1.Psychological evaluation of the patients with spasmodic torticollis

Objective To investigate the psychological factors in the spasmodic torticollis (ST) patients and find the possible relationship between mental disorder and the pathogenesis of spasmodic torticollis. Methods All the 21 ST patients admitted were asked to complete the SCL-90. The result was compared with the norm of Chinese. Results Somatization (1.67 ±0.59) scores, anxiety (1.95 ±0.74) scores, phobic (1.63 ±0.59) scores and psychotic (1.56 ±0.60) scores in ST patients, they were higher significantly than the norm of Chinese [ ( 1.37 ± 0.48 ), ( 1.39 ± 0.43 ), ( 1.23 ± 0.41 ), ( 1.29 ± 0.42) scores](P <0.05). No significant differences were found the other factors between the ST patients and norm of Chinese. Conclusion Somatization,anxiety,phobic and psychotic are correlated with ST,they may likely contribute to the pathogenesis of ST.
2.Microsurgical procedures and the results of supraclinoid internal carotid aneurysms

Objective To analyze the features of supraclinoid internal carotid aneurysms. The experience and mistakes in the microsurgery of supraclinoid internal carotid aneurysms were summarized to improve the results. Methods The data of 24 cases of supraclinoid internal carotid aneurysms that were operated from May 2004 to March 2009 were reviewed. There were 6 cases of small or middle aneurysms, 18 cases of large or giant aneurysms. The operative methods included direct clipping, partial excision of aneurysm with reconstruction of the parent artery, aneurysm trapping with extracerebral/intracerebral (EC-IC) bypass, etc. Results Among the 24 cases, 8 cases had good results. Ten cases occurred hemiplegja post operation immediately, with 5 cases recovered to grade IV ,3 cases had aphasia,5 cases with vision acuity fall,4 cases had abnormal eyeball movement,2 cases had continued coma and died finally. Second operations were performed in 3 cases. The total operation rate of well results was 66.7% (16/24). Mortality rate was 8.3% (2/24). Conclusions The operative technique of the supraclinoid internal carotid aneurysms is difficult because of the location and the large size of the aneurysms. The temporary obstruction' of the parent artery is often needed, so the aneurysm can be partially resected and clipped. The results of clipping of large or giant aneurysms are better than the other methods. But in large or giant aneurysms that had thrombus, the EC-IC bypass is needed to prevent large infarction. The bypass procedure may induce some severe complications that result in some poor outcome.
3.Preoperative diagnosis and microsurgery of the optic gliomas:retrospective analysis of 7 patients

Objective: To summarize the clinical characteristics and therapeutic experience of optic gliomas. Methods: The clinical data of 7 patients of our department in the past 3 years were analyzed. Results: In this series of optic glioma patients, the average age was 18.7 years, the ratio of men to women was 4∶3,and 100% of the tumors were sub-totally removed. Diabetes inspidus was the most comman postoperative complication. Conclusion: The optic glioma is benign intracranial tumor with good prognosis. The key point of treatment is surgical resection combined with proper postoperative radiotherapy.
4.Related factors for the motor recovery after vectriculoperitoneal shunt in normal pressure hydrocephalus

Objective Ventriculoperitoneal shunt (VPS)is preferable in treatment for normal pressure hydrocephalus (NPH). In this study, the factors related to the motor recovery after VPS were analysed.Method Twenty-six patients were treated with VPS, and four factors, that are duration of symptoms, open lumbar puncture(LP) pressure, improvement after LP ,and motor recovery of twelve months after operation,were assessed. Results Twelve months after VPS, the better motor recovery after VPS was related to shorter duration of symptoms, higher open LP pressure, and symptom improvement after LP. Conclusion The patients with shorter duration of symptoms, higher open LP pressure, and symptom improvement after LP are often responsive to VPS.
