in Fuzhou General Hospital were collected.54 patients were operated under regional anaesthesia and 49 under general anaesthesia. All variables of patients including demographic characteristics, vital signs (blood pressure, heart rate, blood glucose) during the operation, duration and type of operation, postoperative complications, difficulty in airway management and anesthesia consumption were recorded and analyzed. Results There was no difference in respect to level of blood glucose, length of hospital stay, discharge time, patient or operator satisfaction rate, anesthesia or operation complications rate between the two groups.Blood pressure (T1:(90 ± 7.0) vs (79 ± 8.3) mmHg,T2:(88 ± 6.8) vs (80 ±7.6) mmHg,P <0.05) and heart rate(T1:(130 ± 18.2) vs(101 ± 12.0)/min,T2:(120 ± 19.7) vs(103 ± 13.3)/min,P < 0.05)level were higher, while the recovery time of anesthesia((0.3 ± 0.14)vs(0.8 ± 0.35)h,P < 0.05)) and anesthesia consumption ((1350 ± 78) vs (2580 ± 220) yuan,P < 0.05)) were lower in the regional anaesthesia group compared with those in the general anaesthesia group. Conclusions Both general anaesthesia and regional anaesthesia for MITS with short-stay discharge are safe and effective.General anesthesia has less psychological trauma, while regional anaesthesia has shorter recovery time and lower anesthetic consumption.

OBJECTIVETo explore material basis of the pharmacological differences between the roasting and pro-roasting Raphani Semen.
METHODThe two new sulfur-containing compounds (A209 and B221) were found changed after processing in the water decoction. The common precursor-C3 of A209 and B211 and the precursor of C3 were seperated and purified. Their transforming relationship was proved.
RESULTThe result showed that glucosinolates could decompose into sulforaphane and transform into A209 and B221 further in the boiling process.
CONCLUSIONThis study provides some experimental evidences for revealing the mechanism of Raphani Semen processing.
